These Home Décor Swing™ gates have turned wooden spindles and were sold in oak or cherry finishes. Only model numbers "1555/6" with manufacture dates before September 2001 are included in this recall. Model numbers and date codes appear on the label located on the bottom of the gate. No other Evenflo gates are affected by this recall.
Only Safety 1st Fold-Up Booster Seats with model numbers 173, 173A and 173B are included in this recall. Booster seats with model numbers 173C and 173D, which have four indentations on the bottom, are not included in this recall. The model number is located on the back of the booster seat, inside the left arm panel. The booster seat is made of blue plastic with green and red arms and includes a yellow feeding tray. The seat insert for the booster seat is made up of two halves that are "press-fitted" together. They have a smooth surface bottom, with no indentations. The booster seats have a restraint system made up of a two-piece waist strap and one crotch strap. "SAFETY 1ST" is molded on the top and bottom of the seat insert.
The recalled playpens were sold under the "Zip n Go," "Okie Dokie," and "Carters" brand names and were manufactured by Cosco. Only model numbers 05-361, 05-362, 05-363, and 05-364 are included in this recall. The model numbers are located on a label on the bottom of the playpen, on the metal tubular frame near one of the corners. The label reads in part, "MANUFACTURED IN CHINA FOR COSCO INC." The 28-inch-by-40-inch playpens have four mesh sides on a folding metal frame. The side rails have the name "Zip n Go, " "Okie Dokie," or "Carters" written on them. The padded floorboard forms a carrying case for the playpen.
The "Runt"™ mini-bicycles are about 24 inches long and 26 inches high with 6-inch wheels. They have adjustable seats and handlebars with quick release levers. The bicycles are red, blue, black or chrome in color with black seats and handle grips. The word "RUNT" and the Runt logo of a dog's face are on the front of the steering column.
The recalled rims are the Salsa Alto models, which have a black anodized finish and are labeled "Salsa" and "Salsa Alto" in yellow, red and white. The rims, which are used on mountain bikes that have disc brakes, were sold individually or built up into wheels with Shimano Deore or Deore XT hubs.
The electric blankets were sold under the model names Controlled Comfort, EH 2000, Starbright, Staywarm, Supreme 21 and Warm Comfort, and had serial numbers A001A to C210C. "Biddeford Textile Corporation," along with the blanket's model name and its serial number are printed on a label near where the control switch is plugged in. The blankets were sold in king, queen, double and twin sizes in various colors, including off-white, hunter green, light blue, navy blue, white, rose, plum, burgundy, French blue, sage green, teal and rawhide.
This recall involves AC adapters with the model series number "PPP003SD," " PPP003" and "PP2012," which is located directly under "Compaq Computer Corporation" on the AC adapter label. Announce Recall of Youth ATVs WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), American Suzuki Motor Corp., of Brea, Calif., is voluntarily recalling about 7,400 youth all-terrain vehicles (ATVs). The drive chain on these ATVs could come off the sprockets and lock the rear axle. This could cause the driver to could lose control of the ATV and crash, possibly resulting in injury or death. CPSC and American Suzuki Motor Corp. have received one report of a chain coming off the sprockets of one of these ATVs. No injuries were reported. These are 2002 model year Suzuki "QuadMaster 50" ATVs designed for children 6 to 12 years old. The ATV has "QuadMaster 50" written on both sides of the bodywork. Suzuki dealers sold these ATVs nationwide from March 2001 through August 2001 for a list price of about $1,800. Consumers should stop using these ATVs immediately and contact their local Suzuki dealer to schedule an appointment for a free repair. Maytag acquired Amana Appliances on July 31, 2001. During the broil and self-clean modes, the range can emit high levels of carbon monoxide (CO), presenting the potential for CO poisoning to consumers. Additionally, during the broil and self-clean modes, the temperature of the storage drawer can become extremely high, presenting the risk of burn injuries to consumers. The ranges are safe to use for baking and for cooking on the surface burners. Maytag has received one report of the temperature on the storage drawer becoming extremely high. No injuries have been reported. The products, which contain methyl alcohol, are not sealed with child-resistant closures as required by federal law. Methyl alcohol, also known as methanol, is toxic and can cause blindness or death if ingested. Blue Coral has received three reports of ingestion, including two children whose parents called the Blue Coral help line and were directed to immediate medical treatment. The recalled products include three brands: Rain-X® Super Glass Cleaner Concentrate, which was sold in 10-oz. and 16.9-oz. plastic bottles, is a blue concentrated liquid that should be added to water; Rain-X® Plus Washer Fluid Concentrate, which was sold in 10-oz. and 16.9-oz. plastic bottles and single-use 1.2-oz. pouches, is a pink concentrated liquid that also should be added to water; and Rain-X® Washer Fluid Additive, which was sold in single-use 2-oz. pouches. The Glass Cleaner and Washer Fluid bottles have a measuring cup built into the cap. A label on the back of the products reads in part, "Blue Coral Slick 50 Ltd." and "Made in USA." Hardware, automotive and department stores nationwide sold the recalled Rain-X® products from September 1998 through August 2001 for between $1 and $5. Consumers should keep these recalled products out of the reach of young children. The TH!NK traveler is an electric, power assisted bike with 20" wheels and a folding frame. The bikes were distributed nationwide from July 2000 through May 2001 by retailers and through TH!NK Mobility's web site. The serial number is stamped into the inside of the left rear "dropout" of the frame (the dropout is the piece of frame that holds the left-side axle of the rear wheel). The TH!NK bike traveler comes in red (TT20001), blue (TT20002), silver (TT20003), and white (TT20004) and sold for about $995.00. PROBLEM: The top of the handlebar stem binder bolt may twist off while being tightened. INCIDENTS/INJURIES: There is no incidence of the bolt breaking during bike use. No injuries have been reported. The latches on the lower sash of the window can unexpectedly open, posing a potential injury hazard to consumers who can be struck by the window or by shattered glass. Andersen has received 11 reports of the latches unexpectedly opening. There have been no reports of injuries. This recall involves only the tilt latches on the lower sash of the Andersen 200 Series Tilt-Wash Double-Hung Window. This window has the Andersen "A/W" logo etched into the lower right corner of the glass (A), has a latch on both sides of the top of the lower sash (B), and has a product label on the inside of the upper left corner of the window frame (C). The majority of windows were installed by home builders in the construction of new homes. Independent dealers and home centers nationwide also sold these windows to contractors between May 2000 through August 2001. Consumers should immediately ensure that the latches are in the closed position. (RTI), of Anderson, S.C., is recalling to repair about 125,000 circular saws. The lower blade guards on these saws can stick in the open position, posing a risk of serious injury to the operator and bystanders. Ryobi Technologies has not received any reports of incidents. This recall is being conducted to prevent the possibility of injuries. The 7 1/4-inch circular saws have a blue plastic body and a metal blade guard. The name "Ryobi" appears on the blade guard and the motor housing. The recalled saws have one of the following model numbers - CSB120, CSB130, CSB1308, CSB130K, or CSB130JS - which can be found on a white label on the motor housing. Some of the recalled saws were sold in kits that included a hard plastic, gray storage case (embossed with "Ryobi Circular Saw") with the saw and blade inside. Home centers (including Home Depot), hardware stores, and catalogs nationwide sold the circular saws from October 1998 through July 2001 for between $60 and $140. Consumers should stop using the saws immediately and contact Ryobi's Consumer Response Team for a free repair kit. The cables on these stands can break, causing the treestand to fall to the ground, posing an injury hazard to consumers. Hunter's View has not received any reports of injuries. This recall is being conducted to prevent the possibility of injuries. The recalled treestands weigh between 16 and 22 lbs., are made of Aluminum and steel, and have a platform and padded seat cushion. The stands are attached to a tree and provide hunters with an elevated view of potential game. The models involved in the recall are the HVTS-100 Wildcat, HVTS-101 Wildcat with camo seat, HVTS-102 Wildcat Comfort Zone, HVTS-104 Wildcat Swivel Seat, HVTS-300 Big Daddy, HVTS-302 Big Daddy Comfort Zone, HVTS-400 Big Horn Ram, HVTS-402 Hoot Owl, HVTS-500 Timber Ghost, HVATS-3000 Big Daddy Lite, and HVATS-4020 Comfort Zone Swivel Seat. A yellow label on the treestand reads, "Hunter's View Ltd., 1-888-878-0440." Hunting and sporting goods stores and catalogs nationwide, sold the treestands from March 2001 through August 2001 for between $50 and $150. Cypert, (313) 792-6646 CPSC, Mill's Pride Announce Recall of Assembled Utility Wall Cabinet WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Mill's Pride, of Waverly, Ohio, is voluntarily recalling about 187,000 "Premier" and "Four Seasons" brand assembled utility wall cabinets. The cabinets can detach from the wall and fall, possibly injuring consumers. Mill's Pride has received 18 reports of cabinets falling from the wall, including four reports of consumers receiving minor injuries such as cuts, scrapes and bruises. The utility cabinets are sold fully assembled and consumer- installed. The cabinets are white and measure 54-inches wide by 24- inches high. Each cabinet has three doors, two adjustable shelves and one divider panel. The cabinets are made of particle board with a white vinyl laminate. There are no identifying labels on the product; however, these were the only assembled white 54-inch utility cabinets sold at Home Depot stores from July 2000 through May 2001. Home Depot stores nationwide exclusively sold the cabinets for about $90. Storybook Heirlooms Recalls Children's Pajamas Recall Alert U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 September 10, 2001 Alert #01-527 CPSC, SBH Restructuring Corp. (formally Storybook Heirlooms) Recall Children's Pajamas PRODUCT: Children's Pajamas - SBH Restructuring Corporation of Foster City, Calif., the former owner of the Storybook Heirlooms children's apparel business, is voluntarily recalling about 13,900 girls' pajama sets. The pajamas were sold as two-piece pajama sets in sizes XS-XL and made of 100% cotton. Style numbers are located on the label attached to the neck or waistband of the garment. Styles included in the recall are Valentine Cuddlies (200) style no. 26789, Thermal Loungewear style no. 26441 and Valentine Cuddlies (2001) style no. 27338. Storybook Heirlooms catalog and www.StorybookHeirlooms.com sold these garments from December 1999 through March 2001 for between $8 and $34. PROBLEM: These pajamas fail to meet the Federal Standards for the Flammability of Children's Sleepwear and as a result these pajamas pose a risk of injury to children due to the possible ignition of the garment. INCIDENTS/INJURIES: None reported. WHAT TO DO: Consumers should immediately stop their children from wearing these pajamas and return them to SBH Restructuring Corporation, 348 Poplar Street, Hanover, PA 17331, Attention: Storybook Recall, for a refund of the purchase price and original shipping costs. Announce the of Recall Certain A/C Adaptors for its ZipVac Hand Held Rechargeable Vacuum PRODUCT: AC Adaptor for certain ZipVac Rechargeable Hand Held Vacuums - Oreck Manufacturing Company, of Long Beach, Miss., is recalling A/C adaptors for approximately 10,000 ZipVac rechargeable vacuums. The recalled A/C adaptor is charger model U090010D that has two circular holes in it's A/C's adapter plug end. The vacuums were sold directly by Oreck or by QVC as a premium for the purchase of an Oreck upright vacuum cleaner. The vacuums with the recalled adaptor were sold from February 2001 to June 2001. PROBLEM: The charger can overheat and melt, presenting a burn or electric shock hazard to consumers. INCIDENTS/INJURIES: Oreck has received 22 reports of the charger overheating. York International Corporation Recall of Gas Furnaces Recall Alert U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 September 6, 2001 Alert #01-526 CPSC, York International Corporation Announce Recall of Gas Furnaces PRODUCT: Gas Furnaces - York International Corporation of York Pa., has initiated a voluntary recall to inspect and possibly repair about 7,000 mid-efficiency 80 percent gas furnaces manufactured between May 1, 2001 and July 16, 2001 to correct possible unsecured heat exchanger tubes in furnaces. The furnaces are marketed under the York, Luxaire, and Coleman brands. York mid-efficiency 80 percent gas furnaces have been installed nationwide by independent installer and contractors. The model families that are involved are P4HU, P1DU, FL8B, G8D, G8T, and L8T. A separated heat exchanger tube could result in leakage of flue gases, including carbon monoxide, which could result in serious injury or death. INCIDENTS/INJURIES: None reported. WHAT TO DO: Consumers should stop using these furnaces immediately and contact York for information in determining if the furnace is part of the recall. Consumers should call York toll-free at (877) 874-7378 between 8:00 a.m. and 4:30 p.m. ET, Monday through Friday, to get information on scheduling a free inspection and repair. All installed furnaces will be inspected and furnaces with improperly fabricated heat exchanger tubes will be replaced at no cost to the consumer. Announce Recall of Children's Toy Toolboxes WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Racing Champions Ertl Inc. (RCE), of Dyersville, Iowa, is voluntarily recalling about 11,600 John Deere Kids Toolboxes. The toy's carrying handle can break, creating small parts and a sharp plastic edge that pose a choking and laceration hazard to young children. CPSC and RCE have not received any reports of injuries. This recall is being conducted to prevent the possibility of injuries. The children's toolbox is a soft-shelled, plastic, green box with a yellow handle and yellow raised letters that read "John Deere." A John Deere Kids logo with a fawn inside a red oval is imprinted on the front of the toolbox. The toolbox is sold with a green and yellow plastic ratchet wrench and pliers inside. Stamped on the bottom of the product is "JOHN DEERE LICENSED PRODUCT" and "MADE IN CHINA." All yellow-handled toolboxes are included in this recall. John Deere dealers and other farm supply retail stores nationwide sold the toys from June 2001 through September 2001 for about $13. Parents and caregivers should take the toy away from young children immediately and contact RCE at (866) 898-4999 anytime for instructions on returning these toys and receiving a free replacement toolbox.
