The toy caterpillar's eyes can break off, presenting a choking hazard to young children.CPSC and Division Sales Inc. are not aware of any injuries involving these toy caterpillars. This recall is being conducted to prevent the possibility of injury. The painted wooden caterpillars, measuring approximately 10 inches long, 4 inches tall, and 3 inches wide, have yellow bodies and green, red, and blue wheels. The toy has a 10-inch long pull string attached to the caterpillar's head. Although the toy itself is unlabeled, it comes packaged in a blue colored cardboard box labeled "WOOD CATERPILLAR ... ITEM NO. 801 MADE IN CHINA FOR JUSTEN PRODUCTS."Various retail stores and discount centers sold the toy caterpillars nationwide from July 1994 to February 1996 for approximately $5 to $10 each.Consumers should take the toy caterpillars away from young children immediately and return the toys to the store where purchased for a full refund. The plastic cover and wire mesh grating basket may crack or shatter if the basket is not properly placed in the machine.HSN Marketing is aware of one incident in which a consumer using the juicer sustained minor scratches when the cover and wire basket seperated from the juicer.HSN Marketing is recalling Juice Master Chef's Choice Juicers with the model number XTIV. These juicers were sold for $19.95 to $29.95 from October 1991 through May 1994 through television shopping channels, catalogs, and discount stores, such as Home Shopping Club (which is not affiliated with HSN Marketing), Spiegel, and Riches Department Store.Consumers should stop using the Juice Master Chef's Choice juicer immediately and return it to the place of purchase for a full refund. The people figurines and their hats are small parts and present a serious choking hazard to young children.The toy is packaged in clear blister packaging on cardboard with multicolored label which states in part "Funtasty *** Made in China *** Justen Products Itasca, IL 60143." The toys are packaged in sets of three boats and three people. The two-inch-long boats are made of solid red, yellow, green, or blue plastic. The people figures are 1.5 inches tall, with a solid red, yellow, green, or blue plastic barrel-shaped body, a round white head, and a small plastic hat.The toys were imported from China and sold nationwide from June 1993 to August 1994 at small discount stores for $1.95.The toy should be taken away from young children immediately and returned to the store where purchased for a full refund. Four models are affected: model 2331 twin/full bunk bed, model 2332 twin/full bed with a "C"-shaped frame, and models 2330 and 2335 twin/twin bunk beds. All beds have 13 crossbars on the top and bottom mattress supports, and a center brace on the bottom mattress support. PROBLEM: The bunk beds may collapse during use. WHAT TO DO: Stop using affected beds immediately. To determine if you have a Coasterbed, contact the retailer for information on the importer or manufacturer of the bed. Consumers with affected Coaster beds should call the company toll-free at (800) 221-9699 for a free repair kit or replacement of the mattress support frame or bed. WASHINGTON, DC - CPSC, Coaster Company of America, Santa Fe Springs, CA, is voluntarily offering a replacement or repair of certain children's tubular metal frame bunk beds. The upper level can fall causing the bunk bed to collapse. Collapse of the top bunk could result in injury to an occupant on the bed. The Coaster Company has received 8 reports of these beds breaking, including one report of an injury. Approximately 13,500 affected bunk beds were distributed from May 1992 through December 1992 in retail stores nationwide. The beds sold for approximately $139 to $199 each (excluding the mattresses). The affected beds distributed in 1992 include the model 2331 twin/full bunk bed, model 2332 twin/full bed with a "C"-shaped frame, and models 2330 and 2335 twin/twin bunk beds. The beds have metal frames made of hollow painted tubes. The glossy painted frame will be either red, blue or white. Coaster Com pany beds also have 13 crossbars on both the top and bottom mattress supports, and a center brace on the bottom mattress support. Consumers should immediately stop using affected beds as bunk beds. To determine if you have a Coaster bed, contact the retailer for information on the importer or manufacturer of the bed. Consumers with affected Coaster beds should call the company toll- free at (800) 221-9699 to obtain either a repair kit, replacement of the mattress support frames or replacement of the bed. The CPSC is investigating numerous other importers and manufacturers of metal bunk beds that may present a similar risk of collapsing. To date, firms have received over 300 complaints of defective bunk beds industry-wide. CPSC urges all consumers to inspect all eight mattress support corners of all metal bunk beds for breaks or cracks in the paint and metal around the weld. Consumers should stop using damaged bunk beds and contact the retailer, manufacturer, or importer. August 13, 1991 Release # 91-110 Western Novelty Company Recalls Butterfly Bracelets WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Western Novelty Company is voluntarily recalling approximately 12,000 Butterfly Bracelets, model number P4136, because the bracelets may break and release small parts that could be a choking or aspiration hazard to young children. The bracelets consist of yellow, orange, green and pink plastic butterflies about three eights of one inch wide, strung together on an elastic string with a black bead between each butterfly. The retail display box held over one hundred unwrapped bracelets and was labeled in part: "Stretchable Bracelets, Made in Hong Kong." The bracelets sold for approximately $.20 each. They were distributed through school and church carnivals and in retail stores in Colorado, Arizona, Kansas, and Illinois. These bracelets were imported from Hong Kong and distributed by Western Novelty Company from July 1990 through February 1991. Neither the CPSC nor the company is aware of any injuries involving this product. This voluntary recall is being conducted to prevent the possibility of injury. Consumers are urged to take these bracelets away from young children immediately and return them to the store where purchased for a refund of the purchase price. Consumers may also return their bracelets to Western Novelty Company, 5670 Washington Street, Denver, Colorado 80216 for a $.50 refund check. The potentially hazardous bracelets were identified during a joint surveillance program conducted between the CPSC and the U.S. Customs Service at the Port of Denver. June 24, 1991 Release # 91-087 Low return rate prompts second notice: Toastmaster Toasters Recalled Because Of Potential Fire, Burn, And Shock Hazards WASHINGTON, D.C. -- Because of a low return rate after the first recall notice a year ago, the U.S. Consumer Product Safety Commission (CPSC) and Toastmaster Inc. of Columbia, MO, are renewing their warning to owners of Toastmaster Model D126T electric toasters. Some toasters have a defective switch that could allow the heating elements to continue operating after the toasting cycle which may create a fire, contact burn, or severe electric shock hazard. Only seven percent of the potentially 8,857 hazardous toasters have been returned in the past year. Toastmaster said its Model D126T electric toaster features a chrome case with a black-and-silver nameplate. The Toastmaster brand name appears on the lower right-hand corner of each appliance. Manufactured in April, May and June of 1989, the toasters sold nationwide between $30 and $35 each. Consumers should check the crumb tray at the bottom of the toaster for the D126T model number and a five letter and digit date code running from EL101 through EL158. While there have been no reports of injuries or fires to date as a result of switch failure, the company is replacing these toasters because of the defective hazard posed by the switch. No other Toastmaster toasters or products are involved in the recall. Consumers having the affected toasters should unplug the appliance and contact the company for instructions on returning the unit for a new toaster. Luxxtra Metal Halide Fixtures (Lam) – Safety Issue (1991)

June 19, 1991 Release # 91-084 WASHINGTON, DC--In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Lam Lighting Systems, Inc., Wakefield, MA today announced the further recall of 829 Luxxtra 1800 and 2400 Series 250 Watt Metal Halide Lighting Fixtures, sold and installed from 1980 to November 1990. The company reports that the extreme temperatures generated by the metal halide lamps could cause severe distortion or deformation of the light fixture's socket to stem attachment hardware. As a result, the lamp housing can separate and fall from the stem attachment. Any person struck by the falling lamp housing could sustain severe head or bodily injuries. Fifteen light fixtures have reportedly fallen from the ceilings of buildings from August 1988 to March 1990. These incidents occurred in New Jersey, Michigan, and Arkansas. These metal halide light fixtures were installed nationwide primarily in commercial office buildings, public and university libraries, and in athletic facilities such as school gymnasiums and indoor swimming pools. The incidents occurred after fixtures were in use from one to five years. Though no injuries have been reported, the company believes that some of 829 unlocated light fixtures may be defective and could eventually fall. A total of 5,514 halide light fixtures were initially recalled in August 1990. As a result, 4,685 fixtures have either been located and corrected or found to be no longer in use. Despite the company's repeated efforts to find and correct all installed light fixtures, 829 light fixtures remain unaccounted for. Lam Lighting Systems, Inc. requests any unidentified contractor or distributor who may know the location of light fixtures which have not already been corrected, to contact the company. This hazard may be present even though the lamp switch is in the "off" position. Brass floor lamp models 2607, 2618 and 2620 sold nationwide since January 1987 are being recalled. They sold for about $50 each. All lamps feature three metal support legs bolted to a metal central post by metal screws or bolts. - Model 2607 features a single light socket atop the center post and three planter supports at the ends of the U- shaped support legs. - Model 2618 features a triple light socket and planter supports at the ends of the support legs. - Model 2620 features a glass tea table which rests atop three C-shaped metal support legs bolted to the central post. The company said certain screws holding the legs to the lamp's center post may be too long and could pierce the power cord insulation, energizing the entire brass frame. The U.S. Consumer Product Safety Commission has learned that one child was electrocuted in Alaska. Consumers with any of the lamps should immediately unplug them from electrical outlets and return the product to the store where purchased for a full refund of the purchase price. May 16, 1989 Release # 89-038 Choking Hazard Prompts Pacifier Holder Recall WASHINGTON -- Some 3,600 "Mother's Helper" pacifier holders sold nationwide in retail stores between October 1987 and August 1988 are being voluntarily recalled by Steven Barrie and Company of Warminster Pa., because of a potential choking hazard to young children.The recalled pacifier holders are various-shaped, two-inch plastic discs with a swivel metal clip attached for fastening the holder to the infant's shirt, sleepwear, blanket, etc. The metal clip may come loose from the disc and could pose a choking hazard to infants.The words "Mother's Helper 1986 Steven Barrie and Co., Inc., Ivyland, PA. U.S.A." are embossed on the back.The company said its recalled holders have raised, multi-colored designs and letters on the front of the disc which read "I'm a 10," "I'M A BOY," or "Hot Stuff." Neither the company nor the U.S. Consumer Product Safety Commission is aware of any injuries associated with the recalled pacifier holders.Re-designed pacifier holders manufactured and sold by the company since August 1988 have a smooth, flat decal with multi-colored pictures and messages on the front of the discs.Consumers having the older "Mother's Helper" pacifier holders should remove the holders from infants and toddlers, then return the product to the store for a full refund of the purchase price. Rear Engine Riding Mowers (Toro) – Safety Concern (1989)

February 2, 1989 Release # 89-008 Toro Recalls Rear Engine Riding Mowers WASHINGTON - The U.S. Consumer Product Safety Commission (CPSC) announced today that it has reached an agreement with the Toro Company, Minneapolis, MN, whereby rear engine riding mowers manufactured by Toro between 1972 and 1979 are being recalled. Toro has learned that there is a possibility that, after starting the mower with the auxiliary rope starter, which is located under the hinged rear body/seat, the user can fall or sit on the rotating starter cup. In addition to the recall program, Toro has also agreed to pay the CPSC a civil penalty of $75,000 in settlement of allegations of the Commission staff which are denied by the company that the company violated the defect reporting requirements of the Consumer Product Safety Act, with regard to the riding mowers. The company has found that there is a possibility that the 1973 mowers can be started in gear if the shift linkage is not properly adjusted, and that 1972-1976 mowers can be accidentally kicked or bumped into gear. Severe laceration injuries to the buttocks have occurred when, after starting the engine, the user climbed on the mower and fell, or simply sat, on the exposed rotating starter cup while the seat was still raised. In cooperation with the CPSC, Toro is offering to install a recoil version of the auxiliary starter at no charge. Consumers should contact their local Toro Authorized or Master Service Dealer to arrange for this service. In addition, Toro is offering to inspect and/or adjust the shift linkage on the 1973 riding mowers.Toro has sent notices to some 19,000 registered owners of the 1974-1979 mowers and has posted notices of this recall program in dealer's showrooms. Consumers who own one of these mowers, but have not received notice, should contact their nearest Toro Service dealer or call Toro toll-free at 1-800-444-8676. The product was sold between October 1986 and June 1987 for a price of $19.95.Acting on consumer reports of more than 700 unconfirmed injuries when the exercise spring broke, Gut Buster has developed a method of repairing the device to reduce chances of the spring breaking and injuring users. In some situations, the spring of the "Gut Buster" may be extended too far to withstand the additional stress and could snap. Consumers have allegedly experienced bruises, cuts and lacerations to the face and other parts of the body. Consumers should stop using the exercise device and are instructed to call Gut Buster's toll-free number at 1-800-325-7292 to receive the repair kit. FOR IMMEDIATE RELEASE Originally issued October 27, 1987, Revised June 14, 2006 Release # 87-066 CPSC Warns of Potential For Carbon Monoxide Poisoning Involving Blueray "BLUE FLAME" Furnaces And Boilers WASHINGTON, DC -- In voluntary cooperation with the U.S. Consumer Product Safety Commission, Blueray Systems, Inc., of Schuylkill Haven, Pennsylvania, has initiated a program to alert oil heat service technicians, oil heat distributors and home owners of the importance of strict service and maintenance of the Blueray "blue flame" furnace or boiler. A total of nearly 21,000 of such furnaces and 14,500 boilers were produced between 1974 and 1983. According to the Commission, since 1979 there have been seven deaths from carbon monoxide poisonings that have been linked to improper maintenance or servicing of the Blueray "blue flame" furnaces or boilers. The firm is working through oil heat distributors and organizations servicing these distributors, to notify all persons living in homes where these furnaces have been installed, Consumers are being reminded that annual maintenance of the equipment is necessary for safety and efficiency. The need for this specialized maintenance is due to the design of the equipment for high-efficiency heating which requires annual servicing. Each household is also being supplied with a label to permanently attach to the furnace/boiler which alerts the home owner and the servicer of the necessity for strict adherence to all service and maintenance procedures prescribed by the firm. Consumers can determine if their furnace/boiler is one involved in this alert program by looking for a blue and white label on the equipment which reads "BLUERAY by Blueray Systems, Inc." The hazard involves a wheel breakage problem which may occur when the tire is inflated significantly beyond the recommended inflation pressure. Roadmaster and the Commission have been informed of two incidents where minor injuries occurred when plastic "mag" wheels broke during tire inflation. Since many air pumps are not metered, bicycle tires are frequently over inflated. Over inflation of the tires may cause these plastic wheels to fragment and pieces to be propelled striking the user and bystanders. This announcement is being issued in conjunction with Roadmaster's replacement program undertaken during the past six months. Roadmaster is replacing, free of charge, all affected sidewalk bicycle wheels. Only models 9693-KM & 7973-KM sidewalk bicycles are involved in this free replacement. Both of the affected models were sold nationwide by K mart retail stores. Roadmaster's engineering analysis indicates that the problem wheels may break when over inflated. Roadmaster stopped shipment of the subject models immediately upon learning of the potential failure. Customers having one of these sidewalk bicycles should contact the Roadmaster Parts and Service Department at 800-626-2811. Replacement steel-spoked wheels will be provided in exchange for the returned plastic "mag" wheels.

Edison Humidifiers Recalled by Toastmaster NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE December 10, 1985 Release # 85-064 Toastmaster Recalls 13,000 Edison Humidifiers Washington, D.C. -- In cooperation with the Consumer Product Safety Commission (CPSC), Toastmaster Inc. of Columbia, Missouri today announced the voluntary recall of approximately 13,000 'Edison Compact Tabletop Humidifiers, Models 534041B and 534042. These electrically operated humidifiers may contain a defective motor which could overheat and present a fire hazard. The Commission staff believes that product ignition could create an increased risk of injury, since humidifiers are often operated at night or while consumers are asleep. In 1984 and 1985, Toastmaster received five reports which indicated that a defective humidifier caught fire or failed. The incidents occurred in the States of Michigan and Wisconsin. There' were no reported injuries. The humidifiers in question were manufactured in June and July, 1984, and were sold to consumers since "June", 1984 in all but Hawaii and the southeastern United States. The product sold retail for about $40 to $45 in certain department stores, chain stores, and a variety of other retail outlets. The model no. 534042 humidifier has an almond cabinet with a pecan wood grain front panel, and the model no. 534041B humidifier has a solid chestnut brown cabinet. Both cabinets are plastic with dimensions of 16 S/8 inches by 14 inches by 8 5/8 inches. The "Edison Humidifier" label appears on the left front panel of each model. Owners can determine if their Edison Humidifier is recalled by checking the metal bracket on the rear for Model No. 534041B or 534042 and any date code from DF152 through DF212. All date code numbers between 152 and 212 are included. No other Edison humidifier models or date codes are involved. Owners of affected humidifiers should stop using their unit and contact Toastmaster Inc. for further instructions. Toastmaster will repair all suspect units free of charge. Small Plastic Animal Toys Recalled by Jack's NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE January 25, 1984 Release # 84-005 Plastic Animals Distributed In Fast Food Firm's Food Promotion May Present Choking Hazard To Small Children In cooperation with the Consumer. Product Safety Commission, Jack's Food Systems Inc., a fast food chain located in Alabama, Flordia, Georgia, Mississippi and Tennessee today announced a voluntary recall of small plastic animals because of the potential danger of choking to small children under 3 years of age. These toys were given away as part of a "Quacker Jack's Surprise Pack" which consisted of a hamburger, french fries, and a carbonated beverage. Distribution of the toys was stopped in September 1983. The toys have not been involved in any choking incidents known to CPSC. The toys have no identifying marks and are made of hard plastic in various colors. The toys vary in length, but most are approximately 2" or smaller. The toys include such animals as a lion, lioness, squirrel, coyote, collie, poodle, pig, hen with chicks, duck with ducklings, calf, alligator, elephant, goat, standing monkey, sitting monkey, horse, ram, rhino. Camel, sheep and hippo. Consumers who obtained these toys for children under three (3) years of age should remove these products from use immediately. The toys should be returned to the Jack's fast food restaurant where obtained. Consumers will receive a discount on their next purchase of a Surprise Pack. Blow-Gun Toys Recalled by Western NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE October 26, 1979 Release # 79-060 Wisconsin Manufacturer Agrees To Recall 180,300 Blow-Gun Toys WASHINGTON, D.C. (Oct. 26) -- Approximately 180,000 "Soft Shot Star Launcher" blow-gun toys which may pose a choking hazard to children are being recalled voluntarily by their manufacturer, the Western Publishing Company, of Racine, Wisconsin. The recall is being conducted in cooperation with the U.S. Consumer Product Safety Commission (CPSC). The toy consists of a white plastic tube and mouthpiece which measure approximately 29 inches in overall length. It is sold with three small cardboard targets and three orange darts molded of soft vinyl, each approximately three inches long. The company is recalling the toys based on consumer reports since July, 1979, of three children who choked on the toy's mouthpiece. In each instance, the mouthpiece reportedly separated from the plastic tube and lodged in the child's throat. No critical injury or illness resulted from any of these incidents, according to the company. A fourth similar incident was reported to CPSC and is being investigated by Commission staff. The company reported to CPSC that, despite the potential hazard, the toy meets or exceeds all existing voluntary product safety standards. Model number 4990 is printed on the toy's package, although the toy itself and its components do not display any distinguishing identification. Manufactured and distributed earlier this year, the toy sold for approximately $2 in toy stores and other variety retail stores nationwide. The manufacturer has directed all retailers and distributors of the toys to stop sales. Parents should return the toy to the retailer from which it was purchased to receive a cash refund.
