Announce Recall of Kid's Meal Toys WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Creative Consumer Concepts Inc., of Overland Park, Kan., and Whataburger Inc., of Corpus Christi, Texas, are voluntarily recalling about 330,000 "Whatagear" compass toys. Creative Consumer Concepts Inc. manufactured these toys, which were distributed with kids' meals at Whataburger Restaurants. The clear, plastic lens covers on these compass toys can come off and pose a choking hazard to young children.Creative Consumer Concepts, Inc. has received four reports of the lens covers of the compass toys coming off. No injuries have been reported.The "Whatagear" compass is a plastic toy with a compass in its center. The toys have varied features, such as flashlights, magnifying lenses or tools. They come in various solid colors. The back of the compass reads, "WHATAGEAR" and "WHATABURGER."Whataburger Restaurants in Alabama, Arizona, Florida, Louisiana, New Mexico, Oklahoma and Texas distributed the toys from March 2001 through April 2001.Parents should take these toys away from children immediately, and return the toy to any Whataburger Restaurant for a free replacement toy. The regulator's first stage spring can break causing restricted airflow, posing a drowning hazard to consumers.Atomic Aquatics has received 14 reports of regulator springs breaking, four of which occurred while the scuba equipment was in use. No injuries have been reported.The recalled regulators have a black body with either a gray, blue, red or yellow faceplate. Printed on the faceplate of the regulator are the words, "Atomic Aquatics"; the model name, "Titanium"; and the model number, T1, T1x, B1 or Z1.The regulators were manufactured between May 1999 and October 1999, and have serial numbers that range from A07751 to A11276. The serial numbers can be found on a label beneath the rubber mouthpiece.Specialty dive equipment stores nationwide sold the regulators from May 1999 through February 2000 for between $450 and $1,600.Consumers should stop using these regulators immediately and return them to Atomic Aquatics or an authorized Atomic Aquatics dealer for repair. Under certain conditions, these locks can open without the use of a key. This can give unauthorized access to a firearm. The NSSF and its lock supplier, Adstar Inc., of Merrick, N.Y., are unaware of any instances of unauthorized access by adults or children. The locks resemble a bicycle cable lock and have a red cable with a black padlock. Red vinyl bands around the top and bottom of the locks read, "PROJECT" and "HOMESAFE." "MADE IN CHINA" and "30mm" are imprinted on the bottom of the locks. The gun locks were distributed in clear plastic cylinders with Project HomeSafe literature. The gun locks were distributed nationwide by NSSF as part of its Project HomeSafe (now Project ChildSafe) Campaign for safe firearm storage. Law enforcement agencies distributed the gun locks free of charge from September 1999 through October 2000. Recipients of the Project HomeSafe (now Project ChildSafe) gun locks should call (203) 426-1320 or (203) 270-2360 anytime to receive a free replacement gun lock. Until the replacement gun lock is received, keep the original Project HomeSafe lock installed on the firearm. Consumers are reminded to never put a gun lock on a loaded gun, and to take extra precautions to make sure the gun is secure to keep kids safe. The replacement lock has an improved locking mechanism and a thicker braided-steel cable. With the announcement of this replacement lock program, NSSF is resuming Project HomeSafe (Now Project ChildSafe) and encourages communities to enroll in the nationwide program by having a law enforcement official contact the NSSF. NSSF urges consumers to comply with all state and local laws requiring the use of gun locking devices. Because the bikes' pedals are too low to the ground, they fail to meet federal bicycle standards. Children can lose control when a pedal strikes the ground, causing falls and injuries. Seattle Bike Supply has received one report of a 4-year-old boy whose pedal struck the ground while riding one of these bikes causing him to fall. No injuries were reported. Bicycles that are part of this recall are all model-year 2001 Torker Blaster 16-inch Boy's bicycles in red, black, blue, or chrome, and Sunflower 16-inch Girl's bicycles in yellow, pink or florescent green. The Torker brand name and the model names are on the frames. Bicycle stores sold these bicycles nationwide from September 2000 through December 2000 for about $130. Consumers should stop using the bicycles immediately, and return them to the store where purchased for a free repair. The freezers that are part of this program were made before 1970 by more than 40 manufacturers, a number of whom do not exist today. Consumers can determine if their chest freezer poses a hazard by trying to open the freezer without using the handle. If the freezer can be opened by pulling up on sides of the lid, it is not a hazard. If the lid only opens by using the handle, it needs to be properly disposed of or disabled. Up to 9 million chest freezers were manufactured between 1945 and 1970 before voluntary safety standards went into effect, allowing freezers to be opened from the inside. CPSC Warns About Defective Furnaces in California WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is warning consumers in California that certain gas-fired horizontal forced-air furnaces manufactured by Consolidated Industries (formerly Premier Furnace Company) present a substantial risk of fire. There have been about 30 reports of fires and damage to homes associated with these furnaces, as well as failures of burners and heat exchangers that can lead to fires. The furnaces were installed exclusively in California.Consolidated manufactured approximately 190,000 of these furnaces from 1983 through 1994 under many different brand names. Most of the furnaces were manufactured under the Premier/Consolidated labels. All of the furnaces can be identified by the fact that they are equipped with steel control rods installed above the burners. These steel rods were required to satisfy California air quality regulations for nitrous oxide emissions.Many of these furnaces are still in use. Normally, the furnaces are installed in attics, although some may be installed in crawl spaces. The Commission is warning consumers to have their gas-fired furnaces inspected by a licensed heating contractor to determine whether the furnaces are subject to this safety alert. The contractor also should determine whether the burners and/or heat exchangers of units are damaged, or whether wood under or near the furnaces shows signs of damage, such as charring or blackening. If this is the case, the furnace should be replaced immediately or repaired.Because Consolidated is currently in bankruptcy liquidation, the availability of repair parts is at this time unresolved. However, there is on-going private litigation which could enable consumers to recover at least some of their out-of-pocket expenses for replacement or repair of the furnaces. Suggestions for improving the fire safety of all horizontal forced-air furnaces in attics are: - Protect the wood deck or rafters on which the furnace is mounted by covering it with a non-combustible material such as cement board that extends a minimum of 12 inches past the side of the furnace. - Provide an air space beneath the furnace. A licensed contractor can perform this work and can offer the homeowner several ways to accomplish this. - Make sure the furnace is installed correctly and serviced at least annually. The inspection should include a safety inspection of the burner assembly and heat exchanger. - Install an ionization-type smoke detector inside the attic to provide an early warning of smoke or fire. CPSC, Apollo Presentation Products Announce Recall of Light Boxes WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Apollo Presentation Products, of Ronkonkoma, N.Y., is voluntarily recalling about 30,000 portable light boxes, used to view film, slides and transparencies. The light boxes have loose wires and lack adequate grounding, presenting fire, electrocution and shock hazards to consumers. CPSC has received two reports of loose wires, one in which the light box caught fire, resulting in $5,000 property damage. No injuries have been reported. The recalled Apollo light boxes are models LB100, LB101, LB102 and LB110. The model number is located on a sticker affixed to the unit's side. The sticker also reads in part, "Portable Light Box...CSA." The light box consists of a white metal box, containing a fluorescent light, and a Plexiglas viewing plate that slides into two black metal guides on top of the metal box. The light box has a folding handle and tilt stand, and it can be wall mounted. The light boxes were sold in a box labeled in part, "APOLLO PRESENTATION PRODUCTS, GLOWPRO PORTABLE LIGHT BOX, Made in China." A photo of the product and the model number are on the box. Office supply stores nationwide sold the light boxes from June 1997 through April 2000 for $66 to $115. Consumers should stop using the recalled light boxes immediately, and call ATVS, Inc. (which is now handling this recall) for instructions on returning the projectors to the company for a refund. Inc. Announce Recall of Cigarette Lighters WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Shine International Trading Co. Inc., of Flushing, N.Y., is recalling about 2.4 million cigarette lighters. The lighters child-resistant mechanisms fail to meet the requirements of the Consumer Product Safety Act. Young children could ignite the lighters, presenting fire and burn hazards. Additionally, the lighters can leak fuel and remain on after consumers have used them, which also presents fire and burn hazards. CPSC and Shine International Trading Co. Inc. have received three incident reports with these lighters. One incident involved two lighters that continued to burn after the consumer put them down, one involved two lighters that spontaneously blew apart while unattended, and one involved a lighter that burst into flames resulting in minor property damage. No injuries have been reported. The lighters being recalled are non-refillable, oval-tube-shaped, with a transparent solid-colored (red, green, yellow, orange or purple) plastic body and a metal top. The lighters measure about 3.25 inches high and 1 inch wide. "SHINE" is imprinted into the metal top of the lighter. There is a warning label on the lighter. "SHINE" and "MADE IN CHINA" are written on the label. The lighters operate with a roll and press type of ignition mechanism to produce a flame. Small stores and souvenir shops nationwide sold these lighters from August 1998 through July 1999 for about $1. Consumers should stop using these lighters immediately and return them to the store where purchased for a full refund. These doll sets come with a small ball, which poses a serious choking hazard to children under 3 years old. CPSC and Toys "R" Us are not aware of any injuries involving these doll sets. This recall is being conducted to prevent the possibility of injuries. The Bathtub Baby doll set includes a blue, plastic bathtub; a 5.5 inch soft vinyl doll wearing a blue diaper; a pink, fish-shaped strainer; and a pink ball about 1.25 inches in diameter. The set is packaged in a clear plastic bag with a hangtag. Writing on the hangtag includes, "BATHTUB BABY," WHAT A DOLL!" "FISHEL" and "MADE IN CHINA." Toys "R" Us stores nationwide sold these doll set from October 1998 through July 1999 for about $2. Consumers are urged to take the doll sets away from young children immediately and return them to the stores where purchased for a full refund. Great American Toy Co. Inc., of Port Washington, N.Y., is recalling about 18,700 stuffed crabs. This recall involves only stuffed crabs that have antennaes with wires and that have the letters "BSW" on the Great American Toy Co. label attached to the toy. The stuffed crabs are either blue and green or red and orange. They come in a variety of sizes. Traveling fairs and carnivals gave the crab toys away as prizes from March 1998 to July 1998. The toy has a retail value of about $1 to $27, depending on the size of the stuffed crab.

Announce Recall to Replace Lids on Pasta Machines WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Creative Technologies Corp. of Brooklyn, N.Y., is voluntarily recalling about 50,000 pasta machines to replace some of their lids. When the safety cutoff switch on the lid does not activate, consumers' fingers can be severely injured if their hand is placed into the mixing bowl. CPSC and Creative Technologies are aware of 11 incidents of the machine unexpectedly turning on, resulting in nine injuries including cut, crushed and severed fingers. The lid replacement program involves the Takka Pasta and Dough machine (model X1000), and the Pasta Express machines (models X2000, X3000 and X4000). Only pasta machines that have serial numbers up to 78209 are subject to this replacement program. The model and serial numbers are located on a label on the bottom of the machine. "Pasta Express TM by CTC" or "Takka Pasta and Dough by CTC" is on the front of the machine. The pasta machines are beige or white. Some machines have chrome-plated front panels. Through this voluntary recall, Creative Technologies will provide consumers with a replacement lid. The machines are sold in cardboard boxes identifying the model of the machine. A photograph of the machine is on the front of the box. Department stores, mail order catalogs and warehouse clubs nationwide sold the pasta machines from 1989 through 1992 for between $90 and $200. Consumers should stop using the pasta machines immediately. The company will help consumers determine whether their pasta machine is subject to this replacement program and, if necessary, will provide a free replacement lid.

Announce Recall of Toy Jewelry Sets WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Creative Products Inc. of Skokie, Ill., is voluntarily recalling about 28,000 toy jewelry sets. The necklaces and bracelets of two different sets can break, releasing small beads. The earring clasps of a third set can come off. Young children can choke on the earring clasps and inhale the beads into their lungs which could lead to death. CPSC and Creative Products Inc. are not aware of any injuries involving these toy jewelry sets. This recall is being conducted to prevent the possibility of injury. There are three different jewelry assortments involved in this recall. One set contains a 16-inch brown bead necklace and 4 brown hair combs. The beads are grouped in threes and are separated by black and white discs. Another set contains a brown 8-inch bead bracelet that matches a brown 18-inch bead necklace. The beads are separated by bamboo- shaped pieces. At the center of each necklace and bracelet are several black and brown triangle-shaped ornaments. The third set contains earrings along with a matching multi- colored 18-inch bead necklace and 6-inch bead bracelet. An orange sunburst ornament with a face embossed in the center is attached to the yellow earring clasps, three sunbursts are attached to the necklace, and one sunburst is attached to the bracelet. Dollar-type and discount stores in the East, Midwest, and South sold the jewelry sets from August 1995 through November 1995 for about $1. Consumers should take the jewelry sets away from young children immediately and return them to the store where purchased for a full refund. Waters has received three reports of overheating occurring in K-9 Electric Fence Controllers used in an unprotected outdoor setting where they are subjected to high humidity or water. Two controller cases showed signs of melting and one controller installed under a porch caused charring of the wooden flooring. The K-9 Electric Fence Controllers with serial numbers starting with MT08096 and MT09096 have a yellow, black, and red label that reads, "American FarmWorks K-9 ELECTRIC FENCE CONTROLLER." The K-9 Electric Fence Controllers sold individually came packaged in a plastic shell with a yellow backer card. Waters also sold the units as part of a pet and garden fence kit, packaged in a yellow, black, and red box with a picture of the K-9 Electric Fence Controller printed on the box. Hardware and farm supply stores sold the K-9 Electric Fence Controllers nationwide beginning in April 1996 for about $28. Consumers should stop using the K-9 Electric Fence Controllers immediately and call Waters at (800) 962-2880 for information on returning the merchandise for replacement with a unit not adversely affected by climate.

The knit dress with leggings (style 16500), infant dress (style 18500), and toddler dress (style 19500) each have embroidered appliques of Minnie Mouse on the front. The short set (style 17214H) features a beach scene with Minnie Mouse on the front. All the outfits have long sailor ties at the neckline, which are held together with small, beige fabric loops that read "Minnie Mouse" in red lettering. The dresses and dress with leggings each have a sewn-in label which reads in part, "MICKEY'S STUFF for kids ... Catton Brothers...70% Cotton/30% Polyester...Made in Thailand...." The short set has a label that reads in part, "Catton Brothers...60% Cotton/40% Polyester...Made in Hong Kong...." Independent retailers sold the garments nationwide from November 1995 through April 1996 for between $10 and $14. The books were sold under two titles, "KITTY AND FRIENDS" and "PUPPY AND FRIENDS." These books were sold under the Playskool name, which is licensed by Dutton. Straight pins used during the manufacturing process were mistakenly left in some of the books, creating a hazard to young children. Neither CPSC nor Penguin USA is aware of any injuries involving these books. This recall is being conducted to prevent the possibility of injury. The "Kitty and Friends Color and Contrast Crib Book" and "Puppy and Friends Color and Contrast Crib Book" each consist of five attached plush fabric panels. Each panel has a black and white picture on one side and a color picture on the other. The panels can be folded into one another, creating a "book." The first and last panel have Velcro tabs attached so that the unfolded panels can be attached to crib slats and used as a crib decoration. The books measure 5.25 inches by 5.25 inches when folded and 5.25 inches by 26 inches when extended. The "Kitty and Friends" book includes colored panels with pictures of a house, a bear, a duck, a flower, and a locomotive. The book's black and white panels include pictures of a rocking horse, four boxes with a quarter moon, heart, triangle, and star, a boy's face, a target, and a cat. The "Puppy and Friends" book includes colored panels with pictures of a fish, a ball, a butterfly, a car, and an apple. The book's black and white panels include pictures of a sailboat, four boxes with two circular mazes and two square mazes, a girl's face, a design with a triangle, a heart, a circle and a star, and a dog. A label on the fabric flaps reads in part, "Dutton Children's Books, a division of Penguin Books USA Inc. ... Playskool ... Hasbro, Inc." and "ISBN 0-525-45468-3" or "ISBN 0-525-45469-1." (Playskool licenses its name to Penguin Books USA Inc.) The books were packaged in a clear plastic bag with a cardboard insert and a cardboard header. January 2, 1996 Release # 96-058 WASHINGTON, D.C. - CPSC, Atlas Electric Corporation of Burlington, N.C., is voluntarily recalling approximately 15,600 portable halogen indoor/outdoor work lights, models CLP150, PQ150WS, PQ500WS, ST500 and TST500. Water may leak into the lights, component temperatures may exceed set limits, or the junction box may break away from the main bulb housing to expose live wires. Models ST500 and TST 500 may also tip over easily. As a result these work lights may pose a serious shock, burn or fire hazard.The models subject to this recall were sold in white boxes and were printed in part with the Atlas address, Atlas logo, and model number, and "Made in China." Model ST500, sold since February 1995, is a single work light mounted on a tripod. Model TST500, sold since June 1995, consists of two work lights mounted on a tripod. Model CLP150, sold since July 1995, is a clip-on work light. Models PQ150WS and PQ500WS, sold since February 1995, consists of a light mounted on a small frame with a handle on top. All of the lights involved in this recall are black and yellow. December 19, 1995 Release # 96-049 WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Tange, Inc, of Osaka, Japan is voluntarily recalling 808 EXP 120 front suspension forks. The fork crown may crack during use, which could cause the fork to separate from the bicycle. As a result, riders could lose control of the bicycle and fall, causing serious injury or death. To date, Tange has not received any reports of injuries associated with cracked forks.The silver-gray forks are identifiable by decals on the front sides of the fork legs which read "Tange EXP 120 Suspension." Recreational Equipment Inc. (REI) stores and mail-order catalogues sold the suspension forks nationwide from October 1994 through November 1995 as original equipment on 1995 REI Novara Ponderosa mountain bikes.Consumers who own 1995 Novara Ponderosa mountain bikes with Tange EXP 120 forks should stop using the bicycles immediately and call REI customer service at (800) 828- 5533.
