The recalled strollers have a metal frame and a cloth seat with a sun canopy. The strollers were sold in a variety of solid colors, including navy, red, black, silver and orange. A metal plate above the footrest shows the Mountain Buggy logo with "Mountain Buggy" written underneath. "URBAN" also is written on the metal plates. The recalled Mountain Buggy Urban Single strollers have item number U1204-002, and serial numbers between 000000 and 0003342. The recalled Mountain Buggy Urban Double strollers have item number U2204-002 and serial numbers between 000000 and 006957. The serial number and model number are on the back of the metal plate.
The recalled Duo Tandem strollers, manufactured between 1994 and 1999, have two seats, one seat in front and one seat in back. The strollers have a blue, white or green plastic and steel frame. They have four wheels in the front and two wheels in the rear. The cloth seats and tops have various colors and patterns. There is a label with the model and serial number on the stroller's frame. Only strollers with serial numbers and model numbers listed below are included in this recall. The cloth seat and top have various colors and patterns. The strollers have four wheels in the front and two wheels in the rear. These strollers were sold as a stand-alone stroller and also as part of a travel system that included an infant car seat/carrier and a base. The car seat/carrier is not affected. There is a label on the cross bar under the foot rest containing the model and serial number. Only strollers with serial numbers between 10012000 and 12312001 in the first 8 digits and the following model numbers are included in this recall. The recalled Baby Connection Fun Frog Soft Gym is comprised of a multi-colored, frog-shaped padded fabric mat, from which blue and orange support arms extend. The support arms consist of thin steel wire covered by foam and fabric. They can be arched, allowing for three stuffed infant toys to hang above the product's mat. A label with the word "Infantino" is attached to each of the stuffed toys as well as the underside of the mat. Products sold under item 150-761, Infantino Activity Soft Gym, which have a stamp on the back of the product showing 150-761 Production Date June 2005, are not affected by this recall.
The recalled pacifier measures about 2-1/4 inches in width and 1-1/2 inches in length and has button or hinged handles. The pacifiers are white with various colored handles. Designs are imprinted on some pacifier shields. One pacifier shield is decorated with a pink elephant, a purple giraffe with green polka dots, a blue lion with yellow fur and a blue cloud and yellow sun. Another pacifier shield has blue stars with Santa shapes. The amber colored nipple, imprinted with the words "caoui," "chouc" and "pur," is about 1-1/8 inches long. "Lov 2-Pack," "Decorated Orthodontic Pacifier," and "97705" are printed on the front packaging of the pacifiers. "Delta Enterprise Corp., Brooklyn NY 11212 Made in Thailand" is printed on the package back.
The recalled non-abrasive "Wet System" Video Head Cleaner was sold in a box labeled "Video Head Cleaner," "RadioShack" and "44-1230" or "44-1213." The box contains a cleaning video cassette and a bottle of cleaning solution. The 4-inch-high white plastic cleaning solution bottle is labeled "Video Head Cleaning Fluid" or "Cleaning Liquid for HEAD CLEANER." Other writing on the bottle includes "Contents: Isopropanol," and the item number "44-1230" or "44-1213."
The recalled cribs are made of wood and painted white. Model numbers 8554W-PT and 8554WW are printed inside the headboard and on the envelope attached to the mattress support. The Simplicity name, address, manufacturing date and model number are written on a label found on the inside bottom of the headboard.
The recall includes one type of pacifier (Light-up Pacifiers) and two types of toys (New Style Pianos and High Toys Choochoo Toy Trains). The Light-up Pacifier is a plastic pacifier, which also acts as a whistle. When a button on the pacifier's handle is pushed, the pacifier lights up with flashing LED lights. The New Style Piano is a battery-operated musical piano, which measures about 6 inches across and comes in orange and blue colors. The High Toys Choochoo Toy Train is a battery-operated train that measures about 8 inches across and 5 inches high. The main body of the train is light blue with red wheels, with tiny foam balls contained inside its pink smokestack.
The recalled overalls are pink corduroy with decorative snap fasteners at the waist and shoulder straps. The light pink overalls (style 435-092) have a floral embroidery design on the chest. The dark pink overalls (style 435-646) have a single patch pocket on the chest. "Carter's" is printed on a tag sewn into the back of the garment.
The recalled pacifier necklace consists of a 28-inch multicolored cord with a 3-inch plastic pacifier that comes in assorted colors. A hole at the tip of the nipple is used as a blow hole for the whistle. The pacifier handle operates as the on-off button for the flashing light on both pacifiers. "Flashing Pacifier" or "2-in-1 Flashing Pacifier with Whistle Necklace" is printed on the packaging of the pacifiers.
The recalled Graco toddler beds are white plastic and steel with openings between the slats in the guard rails and footboard. The beds were sold under the names "Cozy Toddler Bed," "Glow-in-the-Dark Toddler Bed," and "Classic Toddler Bed." There is a label on the guard rail or leg containing one of the following model numbers: "8801, 8801WR, 8821, 8824, 8828, 8833, 30066, 34434 and 11030," a serial number, and the manufacturer's address. Model 8828's headboard has a blue sky with a yellow moon and stars. Model 8801WR has red legs. "Graco" is printed on all of the beds' footboards.
The cribs are made of wood, sold in three finishes, including: natural, cherry and white. Model numbers 4464-1 (white), 4464-2 (natural) and 4464-4 (cherry) and "Taiwan" are printed on the mattress pad support board.
The recalled pacifiers measure about 2-1/2 inches in length and 2 inches in width. The pacifiers are blue, green, yellow and pink and have a tan tinted nipple with a curved plastic handle and shield. "SOOTHER BABY PACIFIERS" is printed on the back of cardboard packaging with a clear blister-bubble cover.
The recalled Lands' End outer wear have a rubber tab located at the end of the fabric zipper pull located on the front of the jacket. The recall includes the garments and style numbers listed below. The style number is written on a label in the left side seam of the garment. The recalled infant carriers were sold under the name "Playtex Hip Hammock," which is sewn into the front of the carrier. The model number is sewn into the inside panel below the instructions for use. The toddler cups are made of clear plastic with two handles and have a blue lid with a spout for young children to drink from. The dome-shaped container inside the cup contains blue and clear liquid, along with two toy dolphins, which appear to be swimming in the liquid. "MAUI OCEAN CENTER" is written on the side of the cups.
This recall involves Baby Connection Reef Rocker infant toys. The floor toy is intended for children ages 3 months and above. The toy's water-filled dome and four toy balls have colorful sea creatures inside. Lights and music are activated when the toy is shaken.
Only Big Save baby walkers with model numbers 9026, 9028, 9090, 90110 and 90111 are included in this recall. The model number is printed as the final digits on a bar code number and may appear on the front or back of the walker. Each walker has six caster wheels and comes in various colors: light green, pink and blue.
The recalled pacifier necklace consists of a 28-inch multi-colored cord with a 3-inch plastic pacifier that comes in assorted colors. The cribs included in this recall are the Daniella, Leeat, Noa, Naomi, Gabriella, Amber and Series 300 model cribs. The cribs are wood and come in a variety of painted and stained finishes including natural, cherry, blonde, or white wood. Some of the cribs have built-in storage drawers on the side of the crib or underneath the crib. The crib model name may be printed on the lower part of the headboard of each crib and the manufacturer name is printed on each set of crib warnings when provided.
The recall includes toddler swings with model numbers 7336KR, 733604 and 733600. The model numbers are located on a label on the back of the swings. The swings are made from green plastic and the red Step 2 logo is located on the front of the swing. The straps on the recalled swings are gray and blue colored.
