Inc. Announce Recall to Repair Child Carriers WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Evenflo Co. Inc., of Piqua, Ohio, is recalling about 22,000 Hike 'N Roll child carriers for repair. Small children can slip sideways into the leg openings, creating a potential strangulation risk. Evenflo has received 16 reports of children slipping into the leg openings, with some reports of red marks on the child's neck. No serious injuries have been reported. Evenflo is offering a free seat insert to reduce the size of the leg openings and provide a more secure fit. The Hike 'N Roll carriers being recalled are model numbers 522101 and 522102. They were manufactured between June 1, 1996, and October 31, 1997. Both the model number and the manufacturing date are found on a white tag attached to the bottom of the carrier. These carriers are green and blue or blue and beige. They can be used both as a backpack carrier and as a stroller. "Evenflo" is printed on the front and back of the carrier. Juvenile product stores nationwide sold these carriers from June 1996 through June 1998 for about $65. Consumers should stop using the carriers immediately and call Evenflo at (800) 649-0071 anytime to receive the free seat insert. Consumers can also view a video clip about this recall (Transcript). FOR IMMEDIATE RELEASE CONTACT: Originally issued June 25, 1997, Revised March 8, 2006 (301) 504-7908 Evenflo Contacts: Dawn Dover, Mark Semer (212) 521-4817 or 4802 Release # 97-146 CPSC, Evenflo Announce Recall of Happy Camper Portable Play Yards WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Evenflo Company Inc., of Piqua, Ohio, is offering free hinge cover kits to owners of fully operational portable play yards sold under the Happy Camper, Happy Cabana and Kiddie Camper model names. If the hinges on the product are not fully rotated, the product can collapse, possibly trapping the child in the "V" formed by the folded top rails. Additionally, if leaned or sat upon, the rotating plastic hinges in the middle of the folding top rails can crack or break, presenting a sharp edge or possibly allowing the child to escape. Evenflo and CPSC are aware of three deaths involving the play yards. In two incidents, the hinges collapsed and entrapped the child and in one incident an infant was placed into a broken play yard. Evenflo has received 107 reports of children receiving cuts and bruises from broken hinges. The hinge covers fit over the hinges of the play yards only when the hinges are fully rotated into place, assuring that the play yard is properly set up. The hinge covers also provide additional reinforcement to the sides of the play yard. They snap on easily but firmly and can be moved by an adult to allow normal folding. Evenflo urges consumers to fully rotate the hinges, to inspect the hinges regularly for signs of cracking or breaking and to avoid leaning or sitting on the top rails. The play yards subject to this action are the Happy Camper and Happy Cabana models, which are rectangular-shaped, and the Kiddie Camper model, which is square-shaped. "Evenflo" and the model name of the play yard are printed on the top side rail of each play yard. The play yards come in a variety of colors and fabrics. Each play yard folds compactly and comes with a carrying bag and mattress. Major department, discount and juvenile products stores nationwide have sold these Evenflo play yards since January 1990 for about $60 to $130. Consumers who have one of these recalled play yards with a broken hinge should immediately discontinue using it and properly dispose of it. Evenflo will not be providing a fix (hinge cover kit) in such cases. The kits are now only being offered to consumers that have a fully operational play yard. Consumers who own a fully operational Happy Camper, Happy Cabana or Kiddie Camper should call Evenflo toll free at 1-800-447-9178 to receive the free hinge covers and should be sure to fully rotate the top rail hinges when setting up the play yard. Houdini Portable Playyards Hazard NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE CONTACT: Nychelle White December 17, 1996 (301) 504-7063 Release # 97-047 CPSC and Evenflo Warn of Potential Choking Hazard on Houdini Portable Playyards WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Evenflo Company Inc. of Piqua, Ohio is warning consumers that some of its Houdini Portable Playyards may present a potential small parts choking hazard and a sharp edge hazard. Approximately 205,000 of these playyards were sold. Small plastic covers from "push caps" used to secure various parts of the playyards together may crack and separate. Evenflo and the CPSC are aware of a number of reports of the plastic caps separating. Although no injuries have been reported, this action is being taken to prevent young children from choking or cutting themselves. All Houdini model numbers beginning with 332 are involved in this warning. The model number is located on a tag attached to the bottom of the unit. The playyards sold nationwide for about $45 from 1994 through 1996. CPSC and Evenflo are advising all Houdini owners to inspect the pushcaps on their playyards. The push caps on Houdini Playyards are small metal washers covered by a plastic clip. In some cases, these plastic clips have cracked and separated from the metal washer. The plastic clip could then present a choking hazard, and the exposed metal washer may be sharp enough to cut a child. The exposed metal washer will continue to hold the playyard securely together. Each playyard contains 16 push caps with three located in each corner of the top rail, and one in the center of each top rail. Consumers should look for loose, cracked, or missing plastic covers. If any are found, consumers should call Evenflo at (800) 490-7549 immediately to obtain free replacement push caps and installation instructions. Even if no loose, cracked, or missing plastic covers are found, consumers should call for a free warning label.
August 7, 1991 Release # 91-107 Evenflo Disney Pacifiers Recalled WASHINGTON, DC -- Evenflo Products Company, Ravenna, OH and the U.S. Consumer Product Safety Commission (CPSC) announced today the recall of all Evenflo pacifiers decorated with Donald Duck, Daisy Duck, Mickey Mouse, or Minnie Mouse characters. In some instances, the characters can separate from the pacifier and present a choking hazard. Evenflo sold the pacifiers between December 1990 and July 1991. The pacifiers retailed for $2.00 to $3.00. Evenflo has sent letters to all distributors, retailers, and wholesalers of these pacifiers ordering their immediate recall. Additionally, retailers will be asked to display warning posters informing purchasers that the product has been recalled. If a consumer has an Evenflo pacifier with Mickey Mouse, Minnie Mouse, Donald Duck, or Daisy Duck character attached to it, the pacifier is part of this recall. Consumers having a pacifier decorated with one of these figures should discontinue use and mail the pacifier to Evenflo Products Company, P.O. Box 1206, Ravenna, OH 44266-1206 and request replacement or refund. Customers may contact Evenflo at 1-800-356-2229 or at 1-800-233-2229 (Ohio residents). Crib Brackets & Mattress Support Hangers Replaced by Baby Line NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE June 26, 1985 Release # 85-033 "Questor" Crib Brackets And Mattress Support Hangers To Be Replaced WASHINGTON, D.C. -- In cooperation with the U.S. Consumer Product Safety Commission, Baby Line Furniture Company, a division of Spalding & Evenflo Companies, Inc., today announced a voluntary replacement program of slotted mattress support hangers and plastic brackets for Questor "Baby Line" cribs manufactured from 1970 through 1982 because the Commission believes they can cause suffocation hazards. The hangers and brackets hold the mattress and spring to the corner- posts of the cribs. Two deaths and 14 other instances where the slotted hangers became unhooked and 17 instances of plastic brackets breaking or bending have been reported to the Commission. If the hanger becomes unhooked or a plastic bracket bends or breaks, the mattress can tilt down when the child moves to that corner. The child can then slide down and become caught between the mattress and the side rail or end panel of the crib. In 19 of the 33 reported incidents, including the two deaths, the child became entrapped between the mattress and the crib. The Commission believes the hangers may become unhooked when the crib is moved, the bed is made of when a child jumps in the crib. While the firm believes its cribs do not present a hazard if assembled properly and used in accordance with instructions, a failure of the plastic hanger bracket, or an unhooking of the slotted hangers could cause the crib spring and mattress assembly to pivot downward when the child moves to that corner, resulting in potential for injury. The cribs can be identified by the name "Questor" on a stick-on-label underneath the mattress on the headboard or footboard of the crib. The cribs can also be identified by the slotted hangers and a tubular mattress support. More than 900,000 of the cribs were manufactured. Many of them are now being used by second or third owners. Fifty percent or more are believed by the Commission to be in storage. The cribs were sold in several different styles and come in white, maple, pine and sometimes in yellow and walnut finishes. They sold at retail for $77.00 to $140.00. The plastic brackets were previously recalled in 1979 and 1981, but the Commission has continued to receive reports of breakage and bending. Baby Line has since changed the design of its brackets and also of its mattress support hangers. If you own one of these cribs having slotted mattress hangers or plastic brackets, whether you are using it or have it in storage, call the manufacturer toll-free to obtain replacement kits for the slotted hangers and plastic brackets. Li'l Squeaker Pacifiers Recalled by Evenflo NEWS from CPSC U.S. Consumer Product Safety Commission Office of Information and Public Affairs Washington, DC 20207 FOR IMMEDIATE RELEASE February 7, 1984 Release # 84-011 Evenflo Li'l Squeaker Pacifiers Recalled Because Of Possible Choking Hazard Washington, D.C. --In cooperation with the U. S. Consumer Product Safety Commission, Evenflo Products Company, Ravenna, Ohio, today announced the voluntary recall of "Li'l Squeaker" pacifiers. CPSC laboratory tests indicate that on some pacifiers from Taiwan it is possible to pull the pacifier from its shield which could present a potential suffocation and choking hazard. The suspect pacifiers can be identified by "Taiwan" printed on the under side of the shield, or by "Made in Taiwan" printed on their blister cards. The shields are orange, blue, or yellow, round, and have three vent holes. There is a white squeaker on the handle end of the pacifier. Approximately 16,000 pacifiers were sold during October, November, and December, 1983. "Li'l Squeaker" pacifiers made in Hong Kong are not involved. Although there have been no incidents involving these particular pacifiers, CPSC and Evenflo have determined that because of the potential for injury, recall is in the public interest. Consumers are advised to return these pacifiers to their place of purchase for a full refund. They may also be returned directly to Evenflo Products Company, 771 North Freedom Street, Ravenna, Ohio 44266, for a full refund including postage.
