OCTOBER 29, 1993 Release # 94-007A PRODUCT: Ceramic Halloween pumpkin candle holder, item number 24688. PROBLEM: Use of this product may result in fire and/or burn injuries. WHAT TO DO: Consumers should stop using the ceramic pumpkin candle holders and return them to the place of purchase for a full refund. WASHINGTON, DC -- CPSC, World Bazaars, Inc., Los Angeles, CA, is voluntarily recalling its imported ceramic Halloween Pumpkin Candle Holder. World Bazaar has received reports that this candle holder may catch fire and shatter. The candle holder is described as a bright orange, ceramic, bag with a Jack O' Lantern's face. It is 5-1/2 inches in height, 3-1/2 inches in width, and 2-1/2 inches in depth. The candle holder's box is marked with the item number 24688. World Bazaars, Inc. received reports of three fire incidents. None of the incidents resulted in injury to users. The incidents occurred when the candle holder was used with the metal contained tealight candle that accompanies the candle holder. Approximately 37,493 candle holders were distributed nationwide from August 1993 to October 1, 1993 in retail stores and specialty stores. Approximately 10,000 products are believed to be in use by consumers. Consumers should stop using the ceramic pumpkin candle holders and return them to their retailers for a full refund. OCTOBER 14, 1993 Release # 94-002 PRODUCT: Builders Square fluorescent "Wall Bracket Lights" SKU number 9300117 and Pay-N-Pak, Inc. "Deck Lite Fixtures" model numbers 302048 and 302049; 9,300 Builders Square and an unknown number of Pay-N-Pak, Inc. light fixtures are involved. All the light fixtures were sold from July 1991 through March 1992 for $14.97 each. PROBLEM: The outdoor light fixture does not have a weatherproof ballast and is not grounded. Rain water could seep inside and short circuit, creating an electric shock hazard. WHAT TO DO: Consumers should immediately discontinue use of both products. Consumers with the Builders Square fixtures should return the fixtures to any Builders Square store for a full refund. Consumers with the Pay-N-Pak light fixtures should disconnect the fixtures and throw them out as Pay-N-Pak, Inc. is out of business. WASHINGTON, DC -- CPSC, Builders Square, San Antonio, TX is voluntarily recalling 9,300 "SKU No. 9300117 Wall Bracket Lights" sold nationwide from July 1991 to March 1992 for $14.97 each. This outdoor fluorescent light fixture does not have a weatherproof ballast and does not provide for electrical grounding. Rain water could seep inside and short circuit the energized ballast terminals to the metal fixture, creating an electric shock hazard. At the same time, Pay-N-Pak, Inc., Bellevue, WA sold in the western United States an unknown number of similar "Deck Lite Fixtures" model numbers 302048 and 302049. These outdoor light fixtures may present an electric shock hazard for the same reasons. Pay-N-Pak, Inc. is now out of business. The Builders Square and Pay-N-Pak outdoor fluorescent light fixtures were imported from the Sam Hee International Corporation, South Korea. No electric shock incidents have been reported to Builders Square or to CPSC. The Builders Square SKU No. 9300117 light can be identified by its black painted aluminum diecast fixture and its clear plastic globe. The original nine watt "U" shaped fluorescent lamp is labeled "DLS 9W, LIGHT 21." Also, the back of the metal fixture has three labels as follows: "UL Incandescent Fixture, Issue No. 696089, Suitable For Wet Locations," "E117402, Made In Korea," and "Wall Mount Only." Consumers who own one of these Builders Square light fixtures should stop using it and disconnect the power. The fixture should be removed and returned to any Builders Square store for a full refund. Consumers who identify a Pay-N-Pak light fixture should discontinue use, remove and discard it. October 8, 1993 Release # 93-111 PRODUCT: Baby Trend and Baby Express "Home and Roam" portable mesh-sided playpens in two sizes, 40" x 40" and 28" x 40". PROBLEM: The playpen may appear to be set up properly, if the top rails are not securely locked. If the playpen is not set up so that each top rail is securely locked in position, the top rails may collapse with an infant in the playpen. If the infant is holding onto a top rail when it collapses, he or she may be caught by the neck and strangle. WHAT TO DO: Make sure each of the four top rails is fully locked before using the product and call Baby Trend immediately at 1-800-234-1879. Baby Trend will provide corrective action AND a free gift valued at $5.00 for each playpen owned. WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Baby Trend, Inc., Pomona, CA is issuing an urgent warning to owners of Baby Trend and Baby Express "Home and Roam" playpens. There have been four reports that these products have collapsed with infants inside the playpens. One incident resulted in the death of a one-year-old child. In two of the incidents the child was found not breathing but was revived. Baby Trend sold approximately 65,000 Home and Roam playpens nationwide during 1992 and 1993. Some additional Home and Roam playpens were sold under the Baby Express label. The playpens come in two sizes and a variety of colors and fabrics. The larger one is 40" x 40" and the smaller one is 40" x 28". The words "Home and Roam" and "Baby Trend;" or "Home and Roam" and "Baby Express" and appear on two of the four top rails. Retail prices range from $68 to $130. All of the playpens are set up by rotating a pivoting hinge in the center of each top rail. To lock the rails in place, the pivoting hinge must be rotated a full 180 degrees from the collapsed position. Because the hinges are under the protective fabric covering the top rails, consumers may rely on their sense of touch to determine if each hinge has been rotated completely. It is possible to rotate the hinge less than 180 degrees but have the top rail look and feel locked. If the pivoting hinge has been turned less than 180 degrees, IT IS NOT LOCKED and the top rail can collapse. If a baby is holding onto the top rail when it collapses, he or she can be caught by the neck and strangle. Consumers should check to make sure the top rail hinges are fully engaged before each use. Consumers who own Baby Trend "Home and Roam" playpens should call toll free at 1-800-328-7363 so the company can provide them with a remedy and a free gift valued at $5.00. OCTOBER 1, 1993 Release # 94-001 PRODUCT: Approximately 10,000 "Professional Styler" hair dryers - model 600-53000, sold by MBR Industries Inc. primarily in Florida from 1990 through August 1993. The words "Professional Styler" or "Professional Styler by Pomair" are printed on the nozzle and the model number is embossed on the handle. PROBLEM: The hair dryer contains a device intended to protect the consumer from electrocution in the event that the hair dryer falls into water while in the "power off" position. Thirty-six percent of the hair dryers tested by Commission staff revealed large amounts of current leakage during immersion while the power switches were in the "off" position. WHAT TO DO: Consumers should immediately stop using the hair dryers and return them to: Customer Service, P.O. Box 640364, N. Miami, FL 33164. WASHINGTON, DC -- CPSC, MBR Industries Inc., Miami, FL, is voluntarily recalling the "Professional Styler" hair dryer model 600-53000. The hair dryer contains a protective device intended to prevent electrocution in the event the hair dryer falls into water while in the "off" position. However, 36 percent of the hair dryers tested by Commission staff leaked large amounts of electric current during immersion while in the "off" position. A consumer who attempts to retrieve an immersed dryer in the "off" position may be electrocuted. No incidents have been reported to the firm to date. The hair dryers - "Professional Styler" model 600-53000 - were sold from 1990 through August 1993 primarily in Florida through specialty stores. The words "Professional Styler" or "Professional Styler by Pomair" are printed on the nozzle and the model number is embossed on the handle. September 28, 1993 Release # 93-108 : 28,800 "Mini Racer F-1" racing car sets, item number 3360, imported and distributed by McCrory and Crest Industries nationwide between November 1, 1992 and May 31, 1993. The car sets were sold at a number of independent variety stores for $.99 each. The car sets contain six brightly-colored plastic racing cars in a cardboard blister package. Each racing car has a number on the hood and a decal on the air foil, behind the engine, that reads "Fire Bird." PROBLEM: During routine testing of the cars by the Connecticut Department of Consumer Protection, the wheels and axles separated, presenting a potential choking hazard to young children. WHAT TO DO: Consumers are urged to take the cars away from young children immediately and return the cars to the stores where purchased for a full refund. The car sets contain six brightly-colored plastic "Mini Racer F-1" racing cars in a cardboard blister package. The racing cars measure 3-1/2 inches long and 1-1/4 inches wide. Each car has the following: a number on the hood, two sets of wheels in the front and one set in the back, a silver engine, an air foil with a decal that reads "Fire Bird," and a driver sitting in the seat. The label on the blister package reads in part: "ITEM NO. 3360 . . . MADE IN CHINA." The "Mini Racer F-1" racing cars were imported by Crest Industries from China and distributed by McCrory nationwide between November 1, 1992 and May 31, 1993. The racing car sets were sold at Britts, Elmore, McCrory, H.L. Green, Kress, McClellan, T. G. and Y., Silver, J.J. Newberry, G.C. Murphy, and some independent variety stores. They sold for $.99 each. Consumers are urged to take the cars away from young children immediately and return the toys to the stores where purchased for a full refund. September 1, 1993 Release # 93-106 : Modular playground equipment with solid metal decks manufactured by Miracle Recreation Equipment Company and installed from 1950 to the present at parks, schools and fast-food restaurants. PROBLEM: If the decks are located in the direct sun, the metal can heat to temperatures in excess of 120 degrees and young children who climb or sit on the hot decks can receive severe burns. The U.S. Consumer Product Safety Commission knows of 201 children whose hands, legs, or buttocks were seriously burned on the hot metal play equipment. WHAT TO DO: If you own or think you recognize one of these decks, call Miracle at 1-800-523-4202 to identify the piece of equipment and to receive a repair kit designed to cover the metal deck. WASHINGTON, DC -- Miracle Recreation Equipment Company, Monett, MO in cooperation with CPSC is offering "deck-coverings" to owners of its modular playground equipment with solid metal decks, to prevent the metal from becoming hot and causing serious burns. Young children can receive severe burns from contact with the bare metal decks, if the decks are located in the direct sun. The solid metal decks on modular play equipment in direct sun can heat to temperatures in excess of 120 degrees, even on a 70-80 degree day. The Commission staff has received reports from consumers of 201 children who suffered serious burns to their hands, legs, or buttocks when they climbed or sat on the hot metal play equipment. The injuries occurred on bare metal play equipment installed from 1950 to the present at parks, schools and fast-food restaurants. To prevent young children from being burned on bare metal decks, the Miracle Recreation Equipment Company is offering owners of Miracle modular play equipment a covering for all its solid metal decks. Parks, schools and restaurant owners/managers who have Miracle modular play equipment with solid metal decks should call Miracle at 1-800-523-4202 to receive a covering which is designed to prevent young children from being burned on the metal deck. Also, consumers who notice unshaded bare metal decks on public play equipment should contact Miracle to identify equipment that needs to be covered. AUGUST 30, 1993 Release # 93-105 PRODUCT: Approximately 7,900 16-inch model B-1601 Zenital sidewalk bicycles with training wheels. The bicycles, labeled with the name Zenital were imported from China and sold at swap meets in southern California, Arizona, and Texas between September 1992 and April 1993. PROBLEM: The bicycles have handbrakes but no footbrakes and lack handlebar stem insertion marks. Federal regulations require the bicycles be equipped with both handbrakes and footbrakes and handlebar stem insertion marks. Sidewalk bicycles with handbrakes, but no footbrakes, may present a risk of injury to young children who might not be able to stop the bicycles using handbrakes. WHAT TO DO: Consumers with these bicycles should contact Zenital, 10135 Rose Avenue, El Monte, CA 91731 or on 1-800-888-3558 to arrange for the bicycle to be modified for free at a local bicycle dealer. WASHINGTON, DC -- CPSC, Zenital, Inc. of El Monte, CA is voluntarily recalling about 7,900 16-inch model B-1601, Zenital sidewalk bicycles with training wheels. The bicycles are equipped with handbrakes, but no footbrakes and lack handlebar stem insertion marks. Federal regulations do not permit sidewalk bicycles to be equipped with handbrakes only and all bicycles must have handlebar stem insertion marks. Sidewalk bicycles with handbrakes, but without footbrakes, may present a significant risk of injury to young children who might not be able to stop the bicycles using handbrakes only. The handlebar stem insertion mark is required to place the stem at the proper depth in the head tube of the bicycle. The bicycles, labeled with the name Zenital were imported from China and sold at swap meets in southern California, Arizona, and Texas between September 1992 and April 1993. The bikes sold for approximately $30.00 each. The potentially hazardous bicycles were identified during a joint surveillance program conducted by CPSC and the U.S. Customs Service at the Port of Los Angeles. Consumers who have these bicycles should write to Zenital, Inc., 10135 Rose Avenue, El Monte, CA 91731, or they may call the firm at 1-800-888-3558 to arrange for the bicycle to be modified for free at a local bicycle dealer. Zenital, Inc. will have the dealers replace the rear wheel with a coaster brake and stamp the handlebar stem with a properly placed permanent minimum insertion mark. AUGUST 27, 1993 Release # 93-100 PRODUCT: Five toys/children's jewelry items distributed by Oriental Trading Company, Inc. The items include plastic toy maracas, item #16/008; plastic toy baby bottles, item #16/16; plastic heart-shaped stetchable bracelets; plastic stretchable charm bracelets; and beaded charm necklaces. PROBLEM: During CPSC testing, small parts detached from each of these toys and jewelry items, presenting a choking hazard to young children. WHAT TO DO: Consumers are urged immediately take the recalled products away from young children and return them to the stores where purchased for a full refund. The maracas were sold two to a package in a clear, plastic bag with a cardboard header labeled, in part "Rock 'N Roll CHA CHA." - Item #16/16, plastic toy baby bottles, 216,000 were sold between June 1985 and July 1992. The bottles were sold three to a package. Each package was labeled in part "*** Bottles with Real Rubber Nipples - Made in Hong Kong***." - Item #24/329, 4-inch heart-shaped stretchable toy bracelets, 42,000 were sold between September 1989 and July 1992. Each bracelet consists of 12 various colored hearts strung together on a elastic cord. The bracelets were sold 12 to a package and were sold in a clear, plastic bag with an affixed adhesive label reading, in part "***Made in China***." - Item #24/394, 15-inch plastic beaded charm necklaces, 39,600 were sold between May 1991 and July 1992. Each necklace consists of a beaded strand with 10 various colored shaped hearts, stars, and other similar items attached. The necklaces were sold 12 to a package and were sold in a clear, plastic bag with an affixed adhesive label reading, in part "***Made in China***." - Item #24/395, 6-inch stretchable charm toy bracelets, 42,000 were sold between June 1991 and July 1992. Each bracelet consists of a beaded strand with various shaped objects, such as hearts, attached. The charm bracelets were sold 12 to a package and were sold in a clear, plastic bag with an affixed adhesive label reading in part "***Made in China***." Neither CPSC nor Oriental Trading Company, Inc. is aware of any injuries involving these products; this recall is being conducted to prevent the possibility of injury. During CPSC testing, small parts detached from each of these toys presenting a potential choking hazard to young children. These toys were found during an inspection of the firm by CPSC. The recalled toys were sold in toy stores, discount stores, and various other retail stores nationwide and ranged in price from 20 to 80 cents each. Consumers are urged immediately take the recalled items away from young children and return them to the stores where purchased for a full refund. AUGUST 19, 1993 Release # 93-104 PRODUCT: Approximately 13,000 BISSELL Trio Vac model 3005 (single speed) and model 3005w (canister kit) vacuums manufactured during 1992 and sold nationwide by major retailers through June 1993. PROBLEM: The vacuum's plastic fan may break into pieces causing the housing to crack open and the fan pieces to propel from the vacuum's housing. BISSELL has received 40 reports of such incidents including 12 reported injuries. WHAT TO DO: Consumers who bought BISSELL Trio Vacuums through June 1993 should immediately stop using them and call BISSELL Customer Service at 1-800-237-7691, Monday through Friday, between 8:00 a.m. and 5:00 p.m. (Eastern Time). WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), BISSELL, Inc., Grand Rapids, MI, is voluntarily recalling the BISSELL Trio Vac model 3005 (single speed) and model 3005W (canister kit) vacuums manufactured in 1992. These models convert from full-length to hand-held vacuums and have plastic fans. The vacuum's plastic fan may break into pieces causing the housing to crack open and the hard, sharp fan pieces to be propelled from the vacuum's housing. Models with plastic fans were sold through June 1993. BISSELL has received 40 consumer reports of product failures including 12 reports of injuries. While most injuries were minor cuts and bruises to the lower body, two injuries occurred at eye level. Approximately 13,000 vacuums were sold nationwide through major retailers for about $50 or $60, depending on the model. After receiving retailer returns and direct calls from consumers regarding failed fans, BISSELL began a product recall and reported the problem to CPSC. Consumers who bought BISSELL Trio Vacuums through June 1993 should immediately stop using them and check the date stamp. To check the date stamp, remove the dirt collection tank and read the six digit number which appears on the inside of the bottom motor housing. The first two numbers are the year. If the numbers are 93, the vacuum has a metal fan design and does not need to be returned. If the numbers are 92 or no numbers are visible, the vacuum must be returned to BISSELL Inc. Please call BISSELL Customer Service: 1-800-237-7691, Monday through Friday, between 8:00 a.m. and 5:00 p.m. Eastern Time. BISSELL will replace the 1992 vacuums with 1993 vacuums containing a metal fan or provide a full refund of the purchase price. August 17, 1993 Release # 93-103 PRODUCT: Approximately 100,000 "Fisher-Price Kiddiecraft Racing Rover" cars sold nationwide during 1991 and 1992. The cars are bright yellow with a white and black dog as the driver. The car is propelled forward when the dog's head is pushed down and released. PROBLEM: Some children have removed the top part of the dog's head from the toy, creating a small part that could become a choking hazard. WHAT TO DO: Consumers should cut the plastic ears off of the dog and discard the rest of the toy. Both ears should then be sent to: Fisher-Price, Consumer Affairs, 636 Girard Avenue, East Aurora, NY 14052. Consumers will receive a $10.00 refund. For more information, consumers may call 1-800-233-4004. WASHINGTON, DC -- CPSC, Fisher-Price, Inc., East Aurora, NY is voluntarily recalling its Kiddicraft Racing Rover car after learning of several instances in which children removed the top part of the dog's head from the product. This creates a small part that could become a choking hazard. The recall affects approximately 100,000 Racing Rover cars sold nationwide during 1991 and 1992. The Racing Rover toy is a bright yellow race car with a white and black dog as the driver that propels the car forward when its head is pushed down and released. The toy is recommended for children ages one to four years. Fisher-Price has learned of two instances of children choking on the small part created after the top part of the dog's head was removed. CPSC received reports of two additional incidents. No injuries were reported as a result of this problem. To eliminate the potential choking hazard, Fisher-Price is asking consumers to cut the plastic ears off of the dog and discard the rest of the toy. Consumers should then send both ears to Fisher-Price, Consumer Affairs, 636 Girard Avenue, East Aurora, NY 14052, for a $10.00 refund, the approximate retail price. August 17, 1993 Release # 93-101 PRODUCT: Five iron-containing dietary supplement products manufactured and sold by Solgar Company, Inc. The products are: "Natural Amino Acid Chelated Iron," "Gentle Iron Vegicaps," "Hematinic Formula," "Prenatal Nutrients," and "Trace Elements." Approximately 17,500 bottles of the recalled products were sold in brown glass bottles with yellow screw-on caps between January 1, 1993 and mid-April 1993. The products were sold in health food and nutrition stores nationwide. PROBLEM: The products were not put in child-resistant packaging as required by the Poison Prevention Packaging Act. Even though the firm and CPSC know of no poisoning incidents due to the Solgar brand iron tablets and capsules, CPSC is aware of many ingestions and at least 11 deaths in 1991 from other iron-containing products. WHAT TO DO: Consumers with any of the recalled products should immediately return the unused portion of the bottles to the stores where purchased or return the product to Solgar Co., Inc. 410 Ocean Avenue, Lynbrook, NY 11563, Attention: Recall, for a complete refund of the purchase price plus postage. These five iron-containing products were not put in child-resistant packaging as required by the Poison Prevention Packaging Act. The lack of child-resistant packaging means that a child could easily access and accidentally swallow the iron tablets or capsules. Each bottle of the recalled products contains enough iron that, if consumed by a child at one time, could cause serious illness or death. The five recalled products were sold in brown glass bottles with yellow screw-on metal caps. Each product comes in several package sizes ranging from 50 to 1000 tablets or capsules per bottle. This recall does not include any other products manufactured by Solgar which contain iron, nor does it include any of the above products packaged with the child-resistant white plastic "push-and-turn caps." Approximately 17,500 bottles of the five recalled iron products were sold to consumers through retail health food and nutrition stores nationwide between January 1, 1993 and mid-April 1993. These products were manufactured and sold by Solgar Co. Inc. for several years. Consumers should immediately return the unused portions of any bottles of the recalled iron products (including bottles which were purchased before January 1, 1993) to the stores where purchased for a refund of the purchase price. PROBLEM: The single buckle that secures the leg strap under the high chair seat needs to be replaced. The original leg strap could be pulled through the slot in the seat which would enable a child to fall from the high chair. The affected bicycles were manufactured and distributed nationwide between February 1993 and March 1993. Affected models are listed on the attached news release. PROBLEM: Affected bicycles have a defect that may cause the plastic hand brake levers to break during use. If the lever breaks, the rider could fail to stop and fall from the bicycle. The police car is red, white and blue and the dumper is red, white and yellow. The toys also came in gift packs with either one or both of the vehicles. PROBLEM: Small parts may separate from the vehicles, presenting a potential choking and aspiration hazard to young children. WHAT TO DO: Consumers are urged to take the toys away from young children immediately and return them to the stores where purchased for a full refund. PROBLEM: The gas tanks on the mowers may split along the seams, leaking gasoline. The leaking gasoline presents a fire hazard. WHAT TO DO: Mowers purchased after September 1, 1989 should have the tank replaced immediately. Those mowers purchased prior to September 1, 1989 should be regularly inspected for leaks and if a leak should actually occur, the tank should be replaced. The latch pin assembly consists of a small, moveable knob attached to a shaft which, when pulled outward, allows the crib dropside to be lowered. WHAT TO DO: Replace the knob if it comes off or can be unscrewed easily. Consumers should contact the retail store where the crib was purchased, or another baby furniture store, for assistance in getting a knob replacement. Lewis of London is not conducting a recall because the company is in bankruptcy. Most of the retail stores known also as "Lewis of London" are franchisees and are not affiliated with the manufacturer of Lewis of London brand cribs. WASHINGTON DC -- CPSC is warning parents about a potential choking hazard involving the crib latch pin assemblies for all of the approximately 8,000 "Lewis of London" brand cribs purchased prior to 1990. The latch pin assembly consists of a small, moveable knob attached to a shaft which, when pulled outward, allows the crib dropside, or headboard if also moveable, to be lowered. The Commission is recommending replacement of the knob portion of the assembly if it's loose. The knob portion of the assembly on an unknown percentage of the cribs sold nationwide prior to 1990 may be removed using very little force. If removed, such knobs pose a choking hazard to children under 3 years of age. CPSC is aware of instances where toddlers placed these knobs in their mouths. The children either found them loose near the crib or were able to remove them from the cribs themselves. CPSC is not aware of any injuries involving these knobs. This warning and suggested correction is being issued to prevent the possibility of injury. Consumers are urged to inspect all Lewis of London brand crib knobs on cribs purchased before 1990 for looseness. If a knob comes off or can be unscrewed easily, consumers should contact the retail store where the crib was purchased or another baby furniture store for assistance in getting a knob replacement. Lewis of London is not conducting a recall because the company is in bankruptcy. The firm was located in Hauppauge, NY. PROBLEM: When pressure is applied, the handrails of the ladders may bend outward, creating a gap between the handrail and the side plate. This gap allows room for fingers or hands to become caught and injured. WHAT TO DO: Consumers with these ladders should call 1-800-235-0185 to receive replacement parts that will connect the sideplate to the handrails; this will prevent a gap from forming. WASHINGTON, DC -- CPSC, Delair Group, Inc., Delair, NJ, is reissuing notice of a repair program for their 52-inch "A" frame pool ladders, model numbers X4932-01 and X4932-06. Delair Group, Inc. received a total of 10 consumer reports of children's fingers or hands being caught in the ladders since the ladders were first distributed in April 1990. Injuries included minor scrapes, severe lacerations, and a severed tendon. Approximately 7,500 pool ladders were distributed from April 1990 through October 1, 1991. The ladders were sold by Delair under the "Patriot" trade name to swimming pool supply distributors and retailers predominantly in the eastern United States. The Patriot trademark is not affixed to the ladder. However, consumers can identify the affected ladders, model numbers X4932-01 and X4932-06, by checking the model numbers listed on the instruction sheets included with the ladders' packaging. Consumers who purchased the affected pool ladders will be sent two new ladder sideplates, two new handrails, and instructions on how to install the parts. The replacement parts are designed to connect the sideplates to the handrails. This will prevent a gap from forming when pressure is exerted on the handrail. Consumers who purchased the affected pool ladders but have not already received replacement parts from Delair Group, Inc. may write to Patriot Manufacturing Co., 8600 River Road, Delair, NJ 08110. Consumers should give their name, address, phone number, the date they purchased the pool ladder, and enclose a copy of the sales receipt or a photo of the ladder for positive identification. PROBLEM: The fireworks display devices may tip over during operation and fire the charges horizontally or at various angles, resulting in death or serious injury to persons nearby. Although the Commission is not aware of any incidents involving these particular fireworks, the Commission knows of two deaths involving other multiple-tube aerial fireworks devices that tipped over, struck, and killed the victims. The Night Shell, Gorilla Bomb, and Gorilla Bomb II devices were imported beginning in December 1992. The Gorilla Bomb device consists of seven 10-inch mortar tubes with a connecting single fuse. The Night Shell and Gorilla Bomb II devices have seven 11-inch mortar tubes with a connecting single fuse. All three devices are 6-1/2 inches in diameter. The Night Shell device is labeled: "Link Triad Brand Night W-800 *** Made in Guangxi, China." The Gorilla Bomb device is labeled: "Gorilla Bomb Suger Star Shell Kg 707 Made in Jiangxi, China."
