January 11, 1994 Release # 94-032 PRODUCT: "Pacifier Thermometers," brand name "Dubby." About 340 pacificer thermometers were imported from Taiwan by M.J. Harris and Associates and distributed nationwide through pharmacies and mail-order companies between April and August 1993. The pacifier thermometers are pink or blue with a clear silicon rubber nipple. They are sold in blue boxes with a window front; the name "Dubby" is printed on the top panel. PROBLEM: Because the product is both a pacifier and a thermometer, it is subject to FDA requirements and CPSC's pacifier regulations. During CPSC testing, the nipples separated from the base of the pacifier thermometers presenting a potential choking hazard to infants and young children. WHAT TO DO: Consumers should take the pacifier thermometers away from infants and young children immediately and return them to the stores or mail-order firms where purchased for a full refund. The pacifiers have a built-in electronic L.C.D. thermometer that displays the temperature of the baby who is sucking on the nipple. The pacifier thermometers are blue or pink with a clear silicon rubber nipple. The pacifier thermometers are sold in blue boxes with a sticker that reads "Made in Taiwan." The boxes have a window front and the name "Dubby" printed on the top panel. A clear plastic storage cover is provided with each pacifier thermometer. The pacifier thermometers were imported by M.J. Harris and Associates from Taiwan and distributed through pharmacies and mail-order firms nationwide between April and August of this year. Each pacifier thermometer sold for as much as $12.50. Consumers are urged to take the pacifier thermometers away from babies and young children immediately and return them to the stores or mail-order firms where purchased for a full refund. February 8, 1994 Release # 94-037 PRODUCT: "High Back Toddler Swing" Model 4309 manufactured by The Little Tikes Company and sold beginning January 1993. These swings have no seat belt. Beginning in September 1993 seat belts were pre-installed on all High Back Toddler Swings. PROBLEM:Children may push up or stand in the swing and fall out of the swing. A fall could result in serious injury. Twelve injuries that included facial cuts, bruises and a broken elbow were reported to the company. WHAT TO DO:Consumers should not use or install a Little Tikes High Back Toddler Swing without a seat belt. A free seat belt can be obtained by calling Little Tikes at (800) 868-2276. WASHINGTON, DC -- CPSC, The Little Tikes Company, Hudson, OH is offering consumers a free seat belt for its "High Back Toddler Swing," Model 4309 for children ages 9 months to 3 years. High Back Toddler Swings manufactured after September 1993 have a seat belt installed on the swing. Consumers should not use or install a High Back Toddler Swing Model 4309 which does not have a seat belt. February 8, 1994 Release # 94-036 PRODUCT: Approximately 18,400 Kouvalias Musical Toys, model number 960. The wooden toy boxes consist of a wooden platform with a ball connected to the platform and nine more balls in various colors attached by metal coil springs. The boxes were distrubuted from 1989 through mid-July 1993 at various retail stores nationwide. They sold for $44.95 each. PROBLEM: The colored wooden balls and pieces from the music box underneath separated during routine CPSC testing, presenting a potential choking hazard to young children. WHAT TO DO: Consumers should take the toys away from young children immediately and return the toys to the stores where purchased for a full refund. WASHINGTON, DC -- CPSC, Reeves International, Inc., Pequannock, NJ is voluntarily recalling 18,400 "Kouvalias Musical Toy" model number 960. During routine CPSC testing, colored wooden balls and gear components from the musical box underneath the toy separated, presenting a potential choking hazard to young children. The musical toy consists of the following parts: a wooden base (5 inches in diameter) that houses the musical box; a wooden platform that measures 3-1/2 inches in diameter and is attached to the base; and a wooden ball (2 inches in diameter) connected to the platform. Nine wooden balls in various colors are attached to the 2- inch diameter wooden ball by 1-1/2 inch metal coil springs. The music box is activated when the platform is turned; a song is played while the toy rotates. The label underneath the base reads: "A. Kouvalias S.A. . . .MADE IN GREECE." The "Kouvalias Music Toy" was imported by Reeves from Greece and distributed nationwide at various retail stores from 1989 through mid-July 1993. The musical toys sold for $44.95 each. Consumers are urged to take the toys away from young children immediately and return the toys to the stores where purchased for a full refund. January 31, 1994 Release # 94-035 PRODUCT: Toy charm necklaces and bracelet, imported by Dillon Importing Company from China. The necklaces and bracelet are plastic colored figures or plastic square pieces with "charms" attached, strung on a piece of thin elastic. The jewelry was sold in retail chains, discount stores, catalog outlets, toy stores, and other outlets in 24 states from August 1992 through July 1993. The jewelry was sold without packaging, usually hanging from a store display rod or placed in open bins or baskets. The jewelry sold for about $1.00 each. PROBLEM: When tested, CPSC found that small parts separated when the elastic band broke, presenting a potential choking hazard to young children. WHAT TO DO: Consumers are urged to take the toy jewelry away from young children immediately and return the jewelry to the place were purchased for a full refund. The 18-inch plastic necklaces (#16-791 and #16-1135) consist of small figures (bear, duck, butterfly, fish, cross, child, rabbit, dog, star, clown, flower) in assorted colors separated by white beads strung on a piece of thin elastic. The necklaces were sold without packaging, usually hanging from store display rods, or placed in open bins or buckets. The 6-inch plastic stretch bracelet (#16-1133) consists of six square assorted pieces separated by two white beads with one colored charm attached to each square. The charms include a heart, cross, rabbit, and fish. The bracelet was also sold without packaging, usually hanging from store display rods or placed in open bins or buckets. Only the bracelets carries a label that reads, "MGNOV MADE IN CHINA." The toy jewelry was imported by Dillon Importing Company from China and distributed in 24 states (Ohio, California, Nevada, Oregon, Utah, New Jersey, Massachusetts, New York, Pennsylvania, Florida, Tennessee, Oklahoma, Texas, Minnesota, Illinois, Colorado, Louisiana, Wisconsin, Washington, Indiana, Iowa, North Carolina, Virginia and Maryland) through retail chains, discount stores, catalog outlets, party stores, toy stores, flea markets, fund raisers, and premium offers. Approximately 32,000 pieces of toy jewelry were distributed from August 1992 through July 1993 for approximately $1.00 each. Consumers are urged to take the toy jewelry away from young children immediately and return the jewelry to the place where purchased for a full refund. December 23, 1993 Release # 94-029 PRODUCT: Single light holiday electric candle decoration manufactured for Kmart and sold at Kmart and Builders Square stores during the 1993 Holiday season. The 300,000 Candoliers may be improperly wired and may also have untrimmed plastic around the metal portion of the plug. PROBLEM: The improper wiring could cause a shock. The Candoliers with untrimmed plastic could pose a fire or shock hazard. The product may function properly but should not be used. WHAT TO DO: Do not attempt to repair defective units. Consumers should immediately discontinue using the product. Beginning on Sunday, December 26, customers may take them back to any Kmart store or Builders Square for a full refund. WASHINGTON, DC -- Kmart, CPSC urge consumers who bought single holiday candle lights called "Candoliers" from Kmart during the 1993 holiday season to stop using them because the Candoliers may be improperly wired; they may also have untrimmed plastic around the metal portion of the plug. The improper wiring and untrimmed plastic on the plug could lead to shock, thermal burn or fire. Consumers may return the lights to any Kmart store or Builders Square for a full refund.When sold at Kmart stores, the single Candoliers were packaged in a clear plastic bag with a red cardboard package top reading "Trim A Home." The 10-inch single-light, cream colored candoliers item #04-12-11 sold for 99 cents and have a clear bulb. The identical Candoliers, sold at Builders Square, were packaged two in a single poly bag and sold for $1.97. The code number of the Builders Square model is #112 3397. Kmart reported problems to the CPSC after testing the product in its Quality Assurance and Technical Design facility. Kmart has had one report of a house fire, but no injuries attributed to the Candolier. Kmart has been unable to confirm any additional information or details about the fire incident. Builders Square has received no reports of injury or fire. Both Kmart and Builders Square stores have been instructed to remove the merchandise from the stores' shelves. When plugged in, the Candolier may present a fire hazard due to an excessive amount of plastic around the metal portion of the plug which could prevent a proper fit into the electrical socket. The Candoliers which have been improperly wired will continue to work but may present an electric shock hazard. Approximately 300,000 Candoliers were distributed at Kmart and Builders Square during the 1993 holiday season. Consumers should return all single-light Candoliers to any Kmart or Builders Square store beginning Sunday, 12/26/93 for a full refund. Consumers should note that this is the second year that Kmart has recalled the single candle Candolier. Kmart also recalled last year's Candoliers. November 17, 1993 Release # 94-012 PRODUCT: Graco Carrier Swings and Convert-a-Cradle Swings manufactured between November 1991 and October 1992. The swings have detachable carrier seats with a three-position handle to control the recline position. PROBLEM: In the fully reclined position, infants can push themselves up and over the back of the seat, resulting in falls, loss of consciousness, or restricted breathing. WHAT TO DO: Consumers should call 1-800-345-4109 for a free retrofit kit. Consumers should NOT use the fully recline position of the carrier seat while it is attached to the swing. WASHINGTON, DC -- CPSC, Graco Children's Products, Inc., Elverson, PA, is voluntarily announcing a retrofit program for certain Graco Carrier Swings and Convert-a-Cradle Swings. In the fully reclined position, infants have pushed themselves up and over the back of the seat. Five infants have fallen backward out of the swing onto the floor, and one was found unconscious suspended upside down. No permanent injuries resulted. To protect children from falls from the swings and possible injury, Graco is offering a free, easily installed attachment that will disable the fully recline position. Approximately 208,000 Carrier Swings and Convert-a-Cradle Swings were produced between November 1991 and October 1992 and distributed nationwide. The swings have detachable carrier seats with a three-position handle to control the recline position and retailed for approximately $60 or $100 each. Graco Carrier Swings and Convert-a-Cradle Swings with carrier seats having only two positions do not need a retrofit. December 14, 1993 Release # 94-025 PRODUCT: Approximately 1,500 "Greatland 7' Lighted Aleutian Pine Christmas Trees," and accompanying extension cord sold nationwide by Target Stores from September 1993 through December 2, 1993. The artificial trees feature pre-attached miniature lights. PROBLEM: The extension cords supplied with the lighted trees may overheat or melt. A potential fire hazard in or near the extension cord is present. WHAT TO DO: Consumers should immediately stop using the extension cord and return the cord and tree to their nearest Target store for a full refund. WASHINGTON, DC -- CPSC, Target Stores, Minneapolis, MN, is voluntarily recalling "Greatland 7' Lighted Aleutian Pine Christmas Trees" and accompanying extension cord. Four Target stores have reported overheated or melted extension cords attached to the artificial trees in store displays. Also, two consumers have reported similar electrical problems with their trees. No fires or injuries have been reported to date. The Greatland 7' Lighted Aleutian Pine Christmas Trees and accompanying extension cord were sold nationwide in Target stores from September 1993 to December 2, 1993. The artificial trees, which sold for either $149.99 or $199.99, feature "1,280 pre-attached, evenly spaced clear, or colored miniature lights." Approximately 1,500 trees are believed to be in use by consumers. Consumers should immediately stop use of the extension cord supplied with the trees and return the cord and tree to their nearest Target store for a full refund of the purchase price. CPSC learned of this problem from representatives of Target Stores. The trees were manufactured in China and sold nationwide by Target Stores. December 13, 1993 Release # 94-018 PRODUCT: "Sweda Juice Factory" electric juicers sold at Jamesway and Caldor retailers after April 1, 1992. About 6,000 juicers are believed to be affected. PROBLEM: If the juicer is not reassembled properly after cleaning, during use the plastic upper body may possibly malfunction, causing injury to consumers nearby. WHAT TO DO: Consumers should stop using the juicers and call Telebrands at 1-800-269-1572 for complete instructions on how and where to return the juicer with proof of purchase for full refund of purchase price and postage. WASHINGTON, DC -- CPSC, Telebrands of Roanoke, VA is voluntarily recalling Sweda Juice Factory electric juicers. This recall is limited only to the Sweda Juice Factory electric juicers made in China and sold at Jamesway and Caldor retailers after April 1, 1992. If the juicer is not reassembled properly after cleaning, during use the unit may malfunction causing the plastic upper body to shatter, possibly causing injury to consumers nearby. DECEMBER 13, 1993 Release # 94-020 PRODUCT: Coaster Company's high gloss, tubular metal frame, twin size/full size and twin size/twin size children's bunk beds. Four models are affected: model 2331 twin/full bunk bed, model 2332 twin/full bed with a "C"-shaped frame, and models 2330 and 2335 twin/twin bunk beds. All beds have 13 crossbars on the top and bottom mattress supports, and a center brace on the bottom mattress support. PROBLEM: The bunk beds may collapse during use. WHAT TO DO: Stop using affected beds immediately. To determine if you have a Coasterbed, contact the retailer for information on the importer or manufacturer of the bed. Consumers with affected Coaster beds should call the company toll-free at (800) 221-9699 for a free repair kit or replacement of the mattress support frame or bed. WASHINGTON, DC - CPSC, Coaster Company of America, Santa Fe Springs, CA, is voluntarily offering a replacement or repair of certain children's tubular metal frame bunk beds. The upper level can fall causing the bunk bed to collapse. Collapse of the top bunk could result in injury to an occupant on the bed. The Coaster Company has received 8 reports of these beds breaking, including one report of an injury. Approximately 13,500 affected bunk beds were distributed from May 1992 through December 1992 in retail stores nationwide. The beds sold for approximately $139 to $199 each (excluding the mattresses). The affected beds distributed in 1992 include the model 2331 twin/full bunk bed, model 2332 twin/full bed with a "C"-shaped frame, and models 2330 and 2335 twin/twin bunk beds. The beds have metal frames made of hollow painted tubes. The glossy painted frame will be either red, blue or white. Coaster Com pany beds also have 13 crossbars on both the top and bottom mattress supports, and a center brace on the bottom mattress support. Consumers should immediately stop using affected beds as bunk beds. To determine if you have a Coaster bed, contact the retailer for information on the importer or manufacturer of the bed. Consumers with affected Coaster beds should call the company toll- free at (800) 221-9699 to obtain either a repair kit, replacement of the mattress support frames or replacement of the bed. The CPSC is investigating numerous other importers and manufacturers of metal bunk beds that may present a similar risk of collapsing. To date, firms have received over 300 complaints of defective bunk beds industry-wide. CPSC urges all consumers to inspect all eight mattress support corners of all metal bunk beds for breaks or cracks in the paint and metal around the weld. Consumers should stop using damaged bunk beds and contact the retailer, manufacturer, or importer. To assist CPSC in its continuing bunk bed investigations, consumers are urged to call the CPSC toll-free hotline weekdays at (800) 638-2772 to report incidents or injuries involving metal bunk beds. The products are made of wooden blue, pink, purple and yellow colored beads with white wooden rabbits and orange wooden carrots strung on elastic strings. Approximately 26,000 necklaces and 39,000 bracelets were sold at retail stores nationwide from December 1992 through October 1993. PROBLEM: The paint on the beads contain levels of lead that exceed government standards, presenting a safety hazard to children. WHAT TO DO: Consumers should immediately take the necklaces and bracelets away from children and return them to the store where they were purchased for a full refund. Testing by the company revealed that the levels of lead in the products' paint exceeds government standards, presenting a safety hazard to children. Even though CPSC and the company know of no poisoning incidents, this recall is being conducted to prevent the possibility of poisoning. Both the necklaces and bracelets are constructed of wooden blue, pink, purple, and yellow colored beads with white wooden rabbits and orange wooden carrots strung on elastic strings. Each product is packaged in an 3-inch by 3-inch cardboard box with a transparent plastic lid. The boxes are not labeled except for a small adhesive tag attached to an elastic string which says "Made in China" and bears the Cannon Falls copyright. The products were imported from China and sold at retail stores nationwide from December 1992 through October 1993. The necklaces sold for approximately $3.00 each and bracelets sold for approximately $1.00 each. Consumers are urged to immediately take the necklaces and bracelets away from children and return them to the store where they were purchased for a full refund. WASHINGTON, DC -- CPSC and Casablanca Fan Company, City of Industry, CA are jointly announcing the voluntary recall of 3,264,000 Casablanca brand ceiling fans manufactured from 1981 through 1993. The fans can separate from the canopy on which they are mounted to the ceiling. If a fan separates from its canopy, it will fall and possibly injure a bystander. In addition, a fallen fan may leave exposed wires in the ceiling possibly posing an electric shock hazard. Casablanca Fan Company has received approximately 50 reports of fans falling from ceiling mountings. One incident has been reported in which a consumer received an injury. CPSC learned of this problem when Casablanca notified the Commission of its intention to initiate a recall. The affected fans have a ball and canopy mounting system and were manufactured and sold nationwide from January 1981 through September 1993. The retail prices for the fans vary by model and time period sold. The prices generally range from $200 to $2,500. A ceiling fan owner can identify the recalled fan by looking at the metal nameplate located on the exterior of every Casablanca fan. A fan subject to the recall will say "Casablanca" on the nameplate. In addition, the ball and canopy mount is visible from the bottom of the fan. Also, the second letter of the serial number on the nameplate will be A, B, C, O, P, R, S, T, U, V, W, X, or Y. This recall does not cover Pasadena brand fans. Casablanca has designed a retrofit part to be installed by the consumer to prevent the fan from falling from its mounting. Step by step instructions will be provided with the part. DECEMBER 1, 1993 Release # 94-015 PRODUCT: Holiday electric candle decorations with either three bulbs or a single bulb, manufactured by Willis Mfg. and sold at Kmart during the 1992 Christmas season. The affected single and three candle units are improperly wired. The three candle units may also have improperly aligned metal contact tabs. PROBLEM: The improper wiring could cause a shock hazard. The improperly aligned contact tabs will immediately short circuit and present a potential fire, thermal burn or shock hazard to users. WHAT TO DO: Do not attempt to repair defective units. Take them back to any Kmart store for a free refund or replacement.. WASHINGTON, DC -- Kmart Corporation, of Troy, MI, and the U.S. Consumer Product Safety Commission (CPSC) urge consumers who bought holiday candle lights called "Candoliers" from Kmart during the 1992 holiday season to return the lights to any Kmart store for a full refund or replacement. The Candoliers may be improperly wired; three-light Candoliers may also have improperly aligned metal contact tabs. The improper wiring and alignment could lead to shock, thermal burn, or fire. The Candoliers came in single and three-lights. The single-light Candoliers were packaged in a plastic bag with a cardboard package top reading "Single Candolier." The candle lights are about 11 inches high. They have an off-white plastic stem and base, and either a clear or an orange bulb. Single- light Candoliers sold at Kmart for $.99 each. Three-light Candoliers were packaged in a cardboard box. The stems and base are off-white molded plastic. The outer two candles are six inches high and the center candle is seven inches high. The three-light Candoliers were packaged with either clear or orange bulbs. They sold at Kmart for $4.59 each. To avoid injury, Kmart is recalling all affected products. Candoliers with improperly aligned metal contact tabs will immediately short circuit. If the household branch circuit breaker or fuse does not interrupt (trip) the circuit, the short circuit could create a fire, thermal burn, or electric shock hazard for anyone nearby or for anyone holding the product or its power cord. The Candoliers which have been improperly wired will continue to work but may present an electric shock hazard. Even if you have used your Candolier without it short- circuiting, you should discontinue use and return the product to any Kmart to prevent potential injury. All Candoliers sold during the 1992 holiday season are being recalled. Kmart reported two incidents to CPSC of Candoliers failing. Both incidents involved three-light Candoliers. One incident resulted in slight burns to a consumer's hand when the cord caught fire. Approximately 13,000 NEC PC-17-01 and PC-17-02 Laptop Computers were sold in the United States and Canada from December 1988 to April 1990. Model numbers can be found on the bottom of the computer. Only the portable PC-17-01 and PC-17-02 models are involved. No other NEC computers are involved. NECT is aware of seven incidents of smoke and/or fire which occurred during battery recharging. Although there are no known injuries to date, NECT and CPSC view this as an important matter and are making every effort to insure consumer safety. The grills were sold between September 1992 and June 1993. Affected grills have a "1,000-watt" designation and the number "E992" stamped on the portion of the grill's heating element that attaches to the variable temperature control. PROBLEM: The grill can cause grease fires if users repeatedly grill food containing a high fat content without emptying accumulated grease between batches. The company has received four returns to date due to smoking of the indoor grills; no grease fires or injuries have been reported. WHAT TO DO: Consumers who purchased the 1,000-watt indoor grills are urged to discontinue use and return them to Sunbeam-Oster for a replacement or a refund (at the consumer's option). WASHINGTON, DC -- Sunbeam-Oster Household Products, Schaumburg, IL, CPSC, today announced the voluntary recall of its discontinued 1,000-watt electric indoor grill with the gray Nu-Stone plastic base, which can cause grease fires under certain circumstances. The affected grills were sold between September 1992 and June 1993. The recall does not involve any other Sunbeam or Oster grills or products. The recall is limited to the 1,000-watt variable temperature electric indoor grills with the gray Nu-Stone plastic base and patented "Griffo" porcelain grid, model numbers 4757 (Sunbeam brand) and 4772 (Oster brand). The Sunbeam brand sold for about $30.00 and the Oster brand sold for about $45.00. Sunbeam-Oster no longer manufactures the 1000-watt indoor grill with gray Nu-Stone base. Current versions are sold with an 800-watt element. The company believes that while the product is not defective, repeatedly grilling food containing a high fat content without emptying accumulated grease between batches could cause a fire. Each bottle of Vitabots has 90 light brown colored tablets; each tablet has five milligrams of iron. The supplements were sold nationwide in retail health food and nutrition stores between July 1, 1992 and July 31, 1993. PROBLEM: The product was not sold in child-resistant packaging as required by the Poison Prevention Packaging Act. The lack of child-resistant packaging means that a child could easily access and accidentally swallow the iron tablets. Each bottle of the product contains enough iron that, if consumed by a child at one time, it could cause serious illness requiring hospitalization. WHAT TO DO: Immediately return the unused portions of any bottles of the recalled Vitabots to the store where purchased for a refund of the purchase price. This iron-containing vitamin and mineral supplement was not put in child-resistant packaging as required by the Poison Prevention Packaging Act. Each bottle of the recalled product contains enough iron that, if consumed by a child at one time, it could cause serious illness requiring hospitalization. The recalled Vitabots Multiple Vitamins and Minerals (for children over 4 years) were sold in amber-colored glass bottles with white screw-on metal caps. Each bottle contains 90 light brown colored tablets with five milligrams (mg) of iron per tablet. The product, wrongly labeled "Non-Toxic," is promoted as a dietary supplement for children over four years of age. The bottle's label also carries the statement "Manufactured by SYNERGY PLUS, Union, N.J. 07083 USA." This recall does not include any other iron-containing products manufactured by International Vitamin Corp. Approximately 2,500 bottles of the recalled iron products were sold to consumers through retail health food and nutrition stores nationwide between July 1, 1992 and July 31, 1993 for approximately $9.00 - $10.00 per bottle. Consumers should immediately return the unused portions of any bottles of the recalled Vitabots to the store where purchased for a refund of the purchase price or call International Vitamin Corp. at 201-371-7300 for instructions on where to send the recalled products for a refund, including shipping costs. York manufactured approximately 10,000 HeatPipe furnaces between 1984 and 1988 under the brand names Borg-Warner, Luxaire, Fraser-Johnson, and Moncrief. These mid-efficiency furnaces can be identified by the following model numbers: Luxaire/Frasier- Johnson/Moncrief (models PAUT-LD08N073, PAUT-LD12N073, PAUT- LD12N105, and PAUT-LD16N105); Borg-Warner/York (models P1NUD08N06301, P1NUD12N06301, P1NUD12N08901, and P1NUD16NO8901). York estimates approximately 4,000 HeatPipe furnaces remain in use. Under York's warranty program, HeatPipe furnace owners may obtain a free replacement furnace of similar capacity and efficiency to their HeatPipe. Replacement of the HeatPipe furnaces will require professional installation and replacement of some or all of the furnaces' venting components. York will provide a $200 allowance toward the cost of installing the replacement furnace. CPSC urges anyone with a HeatPipe furnace to participate in the HeatPipe replacement program. This inspection should include checking the furnace's vent system for corrosion or openings through which carbon monoxide could leak, as well as checking to make sure the furnace is operating properly. Only a qualified service technician should perform repairs or maintenance. CPSC recommends that consumers also purchase and install carbon monoxide detectors that meet the requirements of Underwriters Laboratories (UL) Standard 2034.
