Plastic Toy Storage Chests Recalled by IRIS U.S.A. Announce Recall of Plastic Toy Storage Chests WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), IRIS U.S.A. Inc., of Pleasant Prairie, Wis., is recalling about 100,000 plastic chests. These chests, that were originally marketed for the storage of toys, lack a lid-support device, do not have adequate ventilation when the lid is closed and have latches that secure the lids. Children could be entrapped and suffocate inside the chests. CPSC and IRIS U.S.A. are not aware of any injuries involving these chests. This recall is being conducted to prevent the possibility of injury. These clear, plastic chests have either red or blue lids, and were sold under three model numbers: Model #WT-80 is 19.75 inches long, 21.875 inches wide and 18.75 inches high. Model #WT-120 is 19.25 inches long, 30.75 inches wide and 18.75 inches high. Model #WT-175 is 19.25 inches long, 30.75 inches wide and 28.25 inches high. The model numbers are listed on the instruction sheet and on the label. The labels also show the chest being used for the storage of toys, and contain the warning, "CAUTION: Do Not Sit On Lid. Do Not Climb In Trunk." Retail and department stores, such as Target, sold these chests nationwide since January 1996 for about $25. Consumers should immediately prevent children from having access to these chests, and stop using them for toy storage. Consumers in households with young children can return the chests to the store where purchased for a refund. Announce Recall of Icy Rider TM Toboggan WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Rubbermaid Inc., of Wooster Ohio, is recalling about 60,800 Icy Rider TM Toboggans. The flexible, bowed front of the toboggan can break away while in use, causing loss of control and injury. Rubbermaid is aware of six reports of the fronts of these toboggans breaking off, resulting in two injuries. These injuries were a broken leg suffered by an adult, and facial bruises and lacerations suffered by a 4-year-old girl when the broken piece hit her in the face. The Icy Rider TM Toboggan, Model #2108, is made of purple plastic, and has a yellow rope strung around the sides. The toboggan is made from two separate pieces which fit together, and are secured with three yellow bolts. It measures 1.5 feet wide and 5.5 feet long when assembled, and has a red, Rubbermaid logo molded into the bowed front. Toy and warehouse club stores nationwide, including Toys R Us, Sam's Club and BJ Warehouse Clubs, sold these toboggans beginning in September 1997 for about $50. Consumers should stop using these toboggans immediately. The saw's lower blade guard can fail to fully close during use, leaving the blade exposed and presenting a risk of laceration. DeWalt is aware of 15 reports of the saw's lower blade guard failing to fully close. Of these incidents, there have been eight reports of lacerations, with at least three cases requiring stitches. These are DeWalt 7 1/4-inch framing saws have model number DW378G, which is located on a black name plate on the side of the saw. These saws have date codes 9813F through 9820F, located on the bottom of the saw below the power cord. The saws are yellow with black accents and have a silver-colored blade guard. "DEWALT" is written on the blade guard. Major home centers and hardware stores, as well as local hardware stores, sold these saws nationwide from May 1998 through September 1998 for between $149 and $175. Consumers should immediately stop using these framing saws and take them to a DeWalt/Black & Decker service center for a free repair. Saws with date codes ending with a letter "X" have already been repaired.
The Bilt-Rite, Gerry, Graco, Kolcraft, Playskool, Pride-Trimble and Strolee playpens are drop-sided, mesh playpens that fold in half for storage. Evenflo's Houdini playpen is a portable, mesh playpen that folds up for storage and travel. All Houdinis manufactured between 1994 and 1996 are subject to this recall even if they have plastic covers over the rivets. The rivet covers on these playpens must be replaced because they are defective and can crack, break or come off exposing the rivet and presenting a risk of strangulation.

An electrical short can occur in the treadmills' motors, presenting electrocution, shock and fire hazards to consumers. CPSC and Pacific Scientific are not aware of any injuries involving these treadmills. This recall is being conducted to prevent the possibility of injury. The treadmills being recalled have been manufactured as various models by eight companies. Pacific Scientific Co. manufactured the motors used in the recalled treadmills. These cribs were recalled in 1993, yet two children died in the cribs this summer, a New Jersey infant and a Chicago toddler. The cribs, which were manufactured by Kolcraft from 1990 through 1992, have rotating top rail hinges. These top rails can collapse, entrapping children and suffocating them. In all, six children have died in Playskool Travel-Lite portable cribs. Despite a nationwide effort over the past five years to alert the public about this recall, some of the 11,000 cribs originally sold could still be in use. To get these products back, Kolcraft and Playskool are doubling the reward for each returned crib from $60 to $120. In addition to the $120 reward, Kolcraft and Playskool, a subsidiary of Hasbro, are taking further steps to get the word out, including mailings to daycare centers, notices in new products and posting of details regarding the reward on Hasbro's web site at www.hasbro.com under Consumer Affairs Safety Message. The Playskool Travel-Lite portable cribs have two nylon mesh sides and two blue solid plastic ends. "Playskool" appears in white letters on a red background on each end. The portable crib folds in the center for storage and handling. Consumers who own these cribs should stop using them immediately. Anyone who sees one in a day care center, at a baby-sitters, in the home of a friend or relative, or at a garage sale or secondhand store, should inform the caregiver or seller about the recall and reward. CPSC, Manufacturers Announce Recall of Wooden Bunk Beds WASHINGTON, DC - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), five bunk bed manufacturers are recalling more than 37,000 wooden bunk beds. The beds have spaces on the top bunk that present potential entrapment and strangulation hazards to young children. The spaces can be large enough for a child's body to pass through, but small enough to entrap a child's head. CPSC is not aware of any deaths associated with the beds being recalled by these five manufacturers. However, since 1990, 39 children have died from becoming entrapped in wooden or metal bunk beds with similar spaces on the top bunk. Thirty-seven of those children were 3 years old or younger. Since 1994, more than half a million hazardous bunk beds have been recalled. The American Society for Testing and Materials (ASTM) voluntary standard for bunk beds requires that, in addition to having guardrails on both sides of the top bunk, any spaces between the guardrail and bed frame, and in the head and foot boards on the top bunk, be less than 3.5 inches. The manufacturers are participating in this recall to improve the safety of the beds and to meet the requirements of the voluntary standard. The manufacturers also will send consumers permanent labels to be placed on the beds to warn not to put children under the age of 6 on the upper bunk. CPSC and the manufacturers continue to urge parents and caregivers not to put children under the age of 6 on the top bunk. Consumers should stop using the recalled beds immediately and call the company for a replacement guardrail, retrofit kit or instructions that will eliminate the potential entrapment hazard. The recalled beds were sold through flea markets and in specialty and furniture stores, including Workbench furniture store. The power cords on these vacuums extend out of the very top of the handle, and the cord is often used as an extension of the handle. The power cord can break inside the handle or the casing around the cord can break, and cause shocks or burns to the user's hand. Even when there is no apparent breaking of the casing around the cord, these vacuums can still cause injuries. Electrolux is aware of at least 75 reports of the cords on these vacuums breaking or the casing around the cord exposing wires. About half of these incidents reportedly resulted in burns or shocks to users. Of these reports, there were 11 serious injuries which included consumers suffering second or third degree burns, shocks, or a combination of these injuries. The Ponytail vacuums were sold under the following names: Genesis Prolux S/R Genesis LX Prolux Warehouse Genesis LXE Regency Prolux Special Edition Prolux Plus Only Electrolux upright vacuums with the cord coming out of the very top of the handle are included in this recall. "Electrolux" and the model name are written on each vacuum. Most of these vacuums were sold through direct sales and retail stores, including Sam's Club, Service Merchandise and Macy's, between May 1991 and September 1993 for between $150 and $550, depending on the model and accessories. Some vacuums were sold directly to corporate customers. Consumers should immediately stop using these vacuums, and return them to an Electrolux store for a free repair. The handle on the vacuum will be replaced with a new model handle. Announce Recall of Gas Grills With Side Burners WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Sunbeam Products Inc., of Neosho Mo., is recalling for repair approximately 80,000 Grillmaster gas grills with side burners. The side burner's propane gas hose can twist up toward the aluminum casting of the grill, causing overheating and melting of the hose. Gas leakage or a fire could result from the hose damage. Sunbeam Products received one complaint from a consumer who requested a replacement hose due to damage caused from the heat of the grill casting. There have been no reports of injuries associated with these grills. The recalled grills are black and have a side burner for cooking, similar to a gas-range stove top. The model numbers of the grills are: 56GR8, 560R8, 562D1, 562D9, GC548517, GG546517, GS544417, GS544717, GS546517, GG546537 and GG547517. The complete model numbers can be found on the identification label on the back of the grill, and on the assembly instructions and parts list. The word, "GRILLMASTER" is written on the front of these grills. Mass merchandise stores, such as Bradlees, Caldor, the Home Depot, Service Merchandise and Wal-Mart sold the grills nationwide from 1996 through July 1998 for about $200 to $250. Consumers should stop using the recalled grills immediately and call Sunbeam toll-free at (800) 200-2300 7:00 am to 5:00 pm, Monday through Friday, to receive a free retrofit kit. The retrofit kit contains a wire bracket guard and instructions for easy installation that will correctly position the side burner's propane gas hose. In March 1998, Sunbeam Products recalled grills with model numbers GG 461 EPB, GG 560 EPB, HG 560 EPB and GG 560 EPBS because of a similar hose placement hazard.

Announce Recall to Repair Electric Screwdrivers WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), DeWalt Industrial Tool Co., of Hampstead, Md., is recalling to repair about 660,000 corded electric screwdrivers. The screwdrivers' wiring design can result in the user being shocked. DeWalt has received reports of 40 of these screwdrivers causing shocks during use. There have been three reports of injuries resulting from these shocks. The screwdrivers were sold under the DeWalt , GRABBER , and Black & Decker Industry & Construction brandnames. The DeWalt , model numbers are DW251, DW252, DW257, DW260K, DW265, DW266, DW267, DW268, DW268G, DW269, DW272, DW274, DW274-220, DW274W, DW275QD (Quik Drive ), DW276, DW281, DW282 and DW284. The GRABBER model numbers are 4060 and 4063. The Black & Decker Industry & Construction model numbers are 2121, 2125 and 2141. Only numeric date codes from 9601 to 9833 are included in this recall. The date code is followed by various letters. Those date codes ending with an "R" have already been repaired. The model number and date code are located on the name plate, which is on the right hand side of the screwdriver when viewed from the back. The DeWalt and Quik Drive corded screwdrivers have a yellow housing with black accents. The GRABBER models have a black housing. The Black & Decker Industry & Construction models have a charcoal-gray housing. Major chain home centers and hardware stores, as well as local hardware stores, nationwide sold these screwdrivers from November 1996 through September 1998 for about $69 to $169, depending on the model. Consumers should stop using these screwdrivers immediately and take them to the nearest DeWalt /Black & Decker service center for a free repair. The basketball shoes have a small, decorative metal rivet located on the outside of the shoe that can bend and form a sharp edge. NIKE has received about 35 reports of high school and college age basketball players receiving cuts to the lower legs, including two reports of players requiring stitches. The NIKE Air Face Up men's basketball shoes being recalled are model numbers 830080, 139358 and 630327. The model number is located on the label on the inside tongue of the shoe, above the UPC code. The shoe is a mid-cut men's basketball shoe that comes in multiple color combinations. The shoe has a visible window, about 2 inches long, in the heel of the shoe. The words "NIKE AIR" are on the outside tongue of the shoe. The word "TEAM" is on the back of the shoe. Major athletic shoe stores and department stores nationwide sold the NIKE Air Face Up basketball shoes from June 1998 through October 1998 for about $85. Consumers should stop using these basketball shoes and should immediately return them to the store where purchased to receive a substitute product or a store credit. November 3, 1988 Release # 88-093 "Love Me Tender" Dolls Recalled By Tonka Products WASHINGTON-Tonka Products, Division of Tonka Corporation, is voluntarily recalling its "Love Me Tender" dolls because the plastic eyes may come off and could present a choking hazard to young children. Tonka instituted the recall in cooperation with the U.S. Consumer Product Safety Commission (CPSC). The company acknowledged having received 76 reports from consumers about model #8900 and #8901 dolls where the eyes had come off. While no injuries have been reported, the company said it is recalling the dolls to prevent any ingestion of small parts. More that 100,000 of these dolls were sold nationwide since July 1987 for $18-$30. Speed Queen will supply all consumers with a permanent guard to install at the base of the Contempo Mate washer-dryer, which are equipped with caster-rollers. Casters permit consumers to roll the appliance to the water and drain source for clothes washing, and then to return the unit to another area for convenient storage. FOR IMMEDIATE RELEASE Originally issued November 1, 1988, Revised June 21, 2004 Release # 88-9l Siemens Announces Free I-T-E Circuit Breaker Box Inspection Program WASHINGTON, DC - In cooperation with the U.S. Consumer Product Safety Commission, the Circuit Protection Division of Siemens Energy & Automation, Inc. of Alpharetta, Georgia is voluntarily offering free inspection and correction of certain of its I-T-E brand circuit-breaker boxes (load centers) installed between September 1986 and May 1987. Siemens is extending a program begun in November 1986 when it discovered that some of the circuit-breaker boxes were manufactured using components of an incorrect material, which could cause overheating and malfunction. The involved ITE circuit-breaker boxes, or "load centers" can be identified by a five digit code marked on the top of the wiring diagram label inside the door which is opened to reset the circuit breakers. The specific date codes involved are as follows: H2586-H3186 (H = August, days 25 through 31, 86 = year) J0186-J3086 (J = September, days I through 30, 86 = year) K0186-K2486 (K = October, days I through 24, 86 = year) Example: A product manufactured on September 24, 1986 bears the code J2486. Anyone locating such a load center which does not have an "inspected lugs" sticker should call Siemens at 1-800-241-3138 to arrange for free professional inspection of the unit. If the wiring diagram label has an "inspected lugs" stamp, the load center has already been inspected and corrected if necessary.

Inc. Announce Recall of Children's Vampire Capes and Witch Brooms WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Rubie's Costume Co. Inc., of Richmond Hill, N.Y., is recalling about 6,000 children's vampire capes and about 6,000 children's witch brooms. The capes and the brooms, which fail to meet federal mandatory standards for flammability, could ignite readily and present a serious risk of burn injuries to children. CPSC and Rubie's Costume Co. Inc. are not aware of any injuries involving these capes or brooms. This recall is being conducted to prevent the possibility of injury. The vampire capes are made of black vinyl. The edges of the cape are scalloped. The capes are packaged in a plastic sleeve with a cardboard insert, which reads in part, "Child's Vinyl Vampire Cape...No. 50282...Made in China." The Rubies logo and a picture of a boy wearing the cape are also on the insert. The witch brooms are 27 inches long with a bamboo handle and brush made of twigs. The brooms were sold with a sticker around the handle, which reads in part, "Child's Authentic Witch Broom...Style No. 85." The Rubies logo is also on the sticker. Toy, party, costume, discount and drug stores nationwide sold the capes and brooms for about $2 each from September 1997 to October 1998. Consumers should not let their children wear the vampire capes or use the 27-inch witch brooms this Halloween. Consumers should return the products to the stores where purchased for a refund. The vehicles' electrical components can overheat and cause fires. Children can suffer injuries from fires and house fires can occur. Additionally, wiring problems can prevent the vehicles from stopping. CPSC and Fisher-Price have received approximately 700 reports of electrical components failing and overheating while the cars and trucks were being ridden, charged, parked or stored. About 150 fires have been reported. Nine children suffered minor burns to the hands, legs, or feet; and up to $300,000 in property damage to 22 houses and garages have been reported. There have also been 71 reports of vehicles not stopping. Six children suffered bruises, scratches or bumps when their vehicles hit a car, truck, pole, window or fence. Power Wheels Service Centers will repair the vehicles. These service centers will install the new parts free of charge and give all recalled vehicles a free safety check-up and tune-up. The recalled Power Wheels cars and trucks have been sold under nearly 100 model names. The Power Wheels logo and the model name are on each vehicle. All models with two batteries are recalled, and certain models with one battery are recalled. The company will help consumers identify if their model is part of the recall. Power Wheels cars and trucks are intended for children 2 to 7 years old, and the vehicles' speed ranges from 1 to 5 mph, depending on the model. Toy and mass merchandise stores nationwide sold the cars and trucks since 1984 for about $70 to $300. Consumers should remove the vehicles' batteries right away and not let children use these Power Wheels vehicles until the repair has been made at the service center. To schedule the repair, consumers should call Power Wheels at (800) 977-7800 anytime. Again, the company will help consumers identify if their vehicles are part of the recall. Power Wheels sold after October 1998 are not involved in this recall.
