Omix Ada, Inc. (Omix Ada) is recalling certain Rugged Ridge All Terrain floor mats, Cabela's Custom-Fit floor mats, Line-X Truck Gear floor mats, and Tread Lightly aftermarket floor mats manufactured November 1, 2009, to August 27, 2014, for aftermarket use in Chevrolet and GMC full size trucks and SUV's. The affected floor mats may move forward unintentionally.
Failure of the primary collimator may result in unintentional movement of the device. The unintentional movement will not result in the generation of an error message.
This recall includes the 10 ft. by 20 ft. multi-purpose shelter with "Code Number: L-GZ761PST." The code number is printed on a label sewn into the top of side panels of the canopy fabric. The fabric canopies are white and cover an area of about 20 ft. by 24 ft. "Sunjoy Industries" and "Made in China" are printed under the barcode label on the product packaging.
The clinician did not realize the monitor was in END CASE mode and was expecting audible alarms. Visual alarms continue to work normally. The firm performed a retrospective analysis of all recalls since 2011 and discovered 222 additional serial numbers were missed and were not included in the previous recall under RES 59456 (recall number Z-3032-2011).

This recall includes Franklin & Ben Mason style 4-in-1 style cribs model number 5601 that were manufactured from January 2012 through August 2012. The model number and the date of manufacture are printed on a label on the inside of a side panel of the crib. Listed on the label as a "serial number," the following purchase order numbers are included in the recall: H004522, H004546, H004548 or H004777. The cribs have solid wood sideboards and wood rails on the front and back of the crib in varying heights and can convert to a daybed, toddler bed or full-sized bed. There is a storage drawer at the base of the crib with two metal knobs. The recalled cribs were sold in weathered gray and rustic brown, and measure 57 inches long by 33 inches wide by 51 inches high.
The product is designed to be used with artists' watercolor paints in small amounts to improve flow and wetting. It is clear and of low viscosity and packaged in a 59 ml clear glass jar with a dropper lid. Product code of 7001000-2 appears on the label. One of the following batch numbers is printed on the bottom of the jar: 365254, 366319, 366405 or 366626.

The firm became aware of a potential problem that was initiated by a customer complaint. After consultation with the manufacturer, it was discovered that a resistor was incorrectly placed within the circuit board on the monitor. This change to the resistor was a planned change to address a product improvement (improve battery drain). However, this modification rendered the monitor's alarm via the nurse call communication port to not function.
