The recall includes Power Plus automotive chargers that plug into automotive cigarette lighters, and Impact wrench kits containing these chargers. A label on the front of the charger reads, "Milwaukee...POWER-PLUS 9.6 Volt - 18 Volt AUTOMOTIVE CHARGERS." A nameplate on the back of the charger lists the catalog number. Impact wrench kits containing these chargers have serial numbers within the range listed below. The serial number is located on the side of the Impact wrench kit. Mongoose Mountain Bicycles (Brunswick) – Fork Defect (2002)

The recall includes only Ballistic 105 forks installed on Mongoose and Roadmaster model bicycles manufactured by Brunswick Corp. The forks on these bikes are black with decals that read "BALLISTIC" and "105" on the sides of the suspension fork legs. A small label on the bottom of the bicycle reads "Brunswick." The forks on the Mongoose A40 model bicycles and forks on bicycles not manufactured by Brunswick Corp. are not included in the recall. By Us International Co. Ltd., of Taiwan, the manufacturer of the bicycle forks, previously announced the recall of 13,500 of these forks in May 2000. The recall was expanded to about 40,000 forks in February 2001. By Us International is no longer cooperating with the recall. Brunswick Corp. is expanding the recall to include all Ballistic model 105 forks sold on the Mongoose and Roadmaster mountain bicycles they manufactured (except the Mongoose A40). The previous recalls were limited to forks with certain serial numbers.

Zowie's 123 Book (Disney) – Design Problem (2002)

The Zowie's 123 book is a twelve page white board book that measures 6.5 inches by 6.5 inches, with a yellow plastic abacus-like toy attached to the back book cover. The abacus toy has five small beads: blue, green, red, yellow and orange. The beads slide along a plastic track. The book is based on the Rolie, Polie, Olie television series and depicts the Zowie character on its cover. The book can be identified with the ISBN code number 078683307-6, found on the back cover in the bottom corner.

The recalled gasoline-powered generators have the model number BSI550-W printed on the fuel tank between the handles. These generators also have the date code "09-10-01" printed on a bar code label on the gasoline tank support rail. The generators are red with a black fuel tank and have the brand "Porter-Cable" printed on the fuel tank. Generators produced on dates other than "09-10-01" are not included in this recall.

Curious George Toys (BRIO) – Detachable Backpack Risk (2001)

These Curious George monkey plush toys are dressed in a yellow plastic space suit with matching gloves. They have a detachable backpack with a red fabric-filled mobile phone attached. The small mobile phone is 1.25 inches by 1.5 inches, and is connected to a gray, mesh backpack by a 4.75-inch string sewn into one of the backpack's seams. A label in the collar of the space suit reads, "Curious George by BRIO." This is an "Activity George," which is written on the packaging, and is labeled for children ages 18 months and older. The back of the box reads: "Removable backpack with mobile phone!" The recalled toy has model number 32900 written on the front of the box.

Announce Recall of Play Sets to Replace Swing Chains WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Rainbow Play Systems Inc., of Brookings, S.D., is voluntarily recalling about 7,000 play sets to replace swing chains. The chains can break while children are swinging and cause the swing seats to fall to the ground. Children can suffer injuries from falling to the ground while swinging. Rainbow Play Systems has received seven reports of chains breaking. A 3-year old boy suffered scrapes and a bruise to his head when he fell to the ground after a chain broke. During inspections, distributors found four defective chains. The swing chains are used on all models of Rainbow Play Systems play set swings and are covered in yellow or green vinyl. Rainbow play sets are made with redwood, and feature more than 50 different models and custom designed sets. In addition to swings, the play sets features include various types of slides, rope ladders, rings, ladders, canvas- covered forts and castles, and fire poles. "Rainbow Play Systems" is written on the slides. Rainbow Play System stores nationwide sold the play sets with the defective chains from March 2000 through June 2000 for between $1,200 and $6,000. Consumers should stop using the swings on these play sets immediately, and contact the company to determine if the chains are defective. If so, remove the chains and return them to the store where purchased for free replacement chains. The nipples on these pacifiers could separate from the base, posing a choking hazard to children. CPSC and Atico are not aware of any injuries involving these pacifiers. This recall is being conducted to prevent the possibility of injuries. The pacifiers have an off-white plastic, oblong shield with a ring handle. The diaper bag gift set included a pacifier, a baby bottle and changing pad. Writing on the side of the diaper bag read, "Doody Bag." The pacifier with clip has a circular moss-green plastic clip which is designed to attach to a child's clothing. The clip has a mauve-colored ribbon designed to attach to the pacifier. The words, "Plug me in & enjoy the silence" are written on the clip. Bargain Wholesale, of City of Commerce, Calif., is recalling about 7,000 units of the Onika Steam/Dry Spray Iron. The unlabeled iron is made of gray plastic. Writing on the electric cord reads, in part, "CH0001708-91." The iron, along with a water measuring cup and a nap brush, is packaged in a red box labeled, in part, "Onika Steam/Dry Spray Iron ... ZQYD150g ... UL ... Made in Taiwan." Discount stores in the western U.S. sold these irons from January 1997 through June 1998 for about $4. PROBLEM: The irons have inappropriate size wiring, which can cause a fire. The plastic housings of the irons are not resistant to ignition. Additionally, the irons present a potential scald hazard during use. If they are tilted downward, a shower of hot water is unexpectedly emitted. The electrical wiring can pull out from the splices of these decorative light sets, exposing live wires and presenting an electrocution hazard. CPSC is not aware of any injuries involving these lights. This recall is being issued to prevent the possibility of injury. The recalled curtain-style strings of lights are different from other decorative holiday lights because they use a horizontal electrical wire and have vertical strings of four to seven lights that hang down to give an icicle effect when hung on a roof's edge or a tree. These lights have a "splicing connector" located at the point where the string of lights hang down from the horizontal electrical wire. The splicing connector can pull apart or break exposing a metal wire, causing the electrical shock. To see if you have these indoor/outdoor lights, check for the names "Curtain," "Icicle" and "Wonder Lights." These miniature lights come in strings of clear bulbs and multicolored bulbs, and have either white or green wires. They were sold in strings of 100 or 150 lights in boxes containing one string, and sets of three or more strings. According to Underwriters Laboratories (UL), unauthorized holographic UL labels, located on each string near the plug, with the following numbers identify the lights being recalled: E115759 E126258 E127357 E127522 E48723 E64444 E65770 E97593 Curtain-style lights without splice connectors are not involved in this recall. The lights are packaged in cardboard display boxes, some with a cellophane window to view the product, and are labeled in part, "Made in China." Many of the boxes feature a decorated home at night with the holiday lights on. Retail stores, including BJ's Wholesale, Builders Square, CVS Pharmacy, Caldor, Frank's Nursery & Crafts, Hechinger, Hobby Lobby, Home Quarters, M J Designs, Montgomery Ward, Rite Aid Pharmacy, Sam's Club, Shopko, Venture, Walgreens and Wal-Mart sold the lights nationwide from September 1995 to November 1997 for between $6 and $19, depending on the size of the sets purchased. Consumers should stop using these light sets immediately and return them to the store where purchased for a refund. CPSC and S-B Power Tool Company are alerting 150 owners of portable Bosch Mini-Grinders that the wrong wheel guard was sold with the grinders. If the misfitting wheel guard loosens and touches the high-speed grinding wheel, consumers could be injured by parts of the shattered wheel. S-B Power Tool urges recent purchasers of portable Bosch Mini-Grinders, models 1347A and 1347AK, to check the inside of the wheel guard collar for a metal bump that would prevent the guard from being clamped on the grinder securely. These guards should be removed and returned for the correct-fitting guards. S-B Power Tool has sold 150 portable Bosch Mini-Grinders with the wrong wheel guard since August 1996 in mass merchandise stores nationwide for about $150. No injuries have been reported. However, the company is alerting customers to check for and replace the misfitting guard to prevent potential serious injuries. Dried flowers inside the candle may catch fire as the candle burns, presenting a fire hazard. Bath & Body Works has received three reports of the lavender material inside the candle igniting. No injuries have been reported. The cylindrical, white wax candles, measuring 4 inches high and 4 inches in diameter, contain small purple lavender flowers. A label on the bottom of the candle reads, "Garden Lavender Botanical Candle." Bath & Body Works stores sold the lavender candles nationwide from May 1996 through August 1996 for about $15. Announce Ceramic Potpourri Burner Recall WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission(CPSC), Bath & Body Works Inc. of Columbus, Ohio, is recalling about 18,700 ceramic potpourri burners. The ceramic potpourri burner may contain hairline cracks that can cause the burner to fracture or shatter while in use. As a result, people can be burned by hot wax and potpourri. Bath & Body Works is aware of nine incidents in which the ceramic potpourri burner cracked, exposing consumers to flames, hot potpourri, and hot wax. Two consumers received burns to the hands as they attempted to extinguish the flames. The white ceramic potpourri burner, a cylinder-shaped container measuring 5.5 inches high and 4.25 inches in diameter, is decorated with various floral designs. To heat the burner, consumers place candles in a round hole in the back of the potpourri burner. Bath & Body Works retail stores sold the ceramic potpourri burners nationwide from April 1995 through April 1996 for approximately $10. ANNOUNCE REMOTE CONTROL SWITCH RECALL WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Bandwagon Inc. of Wilmington, Mass., is recalling about 25,450 remote control, hand-operated switches used to turn on and off electrical appliances, such as lights, televisions, radios, fans, and Christmas tree lights. To operate this switch, an appliance is plugged into the plug of the remote switch, which is plugged into the wall outlet. From a remote location, a consumer can operate an appliance with the remote control switch, which functions as a kind of on/off extension cord. If the switch's cover detaches exposing electrical wires, consumers can receive an electrical burn, a shock, or possibly an electrocution. Bandwagon is aware of a woman who received an electrical shock while operating a bedroom fan. The shock resulted in burn injuries to her right shoulder area, right forearm and biceps, left thumb, left forefinger, and to her left middle finger which was subsequently removed. The remote control switch is a white, rounded ceramic disc with a flat plastic base, measuring about one inch high and two inches in diameter. A black plastic button in the center of the disc turns the switch on and off. A 12-foot white cord runs from the disc to the end of a two-sided plug. One side accepts a plug from an appliance, and the other side plugs into an outlet. Mail-order catalogs sold the remote control hand operated switches in the United States and Canada between December 1993 and 1996 for about $9. The switch is packaged in a white cardboard box labeled in part, "Item No. E8810 ... Made in Taiwan." Consumers should stop using the recalled remote control switch immediately and call Bandwagon at (888) 814-3600 between 9 a.m. and 4 p.m. eastern time to arrange for free delivery of a replacement switch.

Waterwick Humidifiers (Bemis) – performance upgrade (1995)

July 18, 1995 Release # 95-146 Applies only to units sold before July 18, 1995 WASHINGTON, D.C. - CPSC, Bemis Manufacturing Co. of Sheboygan Falls, Wisc., is announcing a recall to upgrade the safety of approximately 400,000 Waterwick whole house humidifiers. Two electrical switches on the humidifier can overheat and possibly catch fire if cleaning, descaling, and disinfecting solutions are misapplied directly onto the unit's electrical controls.Consumers should stop using the humidifiers and call Essick Air Products (which bought Bemis Mfg.) at 800-547-3888 between 7:30 am and 5:30 pm CT, Monday through Friday. Essick will arrange for a free pickup and repair of the humidifiers. To promote consumer participation in this safety upgrade program, a free two-stage Air-Care air filter with each upgraded unit will be included. The humidifiers come in a variety of colors and styles. The control panel in the upper right hand corner of the grill has two control knobs and is labelled in part, "BEMIS WATERWICK." Two 2.6-gallon or greater capacity white water bottles fit inside the humidifier's cabinet. May 30, 1995 Release # 95-126 WASHINGTON, D.C. - CPSC, Gainsborough Hardware Industries of Norcross, Ga., is recalling 8,000 Empire Style Metaline 600 Series porcelain door knob sets. The door knobs may crack or break during use resulting in hand lacerations.Gainsborough has received two reports of laceration injuries to the hand as a result of these door knobs breaking during use.The door knob sets sold from January 1991 until May 1993 for approximately $10 through 38 hardware and home improvement stores located nationwide. This door knob, which was sold in sets of two, is available in white and ivory and has a gold-colored oval backplate with a rope design around its perimeter. The number 178 appears on the underside of the backplate.Consumers should return the door knobs to the retailer for a full refund or a replacement door knob
