Crib Brackets & Mattress Support Hangers Replaced by Baby Line NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE June 26, 1985 Release # 85-033 "Questor" Crib Brackets And Mattress Support Hangers To Be Replaced WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Baby Line Furniture Company, a division of Spalding & Evenflo Companies, Inc., today announced a voluntary replacement program of slotted mattress support hangers and plastic brackets for Questor "Baby Line" cribs manufactured from 1970 through 1982 because the Commission believes they can cause suffocation hazards. The hangers and brackets hold the mattress and spring to the corner- posts of the cribs. Two deaths and 14 other instances where the slotted hangers became unhooked and 17 instances of plastic brackets breaking or bending have been reported to the Commission. If the hanger becomes unhooked or a plastic bracket bends or breaks, the mattress can tilt down when the child moves to that corner. The child can then slide down and become caught between the mattress and the side rail or end panel of the crib. In 19 of the 33 reported incidents, including the two deaths, the child became entrapped between the mattress and the crib. The Commission believes the hangers may become unhooked when the crib is moved, the bed is made of when a child jumps in the crib. While the firm believes its cribs do not present a hazard if assembled properly and used in accordance with instructions, a failure of the plastic hanger bracket, or an unhooking of the slotted hangers could cause the crib spring and mattress assembly to pivot downward when the child moves to that corner, resulting in potential for injury. The cribs can be identified by the name "Questor" on a stick-on-label underneath the mattress on the headboard or footboard of the crib. The cribs can also be identified by the slotted hangers and a tubular mattress support. More than 900,000 of the cribs were manufactured. Many of them are now being used by second or third owners. Fifty percent or more are believed by the Commission to be in storage. The cribs were sold in several different styles and come in white, maple, pine and sometimes in yellow and walnut finishes. They sold at retail for $77.00 to $140.00. The plastic brackets were previously recalled in 1979 and 1981, but the Commission has continued to receive reports of breakage and bending. Baby Line has since changed the design of its brackets and also of its mattress support hangers. If you own one of these cribs having slotted mattress hangers or plastic brackets, whether you are using it or have it in storage, call the manufacturer toll-free to obtain replacement kits for the slotted hangers and plastic brackets. Infant "Crib Cuddle" Recalled by Century & Product Source In cooperation with the U.S. Consumer Product Safety Commission, Century Products, Inc., of Stow, Ohio and Product Source, Inc., Of Laguna Hills, California, had a free replacement program for the infant hammock portion of a product called Crib Cuddle. Approximately 109,000 Crib Cuddles have been distributed since 1982. The firms and the Commission are aware of one death and two near-misses in which a baby under two months of age was found with his head over an edge of the hammock. The Crib Cuddle, which retails for approximately $39.95, is a yellow hammock of soft terry material with a plush pile center and a red heart-shaped, battery-operated pulsator intended to simulate a heartbeat-type sound. The hammock has six straps to suspend it from the side rails of the crib. Product Source distributed the Crib Cuddle under the trade name Infacare from March 1982 to May 1983. Century Products had been distributing the Crib Cuddle under the Century name since June 1983. All Crib Cuddle owners should immediately stop using the product and discard or destroy the product. Replacement products are no longer available. Crib Cuddles sold under the Infacare name, have a small white tag sewn onto the hammock with the Infacare name on it. High-Chairs Repaired by Sassy NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 23, 1985 Release # 85-028 Sassy Inc. Announces Voluntary Repair Program For Model 021 And 023 Portable High-Chairs WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Sassy, Inc. of Northfield, Illinois, today announced a voluntary repair program affecting approximately 40,000 Folding Sassy Seats, Models 021 and 023 manufactured between July 1984 and December 1984. The firm has received approximately 100 reports from consumers stating that their chair's plastic material developed cracks around the restraint horn and locking pin tabs (see drawing) prior to the product's removal from retail shelves in December 1984. If the cracks are present and the seat continues to be used, a laceration, falling hazard, or a pinching condition may be present to a user of the chair. There have been no reported injuries. The Folding Sassy Seat is used as a portable high-chair that is easily transported for convenient use in restaurants and when traveling in place of a conventional high-chair. The product consists of round aluminum tubing rails and bright yellow plastic seat and end caps. Other models of Sassy Seats are not affected (Deluxe Sassy Seat and the Original Sassy Seat). To eliminate the possibility of the Seat developing stress cracks, Sassy, Inc. will provide free replacement parts to all owners of their Model 021 and 023 portable high-chair. Retailers who received the chairs manufactured between July and December 1984 have already received notice of the repair program and have received replacement parts. It is believed that 30,000 of the affected chairs may remain in the possession of consumers. Consumers can identify affected chairs by the round aluminum tubing (an earlier Model 020 used square aluminum tubing and is not affected by this action). Consumers owning a Folding Sassy Seat Model 021 or 023 should send their name, address, telephone number, and store where purchased, to Sassy, Inc., 191 Waukegan Road, Northfield, Illinois to obtain a free repair kit. Proof of purchase and/or receipt is not necessary. Until the kit is received, consumers owning the affected models should inspect the Seat for cracks before each use and if cracks are present discontinue use altogether. Sears Craftsman 22-Inch Hedge Trimmers Recalled by Singer NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 14, 1985 Release # 85-026 Sears Craftsman 22-Inch Hedge Trimmers Recalled WASHINTON, D.C. -- In Cooperation with the U.S. Consumer Product Safety Commission, The Singer Company, Stamford, Connecticut today announced the voluntary recall of a 22-inch hedge trimmer sold by Sears, Roebuck and Company. The product being recalled is a Sears Craftsman 22-inch Hedge Trimmer, Model Number 315.796640 with date codes B4322, B4329 or B4336 without a white "100" label affixed below the data plate. These identifying numbers are on the plate which is located near the handle of the trimmer. Approximately 400 of these hedge trimmers were sold through Sears stores during December 1984 and January 1985 for $65.00 each. Although there have been no reports of accidents or injuries, a manufacturing defect in the plug assembly of some trimmers could, under certain circumstances cause a shock hazard if the operator is grounded. Consumers should immediately stop using the hedge trimmers listed above and return them to the Customer Service Counter of their nearest Sears store for replacement. Revco De-Icer Recalled by Hysan NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 14, 1985 Release # 85-025 Hysan To Voluntary Recall Revco De-Icer WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Hysan Corporation of Chicago, Illinois, today announced the voluntary recall of 47,000 cans of Revco brand spray windshield de-icer that Hysan produced from October through December 1984. Some of the aerosol cans containing this product have holes that allow the contents to leak or spray out. Hysan Corporation has received four reports of consumers who have been sprayed in the face by escaping contents. De-icers contain a flammable liquid, and close attention should be paid to the warning label. The recalled cans display a blue and white label painted on the metal can. Revco de-icer cans with paper labels are not affected. Squeak Pacifiers Recalled by Glenco NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 10,1985 Release # 85-024 Pacifier May Present Potential Choking Hazard WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Glenco Infants Items, Inc., of Northvale, New Jersey, is urging consumers to stop using and to discard the Glenco #3180 Squeak Pacifier because it may present a choking and suffocation hazard if the nipple and handle separate from the plastic shield. Separation of the nipple and handle from the shield occurred during CPSC structural integrity testing which involves boiling the pacifier and then subjecting the handle to pull forces. The pacifier has a rigid plastic shield with two vent holes and the words "MADE IN JAPAN" embossed on the shield. The shields are white, orange or yellow. The nipple and handle are made of a single piece of rubber. The handle, which is bulbous or round, has a white plastic squeaker on the end. The pacifiers were sold between July and December 1984. The Commission also has received three consumer complaints reporting that the squeaker separated from the pacifier. Commission testing did not confirm this problem. While the Commission knows of no incidents which caused Items, Inc., involving the nipple and handle separating from the shield, the pacifiers to fail CPSC testing, CPSC and Glenco Infant have determined that because of the potential for injury this warning is in the public interest. This notification does not include the redesigned pacifier which is identified on the package as the Glenco #3180B Squeak Pacifier. Recalls Ladies Active Sweatshirts WASHINGTON, D.C. -- The U.S. Consumer Product Safety Commission and Bernard Chaus, Inc., Secaucus, New Jersey, today announced a Voluntary recall of Ladies active sweatshirts sold by the firm. Approximately 16,000 units were imported from Sri Lanka and sold nation wide. The sweatshirts are identified as being styles 45F70, 54507 and 65402 and the firm has agreed to provide a full refund to consumers who return the sweatshirts to the retail stores where they were purchased. Although these sweatshirts have not been involved in any incidents known to the Commission or the Chaus, Inc. testing shows that the sweatshirts fail to meet the flammability requirement for general wearing apparel before and after washing. The recalled Ladies active sweatshirts were made in Sri Lanka of 100% cotton with a neck label tag "CHAUS RN 51323". The sweatshirts were sold for $30.00 to $40.00 from August 1984 through February 1985. FOR IMMEDIATE RELEASE Originally issued April 11, 1985; Revised Decerber 3, 2001 Release # 85-018 Perfection Kerosene Heaters WASHINGTON, D.C. -- The U.S. Consumer Product Safety Commission and Perfection Schwank (formerly Perfection Product Company) of Waynesboro, Georgia, had announced a retrofit program for Perfection Model 770 and Model 770-l portable kerosene heaters. These kerosene heaters were originally manufactured for outdoor use only and do not contain a flame extinguishing device to extinguish the flames if the units are tipped over. Some consumers may be I using the units indoors where the risk of fire is much greater. The units are black or beige in color, cylindrical in shape and originally sold for approximately $40.00 to $50.00 dollars. Consumers should check the identification label: on their heater for the model number. Consumers owning one of the heaters are urged to stop using it indoors. Perfection Schwank (706-554-2101) is no longer offering the retrofit program. Consumers should not use these products. They should be discarded or destroyed. Flying Wheels Playground Swings Repaired by Miracle NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE April 9, 1985 Release # 85-017 Miracle Recreation Equipment Company To Repair "Flying Wheels" Playground Swings WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Miracle Recreation Equipment Company of Grinnell, Iowa, today announced a repair program affecting 274 "Flying Wheels" tire swings produced by the firm between 1977 and 1983. A five-year old girl in Lorain, Ohio was killed in June 1984 after part of a Flying Wheels swing fell and struck her. Flying Wheels is a playground swing consisting of four tires suspended from cross beams that rotate about a central metal shaft. The accident in Ohio occurred sometime after a weld on the unit had failed. Flying Wheels swings subject to this program were sold nationally to local park and recreation departments and to some schools and churches. From its sales records, Miracle Recreation Equipment Company will contact directly, by registered mail, each purchaser of a Flying Wheels unit to notify them of the potential hazard and arrange for a field repair intended to prevent the swing assembly from the central shaft. The firm will provide needed charge, for the repair and an allowance of $50.00 for from separating parts, without welding costs. Until the repair has been completed, the Commission and the manufacturer urge that the tire swings be removed from all Flying, Wheels units purchased prior to 1984. Clickclack Baby Rattle Recalled by Brio Scanditoy NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE April 1985 Release # 85-014 Brio Scanditoy Recalls Clickclack Baby Rattle Washington, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Brio Scanditoy Corporation of Milwaukee, Wisconsin, today announced that it is voluntarily recalling its Model #30051 Clickclack baby rattle. The firm reports that some of the Clickclack rattles are poorly glued at the ends. If the end part of the rattle comes unglued, the small blue wooden piece inside the rattle is exposed and presents a potential choking hazard to infants playing with the rattle. No incidents of injury have been reported. Brio Scanditoy distributed approximately 5,000 Clickclack rattles between July 1984 and February 1985. The rattle is made in Sweden and retails in the U.S. for approximately $5.00. The wooden Clickclack #30051 is approximately 4.25 inches long. It has a bright red hemispherically-shaped knob at each end of a hollow body that contains the small blue noisemaker. Glass Carafes Recalled by Hamilton Beach NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR RELEASE March 13, 1985 Release # 85-012 Hamilton Beach Recalls 165,000 Glass Carafes And Agrees To Pay Civil Penalty Washington, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), the Hamilton Beach Division of Scovill Inc., Waterbury, Connecticut, today announced the recall of approximately 165,000 4-cup glass carafes sold with its Model 784 style 4-cup Mini-Drip Coffee and Tea Maker, manufactured between August 1980 and August 1981. Hamilton Beach has also agreed to pay the CPSC a civil penalty of $35,000 in settlement of allegations of the Commission staff, which are denied by the Company, that the Company violated the defect reporting requirements of the Consumer Product Safety Act. Chairman Terrence M. Scanlon and Commissioners Stuart M. Statler and Carol G. Dawson voted to accept the civil penalty settlement. Commissioner Saundra Brown Armstrong dissented from this decision. Under certain conditions, when picked up, the 4-cup glass carafe could slip from its metal handle ring and break, presenting a burn or laceration hazard. CPSC and Hamilton Beach are aware of approximately 260 incidents where a carafe of hot liquid separated from its handle resulting in more than 140 injuries of burns or cuts. The 4-cup coffeemaker and carafe were sold together in both the United States and Canada. While 403,000 of these products were originally produced, it is estimated that 165,000 units remain in use, including approximately 160,000 units in the United States and 5,000 units in Canada. The coffeemaker and carafe were sold in the United States under the Hamilton Beach name as Models 784, 785AL and I 779AL; under the J.C. Penney name as Model 2290, and under the Ambassador name as Model 62360. The coffeemaker and carafe were sold in Canada under the Simpson-Sears name as Models 67001 and 67002 and under the Philips name as Model HD5184 and HD5185. The product sold for approximately $24.00. Carafes subject to recall can be identified by a label on the bottom of the coffeemaker bearing one of the above listed model numbers and a date code stamped into the underside of the coffeemaker. Only date codes stamped 0880 through 0881 are applicable. The first two digits of the code represent the month and the last two the year of production. Either the letter W or C should appear after the four-digit date code. The recall is limited to the carafe only. The coffeemaker itself is not involved. Consumers who identify their carafe as being recalled should discontinue use of the carafe pending replacement by the Company. Electric Space Heaters Recalled by Patton NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR RELEASE March 12, 1985 Release # 85-011 Reminder Of Voluntary Recall Of Electric Space Heaters Washington, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Patton Electric Company, New Haven, Indiana, today issued a reminder to the public of its voluntary recall, originally announced in April 1984, of a particular discontinued model of a "Patton" and "Sears" brand portable electric space heater because of a possible fire hazard. The firm is issuing this reminder because approximately 50,000 of the discontinued heaters may still be in consumer use. The CPSC has investigated over sixty incidents of fire in houses where these heaters were being used. Patton halted distribution of the heaters after reports of a possible fire hazard. In March 1984, Patton notified all retail outlets which sold the heaters to remove them from sale. The recalled electric heaters are small portable plastic models and have a "slide-type" control switch on the top. Consumers may identify the recalled units by looking at the bottom label of their heater for either Patton Model HF-10 or Sears Model 201-7208 and the words " Made in Hong-Kong." Only the above two models were involved in the recall. Consumers who may still have these heaters in their possession are urged to stop using them and to unplug them from electrical outlets. Owners should call Patton at l-800-528-6600 ext. 1722 (Arizona: l-800-352-0458 ext. 1722; Alaska l-800-528-0470 ext. 1722) to verify that their heater is one of the recalled models and to obtain instructions for returning the heater to Patton for replacement at no cost. The heaters were sold nationwide between August 1983 and March 1984 in large chain stores including Sears, as well as in small retail outlets. The units were sold for under $30.00. The company believes the potential fire hazard is related to the sliding type switch assembly and not the heater itself. This switch, which utilizes a diode, was installed only in the recalled heater models. These are also the only heaters manufactured in Hong Kong for Patton. All other Patton models are manufactured by Patton in the U.S. and have proven rotary dial temperature controls. Under the recall program, Patton Electric will replace all suspect model heaters with a new domestically manufactured unit. The replacement as well as all transportation charges will be at no cost to the consumer.
Scroller Saws Recalled by Singer NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE January 18, 1985 Release # 85-004 Recall Of Scroller Saws With Replacement Circuit Boards (Washington, D.C.) -- The Singer Company, Stamford, Connecticut and Ranco Electric, Irving, Texas, in cooperation with the U.S. Consumer Product Safety Commission, today announced the recall of any Sears Craftsman Commercial Auto Scroller Saws, Model No. 315.26750 and Craftsman Multi-Speed Scroller Saws, Model No. 315.26710 which have previously been repaired with this replacement circuit board. Saws which have been repaired with this replacement circuit board could pose an electrical shock hazard. Only saws of these two model numbers repaired since October, 1980 through June, 1984, are subject to recall. The model number for the saws may be found on the saw motor plate. These saws were sold through Sears stores and catalogs between 1968 and 1972 for an estimated $60.00 each. However, the only saws affected are approximately 1,600 which were repaired with the Ranco circuit board between October, 1980 and June, 1984. There have been no reports of accidents or injuries associated with the use of the saws repaired with the replacement circuit boards. Consumers should immediately stop using these saws and return them to the Customer Service Counter of their nearest Sears store. The circuit boards will be replaced at no charge. Brushcutters/Trimmers Recalled by K & S NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE January 15, 1985 Release # 85-002 K & S Industries, Division Of Cosco Inc., To Repair Brushcutters (Washington, D.C.) -- The U.S. Consumer Product Safety Commission in cooperation with K & S Industries, A Division of Cosco, Inc., Fort Worth, Texas, today announced a voluntary repair program of 23,240 K & S Brushcutter/Monofilament Trimmers because the blade may separate from the brushcutter. K & S Industries is aware of 11 occurrences in which the blade separation has resulted in laceration injuries to the feet and legs of the users. The blade will come off if the brushcutter head if not properly aligned with the drive plate or if the retaining bolt is not tight. Only those brushcutter/trimmers identified under the trade name of K & S Industries, Models No. Trim-All TA-1200, TA-1200X, Combo 87-1633, and Estate Trimmer E-1200 are subject to the repair program. The model numbers may be found on a decal located in the middle of the shaft and the brand name may be found on the engine frame above the starter pull handle. The product sold nationwide by K & S Industries from February 1, 1982 to August 9, 1984 through a variety of stores for approximately $189.95. K & S Industries is providing a free replacement kit to all owners of the affected brushcutter/trimmers. The kit consists of a blade assembly, bolt, Allen wrench, and instruction sheet. Consumers may continue to use the string trimmer attachment but are warned not to use the brushcutter attachment until the parts in the replacement kit have been installed. The replacement bolt is a special bolt made for this application and should not be replaced with a substitute. Recalls Sweatshirts (Washington, D.C.) -- The U.S. Consumer Product Safety Commission and Alexanders Inc.,New York, N.Y., today announced a voluntary recall of sweatshirts sold by the firm. Alexanders operates 15 retail department stores in the greater New York metropolitan area and has agreed to provide a full refund to consumers who re- turn the sweatshirts to its stores. Although this sweatshirt has not been involved in any incidents known to the Commission or to Alexanders, testing shows that it fails to meet the flammability requirement for general wearing apparel before and after washing. The recalled sweatshirts were made in China of 100% Cotton with a neck label tag "Michael Richards". The sweatshirts were sold for $8.50 in Alexanders own stores from February 3, 1984 through June 11, 1984. Certain Pacifiers Recalled by Monterey Laboratories NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE December 31, 1984 Release # 84-071 Monterey Laboratories, Inc., To Recall Certain Pacifiers WASHINGTON, D.C.-- In cooperation with U.S. Consumer Product Safety Commission, Monterey Laboratories, Inc., of Las Vegeas, Nevada today announced a voluntary recall affecting approximately 143 dozen pacifiers whose nipple may tear away from the pacifier while in use. The potential defect could result in a child either swallowing the nipple or inhaling the nipple into its windpipe. The design of the INF-920 has now been improved so that this potentially hazardous condition has been eliminated. Neither CPSC or Monterey has received reports of any injuries to consumers resulting from this potential defect. The recall is limited to certain Pur Model INF-920 regular shaped baby pacifiers distributed by Monterey and sold prior to December 11, 1984. Other Pur pacifiers and nipples, and all INF-920's purchased after December 11,1984, are unaffected by the recall. The 920 is made entirely of transparent plastic and silicon. The pacifier's shield has three ventilation holes molded into it, and the word "Pur" and the number "0","1","2" or "3" in raised lettering on the underside. It sells for between $1.49 and $1.69 at retail. There are no date codes or serial numbers by which potentially defective units can be differentiated from units which have no problems, so consumers should check to see if the store where they purchase the unit has a recall notice posted. If a notice is posted, the unit may have the potential problem and should be returned to that store. If there is no notice in the store, units may be returned by mail to Monterey. Any consumer with any doubt about whether an INF-920 he or she has purchased is one of the affected units may return it for a free replacement unit to the place or purchase, if a recall notice is posted there. Parkcycle Front Forks Replaced By Columbia NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE December 28, 1984 Release # 84-070 Replacement Program Offered For Columbia Parkcycle Front Forks WASHINGTON, D.C. -- In voluntary cooperation with the U.S. Consumer Product Safety Commission, Columbia Manufacturing Company of Westfield, Massachusetts announces a continuation of its free replacement program involving the front forks of 15,390 Parkcycles (children's sidewalk bicycles) manufactured between August 1 and November 1, 1983. The Commission is aware of 2 complaints of lacerations and contusions of the face resulting from children falling from these Parkcycles and approximately 30 reports of front fork failure that did not involve injury. All but one of these incidents were brought to CPSC's attention by Columbia. Prior tolerating of any fork failures, Columbia's internal testing program indicated that it would be desirable to strengthen the Parkcycle front forks and Columbia commenced using a heavier gauge steel for the front forks on all Parkcycle production as of November 1, 1983. Because of an inspection and replacement program conducted by Columbia, all Parkcycles sold after February 9, 1994, already have the strengthened front fork. Replacement forks are still being offered free of charge to all owners of Columbia Parkcycles purchased prior to February 8, 1984, and consumers are encouraged to participate in this program.The affected units are 16-inch children's bicycles identified as the boys' "X2 Tiger", Model No. 2122, and the girls' "Rainbow Connection", Model No. 2121. The Parkcycles were distributed nationally and retailed for approximately $75.00. PARKCYCLES PURCHASED AFTER FEBRUARY 8, 1984 ARE NOT AFFECTED. Racquets Replaced/Refunded by DP NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE December 19, 1984 Release # 84-068 Diversified Products (DP) Announces Racquetball Racquet Replacement/Refund Program WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Diversified Products Corporation, known nationally as "DP", and headquartered in Opelika, Alabama, is voluntarily offering to refund or replace certain older aluminum frame racquets sold under the brand name "Leach", and refurbish certain older Leach plastic-molded models used for playing racquetball. It has been reported that, upon the release of the racquet during the swing, the butt-plate (plastic plug located at the base of the handle) of the racquet has on seven occasions, pulled out of the racquet handle and the racquet has struck an opposing player. The majority of racquets in question were marketed by Leach Industries, Inc. ("Leach") before DP purchased the company in March, 1980. The racquets were sold between January 1, 1979, and June 30, 1982. DP will refurbish the butt-plate at no charge. Additionally, at the owners option, a new leather grip and wrist thong will be attached for $4.00. The aluminum racquets involved were sold prior to December 31, 1980. DP is offering a $4.00 refund, or again at the owners option, a new aluminum racquet for $10.00. November 6, 1984 Release # 84-067 Nine Inch Oscillating "Snow" Fan Recalled WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Collins Co. Ltd., Secaucus, New Jersey, to-day announced the voluntary recall of 26,000 model CYHD-103 "Snow" brand nine inch 2-speed oscillating fans. These fans were sold by Wal-Mart Stores, Inc. of Bentonville, Arkansas, and Wal-Mart has agreed to assist in the recall program. The Commission staff believes that some of the fans may have a defective 2-speed control switch which could result in overheating of the fan motor and ignition of the plastic motor case, and that the product therefore presents a fire hazard. Seven fires involving the use of this fan have been reported in 1983 and 1984. Although no personal injuries have resulted, the Commission staff is concerned about the danger to consumers from such fan fires occurring at night or while persons nearby are asleep. Collins and Wal-Mart have discontinued distribution of the fan. The fan was imported in 1983 by Collins Co., Ltd. and was sold through Wal-Mart Stores, Inc. to consumers in about 20 southern and midwestern states. The fan sold for about $20 dollars. Collins through Wal-Mart is offering purchasers a full refund for the return of this recalled fan to any of Wal-Mart's 693 retail stores. If return to a Wal-Mart store is inconvenient due to the distance involved, consumers may obtain a full refund by calling Collins Co. Ltd. at 201-866-9560 for instructions to ship their fans to the company. As an alternative, consumers may write to Thomas C. Anderson, Collins Co. Since similar Snow brand Chin Yung Ho 9" fans have been sold by other retailers nationwide, Wal-Mart Stores Inc. requests that consumers return only those fans purchased from its stores. Wal-Mart advises owners who have purchased Snow brand 9" fans from other retailers to contact those retailers or the CPSC to determine if the product has been recalled or may be hazardous. November 6, 1984 Release # 84-066 Peoples Drug Stores Recalls Certain Nine Inch Oscillating Fans WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Peoples Drug Stores, Incorporated, Alexandria, Virginia, is recalling certain Model CYHD-103 "APL", "SNOW", and "KING" brand nine inch oscillating fans it sold in 1982 and 1983. The Commission staff believes that a fire hazard exists because the 2-speed control switch may fail and cause overheating of the motor and ignition of the motor case and other plastic parts of the fan. Though no injuries have occurred, four fan fires have been reported. Three of these incidents occurred in 1982, and involved the a "King" brand fan. Some "Snow" and "APL" brand fans were previously recalled by Peoples in 1982. A total of 6,366 fans may still be in the hands of consumers, according to the firm. The fans sold for about $20 to $25. All of the fans being recalled were manufactured by Chin Yung Ho Enterprises Co., Ltd., Taichung, Taiwan. They were sold in Peoples, Lane, Haag and Reed drug stores (all of which are owned by Peoples) under three different labels: APL, Snow and King. The APL and Snown brand names are indicated by insignia located in the center of the metal grill an the front of the fans. The King brand label is actually a picture of a king's head and is also located at the center of the fan grill. Most fans sold by Peoples Drug Stores Inc. under the King label were not manufactured by Chin Yung Ho and are not covered by the recall. The nine-inch oscillating "King" brand fans covered by the recall can be identified by a label on the fan bottom with the model number "CYHD-103", and the name of the manufacturer, Chin Yung Ho Enterprises Co., Ltd. All nine inch oscillating fans sold by the firm bearing the APL and Snow labels were also made by Chin Yung Ho and are being recalled. All of the recalled fans have a 2-speed control (slide) switch on the fan base. The fans were listed by Underwriters Laboratories. People, Lane, Haag and Reed drug stores are located in the 14 states of Alabama, Georgia, Illinois, Indiana, Iowa, Kentucky, Mary-land, Minnesota, North Carolina, Ohio, Pennsylvania, Tennessee, Virginia, West Virginia and the District of Columbia. Consumers identifying their nine inch oscillating fans as being covered by this recall should stop using them immediately and should return them to the nearest People, Lane, Haag or Reed drug store. owners will be given a full refund of their purchase price.
