Natural Creations is recalling Glucosamine MSM dietary supplement due to unlabeled allergen (shellfish).
Maxivision MaxiFlex (Registered) Multinutrient Formula, 120 Vegetable Capsules is being recalled because a material review was conducted in April 2015. It revealed some of the Chondroitin Sulfate (Lot 093011 1031) raw material used in the manufacture of Maxiflex Multinutrient Formula Lot 32171215 was Out of Specification.

Labeling: Label Mix-Up: Product is incorrectly labeled as phenylephrine 50 mg in 0.9% sodium chloride rather than phenylephrine 50 mg in 5% dextrose.
Failed Impurities/Degradation Specifications: Product is out of specification for a known degradant.
Failed Stability Specifications: Product failed to meet 12 months long term stability specification for viscosity
Penicillin Cross Contamination: Multiple finished products potentially contaminated with penicillin.
