These Schwinn-brand Toddler Bicycle Helmets have "SCHWINN" written on the back of the helmets. A white label inside the helmets reads, "Lot# 791913." This number is followed by a date from "12-01-03" through "5-15-04." This label also contains the model number SK103, SK107 or SK108.

Wheelbarrows (Ames) – Wheel Assembly Defect (2004)

The wheel assemblies on these wheelbarrows have a black plastic rim and have an approximately 14-inch diameter wheel. They have red, green, or orange tubs or trays made of steel or plastic and were sold under the brand names "Easy Roller," "Homeowner," "Mustang," or "Thoroughbred." The brand name was printed on the label attached to the tray at the time of purchase. Wheelbarrows with metal wheel assemblies are not part of this recall. No "True Temper" or "Jackson" wheelbarrow is part of this recall. In addition to the free replacement wheel assembly and free gardening tool incentive, the company is conducting the recall campaign with new measures to reach home gardeners who are the primary users of these recalled wheelbarrows. These new measures include placing ads in gardening publications, providing recall information to editors and webmasters of specialty gardening magazines and websites, and providing new point-of-sale posters to retailers.

The units are marked "Cord Reel Portable Power" and sold as "SKU 42053." The label on the rear of the unit contains the following information: "Cord Reel, 10 Amps, 125 Volts, 1250 Watts, Caution, Disconnect all plugs inserted into the outlets on this face before winding cord reel in either direction, Made in Taiwan, R.O.C."

December 5, 1995 Release # 96-033 WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), LaNacional, cxa, Hato Rey, P.R. and Dominican Republic, is announcing the recall of three models of cribs distributed in Puerto Rico. These cribs do not comply with safety standards and pose strangulation and entrapment hazards to infants.The distance between the cribs' slats is greater than 2 3/8 inches and some of the cribs' corner posts are one or more inches high. A child's head can get caught in the space between slats, which present an entrapment hazard, or a child could strangle when clothes catch on the corner posts. In addition the cribs lack required identifying marks and instructions and fail to meet other requirements.These cribs were sold assembled without a box. Assembled cribs can be identified by a lack of printed instructions and markings, except for the words "MADE IN THE DOMINICAN REPUBLIC" printed under the mattress support. The three crib styles involved in the recall are the CAMA CUNA R.N.C., the CAMA CUNA No. 2, and the Crib with Drawers. The Crib with Drawers (41" x 78" x 19") has two drawers in its base and corner posts. Its wooden headboard has an oval-shaped cut out with woven straw in the center. Slats on the side are 5/8" round and 19" high. The "CAMA CUNA R.N.C." (42 1/2" x 72' x 31") has corner posts and a carved headboard. Slats on the side are 5/8" round and 19" high. The "CAMA CUNA No. 2" (41" x 77" x 41") has curved head and foot boards without corner posts. Rectangular slats on the side measure 1.5" x .5" and 20.5" high. LaNacional distributed 2,288 cribs from January, 1994 to April 12, 1995. Eighteen retailers, including chain stores, distributed the cribs in Puerto Rico. More cribs may be in consumers' possession, since records for distribution prior to 1994 are not available. September 22, 1994 Release # 94-138 Washington, D.C. - CPSC is working with R.J. Reynolds of Winston- Salem, N.C., which recalled yesterday about two million "Camel Metal Match" cigarette lighters that were distributed as part of a cigarette promotion. Use of the lighter may result in burn injuries. To date, R.J. Reynolds has received 17 reports from across the country that adults have been burned while using the lighters.R. J. Reynolds began distribution of the lighters, which were supplied by CVA Inc., in August. The lighter, which resembles a small pack of Camel cigarettes, was included for free with two packs of Camel cigarettes in a blue box labelled "Camel Metal Match. Free with 2-Pack Purchase." Consumers should stop using the lighter immediately, dispose of it, or return any and all of their free lighters to R.J. Reynolds for one $5 incentive refund check. JANUARY 7, 1993 Release # 93-035 PRODUCT: Four models of "Fuzzy Puzzles": Farm Animals, Pets, Jungle Animals and Circus, distributed by Lights, Camera, Interaction!, Inc. PROBLEM: The peg in the center of each animal puzzle piece could separate from the piece and presents a choking hazard to young children. WHAT TO DO: Return the puzzle to the place of purchase for a free replacement puzzle. WASHINGTON, DC -- CPSC, Lights, Camera, Interaction!, Inc. of Westport, CT is voluntarily recalling four models of "Fuzzy Puzzles." The puzzles come in Farm Animals, Pets, Jungle Animals and Circus Animals. No model numbers appear on the puzzles. The wooden puzzle pegs could separate from the pieces of all four puzzles presenting a fatal choking hazard to young children. These "fit-in" type wooden puzzles are designed in the likeness of various animals with textured bodies resembling fur or feathers. There is a peg in the center of each animal to help remove and replace the puzzle piece. Approximately 14,400 fuzzy puzzles were sold nationwide from November 1991 to January 1992 for approximately $10.00 each. Hand painted on the front, bottom right-hand corner of each puzzle are the words "crafted by hand." A 4-1/2 by 5-1/2 inch piece of printed paper is glued to the back of the puzzle board and reads in part: "FUZZY PUZZLES, Ages 1-1/2 to 4 Made in Taiwan." The potentially hazardous puzzles were reported to CPSC by the company after receiving a consumer complaint. CPSC received six reports of incidents where pegs separated from the fuzzy puzzles. In two reports the pegs were reportedly found in the mouths of children. Neither the CPSC nor the company is aware of any injuries. This voluntary recall is being conducted to prevent the possibility of injury. Consumers are urged to take the puzzles away from young children immediately and return them to the store where purchased for a free replacement puzzle. Campari Butane Cigarette Lighters Warning NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE October 20, 1987 Release # 87-062 CPSC Warns of Potentially Hazardous Disposable Butane Cigarette Lighters WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission, Campari USA, Inc., New York, New York, today announced a program to alert consumers to the potential hazards of certain disposable butane cigarette lighters. The lighters in question are labeled "Campari, the smart mixable" on one side of the lighter body and on the other side will say, "Campari, Soda, the classic" or "Campari Grapefruit Juice, the exotic." These lighters were imported beginning in July of 1983 by Campari USA to be used as promotional items and were given to consumers free through liquor sales outlets. Certain of these lighters can shoot up excessively high flames or spew butane without lighting or before lighting. There have been no injuries reported involving these lighters. Since a consumer cannot readily differentiate between lighters which may exhibit the problem and those which do not, the firm is urging consumers who may still have these lighters in their possession to carefully dispose of them. In disposing of the lighters, consumers are warned to do so in such a way that children will not have access to the discarded lighters. The problem with these lighters was first brought to the Commission's attention by the Dade County Florida Fire Department when they received a box of these lighters from an anonymous source. The fire department forwarded the lighters to the Commission. No fires have been reported related to these lighters.
