December 13, 1993 Release # 94-018 PRODUCT: "Sweda Juice Factory" electric juicers sold at Jamesway and Caldor retailers after April 1, 1992. About 6,000 juicers are believed to be affected. PROBLEM: If the juicer is not reassembled properly after cleaning, during use the plastic upper body may possibly malfunction, causing injury to consumers nearby. WHAT TO DO: Consumers should stop using the juicers and call Telebrands at 1-800-269-1572 for complete instructions on how and where to return the juicer with proof of purchase for full refund of purchase price and postage. WASHINGTON, DC -- CPSC, Telebrands of Roanoke, VA is voluntarily recalling Sweda Juice Factory electric juicers. This recall is limited only to the Sweda Juice Factory electric juicers made in China and sold at Jamesway and Caldor retailers after April 1, 1992. If the juicer is not reassembled properly after cleaning, during use the unit may malfunction causing the plastic upper body to shatter, possibly causing injury to consumers nearby. Telebrands is aware of eight incidents of malfunctions that have resulted in five injuries to users. The injuries include cuts and bruises to the face, arm, and chest area of the body and lacerations requiring stitches. Approximately 6,000 electric juicers were sold by Jamesway and Caldor nationwide. The juicer sells in the range of $29.99 to $49.99. DECEMBER 13, 1993 Release # 94-017 PRODUCT: Children's necklaces and bracelets imported and distributed by Midwest Importers of Cannon Falls, Inc. The products are made of wooden blue, pink, purple and yellow colored beads with white wooden rabbits and orange wooden carrots strung on elastic strings. Approximately 26,000 necklaces and 39,000 bracelets were sold at retail stores nationwide from December 1992 through October 1993. PROBLEM: The paint on the beads contain levels of lead that exceed government standards, presenting a safety hazard to children. WHAT TO DO: Consumers should immediately take the necklaces and bracelets away from children and return them to the store where they were purchased for a full refund. WASHINGTON, DC --- CPSC, Midwest Importers of Cannon Falls, Inc., Cannon Falls, MN is voluntarily recalling about 26,000 24-inch children's toy necklaces and 39,000 6-inch children's toy bracelets. Testing by the company revealed that the levels of lead in the products' paint exceeds government standards, presenting a safety hazard to children. Even though CPSC and the company know of no poisoning incidents, this recall is being conducted to prevent the possibility of poisoning. Both the necklaces and bracelets are constructed of wooden blue, pink, purple, and yellow colored beads with white wooden rabbits and orange wooden carrots strung on elastic strings. Each product is packaged in an 3-inch by 3-inch cardboard box with a transparent plastic lid. The boxes are not labeled except for a small adhesive tag attached to an elastic string which says "Made in China" and bears the Cannon Falls copyright. The products were imported from China and sold at retail stores nationwide from December 1992 through October 1993. The necklaces sold for approximately $3.00 each and bracelets sold for approximately $1.00 each. Consumers are urged to immediately take the necklaces and bracelets away from children and return them to the store where they were purchased for a full refund. DECEMBER 13, 1993 Release # 94-016 PRODUCT: About 23,750 non-refillable "Good Time Troll" cigarette lighters distributed by New York Lighter Company. The lighters sold at convenience stores nationwide from January through July 1993. They are printed with one of four troll designs: "Party Troll" is holding a drink in each hand, "Rockin' Troll" is playing a guitar, "King Troll" is smoking a cigar, and "Lucky Troll" is holding a four-leaf clover. The flue of the lighter is labeled "THAILAND" and "C-LITE." PROBLEM: The "Good Time Troll" lighters, printed with troll characters, are very attractive to children and may invite play. If a child plays with the lighter, a fire and/or serious burn injuries may result. WHAT TO DO: Put the lighters out of reach of children. Then, call 1-800-626-4732 to receive special prepaid packaging and complete instructions for returning the lighters. When the lighter is received, a free gift will be sent to the consumer. December 13, 1993 Release # 94-022 PRODUCT: Approximately 3,264,000 Casablanca brand ceiling fans manufactured from 1981 through 1993. PROBLEM: The fan may fall from its ceiling mounting. WHAT TO DO: Consumers should stop using the fan and not allow anyone underneath the fan. Call (888) 227-2178 to receive a retrofit part to be installed by the consumer to prevent the fan from falling. In the event that a fan falls, consumers should immediately turn off the circuit breaker due to the possibility of exposed wires. WASHINGTON, DC -- CPSC and Casablanca Fan Company, City of Industry, CA are jointly announcing the voluntary recall of 3,264,000 Casablanca brand ceiling fans manufactured from 1981 through 1993. The fans can separate from the canopy on which they are mounted to the ceiling. If a fan separates from its canopy, it will fall and possibly injure a bystander. In addition, a fallen fan may leave exposed wires in the ceiling possibly posing an electric shock hazard. Casablanca Fan Company has received approximately 50 reports of fans falling from ceiling mountings. One incident has been reported in which a consumer received an injury. CPSC learned of this problem when Casablanca notified the Commission of its intention to initiate a recall. The affected fans have a ball and canopy mounting system and were manufactured and sold nationwide from January 1981 through September 1993. The retail prices for the fans vary by model and time period sold. The prices generally range from $200 to $2,500. A ceiling fan owner can identify the recalled fan by looking at the metal nameplate located on the exterior of every Casablanca fan. A fan subject to the recall will say "Casablanca" on the nameplate. In addition, the ball and canopy mount is visible from the bottom of the fan. Also, the second letter of the serial number on the nameplate will be A, B, C, O, P, R, S, T, U, V, W, X, or Y. This recall does not cover Pasadena brand fans. Casablanca has designed a retrofit part to be installed by the consumer to prevent the fan from falling from its mounting. Step by step instructions will be provided with the part. In addition, the consumer may contact Casablanca's 24 hotline to obtain a retrofit kit free of charge and for assistance related to installation at (888) 227-2178. Consumers who are unable to install the part may contact Casablanca to request assistance in arranging for free installation of the retrofit part on fans manufactured during or after March 1991. Casablanca fans manufactured before March 1991 were sold by another company. Owners of these fans who require installation assistance should contact Casablanca's toll free hotline to discuss whether free installation is available. Casablanca will decide on a case by case basis. However, the retrofit part and easy to follow step by step instructions will be provided free of charge to all Casablanca fan owners. A consumer who owns an affected fan should not use it until the retrofit part is installed. In the meantime, consumers should not allow anyone underneath the fan. In the event that fan falls, consumers should immediately turn off the fan's circuit in the circuit box due to the likely possibility of exposed wires. (SEI) #BB-104-2 (red), BB- 105-2 (white), BB-106-2 (blue), and BB-107-2 (black) sold from January 1990 to present. (Twin size on top; full size on bottom). All beds subject to this program have rectangular mattress support side rails measuring 3/4-inch by 1-1/2 inches. PROBLEM: The bunkbed may unexpectedly collapse during use. Bed occupants may be injured by the falling upper bunk. WHAT TO DO: Stop using affected beds immediately. To determine if you have an SEI bed, contact the retailer for information on the importer or manufacturer of the bed. Consumers with affected SEI beds should call the company toll-free at (800) 633-5096 for a free retrofit kit to reinforce the bed. WASHINGTON, DC - CPSC, Southern Enterprises, Inc. (SEI), Dallas, TX, is voluntarily providing reinforcement brackets for certain children's tubular metal frame bunkbeds. The upper level can fall unexpectedly causing the bunkbed to collapse. SEI has sold about 6,000 such beds since January 1990 in retail stores nationwide. The beds sold for approximately $150 to $300 each. SEI has received about 15 reports of these beds breaking, including 6 reports of alleged injury. SEI is offering free reinforcement brackets to avoid further incidents. The bunkbeds subject to this program (models BB- 104-2, BB-105-2, BB-106-2, and BB-107-2) have metal frames made of hollow painted tubes. The glossy painted frame can be either red, blue, white or black. The beds use a standard twin-size mattress on the top bunk and either a twin or full-size mattress on the bottom bunk. The beds have 18 crossbars to support each mattress. The two rectangular mattress support side rails are 3/4-inch wide and 1-1/2 inches high and a metal ladder attaches to the side of the bed as a separate unit. Consumers should immediately stop using affected beds as bunkbeds. To determine if you have a SEI bed, contact the retailer for information on the importer or manufacturer of the bed. Consumers with affected SEI beds should call the company toll-free at (800) 633- 5096. SEI is offering consumers a retrofit kit that will reinforce the upper bunk when assembled on the upper and lower corner joints. The CPSC is investigating numerous other importers and manufacturers of metal bunk beds that may present a similar risk of collapsing. To date, firms have received over 300 complaints of defective bunk beds industry-wide. CPSC urges all consumers to inspect all eight mattress support corners of all metal bunk beds for breaks or cracks in the paint or metal around the weld. Consumers should stop using damaged bunk beds and contact the retailer, manufacturer, or importer. December 3, 1993 Release # 94-019 PRODUCT: Approximately 175,000 high gloss, tubular metal frame, twin size/full size children's bunk bed, Rosalco models 3007 (red), 3027 (blue), 3207 (white), and 3227 (black) sold from January 1990 to present. (Twin size on top; full size on bottom.) Also metal frame twin size/twin size bunk beds, Rosalco models 3006 (red), 3026 (blue) 3206 (white, and 3226 (black). All beds have rectangular mattress support side rails measuring 1 inch by 2 inches. PROBLEM: The bunk bed may unexpectedly collapse during use. Bed occupants may be injured by the falling upper bunk. WHAT TO DO: Stop using affected beds as bunk beds immediately. Discard or destroy the products. Firm is out of business. WASHINGTON, DC - CPSC, Rosalco, Inc., Jeffersonville, IN, has warned of a hazard regarding certain children's tubular metal frame bunk beds. The upper level can fall unexpectedly causing the bunk bed to collapse. Rosalco, Inc., an Indiana importer, has sold about 175,000 such beds since January 1990 in retail stores nationwide. The beds sold for approximately $150 to $300 each. Rosalco has received about 150 reports of these beds breaking; including 10 reports of alleged injury when the beds failed. The Company is out of business. Immediately stop use of these beds. The bunk beds subject to this problem (models 3006, 3007, 3026, 3027, 3206, 3207, 3226 and 3227) have metal frames made of hollow painted tubes. To determine if you have a Rosalco bed, contact the retailer to determine the importer or manufacturer of the bed. Firm is out of business. Consumers should discard or destroy the affected beds.Since December 1992, Rosalco had been distributing bunk beds with a different reinforcement bracket kit. Since the Company included these beds in its affected beds program, consumers should also discontinue use of these bunk beds. DECEMBER 1, 1993 Release # 94-015 PRODUCT: Holiday electric candle decorations with either three bulbs or a single bulb, manufactured by Willis Mfg. and sold at Kmart during the 1992 Christmas season. The affected single and three candle units are improperly wired. The three candle units may also have improperly aligned metal contact tabs. PROBLEM: The improper wiring could cause a shock hazard. The improperly aligned contact tabs will immediately short circuit and present a potential fire, thermal burn or shock hazard to users. WHAT TO DO: Do not attempt to repair defective units. Take them back to any Kmart store for a free refund or replacement.. WASHINGTON, DC -- Kmart Corporation, of Troy, MI, and the U.S. Consumer Product Safety Commission (CPSC) urge consumers who bought holiday candle lights called "Candoliers" from Kmart during the 1992 holiday season to return the lights to any Kmart store for a full refund or replacement. The Candoliers may be improperly wired; three-light Candoliers may also have improperly aligned metal contact tabs. The improper wiring and alignment could lead to shock, thermal burn, or fire. The Candoliers came in single and three-lights. The single-light Candoliers were packaged in a plastic bag with a cardboard package top reading "Single Candolier." The candle lights are about 11 inches high. They have an off-white plastic stem and base, and either a clear or an orange bulb. Single- light Candoliers sold at Kmart for $.99 each. Three-light Candoliers were packaged in a cardboard box. The stems and base are off-white molded plastic. The outer two candles are six inches high and the center candle is seven inches high. The three-light Candoliers were packaged with either clear or orange bulbs. They sold at Kmart for $4.59 each. To avoid injury, Kmart is recalling all affected products. Candoliers with improperly aligned metal contact tabs will immediately short circuit. If the household branch circuit breaker or fuse does not interrupt (trip) the circuit, the short circuit could create a fire, thermal burn, or electric shock hazard for anyone nearby or for anyone holding the product or its power cord. The Candoliers which have been improperly wired will continue to work but may present an electric shock hazard. Even if you have used your Candolier without it short- circuiting, you should discontinue use and return the product to any Kmart to prevent potential injury. All Candoliers sold during the 1992 holiday season are being recalled. Kmart reported two incidents to CPSC of Candoliers failing. Both incidents involved three-light Candoliers. One incident resulted in slight burns to a consumer's hand when the cord caught fire. Approximately 72,000 three-light Candoliers and approximately 292,400 single-light Candoliers were distributed during the 1992 holiday season. November 22, 1994 Release # 94-011 PRODUCT: NEC PC-17-01 and PC-17-02 Laptop Computers. PROBLEM: The computer battery can explode and catch fire. WHAT TO DO: Turn on computer with the AC adapter disconnected and allow battery to discharge. Call (800)237-2913 to arrange for repair. WASHINGTON, DC -- CPSC, NEC Technologies Inc. (NECT), Boxborough, MA, is voluntarily recalling batteries in NEC PC-17-01 and PC-17-02 Laptop Computers. The batteries in these laptop computers can explode and catch fire. Approximately 13,000 NEC PC-17-01 and PC-17-02 Laptop Computers were sold in the United States and Canada from December 1988 to April 1990. Model numbers can be found on the bottom of the computer. Only the portable PC-17-01 and PC-17-02 models are involved. No other NEC computers are involved. NECT is aware of seven incidents of smoke and/or fire which occurred during battery recharging. Although there are no known injuries to date, NECT and CPSC view this as an important matter and are making every effort to insure consumer safety. To encourage participation in this recall, NEC is offering a $100 reimbursement to all consumers taking advantage of this important free battery replacement program. NEC developed, and currently has available, a new battery using state of the art nickel-metal hydride technology to replace the defective batteries. Consumers should turn on the computers with the AC adapter disconnected and allow the batteries to drain completely. NOVEMBER 12, 1993 Release # 94-010 PRODUCT: Approximately 4,500 Sunbeam-Oster Household Products' discontinued 1,000-watt electric indoor grills with the gray "Nu-Stone" plastic base. The grills were sold between September 1992 and June 1993. Affected grills have a "1,000-watt" designation and the number "E992" stamped on the portion of the grill's heating element that attaches to the variable temperature control. PROBLEM: The grill can cause grease fires if users repeatedly grill food containing a high fat content without emptying accumulated grease between batches. The company has received four returns to date due to smoking of the indoor grills; no grease fires or injuries have been reported. WHAT TO DO: Consumers who purchased the 1,000-watt indoor grills are urged to discontinue use and return them to Sunbeam-Oster for a replacement or a refund (at the consumer's option). WASHINGTON, DC -- Sunbeam-Oster Household Products, Schaumburg, IL, CPSC, today announced the voluntary recall of its discontinued 1,000-watt electric indoor grill with the gray Nu-Stone plastic base, which can cause grease fires under certain circumstances. The affected grills were sold between September 1992 and June 1993. The recall does not involve any other Sunbeam or Oster grills or products. The recall is limited to the 1,000-watt variable temperature electric indoor grills with the gray Nu-Stone plastic base and patented "Griffo" porcelain grid, model numbers 4757 (Sunbeam brand) and 4772 (Oster brand). The Sunbeam brand sold for about $30.00 and the Oster brand sold for about $45.00. Sunbeam-Oster no longer manufactures the 1000-watt indoor grill with gray Nu-Stone base. Current versions are sold with an 800-watt element. The company believes that while the product is not defective, repeatedly grilling food containing a high fat content without emptying accumulated grease between batches could cause a fire. Consumers who purchased the 1,000-watt indoor grills are urged to discontinue use and return them to Sunbeam-Oster, which will (at the consumer's option) either replace the unit with a new 800-watt version of the grill or refund the full purchase price. To assist consumers, Sunbeam-Oster has established a toll- free number, 1-800-986-0008, which owners of the 1,000-watt indoor grills are urged to call. The company will send prepaid return mailing cartons to indoor grill owners. About 6,600 indoor grills were shipped to retailers in September 1992 and fewer than 4,500 grills remain in the hands of consumers. To date, the company has received four returns due to smoking of the indoor grills. No injuries have been reported. The grills being recalled are the 1,000-watt indoor electric grills with a gray Nu-Stone base. November 8, 1993 Release # 94-009 PRODUCT: "Synergy Plus Chewables Vitabots" multiple vitamins and minerals. The product was promoted as a dietary supplement for children over four years of age. Each bottle of Vitabots has 90 light brown colored tablets; each tablet has five milligrams of iron. The supplements were sold nationwide in retail health food and nutrition stores between July 1, 1992 and July 31, 1993. PROBLEM: The product was not sold in child-resistant packaging as required by the Poison Prevention Packaging Act. The lack of child-resistant packaging means that a child could easily access and accidentally swallow the iron tablets. Each bottle of the product contains enough iron that, if consumed by a child at one time, it could cause serious illness requiring hospitalization. WHAT TO DO: Immediately return the unused portions of any bottles of the recalled Vitabots to the store where purchased for a refund of the purchase price. Consumers can also call the manufacturer at (201) 371-7300 for instructions on where to send the recalled products for a refund, including shipping costs. WASHINGTON, DC --- CPSC, International Vitamin Corp. (doing business as Synergy Plus), Union, NJ is voluntarily recalling its "SYNERGY PLUS (For Children Over 4 Years) CHEWABLES Vitabots Multiple Vitamins and Minerals with Digestive Enzymes and Oat Bran." This iron-containing vitamin and mineral supplement was not put in child-resistant packaging as required by the Poison Prevention Packaging Act. The lack of child-resistant packaging means that a child could easily access and accidentally swallow the iron tablets. Each bottle of the recalled product contains enough iron that, if consumed by a child at one time, it could cause serious illness requiring hospitalization. The recalled Vitabots Multiple Vitamins and Minerals (for children over 4 years) were sold in amber-colored glass bottles with white screw-on metal caps. Each bottle contains 90 light brown colored tablets with five milligrams (mg) of iron per tablet. The product, wrongly labeled "Non-Toxic," is promoted as a dietary supplement for children over four years of age. The bottle's label also carries the statement "Manufactured by SYNERGY PLUS, Union, N.J. 07083 USA." This recall does not include any other iron-containing products manufactured by International Vitamin Corp. Approximately 2,500 bottles of the recalled iron products were sold to consumers through retail health food and nutrition stores nationwide between July 1, 1992 and July 31, 1993 for approximately $9.00 - $10.00 per bottle. PROBLEM: Parts of the furnace and vent system may fail prematurely. Failure of vent components could allow potentially hazardous carbon monoxide (CO) gas to leak into the home. WHAT TO DO: Owners of affected furnaces should call York at 1-877-874-7378 to obtain a free replacement furnace. York is providing a $200 allowance toward the cost of installing the replacement furnace. York initiated this warranty enhancement program because certain parts of the furnace and vent system may fail prematurely. York estimates approximately 4,000 HeatPipe furnaces remain in use. Under York's warranty program, HeatPipe furnace owners may obtain a free replacement furnace of similar capacity and efficiency to their HeatPipe. Replacement of the HeatPipe furnaces will require professional installation and replacement of some or all of the furnaces' venting components. York will provide a $200 allowance toward the cost of installing the replacement furnace. CPSC urges anyone with a HeatPipe furnace to participate in the HeatPipe replacement program. All the light fixtures were sold from July 1991 through March 1992 for $14.97 each. PROBLEM: The outdoor light fixture does not have a weatherproof ballast and is not grounded. Rain water could seep inside and short circuit, creating an electric shock hazard. WHAT TO DO: Consumers should immediately discontinue use of both products. No electric shock incidents have been reported to Builders Square or to CPSC. The Builders Square SKU No. 9300117 light can be identified by its black painted aluminum diecast fixture and its clear plastic globe. The original nine watt "U" shaped fluorescent lamp is labeled "DLS 9W, LIGHT 21." Also, the back of the metal fixture has three labels as follows: "UL Incandescent Fixture, Issue No. 696089, Suitable For Wet Locations," "E117402, Made In Korea," and "Wall Mount Only." October 8, 1993 Release # 93-111 PRODUCT: Baby Trend and Baby Express "Home and Roam" portable mesh-sided playpens in two sizes, 40" x 40" and 28" x 40". PROBLEM: The playpen may appear to be set up properly, if the top rails are not securely locked. If the playpen is not set up so that each top rail is securely locked in position, the top rails may collapse with an infant in the playpen. If the infant is holding onto a top rail when it collapses, he or she may be caught by the neck and strangle. WHAT TO DO: Make sure each of the four top rails is fully locked before using the product and call Baby Trend immediately at 1-800-234-1879. Baby Trend will provide corrective action AND a free gift valued at $5.00 for each playpen owned. WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Baby Trend, Inc., Pomona, CA is issuing an urgent warning to owners of Baby Trend and Baby Express "Home and Roam" playpens. There have been four reports that these products have collapsed with infants inside the playpens. One incident resulted in the death of a one-year-old child. In two of the incidents the child was found not breathing but was revived. Baby Trend sold approximately 65,000 Home and Roam playpens nationwide during 1992 and 1993. Some additional Home and Roam playpens were sold under the Baby Express label. The playpens come in two sizes and a variety of colors and fabrics. The larger one is 40" x 40" and the smaller one is 40" x 28". The words "Home and Roam" and "Baby Trend;" or "Home and Roam" and "Baby Express" and appear on two of the four top rails. Retail prices range from $68 to $130. All of the playpens are set up by rotating a pivoting hinge in the center of each top rail. To lock the rails in place, the pivoting hinge must be rotated a full 180 degrees from the collapsed position. Because the hinges are under the protective fabric covering the top rails, consumers may rely on their sense of touch to determine if each hinge has been rotated completely. It is possible to rotate the hinge less than 180 degrees but have the top rail look and feel locked. If the pivoting hinge has been turned less than 180 degrees, IT IS NOT LOCKED and the top rail can collapse. If a baby is holding onto the top rail when it collapses, he or she can be caught by the neck and strangle. Consumers should check to make sure the top rail hinges are fully engaged before each use. Federal regulations require the bicycles be equipped with both handbrakes and footbrakes and handlebar stem insertion marks. Sidewalk bicycles with handbrakes, but no footbrakes, may present a risk of injury to young children who might not be able to stop the bicycles using handbrakes. PROBLEM: The vacuum's plastic fan may break into pieces causing the housing to crack open and the fan pieces to propel from the vacuum's housing. BISSELL has received 40 reports of such incidents including 12 reported injuries. WHAT TO DO: Consumers who bought BISSELL Trio Vacuums through June 1993 should immediately stop using them and call BISSELL Customer Service at 1-800-237-7691, Monday through Friday, between 8:00 a.m. and 5:00 p.m. (Eastern Time). The vacuum's plastic fan may break into pieces causing the housing to crack open and the hard, sharp fan pieces to be propelled from the vacuum's housing. Models with plastic fans were sold through June 1993. BISSELL has received 40 consumer reports of product failures including 12 reports of injuries. While most injuries were minor cuts and bruises to the lower body, two injuries occurred at eye level. Approximately 13,000 vacuums were sold nationwide through major retailers for about $50 or $60, depending on the model. After receiving retailer returns and direct calls from consumers regarding failed fans, BISSELL began a product recall and reported the problem to CPSC. Consumers who bought BISSELL Trio Vacuums through June 1993 should immediately stop using them and check the date stamp. To check the date stamp, remove the dirt collection tank and read the six digit number which appears on the inside of the bottom motor housing. The first two numbers are the year. The Commission's objective is to reduce the estimated 28.6 million injuries and 21,700 deaths associated each year with the 15,000 different types of consumer products under CPSC's jurisdiction
