The dry cell battery can rupture and irritate the skin. Safety 1st has received 76 reports of batteries rupturing. One consumer reported discomfort and swelling when the battery ruptured in her hand. The batteries are sold with the model 49226 Safety 1st nursery monitor. The monitor comes with a parent's unit (receiver) with charging stand, baby's unit (transmitter), AC adaptor and battery. The receiver holds the battery and displays the date codes for the units involved. These units have date codes between 00097 and 03097 imprinted on a white sticker on the back of the receivers. The monitor is made of hard white and gray plastic and both the receiver and transmitter are labeled "Safety 1st" and come with flexible antennas. The monitors are packaged in a cardboard display box labeled in part, "Safety 1st...Rechargeable Monitor...Cordless...Model No. 49226." Retail stores nationwide sold the monitors from June 1997 through August 1997 for between $35 and $45. Consumers should stop using the monitors immediately, and call Safety 1st at (800) 964-8489 between 9 a.m. and 5 p.m. EST Monday through Friday for instructions on returning the recalled batteries and receiving replacement batteries. Announce Recall of Wipe Warmers WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Halcyon WaterSpring Inc. of Ontario, Canada, is recalling about 536,000 Dex Wipe Warmers, an electric heating pad that warms baby wipes. The Wipe Warmers, distributed by Dex Products Inc., of Benicia, Calif., could overheat and melt. Halcyon WaterSpring is aware of six incidents involving melting of the product and one alleged fire of undetermined cause where other products were present. The Wipe Warmer is about 27 inches long and 3 1/2 inches wide, and wraps around disposable plastic containers to warm the baby wipes inside. The product's cloth cover is white; covered with pink, blue, yellow and green handprints; and has Velcro fasteners. The tan or off-white, plastic warmer unit inside the cloth cover has a white sticker that lists the model number, WW-01. The warmer comes in a mostly purple box with the words, "Dex Products Wipe Warmer, A must for every nursery!" printed in white. Various retail stores nationwide sold the Wipe Warmers from January 1994 through December 1996 for about $10 to $15. Consumers should stop using the Wipe Warmer immediately and call Halcyon WaterSpring Inc. at (888) 735-5585 between 9 a.m. and 5 p.m. EDT Monday through Friday for information on how to receive a full refund or replacement. Limit switches within these heaters can emit sparks, presenting a fire hazard. Cadet Manufacturing has received 44 reports of fires involving these heaters. Most of these reports resulted in fires that self-extinguished. However, Cadet has received two reports of fires causing considerable property damage. No injuries have been reported from these fires. CPSC has received a 45th report of two deaths from a fire involving a Cadet heater where a sofa was nearby. The cause of the ignition of the sofa remains under investigation. The heaters involved in this recall may have the Cadet or Encore brand name on the heater's grill. The heaters involved in this recall must have the following: 1) a model number beginning with FX, FW, LX or ZA followed by three numbers on a label on the front of the internal heater assembly; 2) a limit switch made of black plastic casings on the heater assembly. These casings look like small black discs and have wires attached. Only heaters with black plastic limit switches are included in the recall. Some models will have a single limit switch and others will have two limit switches. Before checking the markings on the heater assembly, consumers will need to remove the heater's grill and the screw at the top of the heater assembly. Before doing this, consumers must disconnect the power supply to the heater or fuse box. These heaters have been installed in homes from 1985 through 1992 in Oregon, Washington, California, Idaho, Montana and Wyoming. Fewer than one percent were distributed nationwide under the name Encore through the following retailers: Menards, Home Base, Builders Square, Ace Hardware and Fred Meyer for about $100 to $200. Consumers should stop using these heaters immediately and call Cadet at (800) 567-2613 Monday through Friday between 8 a.m. and 5 p.m. PDT. Heaters with white ceramic switch casings are not included in this recall.

CPSC, Dell Announce Recall of AC Adapters for Latitude LM Notebook Computers and Latitude LM Port Replicators WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Dell Computer Corp., of Round Rock, Texas, is voluntarily recalling about 233,000 AC adapters supplied with its Latitude LM notebook computers and Latitude LM port replicators in the United States. Dell also is recalling these adapters throughout the world. The port replicator is a docking station used to connect the notebook computer to an external monitor and keyboard. The AC adapter's connector pins can break off from the adapter and remain in the power cord, posing a shock hazard. Dell has received 111 reports of the adapters' connector pins breaking off. No injuries have been reported. These black AC adapters come with the Dell Latitude LM notebook computers and port replicators. The adapters are 4 inches long by 2.25 inches wide and have one attached cord and one detachable power cord. The bottom of the adapter is labeled in part, "DELL...MADE IN TAIWAN." These adapter can be identified easily because the first eight digits of the "DP/N" bar code are "00097689" or "00099500." The "DP/N" bar code is on the bottom of the adapter. The only versions of the "00099500" adapters that are subject to this recall are those that have "A00, A01, A02, or A03" at the far right of the bar code. Dell's direct sales force, Internet ordering site and toll-free number sold the AC adapters with the Latitude LM notebook computers and port replicators from June 1996 to October 1997. The adapters also were sold separately for about $39. Consumers should stop using these AC adapters immediately and contact the company to receive a free replacement adapter and instructions for the safe use of the computer. The cords have undersized wires that cannot carry the advertised load, plugs that are not polarized properly, poor strain relief and lack of overcurrent protection presenting fire, shock and electrocution hazards. CPSC and In-Mar Trading Inc. are not aware of any injuries involving these extension cords. This recall is being conducted to prevent the possibility of injury. The recalled extension cords are white or brown. The cords have double wires, three female plugs and one male plug, and are available in the following lengths: 6, 9, 12, 15 and 20 feet. The receptacle is labeled in part, "Rated for 125 volts...For indoor use only." The cords are packaged in red, white and black cardboard display sleeves that are labeled in part, "SUPER POWER EXTENSION CORD...MADE IN CHINA." The last 5 digits of the UPC code range from 70027 through 70036. Discount stores mostly in Florida, New York and North Carolina sold the extension cords from October 1996 through December 1996 for between $1 and $4. Consumers should stop using the recalled cords immediately and return them to the store where purchased for a refund. Announce Recall of Guardian Dive Computers WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Mares America Corp. of Norwalk, Conn., is recalling about 1,200 dive computers used to determine dive depth, dive time and other automatic measurements during scuba diving. Guardian dive computers can fail to function when water leaks into the battery compartment during a dive. Also, the computer can malfunction by not automatically switching on during a dive or the computer control buttons on the computer can fail to operate properly. These failures could result in divers suffering decompression sickness, which can lead to significant injury. Mares is aware of 45 incidents of the Guardian dive computer failing to operate properly. There have been no injuries or deaths reported involving the use of these dive computers. The Guardian dive computer comes in two models, a wrist mount and a hose mount. The computer is about 3 inches wide and 3 inches long. It is black and gray with yellow buttons. "Mares" and "Guardian" are written on the frame of the computer around the screen. Selected Mares dive shops sold the Guardian computers nationwide from January through August 1997 for about $400. Divers should stop using the Guardian dive computer immediately and return it to their Mares authorized dealer to receive a free replacement computer. Announce Recall of Stuffed Bears WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Nadel & Sons Toy Corp. of New York, N.Y., is voluntarily recalling about 8,000 stuffed bears. The eyes of the bears can become detached, presenting a choking hazard to young children. CPSC and Nadel & Sons are aware of one incident where a child started to choke as a result of a detached eye from one of these bears. The plush bears involved in this recall are the 12- and 13-inch Pajama Bears. The 12-inch bear is white, with brown eyes, and a pink nose and mouth. The 12-inch bear is wearing one-piece, ruffled-collar pajamas that have gray and mint green or yellow and peach floralprints with elasticized cuffs at the wrist, ankle and neck. The bear's paws have the same print as the pajama. The 13-inch bear also is white, has a black nose and eyes, and its one-piece pajamas have a multicolored fish or dinosaur design with a red bow on a white collar. The pajama is permanently attached, and the bear's ears have the same print as the pajama. A sewn-in label on the pajamas reads in part, "Nadel & Sons Toy Corp...Made in China." Amusement parks and roadside entertainment centers, including Busch Gardens Theme Park in Tampa, Fla.; Dutch Wonderland in Lancaster, Pa.; and Play Day Amusements in Seaside Heights, N.J.; distributed the stuffed bears from October 1995 to May 1997 as prizes at game booths or in game machines. Consumers should take these stuffed bears away from young children immediately and return them to Nadel & Sons Toy Corp. for a $5 refund plus the cost of postage. The screws holding the gearcase to the crankcase on these hedge trimmers can loosen, allowing the clutch to be exposed. If the clutch is exposed, loose internal parts could fly out striking the operator and bystanders. CPSC and Husqvarna are not aware of any injuries involving these hedge trimmers. This recall is being conducted to prevent the possibility of injury. The hedge trimmers involved are about 41 inches long and have the Husqvarna name and model number, 225H, printed on the top of the orange engine casing. The company name, model number and serial number, 6500001 through 7330009, also are located on the serial number plate on the bottom of the engine near the handle. Independent outdoor power product dealers sold the hedge trimmers from February 1997 through September 1997 for about $400. Consumers should immediately stop using the trimmer and return it to their nearest Husqvarna dealer. The dealer will inspect the unit for damage and either repair or replace it free of charge. These bottles have plastic pull-up tops that can separate from the cap, presenting a potential choking hazard to young children. Chiquita has received 10 reports of children pulling the bottle tops off with their teeth. No injuries have been reported. Sports Blast™ drinks are sold in 8-ounce plastic bottles. They come in four flavors: "Rollin Blue Raspberry," "Freestyle Fruit Punch," "Inline Orange" and "Groovin Grape." The Chiquita logo and the words "Sports Blast...Naturally Flavored...Sports Drink" are printed on the bottle's label. The bottles come in a variety pack containing 24 of the 8-ounce bottles. Costco Wholesale, Sam's Club and BJ's Wholesale Club stores nationwide sold cases of Sports Blast™ bottles from March 1997 through August 1997 for about $6. Consumers can safely drink the product by completely removing and destroying the plastic cap. Consumers can return unopened bottles to the store where purchased for a refund. Announce Recall of Wooden Vehicle Toys WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Al-Dan Trading Inc. of Miami, Fla. is recalling over 5,800 wooden vehicle toys. The toys have small parts that could separate and present a choking hazard to young children. CPSC and Al-Dan are not aware of any injuries involving these toys. This recall is being conducted to prevent the possibility of injury. The wooden vehicles include trucks, trains, race cars, motorcycles, airplanes, and cranes with parts painted red, blue, green, yellow and/or white. Vehicles range in sizes as small as 5 1/2 inches long and 2 1/2 inches high and as large as 15 inches long and 7 inches high. The small vehicles (trucks, trains, race cars, motorcycles and airplanes) could have a driver in the cab, movable parts, square and hexagon blocks with painted on numerals and letters, and a clock face. The big vehicles all have movable parts like the crane's operable hook, the truck's removable animals, and the locomotive's drive wheel connectors. Some toys may have a warning sticker stating "/!\ WARNING CHOKING HAZARD - small parts not suitable for children under 3 years. NON-TOXIC AND MADE IN CHINA" and/or another sticker stating "18 DANIEL BRAND" with a UPC code number beginning with 69658. Despite the warning label, these toys present a choking hazard to young children. These toys were mainly sold in the following Florida discount stores: All for 99 Cents, Always 99 Cents, Dollar Store, Dollar Dave's, Daniel Club Wholesalers, and Swamis Trading from June 1996 through June 1997 for about $1. Consumers should take the recalled toys away from children immediately and discard or destroy them, or if possible, return them to the store where purchased for a full refund. CPSC, Multiple Firms Announce Recall of Children's Toy Jewelry WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission(CPSC), three firms are voluntarily recalling nearly 42,000 sets of children's toy jewelry. The necklaces and bracelets involved in this recall can break, releasing small beads that pose a choking and aspiration hazard to children under three years of age. CPSC and the firms involved in the recall are not aware of any injuries involving the toy jewelry. This recall is being conducted to prevent the possibility of injury. A. A. of America Inc. of East Brunswick, N.J., is recalling more than 5,300 sets of 12 bracelets packaged as party favors. The bracelets' beads are in various shapes, including disks, hearts and flowers, and are strung on elastic string. The label on the cardboard header reads in part "FUN TIME PARTY FAVORS, MADE IN CHINA." In a purple box on the left side of the cardboard header is an illustration of children sitting around a table for a party. Almar Sales Co. Inc. of New York, N.Y., is recalling more than 8,600 matching children's necklace and bracelet sets with plastic beads strung on elastic string. The beads are in various shapes, including balls, hearts, smiley faces and teddy bears. The mostly white hang tag has outlines of different-colored flowers. "Expressions BY ALMAR" is written in pink on the tag, and "FUNWEAR JEWELRY" is written in blue. A set will bear one of the following item numbers: NB110, NB112, NB124, NB129 or NB137. Mermaid International Inc. of New York, N.Y., is recalling about 28,000 matching toy necklace and bracelet sets. The three sets include pastel beads with hearts, multicolored beads and silver disks with bows, and beads with fish and cubes. All beads and shapes are strung on elastic string. The sets are held together by a cardboard wrapper that reads in part, "... MERMAID ... Kiddie Necklace Bracelet Set." Each set bears the item number NB2490, NB2491, NB2492 or NB2493. All these toys were sold in children's, dollar-type and discount stores nationwide through March 5, 1997 for about $1. Consumers should take the toy jewelry sets away from young children immediately and return them to the store where purchased for a full refund. The fans' plastic enclosures are extremely flammable and the power cords can come apart and expose live electrical parts, presenting fire and shock hazards to consumers. In addition, these fans carry counterfeit Underwriters Laboratories (UL) marks and do not comply with UL's safety requirements. CPSC is not aware of any injuries involving these fans. This recall is being issued to prevent the possibility of injury. The 3-speed oscillating fans subject to this recall come in either the floor stand or desk top models, which are 8, 9, 12, 16 or 18 inches high. The word "Envirotech" is printed on the fan along with one of the following model numbers: FB-102H, FBD-208, FBD-612, FC-071A, FC-0171A, FC-1071A, FCP-167J, FD-125J, FD-12SD, FD-164J, FD-122L, FS- 124J, FS-161J, FS-164J or FS-181J. The model number is on a silver label on the bottom of the fan's base. The fans are packaged in multi-colored cardboard boxes that read in part, "Envirotech...Made in China." The fans have been sold nationwide from January 1997 through June 1997 for between $15 and $35, depending on the model, through the following retail stores: Hechinger's, All American Home Centers, Home Base, BJ's Wholesale Club, Ace Hardware, Marc's, Menard's, Rose's Stores and Supply One. Consumers should stop using these fans immediately and return them to the store where purchased for a refund. Small pieces could break off the cars, presenting a serious aspiration hazard to young children. CPSC and United Tradeline Inc. are not aware of any injuries involving this toy. This recall is being conducted to prevent the possibility of injury. The Hot Pet Car is a red car driven by a yellow dog with orange ears wearing a purple bow and using a cellular phone. The car measures about 3.5 inches wide, 5 inches high and 8 inches long. When turned on, the wheels turn crookedly, and the toy plays music and sound effects. The sound effects also can be activated by pressing the center of the spare tire on the trunk. The car has decals bearing the words "YAHOO!" and "NO. 1." The toy, made in China, is packaged in a green box labeled in part "HOT PET CAR," and "I.C SOUND." The model number, HK-736, also is written on the sides of the box. Small retail and toy stores sold the toy cars primarily in the Los Angeles, Calif., area from April 1997 through May 1997 for about $3. Consumers should take these toys away from young children immediately, and call United Tradeline Inc. at (888) 898-9296 betwen 9 a.m and 6 p.m. PDT Monday through Friday, or write to the company at 11800 E. Francisca Full-Size Baby Cribs Recalled by J.C. Penney Announce Recall of Francisca Full-Size Baby Cribs WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), J.C. Penney of Plano, Texas, is recalling about 6,000 Francisca full-size wooden baby cribs. The cribs' side rail spindles are too short and can loosen and separate, creating a space greater than 2-3/8 inches between the slats. Babies could be strangled if their heads become entrapped between the slats. In addition, babies could be seriously injured if they fall out of the crib. CPSC is aware of four reports of missing or loose side rail spindles. No injuries have been reported. There are two crib models being recalled: model 343-3935 and model 343-4065 sold under the Francisca brand name. The cribs are available in either a hardwood or cherry finish. The manufacturer's identification, which includes the model number, is located on the bottom of the mattress support. J.C. Penney Catalogs sold the cribs from August 1996 through April 1997 for about $200. Consumers should stop using these cribs immediately. Consumers should return the crib to the nearest J.C. Penney store to receive a refund or replacement crib. If consumers cannot get to a store, they can receive the refund or replacement by calling J.C. Penney at (800) 709-5777. This number is operational 24 hours a day.

Plastic parts of the gate can break off into small pieces, creating a potential choking hazard to young children. BRK has received over 200 reports of plastic parts, including bumpers, hinges and handle pieces breaking off, sometimes into small pieces. No choking injuries have been reported. First Alert® True Fit™ Safety Gates subject to this recall are all model CSSG1 gates made from October 1996 (10-96) through May 1997 (5-97). The model number and date manufactured are printed on a white warning label that also includes the manufacturer's name and address. The gate is an expandable, pressure-mounted child security gate that also can be installed with screw-mounted hinges for a "walk-through" style gate. The gate is made of off-white plastic with a gray handle on top of the gate. The gate fits openings from 28 inches to 47 inches wide. Toys R Us, Hills and other retail stores nationwide sold the gates beginning in October 1996 for about $35. Announce Recall of "Smoke Tote" Cigarette Lighters WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Walgreen Co. of Deerfield, Ill., is recalling about 53,000 disposable cigarette lighters sold as part of the Smoke Tote set. These lighters' child-resistant mechanisms could fail to reset themselves, potentially allowing young children to ignite the lighters. Each year, an estimated 190 deaths are caused by children under 5 years old playing with lighters. CPSC estimates that child-resistant mechanisms on lighters will prevent more than 100 deaths each year. CPSC and Walgreen are not aware of any injuries involving these lighters. This recall is being conducted to prevent the possibility of injuries. The lighters are sold inside a vinyl pouch, along with a matching cigarette case, in a blister pack. The lighters are tinted either red, yellow, blue, white, black or green. The cigarette cases and lighter pouches are either beige, black, red, navy or white. The blister pack is labeled in part, "PERFECTION Smoke Tote...FREE Child Proof Lighter & Fashion Case." Walgreen drug stores nationwide sold the lighters within the Smoke Tote set from August 1996 through August 1997 for about $3. Consumers should stop using the lighters immediately and return them to the Walgreen store where it was purchased for a full refund. The bunk beds have openings on the top bunk that present a potential entrapment hazard to young children. The spaces can be large enough for a child's body to pass through, but small enough to entrap the child's head. Since November 1994, CPSC and 36 manufacturers and importers have announced recalls of more than 514,500 wooden and metal bunk beds with similar entrapment hazards. In response to this action, the manufacturers and importers participating in this recall also have identified wooden and metal bunk beds with similar entrapment hazards. Since 1990, CPSC has received reports of 35 children who died from becoming caught in the top of wooden bunk beds. Thirty-four of those children were 3 years old or younger. In addition, CPSC has received reports of five children who died from becoming caught in the top of metal bunk beds. All five children were 2 years old or younger. A recent death occurred on April 24, 1997. A 3-year-old girl died from becoming caught in a wooden bunk bed manufactured by Heartland Furniture. CPSC is not aware of any additional deaths related to the other manufacturers and importers participating in this recall. The ASTM (American Society for Testing and Materials) voluntary standard for bunk beds requires that, in addition to having guardrails on both sides of the top bunk, all spaces between the guardrail and bed frame, and in the head and foot boards on the top bunk, should be less than 3.5 inches. Bunk beds currently made or imported by the companies listed below now meet the spacing requirement of the voluntary standard. The companies also are providing permanent labels or placing permanent labels on their bunk beds to warn that children under the age of 6 should be prohibited from the upper bunk. CPSC and the manufacturers and importers continue to urge parents and caregivers not to put children under the age of 6 on the top bunk. The manufacturers and importers listed below are participating in this recall to improve the safety of these beds and to meet the requirements of the voluntary standard. Consumers should stop using the recalled beds immediately and call the company or contact the retailer for a replacement guardrail, retrofit kit or instructions to help eliminate the potential entrapment hazard. If this cannot be done (for example, firm out of business) then consumers should discard or destory the product. The heater's cover can separate from the bottom housing and expose live electrical parts, presenting shock and electrocution hazards to consumers. King of Fans has received one report of the heater separating and exposing live electrical parts. No injuries have been reported. The portable electric heater, measuring 8 inches high and 6.5 inches wide, is made of black ceramic and comes with a 6 foot cord. "MAXI-HEAT" is written on the front of the heater next to power and safety indicator lights. The heater has top-mounted controls for temperature and speed and a pull-up handle. A silver label on the bottom of the heater reads in part, "ELECTRIC AIR HEATER, MODEL NO. H-415, MADE IN TAIWAN, UL LISTED." The heaters have serial numbers between 0026000 and 0031000, which also are located on the silver label. The heater is packaged in a cardboard box labeled in part, "MAXI-HEAT SUPREME...CERAMIC HEATER...WITH AUTOMATIC THERMOSTAT CONTROL...MODEL H415." There are pictures of the heater on all sides of the box. Sam's Club stores nationwide sold the portable electric heaters from September 1993 through February 1994 for about $35. Consumers should stop using the heaters immediately and return them to King of Fans, 1951 N.W. 22nd St., Fort Lauderdale, FL 33311 to have the heater repaired or replaced or to receive a refund. The heating elements could accumulate moisture and cause electrical current to leak, presenting a potential shock hazard. CPSC and Tema are not aware of any injuries involving these ovens. This recall is being conducted to prevent the possibility of injury. Only Teba ovens with model number TFM-5 and a 1996 date code are involved in this recall. To determine if your Teba brand oven is involved, check the model number and date code on the back panel of the unit. The model number appears on a silver tag/plate immediately below the UL symbol. The date code appears on a white sticker above the serial number. The oven is a multifunction cooking appliance that bakes, broils, grills and toasts food. The unit is white and measures about 16 inches wide, 11 inches deep and 9.5 inches high. The brand name "Teba" is written on the front of the unit. The ovens are packaged in a carton labeled in part, "Teba Cooking System." There is a picture of the oven on the carton. Department and discount stores, home supply centers, catalogs and QVC (a home shopping television program) sold the ovens nationwide from April 1996 through July 1997 for about $150. The glass in the masks can break into sharp pieces and cut divers. Kmart has received seven reports of masks' glass breaking, resulting in one diver's hand being cut. The Healthways Ocean Side dive mask is made of fluorescent yellow, marine blue or raspberry plastic and clear tempered glass. The mask was part of a dive set that included a snorkel and fins. The words "Healthways Ocean Side...Tempered" are printed on the front of the mask. The dive set is packaged in a blue, yellow and white cardboard box labeled in part, "Barbados Healthways Ocean Side...4-Piece Dive Set...Made in China for Kmart." The dive set is visible through a plastic window on the front of the box.
