Recalls Plastic Racing Cars, Jetplanes and Speedcycles WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Acme Premium Supply-Corp. of St. Louis, Missouri today announced the voluntary recall of Plastic Racing Cars item C805, Plastic Jetplanes item C822 and Plastic Speedcycle item C900 which were distributed as prizes at carnivals and fairs. Each item is contained in a package indicating that it is not recommended for children under three years of age. On all three toys, the small wheels may present a potential choking hazard for young children. The U.S. Customs Service identified the potentially hazardous toys at the Port of Seattle during "Operation Toyland," a joint surveillance program with the Consumer Product Safety Commission. The carnival toys are made of light, bendable plastic in The "Supersonic Jetplane" and the "Speedcycle" are 4 3/4" long, and the "Racinq Car" is 3" long. The small, black wheels an the toys, which measure 1/2" to 3/4", are single or double wheels on an axle, which snap into slots on the underside of the toys. Although the CPSC and Acme Premium Supply Corp. have not received reports of any injuries involving these plastic toys, consumers should take the toys away from young children and discard them. Approximately 22,000 of these toys were imported from Hong Kong and distributed to the amusement industry since January 1, 1983. Hedstrom To Offer Glide Guard Kit For Gym Sets WASHINGTON, DC -In cooperation with the U.S. Consumer Product Safety Commission, Hedstrom Corporation, Bedford, Pennsylvania, voluntarily added a Glide Guard to selected gym set models that have a glide ride assembly. Between 1982 and July 31, 1988, Hedstrom sold approximately 370,00 gyms with glide ride assemblies which have 2-3/8 inch top bars. Hedstrom is aware of 12 incidents where the user has stood on the slide or cross bar, reached over to propel or stop the motion of the ride and has entrapped or pinched a portion of a finger in the bracket mechanism, resulting in severed fingertips or severe lacerations. The affected units have a model identification label located on the top bar. These model numbers are: Hedstrom Brand, model numbers 4-277, 4-377, 4-677, 4-777, 4-778, and 4-877; Sears brand, model numbers 72226, 72026 and 72066. These models incorporate a glide ride assembly with a 2-3/8 inch top bar using a V-bolt bracket attachment. The firm is out of business and a remedy is no longer available. Consumers should discard or destroy the product to prevent injuries.

HAGF Circuit Breakers (Challenger) – Safety Replacement (1988)

Release # 88-095 WASHINGTON, DC- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Challenger Electrical Equipment Corp. of Malvern, Pennsylvania, voluntarily offered free replacement of its 15 and 20 ampere type HAGF single pole ground-fault circuit-interrupter (GFCI) circuit breakers.These type HAGF-15 and type HAGF-20 circuit breakers were manufactured between February 22, 1988 and April 29, 1988, and most were sold nationwide to electrical product distributors during this period. Some may have been sold to consumers by retail hardware or lumber outlets. Approximately 9000 of these devices have not been located, some of which are believed to have been installed in residences. Type HAGF ground fault circuit breakers are devices, which provide protection to consumers against severe electric shock and electrocution as well as providing the normal circuit breaker functions of interrupting short circuits and electrical overloads.Although no electric shock incidents have been reported by Challenger, the firm recalled this product because is has determined, by quality control testing, that a mechanical part may become detached and prevent the ground fault feature of the circuit breaker from functioning. (The normal circuit breaker functions are not affected by this problem.)They protect residential 15 ampere and 20 ampere circuits including receptacle outlets in bathrooms, kitchens, basements, garages and outdoors as required by the National Electrical Code. The code also requires GFCI protection for receptacles in hotel and motel bathrooms. The HAGF circuit breakers, which may require replacement, are located in the electrical service panel. If the name "Challenger" is embossed on the front of the panel (on the door handle) and/or on the label on the back of the panel/door, that panel may contain circuit breakers requiring replacement. Affected circuit breakers have a yellow button and the word "test" in raised letters on one side; on the other side, the number 15 or 20 is printed on the handle in white letters between the raised words "ON" and "OFF." If the number on the handle is black or if there is a label it with white lettering next to the word "test , then the HAGF circuit breaker will not need to be replaced. Consumers should note that the HAGF circuit breakers which require replacement will only be found in dwellings which have been constructed since February 1988 or in which electrical work has been done since February 1988. Consumers who believe that they have installed HAGF circuit breakers requiring replacement should contact a licensed electrician to perform this work. Power to the panel should be shut off before inspecting and removing breakers.) The company is no longer able to be contacted at the phone numbers provided in the original press release.The Consumer Product Safety Commission believes that GFCI's provide a significant level of electrical safety. The Commission recommends that all GFCI's (whether circuit breaker or receptacle type) be tested frequently, at least monthly, using the test button on each unit

An incorrect number was cited for "Jill Doll" #3062. The correct model numbers for the dolls are "Cupid Dolls" #3004, #3005, #3035, #3036, #3057, #3058 and #3220; "Jill Doll" #3027; "Sandi Doll" #3028; "Paula Doll" #3029; "Charlotte Doll" #3062; "Beth Doll" #3068; "Baby Doll" #3087; and "Strawberry Girl" #3101. WASHINGTON, DC - Four rattles and 14 small dolls are being voluntarily recalled by Fibre Craft Materials Corp. of Niles, Illinois because of possible choking hazards to infants and young children. Sold in craft stores, the rattles and dolls were marketed as decorations. The four recalled rattles include: "Baby Rattle #4684," a noise-making blue ball atop a pink beaded handle; "Plastic Spoon Decoration #4685," a pink, blue, or yellow spoon with a painted-on face; "Plastic Ball Decoration #4686," a blue and pink barbell; and "Clothespin Cake Decoration #4687," a noise-making blue clothespin with a painted-on face. The rattles are small enough to fit into an infant's mouth, and long enough to reach the back of the throat and cause choking. The dolls are being recalled because their arms may be pulled off and possibly ingested by a young child. They include: "Cupid Dolls," with numbers 3004, 3005, 3035, 3036, 3057, 3058 and 3220; "Jill Doll" #3027; "Sandi Doll" #3028; "Paula Doll" #3029; "Charlotte Doll" #3062; "Beth Doll" #3068; "Baby Doll" #3087; and "Strawberry Girl" #3101. Importer WASHINGTON, DC - Everlast Industrial Company of New York, N. Y. is recalling its imported Baby Rattle with Whistle because it presents a choking hazard to infants. According to the U.S. Consumer Product Safety Commission, the rattle is being recalled because its handle is small enough to enter a child's mouth, and is long enough to reach the back of the throat. In addition, a metal bell can become separated from the top of the rattle and be ingested by a young child. The rattle is multi-colored plastic, with a handle fashioned of beads and a circle of four bell-shaped noisemakers attached to the handle. The end of the handle is a whistle. The packaging is marked "Made in China" and "Baby Rattle with Whistle." This hazardous rattle was identified at the port of New York during the CPSC-U.S. Customs Service cooperative program to check the safety of imported toys. Parents should take the rattle away from infants immediately, then return it to the retail store where purchased or send it to Everlast Industrial Company (U.S.A.) "Pull Along Bear Brother" consists of two connected seven-inch tall bears of orange and yellow plastic with blue feet that "wobble" when pulled; the noses of both bears can be removed and constitute a small parts hazard to a young children. "Pull Along Happy Bear" is a seven-inch tall yellow plastic bear riding between two plastic half-balls with small colored balls inside. The toy is being recalled because the small plastic balls are an ingestion hazard to young children. "Made in Taiwan" is imprinted on the back of both toys. 6,000 have been sold nationally in September and October 1987. Consumers should remove the toys from children and return them to the store for a refund of the full purchase price.

July 14, 1988 Release # 88-051 Importer Recalls Water Toys; Toy Snake WASHINGTON, DC - Beachcombers International Inc. of Fort Myers, Florida is voluntarily recalling water toys and a squeaking toy snake because they may pose choking and lead poisoning hazards to young children. The imported toys recalled by Beachcombers International are: - "Big Mouth Fish, Hippo, and Frog," plastic wind-up water toys labeled "MASUDAYA 1976 Made in Hong Kong;" some 8,000 toys were sold nationally for about $2 each. - "Devil Fish," an eight-inch wind-up toy in the shape of a fish which was imported from Hong Kong and sold for $2 in New Jersey and Virginia. - "Lively Squeaking Snake," a plastic cobra snake some 21 inches long from China which sold nationally for $1. Consumers should immediately take the toys away from children and return them to the retail store for a refund of the purchase price. The U.S. Consumer Product Safety Commission said both water toys have small parts, which may present a choking hazard to young children; the toy snake contains an excessive amount of lead in the surface coating. The violations were discovered by U.S. Customs' agents and CPSC inspectors during routine import surveillance at the Port of Miami. Propane Grills Modification Kits by Sunbeam Leisure NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE June 2, 1988 Release # 88-034 Sunbeam Leisure Products Providing Modification Kits For Windows on Propane Grills WASHINGTON -- Following reports of spontaneous breakage of glass windows of 1988 Sunbeam Leisure outdoor propane gas grills, the Sunbeam Leisure Products Company of Neosho, Missouri is initiating a modification program to remount the glass windows to prevent breakage. In a voluntary modification program undertaken in cooperation with the U.S. Consumer Product Safety Commission, Sunbeam Leisure said that, because of the way the window was mounted, the glass may spontaneously break from the heat of the grill. Incidents of consumers reporting minor injuries from broken glass are being investigated. A limited number of the suspect grills were sold in the latter part of last year, but most have been sold in 1968. An estimated 320,000 grills are currently in consumer use including Yardmaster models sold by Channel Home Centers and Sears Kenmore models 10663, 10684, 10783 and 10883. Consumers who bought 1988 models of Sunbeam Leisure, Yardmaster and the above Kenmore propane gas grills with a glass window will be supplied with a free modification kit and instructions on remounting the window. The installation can be easily performed by the consumer without special tools. Consumers should take the items away from children and return them to the retailer where purchased for a full refund. The "Sweet Home Shoe House Playset" is a plastic house in the shape of a workboot, measuring 11 by 7 by 6 inches, containing is pieces of dolls and doll furniture. The playsets are labeled "1984 JUMBOS INTERNATIONAL (HONG KONG) CO. NO. 698", and 6,500 were sold nationally for about $5.00. "Ice Cream Doll" is a soft doll about 12 inches tall, with a plastic ice cream cone suspended from a string around the neck of the doll, with a sewn-in label marked "ICE CREAM DOLL, MADE IN HONG KONG." $3.00. CPSC and Wisconsin Toy said they know of no injuries involved with the toys. Umbrella Stroller Recalled by Service Merchandise NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE May 17, 1988 Release # 88-028 Umbrella Stroller Recalled WASHINGTON D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Services Merchandise Company of Nashville, Tennessee, today announced a voluntary recall of 15,000 Umbrella Strollers, Model 1601 PRT, which were purchased in Service Merchandise stores from August, 1986 through March, 1987. The hazard involves sixteen tube end plugs on each stroller that are considered to be "small parts," which may come out and present a choking hazard to children. In addition, when the plugs are removed, the exposed metal tube ends could have "sharp edges." Neither Service Merchandise nor the Commission is aware of any injuries to small children as a result of these small parts and sharp edges. Consumers having Model 1601 PRT Umbrella Strollers should stop using them, and immediately contact Service Merchandise for a modification kit. Service Merchandise is offering a modification kit at no charge to its customers. The Model 1601 PRT Strollers involved are navy blue with red trim. WASHINGTON, DC -- The U.S. Consumer Product Safety commission announces that it has authorized its staff to issue an administrative Complaint against Miracle Recreation Equipment Co., of Grinnell, Iowa, seeking repair, replacement, or refund of the purchase price of approximately 12,000 Flying Animal Swings. The Complaint alleges that Miracle's "Flying Animal Swing", a two, three, four, or six seat swing, because of the weight, size, and composition of the individual animal swing figures and because of a metal support tube which projects to the rear of each swing, contains a defect which presents a substantial risk of injury to children. The Complaint alleges that there have been seven (7) cases of serious head injuries and one (1) fatality when children were struck by the swing. Until this matter is resolved, the Commission staff urges the public not to use these swings which are found primarily on public playgrounds around the country. The Flying Animal Swings may be either a pony, seal, lion, or a squirrel. The Commission's action was necessitated by Miracle's unwillingness voluntarily to remedy this problem. Miracle is currently conducting on-going recalls/repair programs of its "Flying Wheels", "Flying Gym", "Bounce-Around Whirl", and "Bukaroo Whirl". Campari Butane Cigarette Lighters Warning NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE October 20, 1987 Release # 87-062 CPSC Warns of Potentially Hazardous Disposable Butane Cigarette Lighters WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Campari USA, Inc., New York, New York, today announced a program to alert consumers to the potential hazards of certain disposable butane cigarette lighters. The lighters in question are labeled "Campari, the smart mixable" on one side of the lighter body and on the other side will say, "Campari, Soda, the classic" or "Campari Grapefruit Juice, the exotic." These lighters were imported beginning in July of 1983 by Campari USA to be used as promotional items and were given to consumers free through liquor sales outlets. Certain of these lighters can shoot up excessively high flames or spew butane without lighting or before lighting. There have been no injuries reported involving these lighters. Since a consumer cannot readily differentiate between lighters which may exhibit the problem and those which do not, the firm is urging consumers who may still have these lighters in their possession to carefully dispose of them. In disposing of the lighters, consumers are warned to do so in such a way that children will not have access to the discarded lighters. The problem with these lighters was first brought to the Commission's attention by the Dade County Florida Fire Department when they received a box of these lighters from an anonymous source. The fire department forwarded the lighters to the Commission. No fires have been reported related to these lighters. Paula Banned NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE September 3, 1987 Release # 87-046 Stuffed Teddy Bear With Small Parts and Sharp Points The Consumer Product Safety Commission today announced that a brown 7 inch tall stuffed plush sitting teddy bear distributed by C.M. Paula Company, Cincinnati, Ohio is a banned toy. The teddy bear may present a choking and laceration or puncture hazard because of small parts and sharp points. Red flowers of plastic and fabric attached to wire stems, and a red ribbon are attached to the torso of the bear. There may also be a message such as "I love you" on a paper tag also attached to the torso. The bears can be further identified by a label on the bottom of the bear which includes the statement "Made in Korea for the C.M. Paula Company, Cincinnati, Ohio." When the Consumer Product Safety Commission subjected the bears to use and abuse testing the flowers, which are small parts, became detached and the sharp ends of the wire stems were exposed. Consumers who bought these teddy bears for children should immediately remove and discard the flowers and wires to eliminate the choking, puncture and laceration hazards. Retailers should do the same to any of these C.M. Paula teddy bears they have in stock. Annually, approximately 15,000 to 20,000 of these teddy bears are distributed nationwide by C.M. Paula. The bears sell at retail for approximately $8.00. The Consumer Product Safety Commission is not aware of any injuries involving these bears. Ice Cream Scoops Recalled by Associated Merchandising NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE July 09, 1987 Release # 87-039 ICE CREAM SCOOPS RECALLED WASHINGTON, DC --In cooperation with the U.S. Consumer Product Safety Commission (CPSC), the Associated Merchandising Corporation (AMC), New York, New York, today announced a voluntary recall of certain ice cream scoops imported by the company. These scoops have a ceramic ice cream cone-shaped handle which may crack under pressure, resulting in potentially severe cuts from the broken pieces of the handle. Nine complaints have been reported to AMC with two incidents involving serious cuts or injury. Approximately, 6,000 units of the affected ice cream scoops were distributed to retail stores nationwide from November, 1986 to March, 1987. The ice cream scoop handle is ceramic, shaped like an ice cream cone, and comes in four pastel colors - yellow, pink, green, and blue. The box is pink with a clear plastic lid. The side of the box is labeled with the style number L-0971 and "Made in Japan". A list of AMC retail stores that sold the ice cream scoop is attached. Consumers should remove these products from use immediately and return them by mail or in person to the retailer where purchased for a refund. The lawn mowers fail to meet the 3-second blade stopping time requirement and do not have a protective foot shield to prevent contact with the rotating mower blade. The recall covers only those lawn mowers which were purchased by consumers for their personal use and does not include lawn mowers which are intended for industrial/commercial/professional uses. The lawn mowers, which are called "high wheelers" because the rear wheels on the mowers are larger than the front wheels, are available in two models: model B-21, with a 2l-inch cutting blade and a 4 horsepower motor; and model B-24, with a 24-inch cutting blade and a 5 horsepower motor, available in both push type and self-propelled. The suggested retail price ranges from $319.00 to $599.00 each. Approximately 1200 units have been sold to retailers located in Mississippi and Louisiana. While neither the Consumer Product Safety Commission nor Brister's Thunder Karts knows of any injuries involving these lawn mowers, they present a risk of serious injuries to hands and feet as a result of contact with the rotating lawn mower blade. Consumers who purchased the mowers from 1982 through 1986 are urged to bring the mowers back to the retailer where purchased so that they may be modified to comply with the safety standard at no cost. Approximately 103,000 walkers have been imported nationwide since 1983. McCrory is recalling these walkers as a result of 30 incidents of crotch strap slippage or disengagement and tearing or stretching of the seat material, which could allow an infant to fall through the seat to -1 the floor and become caught or entangled. One incident of crotch strap slippage is known to have resulted in a broken collar bone. The walkers involved are round with a silver metal frame and yellow tray with three play dials, with multi-colored animal print vinyl padded seat and six yellow plastic wheels. The affected walkers contain a silver sticker under the tray listing Reference Number PHT417 and York Distribution Co. The models involved have a white vinyl binding tape on the edges of the crotch strap. The hazard involves slippage of the strap through the buckle which secures the front of the walker's seat to its deck. This slippage could result in infant entrapment or injury. The Commission and Service Merchandise are aware of at least 5 incidents involving seat strap slippage, one of which was associated with an infant death. Service Merchandise is offering a free replacement seat at no charge to the consumer and urges all owners of the Model 106 FLP walker to participate in the recall. Small Wonders Crib and Playpen Warning NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE February 11, 1987 Release # 87-008 Baby Furniture Outlet Baby Cribs and Playpens Present Hazard to Infants The Consumer Product Safety Commission today announced that Small Wonders full-size baby cribs imported from 1981 to 1986 by Baby Furniture Outlet Inc., Marathon, Florida are banned. The cribs, distributed nationwide, failed to meet one or more CPSC safety requirements with respect to dimensions, spacing of crib components, hardware, construction, and assembly instructions or labeling. Not all cribs present the same hazard but hazards, which may be present, include strangulation, head entrapment, pinching, laceration and falling. CPSC is aware of 19 incidents with the cribs, several involving bruises and 2 incidents involving the playpens. In addition, two playpens, "Small Wonders Play Yards Models 286 and 391," also sold nationwide during this same period, may have floors that collapse, or sharp staples that come loose or protrude. The playpen may trap children's arms or legs if the floor collapses and staples may cause laceration or choking hazards. Because of the hazard presented by these cribs and playpens, the Commission urges consumers to immediately discontinue their use. In those cases where the consumer chooses to dispose of the hazardous models of cribs and playpens, they should be disposed of in a manner which assures that they cannot be used in the future. Since Baby Furniture Outlet is currently involved in bankruptcy proceedings and liquidation, the company maintains that it is unable to bear the costs of recalling any cribs or playpens. Any consumers who have purchased a Small Wonders crib or playpen from a Baby Furniture Outlet retail store may call CPSC toll-free hotline number at 800-638-CPSC for advice as to what hazard may be present for a particular crib or playpen model and what corrections, if any, can be made. Children can become entrapped in the spaces between the metal ladder rungs and the wooden platform deck. The Commission and he firm are aware of at least 4 instances of head entrapment in these play structures. No injuries have been reported. All the reported incidents occurred during supervised play. TimberForm 4000 Series Play Platforms are installed in various configurations depending upon options ordered by each customer. It is, generally, heavy-duty wooden playground equipment, typically found on public playgrounds and at church, school, or childcare facilities. Using its sales records, Columbia Cascade will contact directly, by first class mail and through sales representatives, each known owner of the equipment involved to advise them of the hazard and arrange for repairs to prevent entrapment. Play Brooms Recalled by Come Play Products Company NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR RELEASE August 28, 1986 Release # 86-54 Play Brooms Recalled Because Of Lead Paint Hazard Washington, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Come Play Products Company of Worcester, Massachusetts today announced a voluntary recall of children's brooms because paint on the brooms' handles contains an excess of lead. Since 1960, Come Play has imported 100,000 of these brooms from Hungary and sold them nationwide for about $2.50 retail value through variety stores nationally. The brooms are approximately 28 inches long, with natural straw bristles. The handles are painted red. On the handle the word "HUNGARY" is stamped in gold-colored letters. This recall does not apply to Brooms with plastic handles and plastic bristles. The Consumer Product Safety Commission knows of one incident involving a child who had ingested paint from the broom's handle but was unable conclusively to attribute the child's elevated blood lead level to the toy. CPSC regulations limit paint on consumer products to .06%. The CPSC found that the paint on the brooms' handles contained an excess of lead many times the legal amount. The brooms are therefore banned. If a child gnaws or sucks on an article coated with lead-containing paint and ingests the paint, the lead will enter the child's bloodstream. The Commission and Come Play Products Company urge teachers and parents to remove these brooms from use immediately and either discard them or return them for a refund to the retailer where purchased. Retailers should remove the brooms from their shelves and return them to Come Play Products Company, 44 Suffolk Street, Worcester, Massachusettes, 01604.
