May 4, 1994 Release # 94-070 PRODUCT: Approximately 14,400 electric potpourri pots sold at Walgreen stores prior to June 1, 1992. The white ceramic pots have a blue colored design on the side of the pots. This design can be a wreath, a flower basket, sea shells, or fruit. The pots have a blue ceramic lid and a white electric power cord. The pot holds approximately one and a half cups of water. The recalled pot has a black plastic bottom and is labeled on the bottom "model WA-00117" and does not have a UL listing mark, although there may be a UL mark on the pot's power cord. PROBLEM: The electric potpourri pot presents an electric shock hazard if a metal heating element retaining bar inside the sealed base of the pot comes in contact with the unit's power cord terminals. This could permit metal nuts on the outside bottom of the pot to become electrically charged. No incidents of electric shock have been reported to either the Walgreen Company or CPSC. Other retailers may have imported the same potpourri pots. Consumers who suspect that they may have purchased the same model electric potpourri pots from different retailers are requested to call the CPSC hotline at (800) 638-2772 to assist the agency in identifying other possible retailers of this hazardous product. WHAT TO DO: Consumers who purchased an electric potpourri pot should examine the bottom of the pot for model number WA- 00117. If the pot is marked as model WA-00117 and was purchased at a Walgreen store prior to June 1, 1992 the pot should be returned to Walgreen for a refund or exchange (consumer's option). About 1,500 clamp lights sold at Northern California Yardbirds stores and 500 clamp lights sold at 182 Sears stores nationwide beginning in July 1993 through August 1993. The yellow clamp lights have a black hook and handle and spring loaded clamp base with yellow vinyl grips. The handle is labeled "the Designers EDGE." At the back of the lamphead molded into the metal is the phrase: "SUITABLE FOR WET LOCATION MISC. FIXTURE QH-250N 1G96." PROBLEM: A potential shock hazard is present on certain lamps. When used without the hook, the lamp rotates 180 degrees to a backward position causing tension on the wires inside the switch junction box. This can cause the wires to chafe and short circuit, posing a shock hazard. WHAT TO DO: If you purchased a Designers Edge clamp light from Sears, immediately stop using the lamp and return it to Sears for a full refund. If you purchased a clamp light from a Yardbirds store, check for a date code on the lamp cord. If a date code is not marked, immediately stop using the clamp light and return it to Yardbirds for a full refund or replacement lamp. The handle is labeled "the Designers EDGE." At the back of the lamphead molded into the metal is the phrase: "SUITABLE FOR WET LOCATION MISC. FIXTURE QH-250N 1G96."About 1,500 clamp lights were sold at Northern California Yardbirds stores and 500 clamp lights were sold at 180 Sears stores nationally beginning in July 1993 through August 1993. The retail price was $19.99. The nine Northern California Yardbirds stores that sold the product are located in: Santa Rosa, Petaluma, San Rafael, Alamo, Vallejo, San Pablo, Rohnert Park, Vacaville, and Fairfield. Designers Edge learned of the problem when a Sears El Paso store reported electrical leakage from a clamp lamp on display. The lamp hook had been removed allowing the lamphead to pivot. It was determined that a small amount of wire was exposed from abrasion inside the junction portion of the lamp. Designers Edge subsequently reported the incident to the Commission. If you purchased a Designers Edge clamp light from Sears, immediately stop using the lamp and return it to Sears for a full refund. If you purchased a clamp light from a Yardbirds store, check for a July 1993 or later date code marked on the lamp cord. If a date code is not marked, immediately stop using the clamp light and return it to Yardbirds for a full refund or replacement lamp. Clamp lights marked with date codes are not affected by this recall. They contain insulated wiring in the junction box and do not present a shock hazard. December 23, 1993 Release # 94-029 PRODUCT: Single light holiday electric candle decoration manufactured for Kmart and sold at Kmart and Builders Square stores during the 1993 Holiday season. The 300,000 Candoliers may be improperly wired and may also have untrimmed plastic around the metal portion of the plug. PROBLEM: The improper wiring could cause a shock. The Candoliers with untrimmed plastic could pose a fire or shock hazard. The product may function properly but should not be used. WHAT TO DO: Do not attempt to repair defective units. Consumers should immediately discontinue using the product. Beginning on Sunday, December 26, customers may take them back to any Kmart store or Builders Square for a full refund. WASHINGTON, DC -- Kmart, CPSC urge consumers who bought single holiday candle lights called "Candoliers" from Kmart during the 1993 holiday season to stop using them because the Candoliers may be improperly wired; they may also have untrimmed plastic around the metal portion of the plug. The improper wiring and untrimmed plastic on the plug could lead to shock, thermal burn or fire. Consumers may return the lights to any Kmart store or Builders Square for a full refund.When sold at Kmart stores, the single Candoliers were packaged in a clear plastic bag with a red cardboard package top reading "Trim A Home." The 10-inch single-light, cream colored candoliers item #04-12-11 sold for 99 cents and have a clear bulb. The identical Candoliers, sold at Builders Square, were packaged two in a single poly bag and sold for $1.97. The code number of the Builders Square model is #112 3397. Kmart reported problems to the CPSC after testing the product in its Quality Assurance and Technical Design facility. Kmart has had one report of a house fire, but no injuries attributed to the Candolier. Kmart has been unable to confirm any additional information or details about the fire incident. Builders Square has received no reports of injury or fire. Both Kmart and Builders Square stores have been instructed to remove the merchandise from the stores' shelves. When plugged in, the Candolier may present a fire hazard due to an excessive amount of plastic around the metal portion of the plug which could prevent a proper fit into the electrical socket. The Candoliers which have been improperly wired will continue to work but may present an electric shock hazard. Approximately 300,000 Candoliers were distributed at Kmart and Builders Square during the 1993 holiday season. Consumers should return all single-light Candoliers to any Kmart or Builders Square store beginning Sunday, 12/26/93 for a full refund. Consumers should note that this is the second year that Kmart has recalled the single candle Candolier. Kmart also recalled last year's Candoliers. DECEMBER 1, 1993 Release # 94-015 PRODUCT: Holiday electric candle decorations with either three bulbs or a single bulb, manufactured by Willis Mfg. and sold at Kmart during the 1992 Christmas season. The affected single and three candle units are improperly wired. The three candle units may also have improperly aligned metal contact tabs. PROBLEM: The improper wiring could cause a shock hazard. The improperly aligned contact tabs will immediately short circuit and present a potential fire, thermal burn or shock hazard to users. WHAT TO DO: Do not attempt to repair defective units. Take them back to any Kmart store for a free refund or replacement.. WASHINGTON, DC -- Kmart Corporation, of Troy, MI, and the U.S. Consumer Product Safety Commission (CPSC) urge consumers who bought holiday candle lights called "Candoliers" from Kmart during the 1992 holiday season to return the lights to any Kmart store for a full refund or replacement. The Candoliers may be improperly wired; three-light Candoliers may also have improperly aligned metal contact tabs. The improper wiring and alignment could lead to shock, thermal burn, or fire. The Candoliers came in single and three-lights. The single-light Candoliers were packaged in a plastic bag with a cardboard package top reading "Single Candolier." The candle lights are about 11 inches high. They have an off-white plastic stem and base, and either a clear or an orange bulb. Single- light Candoliers sold at Kmart for $.99 each. Three-light Candoliers were packaged in a cardboard box. The stems and base are off-white molded plastic. The outer two candles are six inches high and the center candle is seven inches high. The three-light Candoliers were packaged with either clear or orange bulbs. They sold at Kmart for $4.59 each. To avoid injury, Kmart is recalling all affected products. Candoliers with improperly aligned metal contact tabs will immediately short circuit. If the household branch circuit breaker or fuse does not interrupt (trip) the circuit, the short circuit could create a fire, thermal burn, or electric shock hazard for anyone nearby or for anyone holding the product or its power cord. The Candoliers which have been improperly wired will continue to work but may present an electric shock hazard. Even if you have used your Candolier without it short- circuiting, you should discontinue use and return the product to any Kmart to prevent potential injury. All Candoliers sold during the 1992 holiday season are being recalled. Kmart reported two incidents to CPSC of Candoliers failing. Both incidents involved three-light Candoliers. One incident resulted in slight burns to a consumer's hand when the cord caught fire. Approximately 72,000 three-light Candoliers and approximately 292,400 single-light Candoliers were distributed during the 1992 holiday season. November 22, 1994 Release # 94-011 PRODUCT: NEC PC-17-01 and PC-17-02 Laptop Computers. PROBLEM: The computer battery can explode and catch fire. WHAT TO DO: Turn on computer with the AC adapter disconnected and allow battery to discharge. Call (800)237-2913 to arrange for repair. WASHINGTON, DC -- CPSC, NEC Technologies Inc. (NECT), Boxborough, MA, is voluntarily recalling batteries in NEC PC-17-01 and PC-17-02 Laptop Computers. The batteries in these laptop computers can explode and catch fire. Approximately 13,000 NEC PC-17-01 and PC-17-02 Laptop Computers were sold in the United States and Canada from December 1988 to April 1990. Model numbers can be found on the bottom of the computer. Only the portable PC-17-01 and PC-17-02 models are involved. No other NEC computers are involved. NECT is aware of seven incidents of smoke and/or fire which occurred during battery recharging. Although there are no known injuries to date, NECT and CPSC view this as an important matter and are making every effort to insure consumer safety. To encourage participation in this recall, NEC is offering a $100 reimbursement to all consumers taking advantage of this important free battery replacement program. NEC developed, and currently has available, a new battery using state of the art nickel-metal hydride technology to replace the defective batteries. Consumers should turn on the computers with the AC adapter disconnected and allow the batteries to drain completely. NOVEMBER 12, 1993 Release # 94-010 PRODUCT: Approximately 4,500 Sunbeam-Oster Household Products' discontinued 1,000-watt electric indoor grills with the gray "Nu-Stone" plastic base. The grills were sold between September 1992 and June 1993. Affected grills have a "1,000-watt" designation and the number "E992" stamped on the portion of the grill's heating element that attaches to the variable temperature control. PROBLEM: The grill can cause grease fires if users repeatedly grill food containing a high fat content without emptying accumulated grease between batches. The company has received four returns to date due to smoking of the indoor grills; no grease fires or injuries have been reported. WHAT TO DO: Consumers who purchased the 1,000-watt indoor grills are urged to discontinue use and return them to Sunbeam-Oster for a replacement or a refund (at the consumer's option). WASHINGTON, DC -- Sunbeam-Oster Household Products, Schaumburg, IL, CPSC, today announced the voluntary recall of its discontinued 1,000-watt electric indoor grill with the gray Nu-Stone plastic base, which can cause grease fires under certain circumstances. The affected grills were sold between September 1992 and June 1993. The recall does not involve any other Sunbeam or Oster grills or products. The recall is limited to the 1,000-watt variable temperature electric indoor grills with the gray Nu-Stone plastic base and patented "Griffo" porcelain grid, model numbers 4757 (Sunbeam brand) and 4772 (Oster brand). The Sunbeam brand sold for about $30.00 and the Oster brand sold for about $45.00. Sunbeam-Oster no longer manufactures the 1000-watt indoor grill with gray Nu-Stone base. Current versions are sold with an 800-watt element. The company believes that while the product is not defective, repeatedly grilling food containing a high fat content without emptying accumulated grease between batches could cause a fire. Consumers who purchased the 1,000-watt indoor grills are urged to discontinue use and return them to Sunbeam-Oster, which will (at the consumer's option) either replace the unit with a new 800-watt version of the grill or refund the full purchase price. To assist consumers, Sunbeam-Oster has established a toll- free number, 1-800-986-0008, which owners of the 1,000-watt indoor grills are urged to call. The company will send prepaid return mailing cartons to indoor grill owners. About 6,600 indoor grills were shipped to retailers in September 1992 and fewer than 4,500 grills remain in the hands of consumers. To date, the company has received four returns due to smoking of the indoor grills. No injuries have been reported. The grills being recalled are the 1,000-watt indoor electric grills with a gray Nu-Stone base. "Deck Lite Fixtures" model numbers 302048 and 302049; 9,300 Builders Square and an unknown number of Pay-N-Pak, Inc. light fixtures are involved. All the light fixtures were sold from July 1991 through March 1992 for $14.97 each. PROBLEM: The outdoor light fixture does not have a weatherproof ballast and is not grounded. Rain water could seep inside and short circuit, creating an electric shock hazard. WHAT TO DO: Consumers should immediately discontinue use of both products. Consumers with the Builders Square fixtures should return the fixtures to any Builders Square store for a full refund. Consumers with the Pay-N-Pak light fixtures should disconnect the fixtures and throw them out as Pay-N-Pak, Inc. is out of business. WASHINGTON, DC -- CPSC, Builders Square, San Antonio, TX is voluntarily recalling 9,300 "SKU No. 9300117 Wall Bracket Lights" sold nationwide from July 1991 to March 1992 for $14.97 each. This outdoor fluorescent light fixture does not have a weatherproof ballast and does not provide for electrical grounding. Rain water could seep inside and short circuit the energized ballast terminals to the metal fixture, creating an electric shock hazard. At the same time, Pay-N-Pak, Inc., Bellevue, WA sold in the western United States an unknown number of similar "Deck Lite Fixtures" model numbers 302048 and 302049. These outdoor light fixtures may present an electric shock hazard for the same reasons. Pay-N-Pak, Inc. is now out of business. The Builders Square and Pay-N-Pak outdoor fluorescent light fixtures were imported from the Sam Hee International Corporation, South Korea. No electric shock incidents have been reported to Builders Square or to CPSC. The Builders Square SKU No. 9300117 light can be identified by its black painted aluminum diecast fixture and its clear plastic globe. The original nine watt "U" shaped fluorescent lamp is labeled "DLS 9W, LIGHT 21." Also, the back of the metal fixture has three labels as follows: "UL Incandescent Fixture, Issue No. 696089, Suitable For Wet Locations," "E117402, Made In Korea," and "Wall Mount Only." Consumers who own one of these Builders Square light fixtures should stop using it and disconnect the power. The fixture should be removed and returned to any Builders Square store for a full refund. Consumers who identify a Pay-N-Pak light fixture should discontinue use, remove and discard it. PROBLEM: The hair dryer contains a device intended to protect the consumer from electrocution in the event that the hair dryer falls into water while in the "power off" position. Thirty-six percent of the hair dryers tested by Commission staff revealed large amounts of current leakage during immersion while the power switches were in the "off" position. WHAT TO DO: Consumers should immediately stop using the hair dryers and return them to: Customer Service, P.O. Box 640364, N. Miami, FL 33164. The hair dryer contains a protective device intended to prevent electrocution in the event the hair dryer falls into water while in the "off" position. However, 36 percent of the hair dryers tested by Commission staff leaked large amounts of electric current during immersion while in the "off" position. A consumer who attempts to retrieve an immersed dryer in the "off" position may be electrocuted. No incidents have been reported to the firm to date. The hair dryers - "Professional Styler" model 600-53000 - were sold from 1990 through August 1993 primarily in Florida through specialty stores. The words "Professional Styler" or "Professional Styler by Pomair" are printed on the nozzle and the model number is embossed on the handle. June 1, 1993 Release # 93-075 PRODUCT: Approximately 83,000 electric WG6-S and WG8-L "Worm Getter" worm probes, manufactured by Handy Marketing Company. The probes are used by fishermen to shock fishing worms to the soil's surface. The probes have a single energized metal rod, enclosed in a white plastic, spring- activated guard on one end and a black plastic bicycle grip on the other end. The affected probes were sold by Kmart, Cabela's, Bass Pro Shops, Fishing Hot Spots, the Sportsman's Guide, and Gander Mountain. WASHINGTON, DC -- CPSC, six retailers today announced the recall of WG6-S and WG8-L electric "Worm Getter" worm probe models due to the potential electric shock and electrocution hazard. Approximately 83,000 worm probes were manufactured by the Handy Marketing Company, Grand Rapids, MI and sold from 1980 through 1992. The worm probes sold for about $11 to $28 each. Electric worm probes are used by fishermen to shock fishing worms to the soil surface. The WG6-S and WG8-L electric Worm Getter models have a single energized metal rod. The probes are energized by household electricity at line voltage, typically 110 to 120 volts alternating current. The metal rod of the probe is designed to be inserted into the ground with one or more extension cords connecting it to an electrical receptacle. The Handy Marketing Company is now out of business and is unable to participate in this recall. The six identified retailers of these worm probes include: the Kmart Corporation, Troy, MI which only sold model WG6-S; Cabela's, Inc., Sidney, NE which only sold model WG8-L; Bass Pro Shops, Inc., Springfield, MO which sold both models; Gander Mountain, Inc., Wilmot, WI which only sold model WG8-L; the Sportsman's Guide, Eagan, MN which sold both models; and Fishing Hot Spots, Rhinelander, WI which only sold model WG8-L. With the exception of Kmart, the other listed retailers sold worm probes mostly by mail-order catalog. CPSC is aware that other unidentified retailers may have also sold these worm probes. The WG6-S model is 21 inches long and the WG8-L model is 26 inches long. At the end of the handle for both models is an eight-to-nine-inch power cord and plug. The WG6-S model has a two-prong plug and the WG8-L model has a three- prong plug with an indicator light in the handle. Also, both units were sold with a red end cap fitted over the probe, which must be removed to use the product. Owners of the electric "Worm Getter" should stop using the product and return it to the place of purchase for a full refund. If the WG6-S model was purchased from Kmart, for example, the owner should take the probe to any Kmart store for a refund. Owners who purchased from one of the five listed catalog companies should mail the units back to the company for a refund of the purchase price and mailing costs. Only models with a gray antenna wire that were sold between September 1992 and January 1993 and have the number "53774" printed on the front right hand corner of the package and the number "139.53774" printed on the back of the light control are affected. PROBLEM: The Light Control's antenna may be miswired, presenting a shock hazard. During a routine quality audit inspection by the firm, a miswired antenna was discovered. The Plug-In Light Controls were sold separately from garage door openers. The model number "53774" appears on the front right-hand corner of the package. The Plug-In Light Controls are intended to be used with certain model Sears garage door openers; they allow users to activate a light in their home with the garage door opener remote control. Chamberlain is sending letters to more than 3,000 consumers who are known to have purchased the Plug-In Light Controls. If consumers have not received a letter, but own a Plug-In Light Control, they should check their Light Control. If the Light Control has a gray antenna wire, immediately unplug it, being careful not to touch the tip of the antenna wire, and return it to the nearest Sears store for exchange or refund. The Vita-Worth iron tablets were sold without child-resistant closures as required by the Poison Prevention Packaging Act. The lack of child-resistant closure means that a child could accidently swallow the iron tablets. Just eight or ten tablets, if consumed by a child at one time, can cause serious illness or death. The tablets are packaged in a white plastic bottle with a screw-on cap. There are 100 tablets per bottle. The recall does not include the 60 tablet bottles, which have child-resistant packaging. Approximately 22,000 bottles of the iron tablets were sold to consumers through the firm's 1,000 company-owned and 250 franchisee-owned retail stores nationwide. The pills were sold between July 1, 1992 and October 31, 1992. Consumers should immediately return the recalled iron tablets to the store where purchased for a refund. Neither the recalling firm nor the Commission is aware of any poisoning incidents due to the Vita-Worth brand iron tablets. However, the Commission is aware of several ingestions and at least 11 deaths in 1991 from iron containing products, as reported by the American Association of Poison Control Centers. The defective smoke detectors have been marketed under the brand names BRK, First Alert and Family Gard. PROBLEM: The horns on the detectors may fail due to corrosion on the horns' electrical contacts. WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), BRK Electronics of Aurora, IL, is voluntarily recalling certain models of wired-in (AC-powered) smoke detectors marketed under the BRK, First Alert, and Family Gard brand names because the horns may fail to sound during a fire. Approximately 3.5 million of these smoke detectors were manufactured between October 1987 and March 1990. None of the company's battery-operated detectors are involved in this action. BRK determined in its own testing programs that the horns on these detectors might fail due to corrosion on the horns' electrical contacts. These detectors were wired into the electrical circuits of new homes, apartments, and hotels between November 1987 and March 1990. Although BRK has received no reports of incidents or injuries associated with horn failure, the company is recalling the smoke detectors to prevent the possibility that a smoke detector horn might fail in a fire. August, 1992 Release # 92-127 The Coleman Company Recalls The Powerstation II Rechargeable Battery Pack Due To Fire Hazard WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), the Coleman Company of Wichita, KS today announced the recall of its Model #5360A550 Nightsight PowerStation II Rechargeable Battery Pack because of a potential fire hazard. According to Coleman, the 12-volt DC battery and output plug power cord could short circuit if the protective sleeve covering the positive battery terminal is exposed and contacts the metal ring of the output plug. This could occur, for example, when the battery pack is stored inside its carrying case. Of the more than 9,000 units sold from August, 1991 to mid November, 1991, all but 789 have been located and replaced. In the company's report to CPSC, one short circuit incident occurred October 30, 1991, when a Coleman employee stored the battery pack inside its carrying case. The power cord running from the battery to the output plug overheated and melted. No fire or injury resulted. No incidents have been reported with units sold to consumers. The PowerStation II provides portable 12-volt DC electric power to electric or electronic appliances such as TVs, radios, or spotlights. These appliances must be equipped with a connector that can be plugged into a standard motor vehicle cigarette lighter socket. Consumers can identify affected units by checking the packing carton for the 5360A550 model number and the carton and black carrying case for the label "Coleman Nightsight PowerStation II 12V Rechargeable Battery Pack." In addition, the battery itself is labeled "Panasonic Rechargeable Battery LCR12V6.5PAD" or "Panasonic Rechargeable Battery LCR12V6.5P." Only the carton is labeled with the model number 5360A550. The steamer cord presents a possible shock hazard.Affected are certain Norelco Fabric Steamers/Wrinkle Removers, Model TS-60, sold nationwide for approximately $17.00 to $22.00 each. Insulation on the product's cord may have been damaged during the packaging process, leaving a small amount of the wiring exposed. Norelco believes that only fabric steamers sold in blister packaging, not those sold in boxes, may have defective cords. In all, 56,880 steamers were distributed in blister packaging which may have this condition. Consumers are asked to read the date code stamped on the outside of the metal blades of the electric plug. If the first three digits of the four-digit number are 241 through 281, consumers should examine their fabric steamer cords for tears exposing the wiring. WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Amana Refrigeration, Inc., of Amana, IA, is voluntarily recalling the control switches on certain models of its 208/230-volt room air conditioners. The switch may overheat in the "off" position and cause a fire. CPSC was informed of this potential hazard by Amana Refrigeration, Inc. after Amana received notice from the control switch manufacturer that overheating could occur. Amana is aware of 14 switch failures with no reported injuries. The affected room air conditioners were distributed by Amana nationwide from June 1987 through October 1991. Approximately 130,000 units were sold to consumers or remain in dealer inventory. The air conditioners sold for about $500 -$600 each. Any consumer with one of the affected room air conditioners should unplug the product and have the switch replaced by an authorized Amana servicer. Overheating cannot occur if the unit is unplugged. Consumers should call 1-800-262-3121 to find their nearest Amana servicer or to have any questions answered. Amana is also notifying, by direct mail, owners of the affected air conditioners who submitted ownership registration cards.
