December 1, 1994 Release # 95-034 WASHINGTON, D.C. - CPSC, Big Save International Corp., of Los Angeles, Calif., is voluntarily recalling about 1,000 toy trucks.The toy truck was identified during a joint CPSC and U.S. Customs surveillance at the port of Long Beach, Calif. CPSC testing found that the toy contained small parts that present a potential choking hazards to young children.The multicolored plastic truck is approximately 8 inches long with a removable drive and passenger seat set. The truck is equipped with a plastic hammer, screw driver, and wrench and plastic screws, bolts, and nails. A decal on each side of the truck reads, "Play Tool Truck." The underside of the truck is embossed with "No. 3009" and "Made in China." The truck is packaged in a cardboard container labeled in part, "... Educational Play Tool Truck... NO. 3009... MADE IN CHINA." The toy was imported from China and sold in Indiana and Alabama from August 1994 to November 1994 at independent toy and retail stores for $3.95. The toy should be taken away from young children immediately and destroyed, discarded, or if possible, returned to the store where purchased for a full refund. However, the firm, Big Save International Corp., is out of business and cannot be contacted.Neither CPSC nor Big Save International Corp. is aware of any injuries involving these toys; this recall is being conducted to prevent the possibility of injury

Beef Iron and Wine Liquid (Metro International) – Supplement Safety (1994)

March 3, 1994 Release # 94-042 PRODUCT: Approximately 2,000 one-pint bottles of "Beef-Iron and Wine" liquid dietary supplement distributed through health food and nutrition stores nationwide from January 1, 1993 through December 1, 1993 by Metro International Distributors, Inc. PROBLEM: The recalled bottles lack child-resistant packaging. A child could accidentally ingest too much iron at one time, leading to serious illness or death. WHAT TO DO: Return unused portions of the recalled product to the place of purchase for a replacement bottle with child-resistant packaging or for a full refund. For more information consumers may call Metro International Distributors, Inc. at 1-800-851-2342. WASHINGTON, DC --- CPSC, Metro International Distributors, Inc., Carlstadt, NJ is voluntarily recalling its "Beef-Iron and Wine" liquid dietary supplement. Because of its iron content, Beef-Iron and Wine must be sold in child-resistant packaging to comply with the Poison Prevention Packaging Act. The recalled bottles lack child-resistant packaging. A child could accidentally ingest too much iron at one time, leading to serious illness or death. The recalled Beef-Iron and Wine was sold in brown plastic bottles with white screw-on plastic caps. Each bottle contains 16 fluid ounces (1 pint) of liquid product with 112 milligrams (mg) of iron per the recommended dose of three tablespoons. The bottle is labeled "Beef-Iron and Wine *** Hematinic *** Alcohol 10% by volume *** 16 FL. OZ. (1 PINT)." The label also reads, "DISTRIBUTED BY METRO INTERNATIONAL DISTRIBUTORS, INC., 217 Washington Avenue-A, Carlstadt, N.J. 07072." This recall applies to Beef-Iron and Wine packaged in containers with white conventional screw-on caps. It does not include any other iron-containing products manufactured by Metro International Distributors, Inc. Approximately 2,000 bottles of the recalled iron product were sold to consumers through retail health food and nutrition stores nationwide between January 1, 1993 and December 1, 1993. Neither Metro International Distributors, Inc. nor the Commission is aware of any poisoning incidents due to the Beef-Iron and Wine liquid supplement. However, the Commission is aware of several ingestions and at least 25 deaths since 1990 from other iron-containing products, as reported by the American Association of Poison Control Centers. Consumers should immediately return unused portions of any bottles of the recalled Beef-Iron and Wine liquid to the store where purchased. They will receive a replacement bottle with child-resistant packaging or a full refund. Consumers may call Metro International's toll-free number 1- 800-851-2342 for more information. CPSC became aware of the packaging problem through its ongoing poison prevention packaging inspection program. CPSC enforces the requirement that any dietary supplement containing an equivalent of 250 mg or more of iron per package must have child-resistant packaging. The U.S. Consumer Product Safety Commission is announcing this recall as part of its mission to protect the public from unreasonable risks of injury and death associated with consumer products. February 8, 1994 Release # 94-036 PRODUCT: Approximately 18,400 Kouvalias Musical Toys, model number 960. The wooden toy boxes consist of a wooden platform with a ball connected to the platform and nine more balls in various colors attached by metal coil springs. The boxes were distrubuted from 1989 through mid-July 1993 at various retail stores nationwide. They sold for $44.95 each. PROBLEM: The colored wooden balls and pieces from the music box underneath separated during routine CPSC testing, presenting a potential choking hazard to young children. WHAT TO DO: Consumers should take the toys away from young children immediately and return the toys to the stores where purchased for a full refund. For more information, consumers can call Reeves at: 1-800-444-4775. WASHINGTON, DC -- CPSC, Reeves International, Inc., Pequannock, NJ is voluntarily recalling 18,400 "Kouvalias Musical Toy" model number 960. During routine CPSC testing, colored wooden balls and gear components from the musical box underneath the toy separated, presenting a potential choking hazard to young children. The musical toy consists of the following parts: a wooden base (5 inches in diameter) that houses the musical box; a wooden platform that measures 3-1/2 inches in diameter and is attached to the base; and a wooden ball (2 inches in diameter) connected to the platform. Nine wooden balls in various colors are attached to the 2- inch diameter wooden ball by 1-1/2 inch metal coil springs. The music box is activated when the platform is turned; a song is played while the toy rotates. The label underneath the base reads: "A. Kouvalias S.A. . . .MADE IN GREECE." The "Kouvalias Music Toy" was imported by Reeves from Greece and distributed nationwide at various retail stores from 1989 through mid-July 1993. The musical toys sold for $44.95 each. Consumers are urged to take the toys away from young children immediately and return the toys to the stores where purchased for a full refund. November 8, 1993 Release # 94-009 PRODUCT: "Synergy Plus Chewables Vitabots" multiple vitamins and minerals. The product was promoted as a dietary supplement for children over four years of age. Each bottle of Vitabots has 90 light brown colored tablets; each tablet has five milligrams of iron. The supplements were sold nationwide in retail health food and nutrition stores between July 1, 1992 and July 31, 1993. PROBLEM: The product was not sold in child-resistant packaging as required by the Poison Prevention Packaging Act. The lack of child-resistant packaging means that a child could easily access and accidentally swallow the iron tablets. Each bottle of the product contains enough iron that, if consumed by a child at one time, it could cause serious illness requiring hospitalization. WHAT TO DO: Immediately return the unused portions of any bottles of the recalled Vitabots to the store where purchased for a refund of the purchase price. Consumers can also call the manufacturer at (201) 371-7300 for instructions on where to send the recalled products for a refund, including shipping costs. Each bottle of the recalled product contains enough iron that, if consumed by a child at one time, it could cause serious illness requiring hospitalization. The recalled Vitabots Multiple Vitamins and Minerals (for children over 4 years) were sold in amber-colored glass bottles with white screw-on metal caps. Each bottle contains 90 light brown colored tablets with five milligrams (mg) of iron per tablet. The product, wrongly labeled "Non-Toxic," is promoted as a dietary supplement for children over four years of age. The bottle's label also carries the statement "Manufactured by SYNERGY PLUS, Union, N.J. 07083 USA." This recall does not include any other iron-containing products manufactured by International Vitamin Corp. Approximately 2,500 bottles of the recalled iron products were sold to consumers through retail health food and nutrition stores nationwide between July 1, 1992 and July 31, 1993 for approximately $9.00 - $10.00 per bottle. Consumers should immediately return the unused portions of any bottles of the recalled Vitabots to the store where purchased for a refund of the purchase price or call International Vitamin Corp. at 201-371-7300 for instructions on where to send the recalled products for a refund, including shipping costs. Neither International Vitamin Corp. nor the Commission is aware of any poisoning incidents involving the Synergy Plus brand of iron tablets. March 28, 1991 Release # 91-052 Bee International Recalls Ma And Baby Bunny Due To Choking Hazard WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Bee International, City of Commerce, CA is voluntarily recalling approximately 5,000 "Ma and Baby Bunny" toys because the plastic eyes may separate from the Baby Bunny creating small parts that pose a potential choking hazard for young children. "Ma and Baby Bunny" are plush rabbit-shaped toys. The "Ma Bunny" stands approximately 12-1/2 inches tall and is either bright pink and white or yellow and white in color. The "Baby Bunny" is approximately six inches tall and is held in the arms of Ma Bunny. The toy was purchased as part of an Easter basket, model number BK101. The Easter baskets also included a variety of candy and novelty items and were sold nationwide by retail gift and specialty stores during the 1988 and 1989 Easter seasons. The "Ma and Baby Bunny" toy was not included with Easter baskets sold during the 1990 Easter season and will not be included in Easter baskets sold during the 1991 Easter season. A label located at the bottom of Ma Bunny reads in part, "Bee International, Commerce, CA 90040, Made in China." Neither the CPSC nor Bee International is aware of any injuries involving this product. This voluntary recall is being conducted to prevent the possibility of injury. Consumers who purchased the Easter baskets, model number BK101, may keep Ma Bunny, however, consumers are urged to take Baby Bunny away from young children immediately and return the toy directly to Bee International, 6817 Acco Street, Commerce, CA 90040, Attention: Greg Hall, for a $1.00 refund of the price Baby Bunny would have sold for if purchased separately from Ma Bunny plus shipping costs. June 14, 1990 Release # 90-111 Tai Tung International, Inc. Recalls Musical Ice Cream Vans And Lovely Dolls WASHINGTON, DC -- TAI TUNG INTERNATIONAL, INC. of Los Angeles, CA is voluntarily recalling its Musical Ice Cream Vans, Model #TT-28A and Lovely Dolls, Models D13/3 and C1372 because of small parts that could break off the toys if a child drops them. These small parts could be a fatal choking hazard to young children. Some of the small parts also have sharp points that could cause puncture or laceration injuries to children. This recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). Musical Ice Cream Car - Model TT-28A (made in China) resembles a large toy delivery van. It is bright red in color with brightly colored decals pasted on the front, sides, back, and top of the vehicle depicting ice cream treats and identifying the toy as a "Musical Ice Cream Car". In the rear of the van is a crank mechanism to wind-up a moving scene which is displayed through a clear plastic window on the driver's side of the van and the music box which plays as the van moves along the floor. The toy is also equipped with a pull cord that is stored in a compartment under the vehicle. Embossed on the bottom of the van is "1984, U.K. REG. NO. 1022370, U.S. PATENT PENDING..." The body of these plastic vehicles will break if dropped causing small parts with sharp points posing a serious choking and/or laceration hazard to young children. Approximately 48,210 Musical Ice Cream Cars have been sold nationwide for $12.00 each since May 1987. Lovely Doll - Model D13/3 (made in China) consists of three five- inch plastic female dolls with rooted nylon hair (either blond or dark brown in color) with painted-on eyes. They are wearing one- piece brightly colored fabric dresses and plain panties. The set comes with a brush and comb and is sold in a soft plastic see- through bag with fabric handle and trim. The arms of these dolls come off easily and could present a fatal choking hazard to young children.Lovely Doll - Model C1372 (made in China) consists of two dolls (one five inches and one 10-1/2 inches tall) with rooted hair. The larger doll has eyes that open and close and cries when it is tipped over. These dolls are clothed in pastel-colored one-piece fabric dresses with plain white panties and socks. The smaller doll has painted-on eyes and is clothed in a brightly colored one-piece fabric dress with plain white panties and no socks. The dolls are sold in soft plastic see-through bags with plastic carrying straps and come with a plastic comb and brush set. A tag attached to one of the dolls in the package identifies them as "Lovely Dolls" along with the model number. The arms of the small dolls come off easily and could present a fatal choking hazard to young children. Approximately 7,560 Lovely Doll Model D13/3 and 29,376 Lovely Doll Model C1372 have been sold nationwide since February 1989. Both models sold for under $3.00 each. Neither the CPSC nor the company is aware of any injuries involving these products. This voluntary recall is being conducted to prevent the possibility of injury. May 10, 1990 Release # 90-093 O'Brien Recalls Pro Flow Water Ski Bindings On Selected 1989 Skis Washington, D.C.-- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), O'Brien International of Redmond WA has announced a product recall of Pro Flow waterski bindings sold after September 1, 1988 and before December 31, 1989. The Pro Flow bindings are on O'Brien TRC, Competitor X, Pro Circuit and Splendor slalom models skis. Five skiers have reported injuries associated with Pro Flow binding which pulled apart and exposed sharp screws which caused foot lacerations. The Pro Flow waterski bindings have a high wrap, open toe boot design. The words "Pro Flow" and the O'Brien trademark "O" are located above the three horizontal white stripes on the heel of the binding. Only those O'Brien bindings with the white stripes on the boots are involved. Consumers who have this model O'Brien waterski binding should call O'Brien's consumer recall action line (1-800-284-SKIS) with questions and for repair instructions. The consumer will be provided with a repair kit and instructions to fix the potential problem or the owner can have the bindings repaired by a local authorized dealer or O'Brien International. In addition, O'Brien International is offering a corduroy hat to those owners requesting kits and making the repair. The action line is open from 11 a.m. to 8 p.m., Eastern time

Public Telephone Toy (Goldwell) – safety concern (1990)

May 9, 1990 Release # 90-094 Toy Public Telephone Recalled By Goldwell International WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Goldwell International, Inc., Los Angeles, CA is voluntarily recalling 13,583 Public Telephone toys because of small parts (coins and push buttons) that could pose a fatal choking hazard to young children. The plastic toy telephone, Model #WM-3922, is constructed to resemble a coin-operated pay telephone with a gold dial on the front and a receiver on the side. A coin return lever is located on the front left side. A cord is attached to the top so that the unit can be hung on a wall. In the middle of the dial is a picture of a telephone receiver and the word "HELLO." There are three coin slots at the top indicating 25, 10 and 5 cents. There are three multi-colored push buttons located in the lower front which activate a squeaker. The toy comes with plastic coins in a variety of sizes and colors. Embossed on the back of the unit are the words "MADE IN KOREA." The Public Telephones were sold nationwide since March 1989 for $9.99. The box was age labeled for children under three years. Neither the CPSC nor the company is aware of any injuries involving this product. This voluntary recall is being conducted to prevent the possibility of injury. Consumers are urged to take these toys away from young children immediately and return them to the store where purchased for a refund of the purchase price. CONTACT: April 18, 1990 (301) 504-7908 Release # 90-075 Henry Gordy International Recalls Eggscavators Toy Trucks WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Henry Gordy International, Plainfield, New Jersey is voluntarily recalling 40,000 Eggscavators Toy Trucks because the mixer on the cement truck may detach posing a choking hazard to young children. The product known as "Eggscavators" (Item Number 187) is a set of three plastic construction vehicles. Each set includes one of each of the following (colors may vary): - A dump truck with blue wheels, blue body and yellow dumper. - A cement truck with blue wheels, red body and yellow mixer. - A bulldozer with blue wheels, yellow body and red dumper and steering wheel. Embossed on the bottom of each vehicle are the words "MADE IN HONGKONG". The cement and dump trucks have white sticker labels on them that read "EGGScavators". Each vehicle is two inches wide and two and one-half inches long. These egg-shaped construction trucks have been sold nationwide since 1987 for under $2.00 a set. Neither the CPSC nor the company is aware of any injuries involving this product. This voluntary recall is being conducted to prevent the possibility of injury. Recalls 600 Alphabet Block Sets WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), International Marketing Source, Inc., Franklin Park, Illinois, is voluntarily recalling 600 alphabet block sets because the set's frame may separate and result in small parts that could present a fatal choking hazard to young children. The wood toy from China was distributed primarily in the midwestern states during 1989 for about $5.00 each. The product consists of 30 blocks in rows of six with painted pictures, letters, and numbers on each side mounted on a frame with dowels. Plastic ring/washers separate each block. Red letters on the top of the frame read "Learn The Alphabet". Neither the CPSC nor the company is aware of any injuries involving this product. This voluntary recall is being conducted to prevent the possibility of injury. Consumers are urged to take these toys away from young children and return them to the store where purchased for a refund of the purchase price. The 14-inch stuffed bear, sold in gift and other speciality stores, may pose a potential choking hazard for children under age three because of heart-shaped buttons which may detach from the bear's dress. There have been no reports of injury. The brown bear, part of the Kensington Bear collection, is wearing a maroon print dress with white lace bloomers and a straw hat with a pink bow. Three heart-shaped buttons are sewn on the front of the bear's dress. Pink and blue ribbon also trims the bottom of the dress. There is no stock number attached to the bear. The hang tag attached to the product reads "The Kensington Bears". If consumers wish to keep the bear, they can eliminate the potential hazard by removing and disposing of the buttons. Recalls Fruit Drink Because Of Potential Choking Hazards WASHINGTON, DC -- The Consumer Product Safety Commission (CPSC) announced that Mackie International Inc. of Paramount, California is voluntarily recalling their plastic toy gun, "Chilly Bang Bang Fruit Drink" because it may pose a potential choking hazard to young children. The opening tab at the end of the gun barrel separates and is a small part which could be ingested by small children and cause choking. Approximately 30,000 toy guns were sold in retail stores nationwide from April through May 1989 for approximately $.30 each. The toy guns should be taken away from children immediately and returned to the retail store where purchased for a full refund. The possible small parts problem was brought to the Commission's and the company's attention by the Empire State Consumer Association. The Hazardous Product Task Force of the New York Attorney General's office worked together with the CPSC and the company on the corrective action plan. While neither the CPSC nor Mackie International, Inc. is aware of any injuries associated with the toy gun, the recall is undertaken as a preventive measure.

Testing showed that the plastic caps could shatter and release small beads and plastic pieces. A replacement is being offered by Orion International Corporation of Miami, Florida, in cooperaticn with the U.S. Consumer Product Safety Commission. Neither the CPSC nor Orion International is aware of injuries associated with the rattle-style caps. Sold from November 1987 through October 5, 1988 as screw-an caps for Baby Cologne #30618, Baby Bath #30619, and Baby Set #30633, the ballshaped "rattle" caps came in colors of blue and pink, with a gold band around the diameter. Small beads inside the rattle generate noise. Approximately 22,248 pieces of the Baby Cologne, #30618, 1,104 pieces of the Baby Bath #30619, and 1,992 pieces of the Baby Set #30633 were sold in New York, New Jersey, Illinois, Florida, Pennsylvania, Connecticut and Washington, D.C. Consumers should not allow infants to use the rattle bottle caps of "Para Mi Bebe" soap and cologne. The caps should be removed. They can be exchanged at the store of purchase for nonrattle caps. U.S. Customs Service identified the hazardous rattle caps at the Part of Miami, Florida, as part of the "Operation Toyland" surveillance program conducted last year with CPSC.

The lighter may flare-up unexpectedly or may fail to extinguish when the control level is released. The models involved are 700LT and 701LT and were imported since 1987. Laboratory testing found that some of the lighters may have been mis-assembled during manufacturing. There has been one reported serious burn injury associated with this product. These disposable 3-1/2 inches in length with transparent plastic bodies which are either red, green, clear, or yellow/orange in color. The silver colored metal windguards are labeled "Bultina" and Korea". The lighters were sold either loose or blister packed under the name Red-Match. Consumers are urged to immediately stop using the lighters and return them to the place of purchase for a refund. These dolls have arms which may be pulled off and present a choking hazard to young children. Neither CPSC nor Wang's International know of any injuries involving these craft dolls. Wang's International packaged these unclothed soft plastic dolls in clear plastic bags with labels. One doll is a 4 1/2-inch Cupid doll #PDT 3538. The imported toys recalled by Beachcombers International are: - "Big Mouth Fish, Hippo, and Frog," plastic wind-up water toys labeled "MASUDAYA 1976 Made in Hong Kong;" some 8,000 toys were sold nationally for about $2 each. - "Devil Fish," an eight-inch wind-up toy in the shape of a fish which was imported from Hong Kong and sold for $2 in New Jersey and Virginia. - "Lively Squeaking Snake," a plastic cobra snake some 21 inches long from China which sold nationally for $1. Consumers should immediately take the toys away from children and return them to the retail store for a refund of the purchase price. The U.S. Consumer Product Safety Commission said both water toys have small parts, which may present a choking hazard to young children; the toy snake contains an excessive amount of lead in the surface coating. (DUI) of San Diego, California is announcing a recall of its AC (air control) dry suit valve because the valve malfunctions. The DUI AC valve is used on SCUBA diving dry suits. The DUI AC valve deflates or inflates the diving dry suit to allow the diver to equalize air pressure while ascending or descending in the water by pressing on the valve. The AC valve differs from the standard 2-valve system commonly used in dry suits, where air intake and outflow are controlled by two separate valves. The Consumer Product Safety Commission is aware of incidents involving 16 divers which are associated with AC valve malfunction. The valve may free-flow causing air to fill the dry suit resulting in the diver rising rapidly to the surface or failing to provide sufficient air to maintain buoyancy. The possibility exists of serious injury and/or death, especially if appropriate emergency procedures are not followed. Owners of dry suits with AC valves attached should immediately stop using the dry suits. DUI asks that all divers return their suits (at DUI's expense) and have the AC valve replaced at no charge. All dry suits returned to DUI will be tested and DUI will fix any leaks in the suit (except for seals) at no additional charge. Further, if the diver wishes to upgrade the valve system, this will be done at the manufacturer's cost of the valves only. Circus America Playpens Modification by C&T International NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 23, 1988 Release # 88-030 Playpens to be Modified Because of Hazards to Children WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, C&T International, Inc., Moonachie, New Jersey, today announced it is voluntarily making available to consumers a free modification kit for its mesh-sided playpen with drop sides, model number 585, "Circus America." Some of the playpens fail to warn users of a potential suffocation hazard if an infant is left in the playpen with one of the sides down. All of the playpens present a finger crushing hazard at two locations in the drop side locking system. In addition, when the playpens are folded for storage, it is possible for a child to remove the plastic feet which may present a choking hazard. The Commission is aware of a number of deaths and injuries involving mesh-sided playpens with drop sides produced by firms other than C&T. Eleven infants are known to have died when left unattended in playpens with a drop side down. These children suffocated when they rolled into a mesh pocket created by the lowered drop side. The Commission is also aware of several incidents in which children's fingers were crushed when they attempted to climb into a playpen grasping the folded drop side locking system. The playpens involved in these incidents were not manufactured by C&T; however, some of the playpens sold by C&T are similar to the playpens involved in the incidents and present the same hazards. The affected collapsible mesh-sided playpens may be identified by a self-adhesive label on the floorboard which states: Art. 585, CIRCUS AMERICA EX. The playpens measure 39 by 39 by 33 inches and contain a removable foam floor pad. The product has four plastic rings about three inches in diameter on the inside which can be used by a child to pull up on. Four hinges made of plastic must be pushed up in order to fold the playpen. The product, which is imported from Italy, retails for about $89.99. The "Circus Polo" playpen model number 582 is not affected by this action. Approximately, 2,500 playpens have been distributed nationwide since November 1984. Although some of the playpens have the suffocation hazard warning label, all of the playpens present a finger crushing hazard in the folded drop side locking system and a choking hazard in the removable plastic feet. C&T International will provide a free modification kit to consumers who call l-800-537-1922. New Jersey residents call 201-896-2555. The kit contains a Safety Alert warning of the potential suffocation hazard, plastic inserts to prevent the possibility of finger crushing incidents in the folded drop side locking system, and replacement plastic feet which can not be removed by young children. Children should never be left in playpens with the drop sides down. Until owners receive and install the modification kit, the plastic feet should be removed and discarded.
