This recall involves Philips "Lightolier" brand AF4SY decorative glass lenses used on various types of light fixtures. The clear glass lens, used to "soften" light with an additional lens layer, is 4 ¾ inches in diameter and 1/8 inch thick. The lenses were manufactured between January 2007 and December 2013. There are no markings on the product. The product number is on the packaging.

Ergo Series Angle Grinders (Metabo) – laceration risk (2014)

This recall involves Metabo W14-150 Ergo series 6-inch medium angle grinders. The angle grinders have a green body, a black and red handle, and a silver head and wheel guard. The grinders measure about 16 inches long by 4 inches wide by 5 inches deep, have a 6-inch wheel capacity and weigh about eight pounds. Metabo is printed on the handle and Metabo 14-150 Ergo is printed on the side of the grinder. The grinders are used for grinding and cutting metal. Model numbers include 06251421 and 06251441 and serial numbers include 3010000001 through 4020031488 for both models. The model and serial numbers are located on the rating plate on the right side of the tool.

This recall involves Bollinger Fitness Classic and SoftTouch resistance bands with door attachment, model numbers 5771, 5772, 5773, 5774 and 5775. The 4-foot-long resistance bands are made of gray rubber with gray, red, black or blue accents and have black foam handles and a door attachment. A 5-inch-long strap of nylon webbing is looped onto the band with a plastic ball attached or encased that serves as a door anchor. The Bollinger Fitness logo appears on the handles. The model number can be found on the product's packaging.

This recall involves 14 Halco LED bulb models used in recessed lights, as outdoor security lighting and in retail displays. The bulbs have a metal cone-shaped housing, are silver in color, either 14 or 18 watts, and have clusters of 6 to 16 LEDs. "ProLED" is printed on a label on the plastic housing of the bulb along with model numbers, product names, and date codes. The PAR30 bulbs measure about 4.75 inches long and 3.75 inches wide. The PAR38 bulbs measure about 4.75 inches long and 4.75 inches wide. This recall involves the Karambit neck knife and sheath. The half-moon shaped knife measures 7 inches in length and was sold in an urban camo print. "MTech USA" is laser printed on the blade. Model number MT-664UC-SO is marked on the back of the knife. The knife fits into an unmarked molded black sheath that is about 4.5 inches in length. The sheath has a snap clip on the back and also comes with a beaded lanyard chain. Replacement sheaths have "MTech USA" laser printed on the back.

This recall involves Big Sword fireworks devices. The mock sword is a handheld fountain that is intended to emit sparks from the tip of the sword. The blue and yellow sword has the Big Fireworks logo and the words "Big Sword" printed on the front. A yellow cardboard tag attached to the handle of the device has "Big Sword" and a caution statement printed on it. The sword measures 30 inches and has model number 3609 printed above the product's barcode.
