NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2010 HEAVY DUTY TRANSIT BUSES, MODELS D35LFR, D40LFR, D60LFR, DE40LFR, DE60LFR, XD35, XDE35, AND XDE40, MANUFACTURED FROM SEPTEMBER 30, 2010, THROUGH JUNE 3, 2011, EQUIPPED WITH A CUMMINS ISB, ISL, ISC, or ISX DIESEL ENGINE. THE V-BAND CLAMP ASSEMBLY, PART NUMBERS Q187684, Q187685, AND Q187686, USED TO CONNECT THE INLET AND OUTLET CONE SECTIONS TO THE DIESEL OXIDATION CATALYST (DOC)/DIESEL PARTICULATE FILTER (DPF) INCLUDE A T-BOLT WHICH CAN FRACTURE AND FAIL WHEN SUBJECTED TO STRESS OR LOAD. A FRACTURE OR FAILURE OF THE T-BOLT MAY CAUSE THE CLAMP TO LOOSEN AND THE INLET OR OUTLET SECTIONS ATTACHED TO THE DOC/DPF HOUSING TO DISCONNECT.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2005-2010 HEAVY DUTY TRANSIT BUSES. THE THREE RETAINING BOLTS FOR THE OUTPUT FLANGE ON THE ISE PROPULSION SYSTEM ARE SUBJECT TO FATIGUE FAILURE. FAILURE OF THE THREE BOLTS RETAINING THE OUTPUT FLANGE TO THE COMBINATION GEAR BOX ON THE ISE PROPULSION SYSTEM COULD RESULT IN A SEPARATION OF A PORTION OF THE DRIVESHAFT, WITH THE OUTPUT FLANGE ATTACHED, FROM THE VEHICLE.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2007-2011 LOW FLOOR TRANSIT BUSES PRODUCED FROM MAY 24, 2007, THROUGH MARCH 1, 2011 AND EQUIPPED WITH TEETER LEVER ASSEMBLIES. THERE IS INSUFFICIENT ENGAGEMENT BETWEEN THE TEETER LEVER BASE AND THE CAM PLATE.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2008-2009 HEAVY DUTY TRANSIT BUSES. THE RADIUS ROD BRACKET WHICH ATTACHES THE FRONT AXLE RADIUS RODS TO THE VEHICLE FRAME MAY BE MISSING TWO OF THE FOUR REQUIRED WELDS.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2008 C40LFR, D40LF AND DE60LF HEAVY DUTY TRANSIT BUSES, AS WELL AS MODEL YEAR 2009 DE60LFA HEAVY DUTY TRANSIT BUSES. THE PASSENGER ASSIST GRAB STRAP HANDLES HAVE EXPERIENCED MULTIPLE FAILURES IN THE FIELD.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2008 D40LFR AND MODEL YEAR 2009 DE60LFR HEAVY DUTY LOW FLOOR TRANSIT BUSES EQUIPPED WITH TRW IGEN3 INTERMEDIATE STEERING COLUMNS. THE WELD AROUND THE IGEN3 STEERING SHAFT TUBE THAT SECURES THE SPLINE TUBE TO THE INTERMEDIATE TUBE MAY BE MIS-LOCATED.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2009 GE40LFR TRANSIT BUSES EQUIPPED WITH REMOTE RELEASE BELTS SUPPLIED BY AMERICAN SEATING. THESE BELTS ARE THE SUBJECT OF DEFECT REPORT 09E-065. THEY MAY NOT PROPERLY LOCK UP AND AS RESULT MAY NOT PROPERLY RESTRAIN THE MOBILITY DEVICE WHEN THE BUS DECELERATES.
NEW FLYER IS RECALLING CERTAIN MODEL YEAR 2004-2009 TRANSIT BUSES. THESE MODELS HAVE AN INSPECTION PANEL LOCATED ABOVE THE DRIVE SHAFT IN THE PASSENGER COMPARTMENT FLOORING FOR EASY INSPECTION AND REMOVAL OF THE DRIVE SHAFT. THIS PANEL MAY NOT BE OF SUFFICIENT STRENGTH TO PREVENT FOREIGN MATTER FROM ENTERING THE PASSENGER COMPARTMENT IN THE EVENT OF A DRIVE SHAFT FAILURE.
June 23, 1995 Release # 95-139 WASHINGTON, D.C.- CPSC, Radio Flyer, Inc. of Chicago, Ill., is voluntarily recalling 24,975 Tornado and Power Tricycles. A weld on the front fork may fail during use, which may cause a child riding the tricycle to fall.CPSC is aware of one incident in which a child received minor injuries from a fall after the weld on the front fork failed during use. This recall is being conducted to prevent the possibility of injury arising from a defective fork weld.This recall applies to the 300 Tornado (blue with light blue trim), PTG Power Trike (green with red trim), PTP Power Trike (purple with pink trim), and PTB Power Trike (blue with light blue trim) tricycles. Each tricycle's front wheel is approximately 12 inches in diameter. All of the tricycles except the 300 Tornado have Radio Flyer decals on the frame. The Tornado tricycles were sold nationwide through warehouse club outlets located in the Western United States between August 1, 1994 and December 31, 1994 for approximately $29.99. The 3 Power Trike models were sold nationwide through various retailers from August 1, 1994 through June, 1995 for approximately $39.99.Consumers should inspect their tricycle to determine whether it is affected by this recall. If the 1.5 inch washer between the front fork and tricycle frame is the same color as the frame, consumers should take the tricycle away from children immediately. Consumers should call Radio Flyer, Inc. at (800) 621-7613 for a free replacement piece. Consumers may also return the tricycle to the store where purchased for an exchange or refund. Radio Flyer tricycles with black 1.5 inch washers between the front fork and frame are not affected by this recall
Blazon-Flexible Flyer "Gym Sets" & "Horse Attachment" Retrofit NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE July 13, 1988 Release # 88-052 Retrofit Program Announced for Blazon-Flexible Flyer, Inc. "Gym Sets" And "Horse Attachment" WASHINGTON, DC - In cooperation with the U.S. Consumer Product Safety Commission, Blazon-Flexible Flyer, Inc. of West Point, Mississippi, today announced a voluntary retrofit program involving certain model gym sets. The potential hazard involves breakage due to wear of plastic tube end bearing brackets connecting the Teeter Totter, Lawn Swing and Horse Attachment, to the top support bar of the Gym Set. Breakage of this connection may cause the unit to fall while children are seated on it. There were approximately 159,000 units manufactured from November 4, 1986 to May 16, 1987. The company has received three reports of injury associated with the product. The Gym Sets retail for between $60 and $300, and are identified by the following model numbers located on the main cross support. A diagram is attached for easy identification. Those model numbers are 30002, 30012, 41000, 41001, 41121, 41122, 41129, 42021, 42112, 42131, 42132, 42139, 42334, 45131, 61221, 62231, 62431, 62439, 62541, 62624, 62631, 62739, 69231, 69233, 69432, 69461, 69541, 69567, 87538, and horse attachment model numbers 99597, 9565 Manufactured from November 4, 1986 to May 16, 1987. Blazon-Flexible Flyer, Inc. has notified all retail customers of its revised production and the availability of the "fix kits" for consumers. Consumers who identify Gym Sets as being those involved in the corrective action may call Blazon-Flexible Flyer, Inc. direct on their toll-free line 1-800-521-6233. They will be supplied with a "retrofit kit" and revised instruction free of charge.
Flying Toy Helicopter Being Recalled NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR RELEASE August 19, 1986 Release # 86-52 Flying Toy Helicopter Being Recalled Washington, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Go Fly A Kite, Inc., E. Haddam, Connecticut is recalling a flying toy helicopter called Model WL-322 Flying Copter. Approximately 10,000 "Flying Copters" were distributed by the firm during the past year nationwide. This toy has previously been recalled by other companies who have marketed it. The toy, imported by many firms, has been responsible for eye injuries and facial lacerations. Some of the injuries have resulted in permanent blindness. This toy has previously been recalled by other companies who have marketed it. The toy has been imported by many firms and CPSC is involved in contacting each of these firms as quickly as they can be identified. CPSC has investigated accidents in which this toy has been responsible for serious eye injuries and facial lacerations not only to children but adults as well. The "Flying Copter", which retailed for $3.50 is a 7 l/2 inch plastic flying toy helicopter with a hand launching device. The helicopter is mostly white with three red plastic rotor blades. The launching handle is green. Assembly is required before the toy can be flown. It is this assembly which is difficult and if done improperly, could allow the rotor blades to come off and strike the user or bystander. Consumers are urged to discontinue use and to return the toy helicopter to the store where it was purchased for a full refund.
