May 17, 1991 Release # 91-070 WASHINGTON, DC -- The Thermos Company, of Freeport, IL, announced that, in cooperation with the U.S. Consumer Product Safety Commission (CPSC), it will voluntarily replace the regulator and hose assembly on certain Thermos brand gas barbecue grills because of a possible fire hazard. Thermos said that a small percentage of the assembly units supplied by Marshall Gas Controls, Inc., of San Marcos, TX, could leak gas and cause a fire. Thermos discovered the potential leak on the regulator and reported its discovery to the CPSC. The leaking regulators were part of a lot that may have been used to manufacture approximately 130,000 Thermos grills shipped to various locations in the United States between January 24 and March 22, 1991. There have been no reported incidents or injuries involving the regulator on these grills. Grills that may be involved in this replacement program have serial numbers ending in either A1, B1 or C1. Consumers can find the serial number by checking the base of their grill near the wheels (see drawing). Side burner grill models and grills having regulators with a red inspection mark on top of the regulator are not affected. Consumers who have purchased an affected model should contact Char-Broil, which bought the Thermos grill business, at 1-800-241-7548 and provide their name and address and the serial number of the grill. A replacement regulator and hose assembly will be shipped via overnight mail at no charge. The CPSC's mission is to protect the public from unreasonable risks of injury and death associated with consumer products. The CPSC is the Federal agency responsible for consumer product safety. CONTACT: August 21, 1990 (301) 504-7908 Release # 90-146 Certain Virco Stack Chairs May Collapse WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Virco Mfg. Corporation of Los Angeles, CA, today voluntarily announced a recall of its 890 Series upholstered stack chairs manufactured between June 29, 1989 and May 3, 1990. These chairs are being recalled because of possible structural flaws in the frames that may cause the legs to collapse. Virco informed the CPSC that a design change in June 1989 resulted in the distribution of approximately 535,000 chairs that may bend excessively during use causing the user to fall to the floor. The firm is aware of 30 incidents of chair leg failure. At least two incidents resulted in personal injuries, one of which was a knee injury. Chairs involved in the recall have square tubular metal frames with upholstered seats and backs. They are used in homes, restaurants, business offices, churches and other institutions. Virco will replace all defective chair frames with new, redesigned frames from the factory. To determine whether a particular frame is subject to recall, consumers and institutions should contact either the company from which it was purchased, or call the Virco Mfg. Corp. at 1-800-44VIRCO. March 7, 1990 Release # 90-043 WASHINGTON, D.C. -- The Thermos Company, Freeport, Illinois, is voluntarily replacing a component part in some of its "Thermos" and "Structo" brand gas barbecue grills sold since April 1989. The part is a hose, valve and regulator assembly which might leak gas and possibly result in fire. The U.S. Consumer Product Safety Commission is not aware of any fires caused by defective assemblies placed in "Thermos" or "Structo" grills. Consumers who have purchased Thermos or Structo gas grills since April 1989 should inspect their grills before cooking again. Check for the name of the manufacturer of the hose, valve and regulator assembly which is stamped on the round metal regulator which connects to the liquid propane gas tank. If the name "Taiwan Pao-An Ind. Co." is not stamped on the regulator, no further action is required and the grill may be used for normal operation. If the name stamped on the regulator is "Taiwan Pao- An Ind. Co.," look for the serial number located on either end of the bottom base or inside the side panel of the grill. If the serial number is between 721791 and 886390, the hose, valve and regulator assembly may be defective and the grill should not be used until the assembly is carefully examined. If the serial number is not between these numbers, the grill may be used. Consumers with Thermos or Structo grills bearing serial numbers within the suspect range should call Char-broil, which bought the Thermos grill business, toll-free at 1-800-241-7548, to obtain instructions for determining whether the hose, valve and regulator assembly on their grill is in fact defective and, if it is, instructions on how to retrofit their grill with a new assembly from Thermos. According to Thermos, 87,606 of approximately 675,000 grills shipped since April 1989 contain a hose, valve and regulator that may be defective. December 14, 1989 Release # 90-023 JA-RU Offers To Replace Parts Of Play To Learn Furniture Set WASHINGTON, D.C.--In cooperation with the U.S. Consumer Product Safety Commission (CPSC), JA-RU, Inc., Jacksonville, Florida has announced a replacement program for its "Play to Learn" furniture set Model No. 1411. Three components of the set violate CPSC's small parts regulation. The "Play To Learn Furniture Set" consists of two play figures and several furniture components made of plastic. JA-RU, Inc. is offering to replace the plastic lounge chair and two play figures because they are small parts which may present a choking hazard to young children. Neither the company nor CPSC has received any reports of ingestion or injury associated with these components. Approximately 144,000 "Play To Learn Furniture Sets," Model No. 1411 were sold nationwide from January 1987 to the present for $1.59 each. November 16, 1988 Release # 88-096 Wagner To Replace Seals On Power Paint Rollers WASHINGTON, DC - Wagner Spray Tech Corporation of Minneapolis, Minn., is voluntarily offering free replacement seals, labeling and instructions on certain power paint rollers to correct a potential lid ejection problem on units manufactured between May and November, 1984.Used by do-it-yourself homeowners, the power paint roller consists of a pressure canister connected by hose to paint roller. The can of paint is placed inside the canister, the lid attached, and the paint is driven to the roller under pressure exerted by an electric powered compressor. According to the U.S. Consumer Product Safety Commission (CSPC), the lid may be ejected from the canister with enough force to cause injury. There have been 41 reported incidents of the ejected lids causing facial and eye injuries. Paint rollers involved in the repair program are dates coded E84 through L84 on the bottom of the unit, and were identified as model no. 0155001 on the packaging. Consumers should stop using power paint rollers within the above date codes, and call Wagner toll-free, 1-800-365-7650, or write Wagner Spray Tech Corporation, 1770 Fernbrook Lane Minneapolis, MN. 55447 for a free repair kit. The company will provide two replacement seal rings for the pressure canister, a warning label to be affixed to the lid, and new operating instructions. Scissors Desk Set Recalled by K Mart NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE September 21, 1988 Release # 88-77 K Mart Recalling Scissors Desk Set WASHINGTON - K Mart Corporation of Troy, Michigan, is voluntarily recalling some 48,000 scissors desk sets because a razor blade cutting knife in the set may pose a serious laceration hazard to young children. Sold for less than $1 at K Mart stores during July 1988, the set included scissors, pencil sharpener and razor blade cutting knife. The package was labeled "School Days Scissors Desk Set." Consumers should remove the scissors set from young children, and return it to the nearest K Mart store for a refund of the purchase price.
Convertible Ladder-Chair Recalled by Maxton/Cox Furniture NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE June 15, 1988 Release # 88-041 Maxton/Cox Furniture Recalls Convertible Ladder-Chair WASHINGTON, D.C. -- In conjunction with the U.S. Consumer Product Safety Commission, Maxton Industries/Cox Furniture, Inc. of Maxton, N.C., today announced a voluntary recall of the firm's wooden, convertible ladder/chair. The ladder/chair was sold by three catalog companies; Swiss Colony, J.C. Penney and Spiegel. The Commission is aware of 24 incidents in which either the steps of the ladder/chair split or the hinges pulled away from the wood causing users to fall or stumble. The 24 incidents involved 13 injuries which included a broken elbow, several cuts and multiple bruises. Another problem is the inherent instability of the product when climbing the ladder. The step placement and arrangement, and the structural design can cause the user to lose balance and fall. Approximately, 4,500 wooden, convertible ladder/chairs were distributed and sold nationwide through Swiss Colony, J.C. Penney and Spiegel catalog companies from October, 1984 through July, 1987. The catalog companies' reference numbers are as follows: Spiegel's catalogue no: #N7910 7376T Swiss Colony's catalogue no: #3047 J.C. Penney's catalogue no: #783-2736 The ladder/chair involved in this recall has a warning label on the top step of the ladder. Propane Grills Modification Kits by Sunbeam Leisure NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE June 2, 1988 Release # 88-034 Sunbeam Leisure Products Providing Modification Kits For Windows on Propane Grills WASHINGTON -- Following reports of spontaneous breakage of glass windows of 1988 Sunbeam Leisure outdoor propane gas grills, the Sunbeam Leisure Products Company of Neosho, Missouri is initiating a modification program to remount the glass windows to prevent breakage. In a voluntary modification program undertaken in cooperation with the U.S. Consumer Product Safety Commission, Sunbeam Leisure said that, because of the way the window was mounted, the glass may spontaneously break from the heat of the grill. Incidents of consumers reporting minor injuries from broken glass are being investigated. A limited number of the suspect grills were sold in the latter part of last year, but most have been sold in 1968. An estimated 320,000 grills are currently in consumer use including Yardmaster models sold by Channel Home Centers and Sears Kenmore models 10663, 10684, 10783 and 10883. Consumers who bought 1988 models of Sunbeam Leisure, Yardmaster and the above Kenmore propane gas grills with a glass window will be supplied with a free modification kit and instructions on remounting the window. The installation can be easily performed by the consumer without special tools. LP Gas Grills Safety Alert by W.C. Bradley Issues Safety Alert on LP Gas Grills WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, W.C. Bradley Enterprises, Columbus, Georgia, today issued a safety alert for approximately 1.8 million LP gas grills manufactured from 1983 through 1985 under the CHAR-BROIL and PATIO KITCHEN labels. The Commission and W.C. Bradley are aware of 330 reports of loosening of the connection of the gas hose to the gas valve. This could result in gas leaking and possibly igniting. Neither the Commission nor W.C. Bradley is aware of any injuries or extensive property damage involving these grills. These gas grills did not have the hose and regulator pre-assembled to the valve. As part of the assembly of the grill, the consumer had to connect the hose to the valve. The owner's manual did not include instructions specifying the use of a wrench to tighten this connection. Consumers should use a wrench to make certain that the connection between the hose and valve is tight. While facing the front of the grill, turn the wrench in a counterclockwise direction. Lariat II Riding Lawn Mowers Warning NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 Note: Firm out of business FOR IMMEDIATE RELEASE Originally issued November 4, l987; Revised May 2, 2002 Release # 87-067 CPSC Warns Of Potential Braking Problem With Lariat II Riding Lawn Mowers WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Roof Manufacturing Company of Pontiac, Illinois announced a program to alert consumers to a potential braking problem with the company's Lariat II riding lawn mowers. The Commission is aware of one death associated with the Lariat II riding lawn mower. The power and braking for the Lariat II are supplied through the left rear wheel of the mower. This wheel is held onto the axle by retaining ring. This ring slip, over the axle and fits next to and to the outside of the hub of the left rear wheel. This retaining ring has two "set screws" which must be securely tightened to insure the continued braking ability of the mower. If the retaining ring comes loose from the axle, all braking ability is lost and the user might lose control of the mower. Roof Manufacturing contacted all dealers, distributors and known consumers to alert them to the necessity for tightening these screws and to provide them with a decal to attach to the mover to remind the consumer to retighten these screws if the retaining ring is ever removed for maintenance. Dotline Pool Ladder Step Replacement by Mengo NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE July 7, 1987 Release # 87-038 POOL LADDER STEP SAFETY REPLACEMENT PROGRAM WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Mengo Industries, Inc., announced a voluntary replacement program for the plastic steps used on some of its swimming pool ladders sold under the DOTLINE brand between 1976 and 1982 because the steps on those ladders may break during use. These removable ladders are used for home above ground pools. Involved are about 80,000 DOTLINE models SPL-4, SBL-4, and SPL-4000. The model number may be found on the assembly instruction sheet accompanying the ladder. Beginning in 1978, the year of manufacture is designated by a series of numbers located on the bottom of each step near the l-1/2 inch hole which accommodates the ladder side rail. The last two digits indicate the year of manufacture. There have been 9 reported incidents of steps breaking during use and some consumers have claimed resulting laceration and sprain-type injuries. The steps have a useful life of about two years. After two years the steps may begin to show signs of deterioration evidenced by small hairline cracks in the plastic. Mengo will replace the steps on all affected ladder models sold between 1976 and 1982 free of charge to the consumer. Consumers are urged to examine their pool ladder steps for cracks immediately. If cracks are discovered, consumers should discontinue using the ladder immediately until the replacement steps are installed. Kenmore Canister Vacuum Cleaners Recalled by Sears NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE June 23, 1987 Release # 87-036 THREE MODELS OF SEARS CANISTER VACUUM CLEANERS RECALLED WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Whirlpool Corporation of Benton Harbor, Michigan today announced the recall of Sears Kenmore canister vacuum cleaner models 116.2643581, 116.2643583 and 116.2645082 sold between November 1, 1986 and May 1, 1987. The company is recalling the vacuum cleaners to check the canister wiring. If installed improperly, the wiring could create an electrical short, causing the vacuum cleaner hose to catch on fire. There is also a potential for electrical shock. Whirlpool Corporation is aware of seven reported incidents involving overheating, with one of these incidents involving a minor burn. Between November 1, 1986 and May 1, 1987 approximately 33,000 of these canister vacuum cleaners were sold nationwide through Sears, Roebuck and Co. stores and catalogs. Various models sold for between $260 and $420, and carry the Kenmore brand name. Customers who purchased a Kenmore canister vacuum cleaner between November 1, 1986 and May 1, 1987 can check the model number of the vacuum they purchased by looking at the serial plate on the bottom of the canister. IF IT IS MODEL 116.2643581, 116.2643583, OR 116.2645082, THEY SHOULD DISCONTINUE USE IMMEDIATELY. Under no circumstances should these vacuum cleaners be used until they have been checked by a Sears service technician. Sears will inspect the vacuum cleaners and make any repairs that may be required free of charge to customers. Customers with canister vacuum cleaners involved in the recall should take their cleaner to the nearest Sears Service Center or the Customer Service Counter of their nearest Sears store. Champ Lawn Mowers Recalled by Brister's Thunder Karts NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 20, 1987 Release # 87-029 BRISTER'S THUNDER KART LAWN MOWERS RECALLED WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Brister's Thunder Karts, Inc., Roseland, Louisiana, announced today that it is recalling its High Wheel "Champ" walk-behind power lawn mowers because they do not comply with federal safety standards in effect since 1982. The lawn mowers fail to meet the 3-second blade stopping time requirement and do not have a protective foot shield to prevent contact with the rotating mower blade. The recall covers only those lawn mowers which were purchased by consumers for their personal use and does not include lawn mowers which are intended for industrial/commercial/professional uses. The lawn mowers, which are called "high wheelers" because the rear wheels on the mowers are larger than the front wheels, are available in two models: model B-21, with a 2l-inch cutting blade and a 4 horsepower motor; and model B-24, with a 24-inch cutting blade and a 5 horsepower motor, available in both push type and self-propelled. The suggested retail price ranges from $319.00 to $599.00 each. Approximately 1200 units have been sold to retailers located in Mississippi and Louisiana. While neither the Consumer Product Safety Commission nor Brister's Thunder Karts knows of any injuries involving these lawn mowers, they present a risk of serious injuries to hands and feet as a result of contact with the rotating lawn mower blade. Consumers who purchased the mowers from 1982 through 1986 are urged to bring the mowers back to the retailer where purchased so that they may be modified to comply with the safety standard at no cost. Sears/Kenmore Vacuum Cleaners Repair by Whirlpool NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE March 04, 1987 Release # 87-14 SOME SEARS CANISTER VACUUM CLEANERS NEED TO BE MODIFIED TO PREVENT INJURIES WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Whirlpool Corporation of Benton Harbor, Michigan, today announced a modification program involving numerous Sears Kenmore canister vacuum cleaners. The motor in some of these cleaners has an opening which exposes moving parts when the canister is opened, the motor filter removed, and when the motor is running. These moving parts may cause severe lacerations or amputations to fingers placed in the opening. There have been 19 serious injuries involving these motor openings. Since 1976, approximately 1.3 million of these canister vacuums were sold nationwide through Sears, Roebuck & Co. stores and catalogs under the Kenmore name and model number. Models with various features sold for between $50 and $280. An easy-to-install, stick-on motor safety screen kit which covers the opening on top of the motor is being provided free of charge to owners of affected models. However, not all Sears Kenmore canister vacuum cleaners sold since 1976 need the kit. Whirlpool is informing known purchasers by direct mail. Consumers who bought canister vacuum cleaners at Sears since 1976 can find out if their vacuum motor requires a safety screen and receive additional information by calling 800-654- 1820 toll-free (have the model number from the bottom of the canister ready) or by visiting the customer service counter of the nearest Sears store. Consumers are warned to take extra care in handling their canister vacuum cleaners until the safety screen is installed. Whenever using the cleaner, unplug it before opening the canister lid.
Nine-Inch Oscillating Desk Fans Recalled by Tatung NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE April 10, 1986 Release # 86-23 Tatung Recalls 80,000 Nine-Inch Desk Fans In cooperation with the Consumer Product Safety Commission (CPSC), the Tatung Company of America, Inc., of Long Beach, California today announced the voluntary recall of 80,000 nine-inch 2-speed oscillating desk fans. The fans were sold in 1985 under the Tatung brand name and included models LF-9GKA, LF-9BKA, LF-9GKP, LF-9BKP, and under the Crosley brand name as model CF-9T. All of the fans are subject to flexural cord failure after an unknown period of use. Cord failure can result in fan failure or ignition of the plastic case. The Commission staff believes the fan presents a fire hazard. Numerous incidents of fan failure occurred in 1985, including seven reports that the fan caught fire. Although no injuries were reported, several, fires occurred while consumers were asleep. Consumers should check their nine-inch oscillating fans for the Tatung or Crosley brand names on the decorative base plate, and for one of the listed model numbers printed on the fan bottom. Owners of a recalled fan should stop using it. Owners can receive instructions for getting a new fan motor and base assembly free of charge by writing to the Fan Recall Program, Tatung Company of America, Inc., 2350 El Presidio Street, Long Beach, California 90810. The owner's letter should include the return name and address. Tatung and Crosley brand fans were imported by Tatung in 1985, and were sold nationwide by a variety of retail stores. Flavor-Lock Drip Coffeemakers Recalled by Sunbeam NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE February 26, 1986 Release # 86-10 Sunbeam Recalls 86,00 Drip Coffeemakers WASHINGTON, DC - In cooperation with the Consumer Product Safety Commission (CPSC), the Sunbeam Appliance Company of Oak Brook, Illinois today announced the voluntary recall of 8,00 under-cabinet Model 15306 Sunbeam Coffeemaker Flavor-Lock drip coffeemakers, date coded as GJA, GJB, and GJC. Sunbeam has learned that the heating element in some of these coffeemakers can loosen with use and shift position to cause a hot spot. Under certain conditions, the plastic shell of the coffeemaker can ignite to create a fire hazard. From January 1985 to date, Sunbeam received fifteen reports that this model coffeemaker caught fire. These reports involved some property damage to near by combustibles, and included a January 1986 minor incident hand burn in Illinois. No other injuries were reported. Recalled units can be identified by the model number 15306, the service number 15-30AC, and the date codes GJA, GJB, and GJC all stamped into the metal plate on the back of the coffeemaker. High-Chairs Repaired by Sassy NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 23, 1985 Release # 85-028 Sassy Inc. Announces Voluntary Repair Program For Model 021 And 023 Portable High-Chairs WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Sassy, Inc. of Northfield, Illinois, today announced a voluntary repair program affecting approximately 40,000 Folding Sassy Seats, Models 021 and 023 manufactured between July 1984 and December 1984. The firm has received approximately 100 reports from consumers stating that their chair's plastic material developed cracks around the restraint horn and locking pin tabs (see drawing) prior to the product's removal from retail shelves in December 1984. If the cracks are present and the seat continues to be used, a laceration, falling hazard, or a pinching condition may be present to a user of the chair. There have been no reported injuries. The Folding Sassy Seat is used as a portable high-chair that is easily transported for convenient use in restaurants and when traveling in place of a conventional high-chair. The product consists of round aluminum tubing rails and bright yellow plastic seat and end caps. Other models of Sassy Seats are not affected (Deluxe Sassy Seat and the Original Sassy Seat). To eliminate the possibility of the Seat developing stress cracks, Sassy, Inc. will provide free replacement parts to all owners of their Model 021 and 023 portable high-chair. Retailers who received the chairs manufactured between July and December 1984 have already received notice of the repair program and have received replacement parts. It is believed that 30,000 of the affected chairs may remain in the possession of consumers. Rototillers Recalled by Sears & Roper NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 8, 1984 Release # 84-030 New Notice To Identify And Correct Rototillers That Lock In Reverse Washington, D.C. -- The U.S. Consumer Product Safety Commission today announced new efforts by Sears, Roebuck and Co,, and Roper Corporation to notify consumers of potential hazards associated with certain rototillers sold by them from 1963-1980. These tillers have a clutch locking button that allows the tiller to be locked in reverse. The firms have received over 20 reports of injuries, the most severe occurring when the tillers reportedly backed over users causing severe lacerations, fractures, or amputations to their legs. Sears will place posters at various locations in its stores to inform customers of a modification kit to eliminate the clutch locking mechanism. The free kit is easy to install and is designed to allow the consumer to operate the tiller without excess hand fatigue. The Sears effort is aimed at reaching additional consumers who have not yet received word that the kit is available. Last year, Sears and Roper Corporation, the manufacturer of the tillers, undertook an extensive public notice effort that included paid advertisements in national magazines and various farm and garden publications. A recall effectiveness study by the Consumer Product Safety Commission estimated that this notice effort had reached approximately 50 percent of the rototillers owners. The Commission has received a report that, during the past year, at least one additional consumer has been injured by one of the tillers, receiving lacerations to his leg. Slide Ladders Recalled by Carolina Enterprises NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 26, 1983 Release # 83-030 Carolina Enterprises To Recall Empire Slide Ladders WASHINGTON, D.C.-- In cooperation with U.S. Consumer Product Safety Commission, Carolina Enterprises, Inc., Tarboro, North Carolina, today announced a voluntary replacement program affecting approximately 100,000 of its Super Slide Item #9005. The ladder with this Supper Slide presents a potential safety hazard in that the heads of small children may become entrapped between the ladder rungs. The Commission is aware of one instance in which a child's head became entrapped between the top step of the ladder and the platform. No injury occurred as a result of this incident. The Supper Slide consists of a plastic red sliding board with a yellow ladder and comes unassembled. The name "Empire" appears at the foot of the sliding bord. Ladders which have been manufactured since June, 1982, are not subject to replacement. These redesigned ladders can be identified by the pre-formed plastic piece attached to the underside of each ladder rung. The new slides are now blue in color. The Super Slide wassold by numerous retail stores nationwide including Toy "R" Us, K Mart, Woolworth/Woolco, and by J.C. Penney Company in its catalogue. J.C. Penny Company, which was able to identify individual purchasers, has already notified its customers by direct mail and has completed the replacement program. The units being recalled sold for approximately $30.00 and were produced from 1980 until June, 1982, when the design change was made in the ladder. Release # 82-008 WASHINGTON, D.C. -- Creative Playthings had renewed its efforts to replace all estimated 137,000 to 239,000 Creative Playthings Indoor Gym House ladders following a recent strangulation death. The product was manufactured prior to 1980 by Creative Playthings, a unit of Gabriel Industries, a division of CBS Inc., New York, N.Y. Consumers are urged to remove and destroy the ladder immediately. The replacement program was conducted in cooperation with the U.S. Consumer Product Safety Commission which advised the firm of the death of a two-year-old California boy on February 9, 1982, in an accident associated with a Creative Playthings Indoor Gym House. The firm first took corrective action for the product in February, 1980, after learning of an accidental death and an incident causing brain damage which later resulted in death to a child playing with the product. The firm then urged owners of the product to obtain a free replacement ladder because the original ladder posed a strangulation hazard. The Creative Playthings Indoor Gym House consists of a hollow, fiberboard-and-wood box 20 inches in height which supports a two-step wooden ladder on one side and a short slide on the opposite side. The top of the box or "platform" serves as a bridge between the ladder and the slide and is protected on two sides by wooden railings. The product is intended for use by children ages 18 months through 3 l/2 years, according to the manufacturer. It was sold for approximately $29 to $50 in toy stores and other retail outlets nationwide in knocked-down form to be assembled by the owner. The ladder that was being replaced was manufactured from 1962 through 1979. The gym houses with ladders that were being replaced are those with "Creative Playthings" printed on the side or with no lettering. The space between the upper rung of the ladder and the platform was small enough for a child's head to be inserted and become entrapped creating a potential for strangulation. The replacement ladder that had been offered by the firm was redesigned to reduce the likelihood of head entrapment. The Creative Playthings company that conducted the recall no longer exists. The replacement ladders are no longer available, and consumers should discard or destroy the ladder if they have the product. Creative Playthings had undertaken additional efforts to advise owners to destroy the old ladder. The firm sent letters and posters to pediatricians and daycare centers throughout the United States, advertised in magazines, provided a public service announcement to major networks, aimed a news campaign to consumer affairs editors, and sent advisory bulletins to organizations serving parents of young children.
