This recall involves "Windsor" style arm chairs. The chairs measure 40 inches high by 21 inches wide. Made of black wood, the chairs have 9 metal spindles across the rounded back.
The "GET-GO" and "LITTLE GET-GO" children's athletic shoes come in various color combinations and were sold in toddler and pre-school sizes 2C through 3Y. The Nike Swoosh symbol is on the outside and sole of the shoe. All shoes close with a strap. The style name "GET-GO" or "LITTLE GET-GO" is written on the shoe box. The recalled shoes have one of the following six-digit numbers on a label inside the shoe just above the UPC code: 308638, 308639, 308642 and 308647. Only these style numbers are included in this recall.
The scented candle is housed in a clear glass holder and has a crumbled appearance with color variations. It contains slices of dried apple and cinnamon sticks in the wax. The candle is 3 inches in diameter and 3.5 inches high. "Kitchen Krumblers" is labeled at the base of the candle. The candle was sold separately or in a set with another candle not involved in this recall.
The recalled Belkin OmniGuard Rackmount UPS units are used to provide voltage regulation, surge suppression, and back-up power for electronic equipment. The Belkin OmniGuard Rackmount UPS units are black with a silver/gray panel on the front. Recalled units are the OmniGuard 1100 (Model no. F6C110-RKM-2U), OmniGuard 1500 (Model no. F6C150- RKM-2U), OmniGuard 2300 (Model no. F6C230-RKM-2U), and OmniGuard 3200 (Model no. F6C320-RKM-3U). The model number and model type can be found on a label on the rear of the unit.
These down-rod-mount, dual-motor, 36-inch ceiling fans were sold in chrome, stone white, brush nickel, polished brass, antique brass, or weathered patina finishes. Model number 355-6645 is printed on the box and on a label located on the central housing of the ceiling fan. The brand names of the fans are "Aire Tek" and "Turn-of-the- Century Apollo Series" which is written on the packaging only. A label located on the central housing of the ceiling fan displays the File Number "E215078," the UL Listing Mark, and the model number 355-6645. Vaxcel had previously recalled the same model fan sold under the "Aire Tek" brand name.
The recalled "10,000 Lux Light Box" produces light to simulate natural sunlight. The units, which are white with a translucent hinged door, measure 24 inches high, 12 ½ inches wide, and 3 ¾ inches deep. Some of the recalled light boxes may have a small label on the top of the unit with the name "Nu-Yu Light." Only the "10,000 Lux Light Box" sold by Natural Options is recalled.
The recalled models of wooden bunk beds are: Twin/Twin #1000; Twin/Double #2000; and Loft Bunk #3000. There are no model numbers on the bunk beds, but the manufacturer's label is located on the inside of the upper bunk's end structure. The Twin/Twin #1000 has top and bottom bunks of equal size. The Twin/Double #2000 has a twin top bunk and a double bottom bunk that is 15 inches wider. The Loft Bunk #3000 has the bottom bunk perpendicular to the top bunk, making an "L" shape.
The recall involves 13 plush Pokémon characters. All recalled toys have a sewn-in label reading "TOMY." There also is a production code on the toy's label that begins with a letter and is followed by two numbers. The following production codes are included in the recall: A04, B04, C04, D03, D04, E03, E04, F03, F04, G03, G04, H03, I03, J03, K03, and L03. Any production code containing an "S" is not part of this recall.
The recalled toy is an 11-inch purple stuffed frog with a green chin, belly, hands and feet. The stuffed frog has the word "BRAVE" sewn in purple letters onto the underside of its right foot.
The recalled toy is a blue wooden ball with six holes located equally around the ball. Each hole contains a small wooden head with a smiling face in various colors.
The recalled Corningware 4-Quart Slow Cooker (QVC item number K2585) has a removable white ceramic bowl with a glass lid and a plastic storage lid. The enclosure that surrounds the ceramic bowl is made of plastic. "CorningWare" is printed on the front of the unit underneath the digital display and control buttons. The model number, SC-40PL, is located on a silver label on the bottom of the base.
Only Rival Crock-Pot® slow cookers with model numbers 3040, 3735, 5025, 5070 and 5445 manufactured before May 2002 are included in the recall. The model number is printed on the UL label located on the bottom of the base. The recalled Rival slow cooker has a removable ceramic bowl that sits inside of a metal base. The Rival logo is printed on the front of the unit above the control knob. The bases are round or oval in shape and were sold in various colors and designs. A date code is stamped onto the plug at the end of the power cord attached to each unit. The first two digits represent the week of manufacture, and the last two digits represent the year of manufacture. Any plug with a date code from 0199 (1st week of 1999) to 1802 (18th week of 2002) is included in this recall.
The recalled 60-watt B10 medium base light bulbs were sold in packages of two or four units. The bulbs are clear. Some were packaged as "Ceiling Fan" and "Double Life" light bulbs. "SYLVANIA" is printed on the front of the packaging and on the bulb base. The bulbs involved have a UPC bar code on the back of the package, ending in the following five digits: 13323, 13329, 13333, 13442, 13445, 13454, 13565, 13650, and 13721.
The recalled portable electric whole-room heaters are designed for indoor use and have model numbers 180VH®, VH®, Intellitemp®, EVH®, and DVH®. The model numbers are located on the bottom of each unit. Each heater is about 11-1/2 inches long, 9-1/2 inches wide, and about 12 inches tall; weighs about 6 lbs.; and has the "Vornado" name and symbol on the front.
The recalled combination TV/VCRs are gray/black and have these brand names: Panasonic, Quasar, RCA, or JCPenney. The cabinets have ventilation holes in the top rear of the cabinet. Only the 13" and 20" Combination TV/VCR sets manufactured between January 1, 1995, and December 30, 1995, are recalled. These sets can be identified by the model number which appears on the front and rear of the cabinet. The recalled Geartec ESPY and FX-2 (DBX Ravage) model helmets come in carbon yellow, silver, or blue, and have a carbon fiber graphic on the front to middle part of the helmet. The helmets have 15 vent holes, a removable visor, and the logo ("Geartec" or "DBX") printed on both sides of helmet. The recalled VT-3 (DBX Engage) model helmets come in either blue/silver/black, red/yellow/black, or silver and black. The helmets also have 15 vent holes, a removable visor, and the "DBX" logo printed on both sides of helmet.
The "Frosted Grip" eyelash curlers have pink-plastic handles and are sold with a rubber re-fill pad. The eyelash curlers were sold under various brand names, including TRIM and Salon Plus. There are no identifying marks on the eyelash curler. However, item numbers are printed on the packaging. Item numbers involved in the recall include 5-87B (sold at Wal-Mart, CVS and Shopko), 5-87RA (sold at Rite Aid) and 5-87S (sold at Kroger).
The recall includes all PTAC units in the P series with dates of manufacture between November 1, 2002, and December 31, 2003. The model and serial numbers are written on the nameplate, found behind the plastic front cover, directly below the control panel. Units with serial number LBLP00001 through LCMP9999 are included in the recall. Also included in the recall are "P Series" units that were serviced between June 1, 2003, and December 31, 2003, in which service included the field replacement of the heater assembly. The Friedrich "P Series" PTAC is a light beige color with the "FRIEDRICH" logo on the panel door.
The recalled Pottery Barn Bailey Chenille Rugs are "periwinkle blue" color only. Two models/sizes are recalled: 4479028 (8' x 10') and 4479036 (5' x 8'). These rugs are 65% rayon and 35% cotton.
The 15-inch-long water guns are shaped like giant syringes. When placed in a pool, they fill with water when the handle at the top of the device is pulled, and shoot water when pushed. They have either an orange tube with green handle and nozzle or a yellow tube with blue handle and nozzle. There is no writing on the water guns themselves
