CONTACT: August 16, 1990 (301) 504-7908 Release # 90-141 Child Electrocuted Floor Lamps Recalled WASHINGTON, DC -- The Coaster Company of America, headquartered in Sante Fe Springs, CA, is voluntarily recalling some 1,900 brass floor lamps because of a possible electrocution hazard. This hazard may be present even though the lamp switch is in the "off" position. Brass floor lamp models 2607, 2618 and 2620 sold nationwide since January 1987 are being recalled. They sold for about $50 each. All lamps feature three metal support legs bolted to a metal central post by metal screws or bolts. - Model 2607 features a single light socket atop the center post and three planter supports at the ends of the U- shaped support legs. - Model 2618 features a triple light socket and planter supports at the ends of the support legs. - Model 2620 features a glass tea table which rests atop three C-shaped metal support legs bolted to the central post. The company said certain screws holding the legs to the lamp's center post may be too long and could pierce the power cord insulation, energizing the entire brass frame. The U.S. Consumer Product Safety Commission has learned that one child was electrocuted in Alaska. Consumers with any of the lamps should immediately unplug them from electrical outlets and return the product to the store where purchased for a full refund of the purchase price. Recalls Decoy Duck Pull Toys WASHINGTON, DC -- Artsana of America Inc. of New York, NY is voluntarily recalling approximately 54,062 Decoy Duck Pull Toys because of small parts and sharp points that could pose a fatal choking hazard to young children. This recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). When CPSC tested the ducks in accordance with regulations under the Federal Hazardous Substances Act, the screw holding the wing in place separated constituting a small part and a sharp point. The 10-inch Decoy Duck with pull cord is made of hard plastic, yellow and orange in color. It has four wheels, and movable feet, wings, eyes and beak. When pulled along the toy makes a sound that is similar to a real duck's voice. Just above the duck's right wing is the name CHICCO printed in blue. The box was labeled in part: "No. 63002.0 Decoy Duck Made in ITALY." The Decoy Ducks were sold nationwide since 1987 for $10.99 each. Neither the CPSC nor the company is aware of any injuries involving this product. This voluntary recall is being conducted to prevent the possibility of injury. Rowing Action Exerciser (Dynamic Classics) – Retrofit Program (1990)

June 21, 1990 Release # 90-115 Retrofit Program Announced For Spring Exercisers WASHINGTON, D.C.--Dynamic Classics, Ltd. of New York City, in cooperation with the U.S. Consumer Product Safety Commission (CPSC), today announced a repair program for its spring exercisers called the "Rowing Action Exerciser." Early models of the exerciser were involved in injuries resulting mainly from the spring breaking or the footbar separating during use. Some two million exercisers of the earlier model were sold nationwide from 1984 to August 1987. Approximately 60 incidents have been reported to CPSC and the firm, most of which involved a broken spring or a footbar assemble coming apart. Consumers have experienced bruises, cuts and lacerations to the face and other parts of the body when the handlebar or spring stuck the user. Incidents showed that the spring of the Rowing Action Exerciser, if extended too far to withstand the stress, could snap. Dynamic Classics is offering owners of the older models a free retrofit kit, which is designed to prevent the spring breaking and the footbar separating. The retrofit kit includes a safety cord to prevent over-stretching and over-stressing the spring, metal brackets to prevent over-stressing the spring loop, and a new extended footbar to prevent separation. Rowing Action Exercisers sold after August 1987 do not need retrofitting. They can be identified by metal brackets already attached to the top and bottom of the spring and a safety cord installed inside the spring. Earlier models had no brackets and a longer safety cord. Consumers should immediately discontinue the use of older models of the Rowing Action Exerciser, which do not have metal brackets attached to both ends of the spring. June 8, 1990 Release # 90-109 Taco Bell Recalls Sports Water Bottle Because Of Choking Hazard WASHINGTON, D.C.-- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Taco Bell Corporation of Irvine, California, today announced a voluntary recall of 300,000 sports water bottles distributed by the firm in a recent promotion. The bottle's mouthpiece can be pulled apart and swallowed by children, presenting a potential choking hazard. The water bottle promotion at some 2700 Taco Bell stores nationwide began June 4, 1990, and was available to Taco Bell customers for $0.99 with a food purchase. Taco Bell stopped the program June 7, 1990 when they received a report from a customer that his 21-month-old child had dismantled the bottle's mouthpiece and attempted to swallow it. The child was not injured. Taco Bell has decided it would be in their customers best interest to discontinue sale of this bottle. The bottle, marketed for more than ten years by Specialized Bicycle Components, has three pieces: the main container, a cap and a small mouthpiece closure top. The top may be pulled apart and poses a potential choking hazard to children. Since many types of water bottles are very popular and also widely available in stores today, care should be taken not to allow young children to have access to the caps. May 21, 1990 Release # 90-098 San Francisco Music Box Company Recalls Plush Animals Because Of Choking Hazard WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), the San Francisco Music Box Company of San Francisco, CA is voluntarily recalling six plush animals imported from China because the eyes may come off posing a fatal choking hazard to children. The stuffed animals described below have eyes with washers on the back. The company discovered that the washers may be defective allowing the eyes to become detached from the plush animals. The plush animals all have music boxes inside and a silver wind up key protruding from the back. White Jointed Bear, #55-0046 plays "You are My Sunshine" is made of all white fur with a red ribbon tied around its neck. Beige Jointed Bear, #55-0061 plays "Don't Let Me Go," stands 15-1/2 inches tall has all beige fur and a red ribbon tied around its neck. Beige Small Jointed Bear, #55-0038 plays "It's a Small World" stands 9-1/2 inches tall has all beige fur with a red ribbon tied around its neck. Pink Bear, #55-0079 plays "A Baby's Lullabye" stands 9-1/2 inches tall, is extremely soft with all pink fur and a sewn in pink and white polka dot heart on its chest. Pink Monkey, #55-0095 plays "Zip-a-dee-doo-daa" stands 7- 1/2 inches tall is extremely soft with white face, hands and feet and a pink fur body. Beige Articulated Santa Bear, #55-0368 plays "Jingle Bells" stands 17 inches tall, has a sewn on red and white Santa's hat covering the right ear, a red coat with white trim that has a "Velcro" closure in the front, and a black felt belt with a metal buckle. May 8, 1990 Release # 90-091 North American Bear Company Issues Warning About Choking Hazard WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), and the New York State Attorney General's Office, North American Bear Co., Inc. of Chicago, IL is issuing a warning to purchasers of two products in their VanderBear Family line. "Alice VanderBear in High Tea Outfit" (Style #4152) and "Muffy VanderBear in Tree Trimming Outfit" (Style #7144) have accessories that may separate from their outfits and present a small parts choking hazard to young children. Before giving these plush bears to children, consumers are urged to remove the pearl necklace and buttons from the dress on Alice VanderBear, and to remove the gingerbread man from the pocket of Muffy VanderBear. Approximately 3,500 Alice VanderBears were sold nationwide from January 1, 1989 to January 31, 1990 for $62.00 each. The bear stands 18 inches tall and has soft beige fur. Alice is dressed in a pink and white print dress with a white collar and trim. She is also wearing a straw hat with black trim and a pink flower attached to the front. The three buttons on the back of her dress and the pearl necklace around her neck may detach producing small parts that could present a choking hazard to young children. Approximately 20,000 Muffy VanderBears were sold nationwide from July 1, 1989 to September 30, 1989 for $28.50 each. The bear stands 7-1/2 inches tall and has soft beige fur. Muffy is dressed in a red and white stripped long-sleeved Christmas dress with large red plastic slip-on shoes. She is also wearing a red and white apron which has a hard plastic gingerbread man in the pocket that may come loose presenting a choking hazard to children. Neither the CPSC nor the company is aware of any injuries involving these plush bears. This warning is being announced to prevent the possibility of injury. CPSC learned about Alice VanderBear and Muffy VanderBear from the Empire State Consumer Association and the U.S. Public Interest Research Group this past holiday season. March 15, 1990 Release # 90-050 WASHINGTON, D.C. -- Fisher-Price of East Aurora, N.Y., in cooperation with the U.S. Consumer Product Safety Commission (CPSC), today announced that it is asking consumers to modify the No. 1019 Fun Bus to eliminate a possible choking hazard. The Fun Bus, introduced in 1989, is a yellow bus with various activities, including a pop-up puppy, which is activated by rotating a roller on the front of the roof. The roller is wrapped with a thin band of decorative plastic with letters and numbers printed on it. Fisher-Price has received approximately seven reports of children chewing on the toy's roller. A few children have removed the decorative plastic band from the roller and began choking on it. This could result in a life-threatening situation for a child. Consumers owning a Fun Bus should immediately remove this plastic band by tearing it with a ball-point pen and then removing it from the roller. The band should then be discarded. Consumers with questions should contact Fisher-Price at 1-800-233-4004. Fisher-Price has sold about 160,000 of the toys nationwide. The approximate retail price is $16.99. December 6, 1989 Release # 90-022 WASHINGTON -- The Micromar Manufacturing Corporation is recalling all Heat Machine model HMB2000tc/vhc ceramic portable electric space heaters, sold nationwide since September of this year, to correct a possible fire hazard. Micromar of London, Ontario, Canada began the voluntary recall program in cooperation with the U.S. Consumer Product Safety Commission because they found that a potentially defective component had been used in the heaters which could possibly cause a fire. To date, the firm has had no reports of fires or injuries associated with the heaters. Nonetheless, the company has initiated a repair program for all potentially affected heaters in this line. Consumers having a Micromar Heat Machine 2000 model number HMG2000tc or HMG2000vhc, should stop using the heater and unplug it. The 14-inch stuffed bear, sold in gift and other speciality stores, may pose a potential choking hazard for children under age three because of heart-shaped buttons which may detach from the bear's dress. There have been no reports of injury. The brown bear, part of the Kensington Bear collection, is wearing a maroon print dress with white lace bloomers and a straw hat with a pink bow. Three heart-shaped buttons are sewn on the front of the bear's dress. Pink and blue ribbon also trims the bottom of the dress. There is no stock number attached to the bear. The hang tag attached to the product reads "The Kensington Bears". If consumers wish to keep the bear, they can eliminate the potential hazard by removing and disposing of the buttons. June 13, 1989 Release # 89-054 American International Recalling Doll Because Of Potential Choking Hazard WASHINGTON, D.C. -- American International of Brooklyn, New York is voluntarily recalling a toy baby doll because removable arms and legs may pose a potential choking hazard to young children.The product being recalled is a baby doll about six inches in height packaged in a clear carrying case with a smaller doll and a bottle, Item No. 2065PVC. The arms on both dolls are moveable, but the arms on the smaller doll can also come off and may be hazardous.Approximately 960 of these dolls were sold in the New York City area during October 1988 through December 1988 for approximately $1.99 each.Neither the U.S. Consumer Product Safety Commission nor American International is aware of any injuries associated with these dolls.Consumers having these dolls should take them away from children and return them to the retailer where purchased for a full refund of the purchase price. Rear Engine Riding Mowers (Toro) – Safety Concern (1989)

February 2, 1989 Release # 89-008 Toro Recalls Rear Engine Riding Mowers WASHINGTON - The U.S. Consumer Product Safety Commission (CPSC) announced today that it has reached an agreement with the Toro Company, Minneapolis, MN, whereby rear engine riding mowers manufactured by Toro between 1972 and 1979 are being recalled. Toro has learned that there is a possibility that, after starting the mower with the auxiliary rope starter, which is located under the hinged rear body/seat, the user can fall or sit on the rotating starter cup. In addition to the recall program, Toro has also agreed to pay the CPSC a civil penalty of $75,000 in settlement of allegations of the Commission staff which are denied by the company that the company violated the defect reporting requirements of the Consumer Product Safety Act, with regard to the riding mowers. The company has found that there is a possibility that the 1973 mowers can be started in gear if the shift linkage is not properly adjusted, and that 1972-1976 mowers can be accidentally kicked or bumped into gear. Severe laceration injuries to the buttocks have occurred when, after starting the engine, the user climbed on the mower and fell, or simply sat, on the exposed rotating starter cup while the seat was still raised. In cooperation with the CPSC, Toro is offering to install a recoil version of the auxiliary starter at no charge. Consumers should contact their local Toro Authorized or Master Service Dealer to arrange for this service. In addition, Toro is offering to inspect and/or adjust the shift linkage on the 1973 riding mowers.Toro has sent notices to some 19,000 registered owners of the 1974-1979 mowers and has posted notices of this recall program in dealer's showrooms. Consumers who own one of these mowers, but have not received notice, should contact their nearest Toro Service dealer or call Toro toll-free at 1-800-444-8676. HAGF Circuit Breakers (Challenger) – Safety Replacement (1988)

Release # 88-095 WASHINGTON, DC- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Challenger Electrical Equipment Corp. of Malvern, Pennsylvania, voluntarily offered free replacement of its 15 and 20 ampere type HAGF single pole ground-fault circuit-interrupter (GFCI) circuit breakers.These type HAGF-15 and type HAGF-20 circuit breakers were manufactured between February 22, 1988 and April 29, 1988, and most were sold nationwide to electrical product distributors during this period. Some may have been sold to consumers by retail hardware or lumber outlets. Approximately 9000 of these devices have not been located, some of which are believed to have been installed in residences. Type HAGF ground fault circuit breakers are devices, which provide protection to consumers against severe electric shock and electrocution as well as providing the normal circuit breaker functions of interrupting short circuits and electrical overloads.Although no electric shock incidents have been reported by Challenger, the firm recalled this product because is has determined, by quality control testing, that a mechanical part may become detached and prevent the ground fault feature of the circuit breaker from functioning. (The normal circuit breaker functions are not affected by this problem.)They protect residential 15 ampere and 20 ampere circuits including receptacle outlets in bathrooms, kitchens, basements, garages and outdoors as required by the National Electrical Code. The code also requires GFCI protection for receptacles in hotel and motel bathrooms. The HAGF circuit breakers, which may require replacement, are located in the electrical service panel. If the name "Challenger" is embossed on the front of the panel (on the door handle) and/or on the label on the back of the panel/door, that panel may contain circuit breakers requiring replacement. Affected circuit breakers have a yellow button and the word "test" in raised letters on one side; on the other side, the number 15 or 20 is printed on the handle in white letters between the raised words "ON" and "OFF." If the number on the handle is black or if there is a label it with white lettering next to the word "test , then the HAGF circuit breaker will not need to be replaced. Consumers should note that the HAGF circuit breakers which require replacement will only be found in dwellings which have been constructed since February 1988 or in which electrical work has been done since February 1988. Consumers who believe that they have installed HAGF circuit breakers requiring replacement should contact a licensed electrician to perform this work. Power to the panel should be shut off before inspecting and removing breakers.) S. Consumer Product Safety Commission and the Bic Corporation announced that Bic is voluntarily recalling its new regular size fixed flame lighters because, according to estimates of the firm, the flame on approximately 0.04% of these lighters may not extinguish. There have been no reported accidents or complaints about the lighters. Approximately 6.5 million of these new regular size fixed flame lighters have been distributed since July 21, 1988, when Bic began to distribute this new lighter. The majority of these lighters are still in warehouses or in the Bic distribution chain. Bic has already made the manufacturing changes necessary to solve the potential problem with the regular size fixed flame lighter. The regular size Bic fixed flame lighter is packaged in either a plastic pouch containing two lighters or in an open plastic tray. The plastic pouch states "with fixed flame." Consumers can identify the lighter because it does not contain a wheel, which permits the user to adjust the flame height. Bic Mini fixed flame and Bic regular size adjustable lighters, which represent 98% of the Bic lighters on the market, are not affected by this recall. Consumers should immediately stop using the regular size fixed flame Bic lighter and return it to the store where purchased for a full refund.

FOR IMMEDIATE RELEASE Originally issued July 27, 1988; Revised October 29, 2002 Release # 88-58 Fisher-Price To Modify "Pop-Up Playhouse" Toy WASHINGTON, DC - Fisher-Price of East Aurora, N.Y., in cooperation with the U.S. Consumer Product Safety Commission, today announced that it is voluntarily offering a free modification kit for more than 380,000 of its model 2306 "Pop-Up Playhouse" toy. The action is being taken because Fisher-Price and the Commission are concerned that the playhouse could unexpectedly come apart and cause serious eye or other injury to children or adults. Fisher-Price has received approximately 100 reports from consumers about parts of the children's playhouse suddenly coming apart, and parts being hurled across a room with enough force to break glass, stick in drywall or cause other property damage. Although several consumers have reported being struck in the eye or face, none of the injuries required hospitalization, and most of the incidents did not involve injury. The "Pop-Up Playhouse" is made of colorful fabric, supported by red or blue plastic tubes at all four corners. At the top of the playhouse, six yellow fiberglass rods form arches connected by metal sleeves. When assembled, the support structure is held together under tension. If any of the tubes break, or for some other reason the tension is released, the playhouse parts can come apart and be forcefully and rapidly thrown or ejected from the toy. Consumers with the playhouse should immediately remove the toy from children, then fold it according to instructions that came with the product. Consumers should not disassemble the playhouse until they receive the free modification kit and instructions. Consumers should call Fisher-Price toll-free at 1-800-233-4004 for the kit. Following consumer complaints about the toy breaking apart Fisher-Price redesigned the playhouse and revised its assembly instructions. The metal connectors are now permanently attached to the yellow rods, and the plastic tubes are made of stronger material and are held firmly in place. Toy Cars Recalled by Li Peng Enterprises NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE July 14, 1988 Release #88-050 Toy Cars Recalled WASHINGTON, D.C. -- Two models of a toy car which may pose a choking hazard to young children are being voluntarily recalled by Li Peng Enterprises, Inc., of Los Angeles, Cal., in an agreement reached with the U.S. Consumer Product Safety Commission. The plastic toys, sold as wind-up and battery-powered models, consist of a panda pushing another panda in a four-wheel cart. Each cart has an umbrella with plastic discs and balls hanging from the umbrella. The carts are labeled "MUSICAL CAB ST-22 (A or B) MADE IN TAIWAN." The balls on the umbrellas and other small parts can come off and present a choking hazard to young children. Consumers should immediately take the toys away from children, discard the toys or return them to the retail store where purchased for a full refund. Approximately 4,800 of the wind-up toys and 3,600 of the battery-powered models were sold between November 1987 and January 1988 for $2.50 each.

Loud Speaker Components Recalled by Electro-Voice NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE July 11, 1988 Release #88-047 Loud Speaker Components Recalled WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission, Electro-Voice, Inc. has recalled EVX professional sound reinforcement loudspeakers built and shipped between July 31, 1987 and February 8, 1988. The loudspeaker components, model numbers EVX-150, EVX-156, EVX-180, EVX-184, EVX-1500 and EVX-1800 contain uninsulated lead wires and overly - long terminal lugs which may, during maximum excursion, ignite the carbon fiber/paper composite speaker cone. Approximately 65 speakers remain unaccounted for (as of June 28, 1988), sold mainly in New York City and northern California; the majority of these are said to have been sold to customers who took them out of the United States. Consumers should immediately return EVX loudspeakers purchased before March 1, 1988 to their place of purchase. Those customers who return loudspeaker model numbers EVX-150, EVX-156, and EVX-180 will receive in exchange a newly manufactured unit with fully tested modification in place. Credit will be issued to those customers who return loudspeaker model numbers EVX-1500, EXV-1800, and EVX-184. All EVX loudspeakers purchased after the above date have improved performance characteristics and full safety modifications in place.

Umbrella Stroller Recalled by Service Merchandise NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 17, 1988 Release # 88-028 Umbrella Stroller Recalled WASHINGTON D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Services Merchandise Company of Nashville, Tennessee, today announced a voluntary recall of 15,000 Umbrella Strollers, Model 1601 PRT, which were purchased in Service Merchandise stores from August, 1986 through March, 1987. The hazard involves sixteen tube end plugs on each stroller that are considered to be "small parts," which may come out and present a choking hazard to children. In addition, when the plugs are removed, the exposed metal tube ends could have "sharp edges." Neither Service Merchandise nor the Commission is aware of any injuries to small children as a result of these small parts and sharp edges. Consumers having Model 1601 PRT Umbrella Strollers should stop using them, and immediately contact Service Merchandise for a modification kit. Service Merchandise is offering a modification kit at no charge to its customers. The Model 1601 PRT Strollers involved are navy blue with red trim. The model number is located on the ID tag attached at the top of the fabric seat. To obtain a free modification kit, call toll-free, l-800- 251-1389. Surge Suppressors Recalled by GE NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE March 21, 1988 Release # 88-018 GE Recalls 45,000 Surge Suppressors WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, General Electric Company, Warwick, Rhode Island, is recalling an estimated 45,000 GE and RCA brand Electronic Protection Center surge suppressors, sold nationally by a variety of retailers from October, 19S6 to December 4, 1987. Four GE surge suppressor models, EPC-6S, EPC6NS, EPC-4NS, and EPC-CPU and two RCA surge suppressors, models SK416 and SKF416, are being recalled. Some of these surge suppressor- c are defective and may present a severe electric shock hazard. A defective product's metal housing and the metal housing of any 3-wire appliance connected to it could be electrified. Although neither shock incidents nor injuries have been reported, four defective units have been returned to GE after their internal circuit breakers reportedly tripped in separate incidents. The product, which sells for about $4O-$60, resembles a power strip with four or six electrical outlets in a light gray colored metal housing. Each unit has an on/off switch, a power indicator light, a resettable circuit breaker, and a 6-foot power cord. The label on top of each unit includes (1) the GE or RCA brand and logo, (2) the words "Electronic Protection Center Surge Suppressor", "Electronic Protection Center Surge and Noise Suppressor", or "Computer Protector" and (3) one of the above-listed model numbers. The product was designed for use with personal computers, stereos, TVs and VCRs. FOR IMMEDIATE RELEASE Originally issued November 24, 1987; Revised October 29, 2002 Release # 87-073 Fisher-Price To Modify Strollers WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Fisher-Price, East Aurora, New York, today announced it is voluntarily making available to consumers a free modification kit for approximately 41,000 of its Model 9110, combination carriage/strollers manufactured prior to July 1, 1986. A child's head or body can fall through an opening in the stroller when the seat is reclined. Although no serious injuries have been reported to date, the firm is taking corrective action to prevent that possibility. The combination carriage/strollers involved have fabric canopy side panels attached to the seat back with fabric fasteners. The fasteners can detach, leaving an open space when the stroller back is reclined through which an infant's head or body can fall. Fisher-Price has received 17 complaints that children's heads or arms protruded through the opening. In two cases the infant fell from the stroller. Fisher-Price has corrected the problem and models produced after July 1, 1986 are not affected. These models have a fabric liner stitched in place. The strollers were distributed by Fisher-Price between April 1985 and May 1986 and sold for about $100 in retail stores nationwide. Strollers subject to this program can be identified by the date of manufacture printed on the label on the back of the seat. Only those strollers dated prior to July 15, 1986 are affected. Fisher-Price will provide a free modification kit that will close the opening and prevent the possibility of a child's head or body falling through. The modification kit will still allow use of the multi-position feature of the seat back. Consumers can obtain the free modification kit with complete installation instructions by calling 800-233-4004 between 8:30 a.m. and 5:00 p.m. EST or by writing to Fisher-Price Consumer Affairs, PO Box 16, East Aurora, New York 14052. Until owners receive and install the modification kit, the strollers should not be used with the seat back reclined. Bernzomatic TX-900A Heater Recall -- $50 Refund Offered WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, BernzOmatic, once located in Medina, New York (now a brand under Irwin Industrial, located in Wilmington, OH), announced the recall of its model TX-900A portable propane gas radiant heaters last manufactured and sold by the company in 1966. The Commission is aware of 16 deaths and one serious injury that. have occurred since 1972, from asphyxiation or carbon monoxide poisoning that may have been associated with the use of there heaters in poorly ventilated areas. The latest such incident occurred in December 1986. Directions which may be on the TX-900A heater state, among other things: "FOR COMPLETELY SAFE OPERATION, this heater SHOULD BE USED ONLY IN WELL VENTILATED AREAS, since all combustion heaters consume oxygen and exhaust fumes can be harmful. DO NOT USE WHEN SLEEPING." Consumers should immediately stop using these heaters and contact Irwin Industrial (which owns the BernzOmatic brand name). BernzOmatic is offering to refund owners $50 for each model TX-900A heater returned C.O.D. Approximately 33,000 TX-9OCA heaters were produced and sold by the manufacturer between 1962 and 1966. The heater (pictured) is housed in a blue or green metal cabinet approximately 9 inches wide x 14 inches high x 7 inches deep, with room inside for two 34.1 ounce propane gas cylinders. The name BernzOmatic appears on the front of the heater. The name BernzOmatic, the model number TX-SW and the description, Portable Gas Radiant Fester appear on instructions which may be on the inner side of the rear door. Owners of a model TX-90OA heater should call Irwin Industrial (which owns the BernzOmatic brand name) toll-free at 800-654-9011 for information on how to return the heater and obtain a refund.
