CPSC, Apollo Presentation Products Announce Recall of Light Boxes WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Apollo Presentation Products, of Ronkonkoma, N.Y., is voluntarily recalling about 30,000 portable light boxes, used to view film, slides and transparencies. The light boxes have loose wires and lack adequate grounding, presenting fire, electrocution and shock hazards to consumers. CPSC has received two reports of loose wires, one in which the light box caught fire, resulting in $5,000 property damage. No injuries have been reported. The recalled Apollo light boxes are models LB100, LB101, LB102 and LB110. The model number is located on a sticker affixed to the unit's side. The sticker also reads in part, "Portable Light Box...CSA." The light box consists of a white metal box, containing a fluorescent light, and a Plexiglas viewing plate that slides into two black metal guides on top of the metal box. The light box has a folding handle and tilt stand, and it can be wall mounted. The light boxes were sold in a box labeled in part, "APOLLO PRESENTATION PRODUCTS, GLOWPRO PORTABLE LIGHT BOX, Made in China." A photo of the product and the model number are on the box. Office supply stores nationwide sold the light boxes from June 1997 through April 2000 for $66 to $115. Consumers should stop using the recalled light boxes immediately, and call ATVS, Inc. (which is now handling this recall) for instructions on returning the projectors to the company for a refund. Announce Recall of Sky Dancers® Flying Dolls WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Galoob® Toys Inc., of San Francisco, Calif., is recalling about 8.9 million Sky Dancers® flying dolls. Galoob® was purchased by Hasbro in 1998. The hard plastic Sky Dancers® dolls can fly rapidly in unpredictable directions, and can hit and injure both children and adults. Galoob® has received 170 reports of the dolls striking children and adults resulting in 150 reports of injuries. They include eye injuries, including scratched corneas and incidents of temporary blindness, broken teeth, a mild concussion, a broken rib, and facial lacerations that required stitches. The recalled Sky Dancers® dolls were sold in many different styles, including Pretty Lights Sky Dancers, Mini-Sky Dancers and Fairy Flyers. The princess/ballerina-type dolls have hair pulled up into a pony tail and have stiff foam wing-covered arms that propel the doll when it is launched. The launchers, sold in many shapes including dolphins, flowers, moon, ponies, and sun and rainbow, have a molded plastic base and a pull-cord. The launchers were sold in both hand-held and table-top versions. The doll is inserted into the top of the launcher feet-first, and the pull-cord is pulled to launch the doll. The packaging is labeled "Sky Dancers®", "galoob®", "MADE IN CHINA", "Ages 5 and Up Only" and "Not for children under 3 years". Mass merchandise and toy stores nationwide sold the dolls from November 1994 through June 2000 for between $8 to $25. Consumers should stop using these flying dolls immediately and call Galoob® for instructions on how to return the toy sets to receive a product of equal value. The toy phones have a ball-shaped antenna which can detach, presenting a choking hazard to young children. CPSC and Vtech Industries have received nine reports of the plastic ball detaching from these phones. One child reportedly started to choke on one of these plastic balls. The toy is called Little Smart Soft Songs Baby Phone. The cordless baby phone has a mirror, four colorful talking musical buttons, a ringing light-up phone button and a red on/off button. The phone is yellow with a blue padded border and a yellow ball-shaped antenna. A label on these toys reads "Soft Songs Baby Phone 39100." Writing on the phone includes "VTECH" and "Little Smart Soft Songs Baby Phone." Wal-mart stores nationwide sold the toy phones from January 2000 through May 2000 for about $10. Announce Recall to Repair Children's Rocking Chairs Sold at Target Stores WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Jetmax International Ltd., of Stamford, Conn., is voluntarily recalling to repair about 48,000 children's rocking chairs. The rear legs of the chair can separate from the rocker's base causing it to collapse, and the child seated in the chair can fall. Jetmax has received five reports of these chairs collapsing and children falling. One 13-month-old girl suffered a laceration requiring 18 stitches. Jetmax imported the rocker exclusively for sale by Target stores. The wooden rocking chairs come in two styles: one for boys and one for girls. The boy's rocker has a yellow frame, red rockers (chair bases) and a blue canvas seat. The girl's rocker has a white frame, purple rockers (chair bases) and a pink canvas seat. There is no writing on the chairs, but the packaging reads, "restore? & RESTYLE KIDS," "Wooden Folding Chair," "Ages 3 and Up," and "Made in China." The chairs are designed for children between 30 and 100 pounds. They are 23 inches long, 13 inches wide and 16.5 inches high. Target stores nationwide sold the rocking chairs from November 1999 through March 2000 for about $20. These walkers can collapse unexpectedly during use and injure infants. Graco has received 27 reports of these walkers collapsing. Injuries to infants have included a cut toe, a bump on the head, a sprained foot, and scrapes and bruises to legs and feet. The recalled Tot Wheels® Entertainer® Activity Center infant walkers, with "bouncing action" and five-position height adjustment, have model numbers 4032LN or 4032BLA, located underneath the front of the walker's tray. The walker has a removable, green play tray with 12 activities, including a steering wheel, ball spinner and flashing lights. The multicolor walker has an animal-print fabric seat. "Graco Lights & Sounds" is written on a label on the walker tray. "Tot Wheels® Entertainer® Activity Center with Bounce" is written on a label on the walker's base. Mass merchandise, juvenile products and major discount department stores nationwide, including Wal-Mart and Toys R Us, sold the walkers from September 1999 through February 2000 for about $50. Announce Recall of Vinyl Window Blinds WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Ace Hardware Corp., of Oak Brook, Ill., is recalling about 87,000 vinyl window blinds. These blinds contain lead exceeding government guidelines, and do not have required labeling warning they contain lead. Over time, vinyl deteriorates from exposure to sunlight and heat, forming lead dust on the surface of the blind. Young children can ingest lead by wiping their hands on the blinds and then putting their hands in their mouths. Lead poisoning in children is associated with behavioral problems, learning disabilities, hearing problems and growth retardation. CPSC and Ace Hardware are not aware of any lead poisonings involving these window blinds. This recall is being conducted to prevent the possibility of lead poisonings. The recalled vinyl blinds are white miniblinds and woodgrain roll- up blinds. Sizes of the blinds range from 23-inches wide by 42-inches long to 72-inches wide by 120-inches long. Writing on the label of the white miniblinds reads "Safe Nonleaded Vinyl Formulation." Writing on the label of both blinds reads "MANUFACTURED IN THAILAND FOR ACE HARDWARE CORP." Ace Hardware and other hardware stores nationwide sold the window blinds from August 1999 through May 2000 for between $6 and $27. Consumers should remove the window blinds from windows and keep them away from children. Consumers can return the window blinds to the store where they were purchased for a refund. Because the latex is aging faster than normal, the nipple can detach from the shield, presenting a choking hazard to babies. Playtex has received 21 reports of pacifier nipples detaching from the shields. No injuries have been reported. Only Classic Patterns "Cherubs" and Soft Comfort latex pacifiers are being recalled. These robes, when used as sleepwear, fail to meet federal flammability standards for children's sleepwear. The standards require sleepwear, including robes, to be flame resistant, and if the garment ignites, the flame must self-extinguish. Failure to meet the flammability standards presents a risk of serious burn injuries to children. Federated Department Stores has not received any reports of injuries from these robes. This recall is being conducted to prevent the possibility of injury. The recalled robes are made of 100 percent cotton terry cloth sold under the brand names Aegean, Baby Monarch, b kids, Charter Club, Club Room, and Jr. By Monarch. The robes are a wrap style with waist belt, two front patch pockets and a hood. They were sold in boys and girls sizes 12 months to 14 and in sizes small, medium, large and extra large. The robes come in a variety of colors and prints. The brand name, fabric content and size are on sewn-in on labels. The robes were sold at the following stores from July 1995 through January 2000 for about $25 to $52. The balloon tongue on this toy can detach, posing a choking and aspiration hazard to young children. The Promotional Resources Group has received one report of the balloon tongue detaching from the toy. A 3-year-old boy reportedly removed the balloon tongue, placed it in his nose and inhaled it into his sinus cavity. He required medical treatment to remove the part from his nose. The Bug Zapper toy comes in two different styles: a frog and a lizard. When the toy is squeezed, the balloon tongue of the toy rolls out from the mouth about 2 inches. They toys were mainly orange, green and purple. "CHINA" is molded into the toy. Fast food, drive-in and small franchise restaurants distributed these toys with children's meal from November 1998 through April 2000. Consumers should immediately take these toys from young children, and return the toy to the restaurant where the children's meal was purchased for a free replacement toy. Consumers also can receive a free replacement toy directly from the company. Some grinders were shipped without the side handle, which is used to control and guide the tool. Use of the grinder without the side handle can cause consumers to lose control of the tool and be injured.
Children's Picnic Sets - Mervyn's California, of Hayward, Calif., is recalling about 1,200 children's plastic picnic sets contained in clear bags. The clear bag reads "AUSTRALIA" in multicolored lettering, "Down Under" in blue lettering.

The removable toy bar that attaches to the seat can suddenly release and cause injuries to babies. Kids II has received 122 reports of toy bars suddenly releasing from bouncer seats, resulting in 31 injuries to babies, including a black eye, a scraped eye, and facial cuts. Only Kids II bouncer seats with semi-circular toy bars are recalled. These bouncer seats were sold under the names "Soft Toy Bouncer Seat" or "Comfort Me Bouncer." The Kids II logo is embroidered on each seat's crotch strap. The bouncer seats have a ruffled seat pad in three patterns: a black, white and red cow print; a teddy bear, rocking horse and toy box print; and a nursery rhyme print. Each pattern has certain model and lot numbers, which are found on a tag attached to the seat. The Comfort Me Bouncers feature vibration and soothing sounds, including music, waves and heartbeat. Each semi-circular toy bar has three toys. Some of the recalled seats came with white toy bar tethers. Mass merchandise and juvenile specialty stores nationwide sold the seats from October 1997 through April 2000 for about $25 to $35. Consumers should remove the semi-circular toy bars immediately and contact Kids II for an in-home repair kit. Consumers can continue to use the bouncer seat for the baby to sit in as long as the toy bar is removed. The smoke alarms can fail to alarm when smoke or fire is present. A capacitor in the alarm can burn out, releasing smoke and melting the cover. Universal has received three reports of alarms failing, releasing smoke. There has been one report of minor smoke damage to the wall of a mobile home. No injuries have been reported. Consumers can identify the recalled models by removing the alarm cover from its base and looking for the manufacture date code and model number. The smoke alarms are made of white plastic and have the manufacture date code and model number imprinted on the back of the alarm. Studio Store Announce Recall of Tweety Rattles and Sandals WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Warner Bros. Studio Store, of Burbank, Calif., (Warner Bros. Store) is recalling about 3,800 Tweety rattles and 1,000 pairs of Tweety sandals. Both products are being recalled because small parts of each product can detach and pose a choking hazard to young children. CPSC and Warner Bros. Studio Store have received three reports of parents removing detached parts of sandals and a rattle from their babies' mouths. No injuries have been reported. Rattle Recall The rattle is sold as part of a Tweety Bib, Rattle and Spoon Set. The rattle, which is covered with a yellow cloth and is approximately 5 inches long, is a Tweety image and has a small blue and white bib (about 1.5 inches wide and 1.4 inches long) tied around Tweety's neck. Sandals Recall The Tweety sandals are girls' jelly swimwear sandals made of transparent pink plastic with embedded sparkles and a picture on the inside soles of Tweety sitting on flowers. The sandals were sold in infant sizes from 3 to 15 months. The sandals have three round plastic flowers on the top surface. The flowers can detach, posing a choking hazard to a young child. Warner Bros. Stores nationwide sold the sandals from February 2000 through April 2000 for about $10. Consumers should take these products away from children immediately and return them to a Warner Bros. Store for a refund. Because of the high-pressure contents of these fire extinguishers, they can explode and expose consumers to flying debris, causing puncture wounds and blast injuries. CPSC and the firm have received five reports of the fire extinguisher exploding in consumers' homes. One consumer was hit by flying debris, but was not injured. The explosions have resulted in property damage. The fire extinguisher is yellow with a black top and a red spout. "FIRESTOPPER" is written on the side of the extinguisher in orange. The "I" in the name is written to resemble a flame. Other writing on the unit includes "ENVIRONMENTALLY SAFE," "NON-TOXIC," "The Portable Liquid Fire Extinguisher," and "MADE IN THE USA." The extinguisher is about 10 inches high and the net weight is 12 ounces. The product was originally marketed by Bombay Productions Inc. in California. Dollar-type discount stores, including Only Deals and Bargain Wholesale, and wholesalers sold these fire extinguishers nationwide from January 1999 through February 2000 for between $.99 and $4. Consumers should discharge these fire extinguishers into a plastic garbage bag or similar container outside, in an open area. Consumers can return the discharged units to the store where purchased for a refund. These AC adapters can overheat, posing a fire hazard. IBM has received nine reports of the AC adapters overheating, including one report of fire causing minor damage. There have been no reports of injuries. The recall includes AC adapters used only with IBM ThinkPad 310 (type 2600 only) and IBM ThinkPad i Series (type 2611 only) notebook computers, and IBM WorkPad z50 (type 2608 only) companion devices. The AC adapter is a small rectangular black plastic box that, on one end, has a permanently attached wire that connects to the computer, and on the other end has a detachable cord that plugs into an AC outlet. It is approximately 2.5 inches long by 4.5 inches wide by 1.25 inches high. The label reads in part, "AC Adapter," "Made in China" and "Delta Electronics (Dougguan) Co., Ltd." It has a FRU p/n number of 12J0537, 12J0539 or 02K6562. Electronic and computer stores nationwide, and the IBM web site and other on-line retailers sold these AC adapters with mobile devices from May 1997 through November 1999. They also were sold separately for about $50. Consumers should contact IBM to receive a free replacement AC adapter. The bottles, which contain methyl alcohol, are not sealed with child-resistant closures as required by federal law. Methyl alcohol, also known as methanol, is toxic and can cause blindness or death if ingested. CPSC and Filter Tech are not aware of any injuries involving this product. This recall is being conducted to prevent the possibility of injury. This recall involves 1 gallon bottles of Aqua Mist Windshield Washer Fluid with twist-off caps. The bottle's label reads in part, "Aqua Mist Windshield Washer Fluid Protects to -25° F...Manufactured by Filter Tech, Inc. Huntington, WV." The label's UPC number is "6 96632 00001 5." Speedway Superamerica stores and other automotive stores in Kentucky, Michigan, Ohio, Pennsylvania and West Virginia sold the windshield washer fluid from October 1999 through April 2000 for about $1.50. Consumers immediately should return the bottles to the store where purchased for their choice of a full refund, exchange or child-resistant cap.
