ELKHART COACH HAS NOTIFIED NHTSA OF A DEFECT IN CERTAIN TRANSIT BUSES FITTED WITH RICON PUBLIC USE LIFTS. THE RESTRAINT BELT ON THESE LIFTS WAS DESIGNED TO PREVENT THE LIFT FROM MOVING, UNLESS THE TONGUE OF THE BELT IS FULLY ENGAGED AND LATCHED IN THE BUCKLE. RICON HAS DISCOVERED, HOWEVER, THAT IT IS POSSIBLE FOR THE OPERATOR TO INSERT THE BELT TONGUE INTO THE BUCKLE WITHOUT FULLY ENGAGING THE LATCHING MECHANISM. IN THIS CASE, THE RESTRAINT BELT WOULD APPEAR TO BE LATCHED, AND THE OPERATOR COULD RUN THE LIFT. IF THE RESTRAINT BELT TONGUE IS NOT FULLY ENGAGED INTO THE RESTRAINT BELT BUCKLE, AND LATCHED, THE LIFT MAY OPERATE WITH AN OCCUPANT ON THE PLATFORM.
ELKHART COACH HAS NOTIFIED NHTSA ABOUT A DEFECT IN CERTAIN TRANSIT BUSES EQUIPPED WITH CERTAIN RICON S SERIES AND K SERIES L-MODEL WHEELCHAIR LIFTS, DUE TO A PROBLEM IN THE SOFTWARE THAT CONTROLS THE LIFT'S "UP" FUNCTION. IF THE "UP" SWITCH IS RELEASED BEFORE THE FLOOR LEVEL LIMIT SWITCH IS ACTIVATED, THE LIFT MAY CONTINUE ITS UPWARD TRAVEL FOR APPROXIMATELY 1 SECOND BEFORE STOPPING. THE CONTINUED UPWARD TRAVEL MAY OVERRUN THE FLOOR LEVEL CUT OFF POINT.
VAN-CON HAS NOTIFIED NHTSA OF A DEFECT IN CERTAIN MODEL YEAR 2008-2009 HANDICAP SCHOOL BUSES BUILT ON FORD AND CHEVY CHASSIS, FITTED WITH RICON PUBLIC USE LIFTS. THE RESTRAINT BELT ON THESE LIFTS WAS DESIGNED TO PREVENT THE LIFT FROM MOVING, UNLESS THE TONGUE OF THE BELT IS FULLY ENGAGED AND LATCHED IN THE BUCKLE. This recall involves Stabilicers Lite cleats with model number BNX9. The cleats are worn over footwear for traction in winter walking conditions. The recalled cleats were sold in blue, orange, and pink. Black Stabilicers Lite cleats are not included in this recall. The cleats were sold in sizes small through extra large. The name Stabilicers Lite appears in raised letters on the underside of the harness.
This recall involves the Notifier brand operating system firmware control panels in fire alarms. The control panel is black or red with a silver screen. The panels control display and emergency communications. "NOTIFIER" or "NOTIFIER by Honeywell" is printed on the front of the fire alarm unit. This recall includes the following model numbers: Firmware version 11.2.2 for NFS2-640 and NFS-320 control panels and firmware version 3.14.1 for and NFS-640 control panels.
This recall involves the external power adapters sold with the following DYMO LabelWriter® 400 series printers: DYMO LabelWriter 400, DYMO LabelWriter 400 Turbo, DYMO LabelWriter Twin Turbo, DYMO LabelWriter Duo and DYMO Desktop Mailing Solution with housing date codes 2407, 2507, 2607 or 2707. The date code is printed on the plastic housing of the adapter. Power adapters with "RoHS" printed on the top right of the product label are not included in this recall.
This recall involves Gamewell-FCI 7100 Series fire alarm control panels. The panels provide control, display and annunciation information. This recall involves Caterpillar brand steel toe logger boots. The boots were sold in women's and men's sizes. Women's boots were sold in black and have stock number P88030. Men's boots were sold in brown and black and have stock numbers P88033 or P88034. Stock numbers are printed on a label on the tongue of the shoe. The Caterpillar logo and "EH" are also printed on the label. The word "Caterpillar" is printed on the outside of the boots.
The software upgrade involves the Gamewell-FCI model 600 Series control panel using System Sensor compatible line cards, SmartScan. These panels are components of the Gamewell-FCI 600 Series fire alarm system. They provide the signaling line circuits and display information for the system.
This recall involves ceramic, gold-matte finish curling irons. The irons have a 1, 1¼, or 1½-inch barrel, metal counter rest, vertically placed ON-OFF buttons, and a heat set dial that ranges from 0 to 30. A four-digit date code can be found on the plug prong. Date codes included in this recall range from November 2005 through July 2006 (ex. 0706). The name "Conair" is printed on the handle of the iron.
The software upgrade involves Gamewell-FCI model ILI-MB-E3 and ILI-S-E3 loop control boards. These boards are components of the Gamewell-FCI E3 Series fire alarm system. They provide the signaling line circuits and display information for the system.
This recall involves all Segway PT i167, i170 and i180 ("i Series") models, the p133 ("p Series"), XT ("cross-terrain transporter"), GT ("golf transporter") and i2 models. These units were sold to consumers and commercial customers. All e167 ("e Series") models, which were sold to commercial users, also are included in this recall. No other models are involved in this recall. The name, "Segway", appears on the front bumper and/or on the handlebars of the personal transporter.
GeoDeck™ is a composite decking product that looks similar to natural wood and is sold in three colors, including: driftwood, cedar and mahogany. The decking materials were manufactured between April 2002 and October 2003. Each decking and railing component has a manufacturing date stamp (day/month/year) on the cut end, which may still be visible on uninstalled materials.
The recall involves Holmes® models HFH6498-U, HFH6500-U and HFH6500TG-U and Bionaire® model BFH3530-U. The recalled heaters also have date codes beginning with 2604 through 3804 for all units, except for Holmes® model 6500TG-U, which has date codes ranging from 2604 through 4704. The model and date code can be found on the silver label on the back of the unit. The heater fans are 26 inches tall; come in all gray, two-tone gray, or black and gray; and have either the word Holmes® or Bionaire® printed on the front of the base.
The recall includes Rival® Crock-Pot® slow cookers with model numbers 3040, 3735, 5025, 5070 and 5445. The model number is printed on the UL label located on the bottom of the base. The recalled Rival® slow cooker has a removable ceramic bowl that sits inside of a metal base. The Rival® logo is printed on the front of the unit above the control knob. The bases are round or oval shaped and were sold in various colors and designs. A date code is stamped on the side of one prong of the power plug. The first two digits represent the week of manufacture and the last two digits represent the year of manufacture. Any plug with a date code from 0199 (1st week of 1999) to 3504 (35th week of 2004) is included in this recall or the previous recall.
These refillable butane-fueled lighters are silver in color with a black sliding switch at the middle of the product. Two models of the lighters are included in this recall: a 7-inch model (model number 324007) and a 12-inch model (model number 324012). "Aristo Home & Garden" and "Fire Stick" are printed on the products' original packaging.
The Zippo MPL (Multi-Purpose Lighter) uses refillable butane fuel, is child resistant, and measures 8 inches long and 1 inch wide. The utility lighter is finished in black metal or silver metal with a chrome nozzle. The black unit is packaged in a blister pack with black and red graphics and the silver unit is in a silver gift box. Only utility lighters with these date codes are recalled: G03, H03, I03, ZH03. The date code is on the black adjuster knob on the bottom of the lighter.
Only Rival Crock-Pot® slow cookers with model numbers 3040, 3735, 5025, 5070 and 5445 manufactured before May 2002 are included in the recall. A date code is stamped onto the plug at the end of the power cord attached to each unit. The first two digits represent the week of manufacture, and the last two digits represent the year of manufacture. Any plug with a date code from 0199 (1st week of 1999) to 1802 (18th week of 2002) is included in this recall.
The fire alarm control panel includes the central processing unit (CPU-2020 and CPU-2) and Display Interface Assembly (DIA-2020 and DIA-2) components of Notifier model AM2020 and AFP1010 fire alarm control panels with date codes from 0100 to 2003 and firmware version 3.0 and 3.01. The date codes appear on the back outer edge of the PC boards.
These are 12-foot, 13-foot, and 14-foot trampolines, which were sold separately, and also banded together with safety enclosures. They were sold under the brand names Hedstrom and NBF. The brand name is written on the warning labels found on the products. The recalled trampolines have model numbers 10136, 101366, 101442, 10146, 102369, 102949, 10321, 103217 or 10381. They also have four-digit date codes ranging from 0403 through 2103 with the last two digits always being 03. Model I.D. labels showing the model number and date code are located on one of the frame rail legs on the trampoline.
