Choking Hazard

The battery-operated, multicolored toy flip phone is red, blue and yellow with green, blue, yellow and orange dial buttons. Each button has its own sound: a ring, a beeping dialing tune, a busy signal and a "hello." The toy phone has a blue hard-plastic antenna with a ball-shaped tip that makes a clickity-clack sound.

Wolf Gas Ranges – Potential Malfunction (2001)

These gas ranges were sold under the Wolf® and Wolf Gourmet® brand names, which are written on the front of the ranges. These are 30-inch, 36-inch, 48-inch, and 60-inch natural gas and LP ranges with the following serial numbers: 60-10000000 through 60-1099999 and 11000957 through 11006106. In most units, the serial number is located on the left side of the range beneath the removable burner pan on the range top. On some 48-inch models, the serial number is located in the same area on the right side. The ranges were either stainless steel or black steel.

Microwave-Hood Combinations (Whirlpool) – Safety Concern (2001)

They have serial numbers that begin with XC. Open the door to locate the serial and model numbers. Countertop microwaves are not included in this recall.

Cosco Playpens (Dorel) – Safety Problem (2001)

The recalled playpens were sold under the "Zip n Go," "Okie Dokie," and "Carters" brand names and were manufactured by Cosco. Only model numbers 05-361, 05-362, 05-363, and 05-364 are included in this recall. The model numbers are located on a label on the bottom of the playpen, on the metal tubular frame near one of the corners. The label reads in part, "MANUFACTURED IN CHINA FOR COSCO INC." The 28-inch-by-40-inch playpens have four mesh sides on a folding metal frame. The side rails have the name "Zip n Go, " "Okie Dokie," or "Carters" written on them. The padded floorboard forms a carrying case for the playpen.

Announce Recall of Children's Toy Toolboxes WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Racing Champions Ertl Inc. (RCE), of Dyersville, Iowa, is voluntarily recalling about 11,600 John Deere Kids Toolboxes. The toy's carrying handle can break, creating small parts and a sharp plastic edge that pose a choking and laceration hazard to young children. CPSC and RCE have not received any reports of injuries. This recall is being conducted to prevent the possibility of injuries. The children's toolbox is a soft-shelled, plastic, green box with a yellow handle and yellow raised letters that read "John Deere." A John Deere Kids logo with a fawn inside a red oval is imprinted on the front of the toolbox. The toolbox is sold with a green and yellow plastic ratchet wrench and pliers inside. Stamped on the bottom of the product is "JOHN DEERE LICENSED PRODUCT" and "MADE IN CHINA." All yellow-handled toolboxes are included in this recall. John Deere dealers and other farm supply retail stores nationwide sold the toys from June 2001 through September 2001 for about $13. Parents and caregivers should take the toy away from young children immediately and contact RCE at (866) 898-4999 anytime for instructions on returning these toys and receiving a free replacement toolbox. (REI) Announce Recall of Children's Baseball Caps PRODUCT: Children's Baseball Caps - Recreational Equipment Inc. of Kent, Wash., is voluntarily recalling about 1,600 REI Kids Super Caps. The caps are sold in three colors (New Denim, Pumpkin or Lichen) with the letters "REI" embroidered on the back left side of the cap and a decorative compass embroidered on the front center of the cap. There is a size-adjustment cord, located on the rear of the cap that has two parts; a black toggle and a black snap-on plastic pull. The caps were sold nationwide at retail stores, mail order catalogs and on the Internet. They were sold from February 20, 2001 through June 26, 2001 for $10.00. PROBLEM: The adjustment cords, located on the rear of the hats, use a snap-on plastic pull. The snap-on plastic pull could separate from the cord, posing a choking hazard to young children. INCIDENTS/INJURIES: REI Product Quality Assurance (PQA) received one report of a parent finding the snap-on plastic pull in the mouth of her two-year old son. No injuries have been reported. WHAT TO DO: Consumers should stop using the recalled children's baseball caps immediately and return it to the nearest REI retailer to receive a refund. Announce Recall of Infant Seat Pads WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Basic Comfort Inc., of Denver, Colo., is voluntarily recalling about 16,000 infant seat pads. The seat pad has two shoulder straps that create a v-shaped opening around the head and neck, presenting a strangulation hazard to young children. Basic Comfort received one report of an 8-month old girl who was caught by the neck in the shoulder straps of the seat pad after falling sideways off a chair. The child was removed from the seat pad without injury. The seat pad was sold under The Baby Sitter brand name. The infant seat pad is designed to restrain a child on a chair in a sitting position. It has a padded back and seat. The seat pad is attached to a chair by Velcro straps attached to the sides of the pad. The front of the seat pad has two straps attached near the top of the pad that go over a child's shoulders and connect to a single crotch strap. The seat pad is a denim blue color with white restraint straps. Department and juvenile products stores nationwide including JC Penney and One Step Ahead sold these infant seat pads from October 1998 through May 2001 for about $11. Consumers should stop using the infant pads immediately and contact Basic Comfort at (800) 456-8687 between 8 a.m. and 5 p.m. MT to receive a replacement. The noses, bells, and eyes of the bears can detach, presenting a choking hazard to young children. The eyes also present a sharp point hazard. CPSC and Inca Imports have not received any reports of injuries. This recall is being conducted to prevent the possibility of injuries. The recalled bears come in three sizes: small (8 1/2"), medium (12") and large (17"). The recalled bears have plastic eyes and noses, and red ribbon bows with bells. They are made of alpaca fur. The bears have a small sticker on the back side of the ribbon which reads in part, "Made in Peru." Stores in tourist areas throughout Arizona, California, Colorado, Montana, Nebraska, Oklahoma, and Texas sold the bears from July 1997 through February 2001 for between $20 to $50. Consumers should take these stuffed bears away from young children immediately and return them to the store where they were purchased for a full refund. CPSC, Vermont Castings, Majestic Products Announce Recall of Space Heaters WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Vermont Castings, Majestic Products (VCMP), of Ontario, Canada, recalled about 3,900 space heaters. Overheating inside the space heaters can cause the exterior to become extremely hot, posing fire and burn hazards to consumers. VCMP had received three reports of the space heaters overheating, resulting in scorched floors and wall coverings. No injuries have been reported The recalled heaters are white with a silver grill cover. The vent-less heaters have the name "Vermont Castings" on the label directly below the grill. The heater model numbers start with the letters VBVH or VPVH and can be found on a rating plate on the left side of the heater. Independent retailers in the southeastern U.S. sold these recalled heaters from August 2000 to January 2001 for $200 to $400. Consumers should stop using the heaters immediately and discard or destroy them. The company is no longer in business and a remedy is no longer available.

Creative Consumer Concepts Inc. manufactured these toys, which were distributed with kids' meals at Whataburger Restaurants. The clear, plastic lens covers on these compass toys can come off and pose a choking hazard to young children.Creative Consumer Concepts, Inc. has received four reports of the lens covers of the compass toys coming off. No injuries have been reported.The "Whatagear" compass is a plastic toy with a compass in its center. The toys have varied features, such as flashlights, magnifying lenses or tools. They come in various solid colors. Options 5 High Chairs (Cosco) – Multiple Use Defect (2000)

The Cosco Options 5 High Chairs being recalled have five options for use: high chair, infant feeding, youth chair, play chair and booster seat. The chair's seat can be used in a reclined infant position or adjusted into seven upright height positions for toddlers. It has four tray positions, a vinyl seat pad and a removable footrest. "Cosco" is on the chair's leg support bar, tray and on a sticker on the bottom of the seat. The restraint system consists of a waist/crotch belt and a plastic T-bar attached to the tray. The recalled chairs can be identified by model number 03-286 and manufacture date, which are located on the bottom of the seat.

Honda Rancher TRX350 ATVs A mounting bracket on the front suspension of the Honda Rancher TRX350 models could be welded improperly to the frame. If this mounting bracket breaks, the vehicle could lose steering control and crash. These ATVs are red, orange or olive green. "Honda Rancher" is written on the sides of the ATV's fuel tank. Chest Freezers (AHAM) – Vintage Safety Risk (2000)

The freezers that are part of this program were made before 1970 by more than 40 manufacturers, a number of whom do not exist today. Consumers can determine if their chest freezer poses a hazard by trying to open the freezer without using the handle. If the freezer can be opened by pulling up on sides of the lid, it is not a hazard. If the lid only opens by using the handle, it needs to be properly disposed of or disabled. Up to 9 million chest freezers were manufactured between 1945 and 1970 before voluntary safety standards went into effect, allowing freezers to be opened from the inside. The freezers that are part of this program were made before 1970 by more than 40 manufacturers, a number of whom do not exist today. Consumers can determine if their chest freezer poses a hazard by trying to open the freezer without using the handle. If the freezer can be opened by pulling up on sides of the lid, it is not a hazard. If the lid only opens by using the handle, it needs to be properly disposed of or disabled.

Battery Chargers (DeWALT) – Charging Malfunction (2000)

The battery chargers are model number DW9116, which is written on the front and bottom of the charger housing. The chargers are black with yellow lettering. "DeWALT" is written on the front of the chargers. The recalled chargers have date codes from 9927EM through 9952EM and 0001EM through 0031EM located on the bottom of the charger. These chargers are for 9.6- to 18-volt batteries used with DeWALT cordless tools. Chargers with a yellow or green dot sticker near the date code already have been repaired, and are not recalled.

The recalled extension cords and cord reels are equipped with a LockJaw™ locking plug. The LockJaw™ plug has the following words embedded on the red, green or yellow locking button, located on the side of the plug, "LOCK, WOODS®, OPEN" or "LOCK, OPEN". The extension cords were sold under the "Woods LockJaw™" and "Yellow Jacket® LockJaw™" brand names in various wire gauges, lengths and colors. The cord reels are 30 feet long, and were sold under the "Woods' ACDelco" and "LumaPro" brand names in various wire gauges and colors. Some of the extension cords and cord reels have two-prong plugs and some have three- prong plugs. Some have the brand name on the cord. They are UL listed, assembled in the United States or Malaysia with a LockJaw™ plug made in China.

CPSC, Appliance Co. of America Announce Recall of Toaster Ovens WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Appliance Co. of America, of Great Neck, N.Y., recalled about 7,000 Welbilt toaster ovens. The ovens' heating elements can break, causing the glass door to shatter. The toasters could short circuit, causing electrical shocks and fires. Appliance Co. of America has received 14 reports of heating elements breaking. In one incident, the glass door broke, and in another, a countertop was scorched. No injuries have been reported. These Welbilt toaster ovens with rotisserie and griddle have model number TR660 on the back of the oven. The oven is white and black about 17 inches wide and 12 inches high, and has a glass door. Three knobs on the right side of the oven control the temperature; function, such as broil, rotisserie, toast and bake; and length of time. "WELBILT® Toaster Oven With Rotisserie & Griddle" is written on the oven's lower-right front. Club, discount and department stores, and mail order catalogs sold the toaster ovens nationwide from January 2000 through May 2000 for about $100. Consumers should stop using these toaster ovens immediately and discard or destroy them. The firm is out of business and cannot be contacted.
