Announce Recall of Outlet Converters WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), General Electric Co.'s GE Lighting division of Cleveland, Ohio, is recalling about 50,000 outlet converters, also known as current taps. They convert a single electrical outlet into three outlets. The ground connector receptacles are oversized and can cause loose ground contacts. Without grounding, consumers are exposed to serious shock hazards. GE has received one report of a loose ground plug. No injuries have been reported. The outlet converters were sold as "GE Heavy-Duty Grounding Triple Taps." They are made of ivory, orange or green plastic and measure about 4 inches long and 1 inch wide. The GE logo is imprinted on one side of the converter and "15A-125V...CURRENT TAP...MADE IN CHINA" is imprinted on the other side of the converter. A silver UL label also appears on the converter. Retail stores nationwide sold the converters from September 1997 through January 1998 for about $3 to $4. Consumers should stop using the recalled converters immediately and call General Electric at (800) 729-4399 between 7 a.m. and 11 p.m. CDT Monday through Friday. Consumers will be instructed on how to return the product for a free replacement.

Inc. Announce Recall of Disposable Lighters WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), ERA Intermarketing Co. Inc. of City of Industry, Calif., is recalling about 840,000 Elite-2 disposable cigarette lighters. The lighters do not meet the 1994 requirement for child-resistant mechanisms in violation of the Consumer Product Safety Act. This act requires amechanism on disposable and novelty lighters shown to prevent ignition of the lighters by most children under 5 years old. Each year, CPSC estimates 190 deaths are caused by children under 5 years old playing with lighters. CPSC and ERA Intermarketing are not aware of any injuries involving these cigarette lighters. This recall is being conducted to prevent the possibility of injury. The Elite-2 lighters are made of clear plastic tinted in assorted colors. The metal top is labeled "ELITE," and the lighter label bears the bar code: "16229 12001" and the date code, "3/97." Small retailers sold the Elite-2 cigarette lighters nationwide from June 1997 to March 1998 for about $1. The heaters and humidifiers can overheat, presenting a fire hazard. Honeywell has received 56 reports of heaters overheating or catching fire, causing one minor injury, and six reports of humidifiers overheating or catching fire. Some of the fires relating to these heaters and humidifiers caused extensive damage. Only Duracraft model CZ-303, CZ-304, CZ-308, CZ-318, CZ-319 Heat Express and DeLonghi model CER-1 portable electric ceramic heaters are involved in this recall. The model number is located on a silver sticker on the bottom of the heater. The heater is 1500 watts and is colored black. Control knobs are located on the top or front of the unit. "Heat Express " and "Duracraft " or "DeLonghi" appear on the front of the unit. Only Duracraft model DH-950 Moisture SelectTM humidifiers and Honeywell model HCW-3040 Moisture SelectTM humidifiers are involved in this recall. The model number is located on the bottom of the humidifier. The humidifier is colored white and has a clear water tank that holds about two gallons. Control knobs and two output vents (one for cold moisture and one for warm moisture) are located on top of the unit. "Duracraft" or "Honeywell" appears between the two output vents on top of the unit. Retail outlets including discount stores, home centers, club stores and catalogue showrooms nationwide sold the Duracraft heaters from January 1989 through March 1998 for about $50 to $75. The DeLonghi heaters were sold from January 1989 through December 1990. Similar retail outlets nationwide sold the Duracraft and Honeywell humidifiers from May 1995 through March 1998 for about $50 to $80. Consumers should immediately stop using the recalled heaters and humidifiers and call Honeywell at (800) 632-9498 anytime for instructions on returning their product for a free replacement. These balls contain kerosene. If the ball cracks or breaks, kerosene leaking out could poison young children if swallowed. The smaller of these balls also present a serious choking hazard to young children. They lack warning labels required for small balls. CPSC and Atico International USA, Inc. are not aware of any injuries involving these balls. This recall is being conducted to prevent the possibility of injuries. All of the Smiley Face balls are 1.5 inches in diameter. They are yellow with black features. The Halloween Eyeballs were sold in two sizes. They measure either 1.5 or 2.25 inches in diameter. They are white with different colored irises and red streaks (resembling bloodshot eyes). All the balls float in kerosene inside a clear outer ball, and are labeled "MADE IN TAIWAN." These balls were sold nationwide by retail chain stores such as Arbor Drug, Eckerd Drugs, Thrift Drugs, K&B, Inc and Walgreen drug stores from June 1995 through October 1997 for about $1. Consumers should take these balls away from children immediately and return them to the store where they were purchased for a refund. Stores Inc., of Burbank, Calif., is recalling about 2,500 Tweety Water Timer Game Key Rings. The key rings contain ethylene glycol, commonly found in antifreeze, that could leak. Ethylene glycol can poison young children if swallowed. CPSC and Warner Bros. Stores are not aware of any injuries involving these key rings. This recall is being conducted to prevent the possibility of injury. The Tweety Water Timer Game Key Ring is a rectangular-shaped clear plastic container with three compartments containing a blue liquid. The middle compartment also contains a yellow wheel and a picture of Tweety at each end. When the rectangle is moved, the liquid flows freely between the compartments, spinning the wheel. A key ring is attached with a small chain. Tweety appears on the front of the key ring. Warner Bros. Stores nationwide sold the key rings from April 1996 through April 1997 for about $5. When an electrical short occurs, the rechargeable battery can cause the monitor's "parent" unit to smoke and flame. Gerry has received one report of flames and four reports of smoke coming from the monitor's parent unit. No injuries or property damage have been reported. The Gerry Clear Choice rechargeable baby monitor being recalled is Model 618. The model number is found on a silver plate on the front of the monitor's AC adapter. The monitor consists of a "baby" unit, a parent unit, a recharging base for the parent unit and AC adapters. All of the units are white with a light blue accent color. Because it houses the battery, only the parent unit presents the hazard. Mass merchandise and baby stores throughout the United States sold the baby monitors from April 1996 to March 1998 for about $40. Consumers should disconnect and throw away the blue battery pack from the parent unit immediately. Metal fatigue could cause failure at the hitch arm pivot bracket, resulting in the trailer breaking loose from the bicycle, causing injury to the occupant of the trailer. Norco Products is not aware of any injuries involving these trailers. This recall is being conducted to prevent the possibility of injury. These trailers are designed to carry one or two children behind a bicycle. This recall includes Norco Stagecoach, Norco Rambler, Norco Tot Taxi, Axiom Firefly and Bike Mate Trailer models with a folding arm. The model name is located on the back of the trailer in 2-inch letters. Small independent bicycle shops in Washington, Oregon, Northern California, Idaho and Colorado sold the trailers from January 1997 through February 1998 for about $200 to $350. Consumers should stop using these bike trailers immediately and call the store where purchased or Norco Products to arrange to have a safety upgrade kit installed at no charge. The tumbler's motor can overheat, presenting a fire hazard. Midway Arms has received 13 reports of motors overheating and catching fire. Five of these fires caused minor and major property damage beyond the tumbler. No injuries have been reported. Only tumblers sold under the Midway and Past brand names are involved in this recall. The Midway brand tumblers have a blue top and gray base with the words, "Midway...Model 1292...Tumbler" on a sticker on the base. The Past brand tumblers have a tan top and brown base with the word, "Past" on a sticker on the base. Midway catalogs and independent arms distributors and dealers nationwide sold the tumblers from February 1992 through January 1997 for about $45. These converters could short circuit, presenting fire, shock and thermal burn hazards to consumers. CPSC and the firm are not aware of any injuries involving these products. This recall is being conducted to prevent the possibility of injury. This model CH-1600 electric converter is intended for use by U.S. travelers overseas. It converts international 220/440 volt electricity to 110/120 volts and is rated for 50 to 1,600 watts. The body is plastic on three sides and metal on the other three sides, and has "1600 WATT FOREIGN TRAVEL CONVERTER" imprinted on the front plastic panel. It was sold in a box labeled in part, "SOMITO," "1600 WATTS FOREIGN TRAVEL CONVERTER," "Model CH-1600" and "MADE IN CHINA." Small discount and electronic stores in Chicago and the New York metropolitan area sold the converters from May 1997 through June 1997 for about $3. Announce Recall of Gas Grills With Side Burners WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Sunbeam Products Inc., of Neosho Mo., is recalling for repair about 1,800 GrillmasterTM gas grills with side burners. The side burner's propane gas hose can twist up toward the aluminum casting of the grill, causing overheating and melting of the hose. Gas leakage or a fire could result from the hose damage. Sunbeam Products received four complaints from consumers who requested replacement hoses due to damage caused from the heat of the grill casting. There have been no reports of gas leakage, fire or injury associated with these grills. The recalled grills are black and have a side burner for cooking, similar to a gas-range stove top. The models number of these grills are GG 461 EPB, GG 560 EPB, HG 560 EPB and GG 560 EPBS. The complete model numbers can be found on the assembly instructions and parts list. Either "461" or "560" also is located on the front of the grill next to the instrument panel, under the word, "GRILLMASTER." Mass merchandise stores, such as Bradlees, Caldor, The Home Depot and Wal-Mart sold the grills nationwide between October 1997 and February 1998 for about $240. Consumers should stop using the recalled grills immediately and call Sunbeam toll-free at (800) 200-2300 anytime to receive a free retrofit kit. The retrofit kit contains a wire bracket guard and instructions for easy installation that will correctly position the side burner's propane gas hose.
Sanders Recalled by Ryobi America Corp. Announce Recall of Detail Sanders WASHINGTON, D.C.-In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Ryobi America Corp. of Anderson, S.C., is recalling about 1.2 million detail sanders for repair or replacement. If the sander is left plugged in and the on/off switch is not fully in the "off" position, pressure from the switch's rubber dust boot can force the switch into the "on" position. As the sander runs unattended, it can generate heat, which can result in a fire. Ryobi has received three reports of these sanders possibly causing fires. One of these fires resulted in substantial property damage. No injuries were reported as a result of these fires. The recall includes the Ryobi model DS 1000, and Craftsman models 315.11639 and 315.11600 sanders. The model numbers and brandname are located on a data plate on the side of the sander. These blue or black oscillating sanders are hand-held tools weighing 1.6 pounds with a sanding arm that extends forward and down from the front end of the sander. They are about 11 inches long and have a triangular sanding head. Only sanders built between May 1993 and April 1997 are being recalled. These sanders have a serial number or date code located along the bottom edge of the data plate on the side of the tool. Consumers should always unplug the sander from its power source when it is not in use. Announce Recall of Soldering Guns WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), American Presto Corp. of Ontario, Calif., is recalling about 720 soldering guns sold under the Wisdom brand name. The plastic handle can melt during use, exposing consumers to electrocution, shock and burn injury hazards. CPSC has received one report of a soldering gun's handle melting. No injuries have been reported. The soldering gun is pistol-shaped, and made of red plastic with a metal barrel. The item is labeled, "SOLDERING GUN 110V 60 HZ 35W." A black power cord is attached to the bottom of the soldering gun. The soldering gun was packaged in a plastic shell, with a hang tag labeled in part, "SOLDERING IRON...PISTOL TYPE...MANUFACTURED IN CHINA FOR WISDOM TOOLS." Northern Hydraulic Co. stores in Minnesota, North Carolina, South Carolina, Florida, Iowa, Texas, Tennessee, Georgia, Wisconsin and Virginia sold the soldering guns from February 1997 through January 1998 for about $4. Consumers should stop using these soldering guns immediately and return them to American Presto Corp. for a refund. S. Consumer Product Safety Commission (CPSC), GT Bicycles of Santa Ana, Calif., is recalling 10,000 GT Speed Series and Robinson bicycle frames. The frames can crack and separate, causing a rider to lose control of the bicycle, fall and be injured. GT Bicycles has received about 17 reports of frames cracking, resulting in 2 injuries involving cuts and bruises. The recalled bicycle frames came equipped on bicycles or were sold separately as part of a frame kit. 1995, 1996, 1997 and 1998 XL and XXL model GT Speed Series, Speed Series Team and Speed Series Cruiser frames and bicycles are included in this recall. 1997 and 1998 model Robinson Pro, Pro XL, Pro 24 and Amtrac frames and bicycles also are included in this recall. These model names are found on the frame. GT Authorized Dealers nationwide sold these bicycles and frames from November 1994 through February 1998. The frame kits cost about $300 to $450 and the bicycles cost about $600 to $1,450. Consumers should immediately stop using bicycles equipped with these frames and call their local GT Authorized Dealer to arrange to have the frames replaced free of charge. Announce Recall of Climbing Harness WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Misty Mountain Threadworks Inc. of Banner Elk, N.C., is recalling about 5,000 climbing harnesses with a front entry sleeve system. If attached incorrectly, the climber's swami belt can be held by only a small piece of elastic behind the sleeve. Climbers who don't realize this mistake create a dangerous situation where the belay/rappel loop tears through the elastic webbing, potentially causing the climber to flip backwards, possibly causing injury. Misty Mountain Threadworks is aware of at least three incidents where climbers have ripped the elastic webbing, but there have been no injuries reported. The front entry sleeve is used in four different harness styles by the company -- Arete, Finesse, Quantum and Cadillac Big Wall. Announce Recall of Back Massagers WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), JB Research Inc., of Bellflower, Calif., had voluntarily recalled about 15,000 back massagers sold under the Relaxor, Deep Knead? Shiatsu brand name. The motor for the massager's Deep Knead? mechanism can jam and overheat. This will cause scorching to the foam and fabric on back of the unit, presenting a potential fire hazard to consumers. JB Research Inc. has received 46 reports of units overheating. No fires or injuries have been reported. To determine if a Relaxor, Deep Knead? Shiatsu back massager is involved in this recall, check the control wand of the massager. Only massagers that have the words "Deep Knead" on the front of the control wand and the controller model number JDK46H and serial numbers from 000001 to 020000 on the back of the control wand are included in this recall. These massagers come in 2 sizes, a "seat topper" (measuring 46 inches long by 20 inches wide) and a "seat lounger" (measuring 69 1/4 inches long and 20 1/8 inches wide). The massagers are covered in solid black or gray tweed fabric. The word "Relaxor" is imprinted on the front of the unit and elastic straps on the back of the unit attach the massager to a chair. The massager comes with an AC plug-in/auto adapter and the control wand. Specialty stores nationwide, including The Sharper Image, Brookstone and Nordic Track, sold the massagers from October 1997 through December 1997 for about $200 to $260. Consumers should stop using the recalled massagers immediately. Since JB Research Inc. is no longer in business, recalled massagers should be discarded or destroyed to prevent fires and injuries. No other models of Relaxor massagers are involved in this recall.

NHTSA, CPSC Announce Recall of Evenflo On My Way Infant Car Seats/Carriers The National Highway Traffic Safety Administration (NHTSA) and the U.S. Consumer Product Safety Commission (CPSC) announced the recall of about 800,000 Evenflo On My Way infant car seats/carriers because of a defect that can cause serious injuries to children while the seats are being used as infant carriers. Parents should continue using these rear-facing infant car seats, but should not use the carrying handle until new parts are installed to fix the problem, said NHTSA Administrator Ricardo Martinez, M.D. Safety is our highest transportation priority. A child safety seat that is installed and used correctly is the most effective safety device available for young children in automobiles. The recall involves On My Way infant car seat/carrier model numbers 207 and 492 manufactured from Dec. 15, 1995, to July 27, 1997. The model 492 is the On My Way Travel System, which includes a stroller. Model 207 is the car seat/carrier without the stroller. The manufacture date and model number are located on the bottom of the seat. Juvenile product, mass merchandise and major discount stores nationwide sold the car seats beginning in January 1996 for about $60 to $70, and the car seat with stroller for $150 to $175. The red push button assembly on each side of the seat that is used to latch and adjust the carrying handle can unexpectedly release and cause the seat to flip forward. There have been 176 reports in which the carrying handle latch unexpectedly released, resulting in 89 injuries to children, including bruises, concussions and skull fractures. The injuries occurred to both restrained and unrestrained children. This is a serious problem that puts infants at risk, said CPSC Chairman Ann Brown. The latches on these car seat carriers can release without warning. Parents who have this car seat should call for the free repair kit immediately. The recall results from a cooperative investigation conducted by NHTSA and the CPSC. Evenflo will provide a repair kit, free of charge, with redesigned latch buttons that will make the handle latch much stronger. The repair can be done in the home with a Phillips head screwdriver. Until the seats are repaired, NHTSA and the CPSC advise owners not to carry a seat by its handle, but to use the hand holds at the head and foot of the seat. The magnet can come apart and release small plastic pieces, presenting a choking hazard to young children. Part of the mobile can detach and fall into the crib, presenting a risk of injury to young children. Fisher-Price has received 59 reports of cookie magnets coming apart. One report involved a child ingesting plastic pieces and nine reports involved children putting plastic pieces in their mouths, which were removed by adults. Fisher-Price has received 78 reports of mobile parts detaching. Forty-one reports involved the mobile's rotating ring and mirror detaching and falling into cribs, resulting in 15 injuries, such as bumps and bruises. The refrigerator magnet toy looks like a partially eaten chocolate cookie with colorful candy pieces on top. The bottom of the cookie is white. Only cookie-shaped magnets with date codes from 224 through 228, sold after September 1, 1997, are involved in this recall. The date code is imprinted on the back of the magnet and is the first three digits of an alpha-numeric code. The cookie-shaped magnet is sold as part of a three-piece Refrigerator Activity Magnet Set, Fisher-Price No. 71126. The other two magnets look like a carrot and a piece of cheese. Only the cookie magnet is being recalled. The crib mobile toy is a round, rotating mobile with a white plastic ring that is suspended from a plastic arm attached to a crib. The mobile, which plays music, has a mirrored center and has colorful butterflies and beads that move on the ring as the mobile rotates. The mobile also can be removed from the arm for use as a floor toy. The mobile is sold as the Magic Motion Mobile, Fisher-Price No. 71153. Major toy and mass merchandise stores nationwide sold the recalled cookie magnet with the Refrigerator Activity Magnet Set beginning in September 1997 for about $6 and sold the Magic Motion Mobile beginning in June 1997 for about $24. The HTPV pipe used in these vents could be susceptible to corrosion, cracking and joint separation, which could result in the release of carbon monoxide into living areas, presenting a deadly threat to consumers. Although Goodman did not manufacture HTPV pipe, some models of Goodman furnaces were vented using HTPV material produced by other companies. Goodman voluntarily undertook its replacement program to help ensure the safety and comfort of its gas-appliance customers and has already replaced many HTPV systems.For each Goodman furnace that is horizontally vented with HTPV pipe, Goodman will continue to arrange for replacement of the vent with a new, approved vent at no charge. The clacker balls hanging from the center of the toy present a choking hazard to young children. CPSC and The First Years have received 2 reports of children getting one of the clacker balls stuck in the throat and blocking the airway. Nearby adults were able to pull the balls free and prevent the children from choking. The First Years is offering a free repair kit, consisting of easily installed permanent covers for the ball-shaped ends of the clacker toy. The covers will prevent the toy from entering a child's throat. The First Years High Chair Gym is an activity toy consisting of a metal wire frame about 7 inches high and 10.5 inches wide. It attaches to a high chair tray by two large red suction cups. Several brightly colored plastic toys hang from the wire frame. Two 1-inch diameter plastic clacker balls, one black and one white, are suspended from the center of the frame by rigid plastic arms about 3.5 inches long. A yellow plastic spinning toy on one end of the frame has "the first years" molded in one end with a sun on the opposite end. CPSC, Manufacturers Announce Recall Program to Replace Vent Pipes on Home Heating Systems WASHINGTON, D.C. - In a landmark action, virtually the entire furnace and boiler industry together with the manufacturers of high-temperature plastic vent (HTPV) pipes have joined with the U.S. Consumer Product Safety Commission (CPSC) to announce a recall program. This program will replace, free of charge, an estimated 250,000 HTPV pipe systems attached to gas or propane furnaces or boilers in consumers' homes. The HTPV pipes could crack or separate at the joints and leak carbon monoxide (CO), presenting a deadly threat to consumers. CO is a colorless, odorless gas produced by incomplete burning of carbon-based fuel, including natural gas and propane. The initial symptoms of CO poisoning are similar to the flu, and may include dizziness, fatigue, headache, nausea and irregular breathing. High-level exposure to CO can cause death. To determine whether they have HTPV pipe systems that are subject to this program, consumers should first check the vent pipes attached to their natural gas or propane furnaces or boilers. Vent pipes subject to this recall program can be identified as follows: the vent pipes are plastic; the vent pipes are colored gray or black; and the vent pipes have the names "Plexvent ," "Plexvent II" or "Ultravent " stamped on the vent pipe or printed on stickers placed on pieces used to connect the vent pipes together. Consumers should now check the location of these vent pipes. For furnaces, only HTPV systems that have vent pipes that go through the sidewalls of structures (horizontal systems) are subject to this program. For boilers, all HTPV systems are subject to this program. Other plastic vent pipes, such as white PVC or CPVC, are not involved in this program. After checking the vent pipes, consumers should call the special toll-free number (800) 758-3688, available between 7 a.m. and 11 p.m. EST seven days a week, to verify that their HTPV pipe systems are subject to this recall program. Consumers with eligible systems will receive new, professionally installed venting systems free of charge. Additionally, consumers who already have replaced their HTPV pipe systems may be eligible for reimbursement for some or all of the replacement costs. The program came about as a result of mediation among 27 participants manufacturers of HTPV pipes and manufacturers of natural gas or propane-fired boilers and mid-efficiency furnaces. This is the first time that CPSC has used a mediator to bring together all segments of an industry to implement a program for the benefit of consumers. All consumers should have their fuel-burning appliances inspected each year to check for cracks or separations in the vents that could allow CO to leak into the home. In addition, CPSC recommends that every home should have at least one CO detector that meets the requirements of the most recent Underwriters Laboratories 2034 standard or International Approval Services 6-96 standard.
