November 17, 1993 Release # 94-012 PRODUCT: Graco Carrier Swings and Convert-a-Cradle Swings manufactured between November 1991 and October 1992. The swings have detachable carrier seats with a three-position handle to control the recline position. PROBLEM: In the fully reclined position, infants can push themselves up and over the back of the seat, resulting in falls, loss of consciousness, or restricted breathing. WHAT TO DO: Consumers should call 1-800-345-4109 for a free retrofit kit. Consumers should NOT use the fully recline position of the carrier seat while it is attached to the swing. WASHINGTON, DC -- CPSC, Graco Children's Products, Inc., Elverson, PA, is voluntarily announcing a retrofit program for certain Graco Carrier Swings and Convert-a-Cradle Swings. In the fully reclined position, infants have pushed themselves up and over the back of the seat. Five infants have fallen backward out of the swing onto the floor, and one was found unconscious suspended upside down. No permanent injuries resulted. To protect children from falls from the swings and possible injury, Graco is offering a free, easily installed attachment that will disable the fully recline position. Approximately 208,000 Carrier Swings and Convert-a-Cradle Swings were produced between November 1991 and October 1992 and distributed nationwide. The swings have detachable carrier seats with a three-position handle to control the recline position and retailed for approximately $60 or $100 each. Graco Carrier Swings and Convert-a-Cradle Swings with carrier seats having only two positions do not need a retrofit. All owners of Graco brand swings with detachable infant carriers should immediately check to see if the carrier seat handle adjusts to three positions. December 14, 1993 Release # 94-025 PRODUCT: Approximately 1,500 "Greatland 7' Lighted Aleutian Pine Christmas Trees," and accompanying extension cord sold nationwide by Target Stores from September 1993 through December 2, 1993. The artificial trees feature pre-attached miniature lights. PROBLEM: The extension cords supplied with the lighted trees may overheat or melt. A potential fire hazard in or near the extension cord is present. WHAT TO DO: Consumers should immediately stop using the extension cord and return the cord and tree to their nearest Target store for a full refund. WASHINGTON, DC -- CPSC, Target Stores, Minneapolis, MN, is voluntarily recalling "Greatland 7' Lighted Aleutian Pine Christmas Trees" and accompanying extension cord. Four Target stores have reported overheated or melted extension cords attached to the artificial trees in store displays. Also, two consumers have reported similar electrical problems with their trees. No fires or injuries have been reported to date. The Greatland 7' Lighted Aleutian Pine Christmas Trees and accompanying extension cord were sold nationwide in Target stores from September 1993 to December 2, 1993. The artificial trees, which sold for either $149.99 or $199.99, feature "1,280 pre-attached, evenly spaced clear, or colored miniature lights." Approximately 1,500 trees are believed to be in use by consumers. Consumers should immediately stop use of the extension cord supplied with the trees and return the cord and tree to their nearest Target store for a full refund of the purchase price. CPSC learned of this problem from representatives of Target Stores. The trees were manufactured in China and sold nationwide by Target Stores. DECEMBER 13, 1993 Release # 94-016 PRODUCT: About 23,750 non-refillable "Good Time Troll" cigarette lighters distributed by New York Lighter Company. The lighters sold at convenience stores nationwide from January through July 1993. They are printed with one of four troll designs: "Party Troll" is holding a drink in each hand, "Rockin' Troll" is playing a guitar, "King Troll" is smoking a cigar, and "Lucky Troll" is holding a four-leaf clover. The flue of the lighter is labeled "THAILAND" and "C-LITE." PROBLEM: The "Good Time Troll" lighters, printed with troll characters, are very attractive to children and may invite play. If a child plays with the lighter, a fire and/or serious burn injuries may result. WHAT TO DO: Put the lighters out of reach of children. Then, call 1-800-626-4732 to receive special prepaid packaging and complete instructions for returning the lighters. When the lighter is received, a free gift will be sent to the consumer. December 13, 1993 Release # 94-022 PRODUCT: Approximately 3,264,000 Casablanca brand ceiling fans manufactured from 1981 through 1993. PROBLEM: The fan may fall from its ceiling mounting. WHAT TO DO: Consumers should stop using the fan and not allow anyone underneath the fan. Call (888) 227-2178 to receive a retrofit part to be installed by the consumer to prevent the fan from falling. In the event that a fan falls, consumers should immediately turn off the circuit breaker due to the possibility of exposed wires. WASHINGTON, DC -- CPSC and Casablanca Fan Company, City of Industry, CA are jointly announcing the voluntary recall of 3,264,000 Casablanca brand ceiling fans manufactured from 1981 through 1993. The fans can separate from the canopy on which they are mounted to the ceiling. If a fan separates from its canopy, it will fall and possibly injure a bystander. In addition, a fallen fan may leave exposed wires in the ceiling possibly posing an electric shock hazard. Casablanca Fan Company has received approximately 50 reports of fans falling from ceiling mountings. One incident has been reported in which a consumer received an injury. CPSC learned of this problem when Casablanca notified the Commission of its intention to initiate a recall. The affected fans have a ball and canopy mounting system and were manufactured and sold nationwide from January 1981 through September 1993. The retail prices for the fans vary by model and time period sold. The prices generally range from $200 to $2,500. A ceiling fan owner can identify the recalled fan by looking at the metal nameplate located on the exterior of every Casablanca fan. A fan subject to the recall will say "Casablanca" on the nameplate. In addition, the ball and canopy mount is visible from the bottom of the fan. Also, the second letter of the serial number on the nameplate will be A, B, C, O, P, R, S, T, U, V, W, X, or Y. This recall does not cover Pasadena brand fans. Casablanca has designed a retrofit part to be installed by the consumer to prevent the fan from falling from its mounting. Step by step instructions will be provided with the part. In addition, the consumer may contact Casablanca's 24 hotline to obtain a retrofit kit free of charge and for assistance related to installation at (888) 227-2178. Consumers who are unable to install the part may contact Casablanca to request assistance in arranging for free installation of the retrofit part on fans manufactured during or after March 1991. Casablanca fans manufactured before March 1991 were sold by another company. Owners of these fans who require installation assistance should contact Casablanca's toll free hotline to discuss whether free installation is available. Casablanca will decide on a case by case basis. However, the retrofit part and easy to follow step by step instructions will be provided free of charge to all Casablanca fan owners. A consumer who owns an affected fan should not use it until the retrofit part is installed. In the meantime, consumers should not allow anyone underneath the fan. In the event that fan falls, consumers should immediately turn off the fan's circuit in the circuit box due to the likely possibility of exposed wires. November 1, 1993 Release # 94-004a PRODUCT: Approximately 4,000 Borg-Warner, Luxaire, Fraser- Johnson, and Moncrief "Heatpipe" furnaces manufactured and sold by York International Corporation between 1984 and 1988. PROBLEM: Parts of the furnace and vent system may fail prematurely. Failure of vent components could allow potentially hazardous carbon monoxide (CO) gas to leak into the home. WHAT TO DO: Owners of affected furnaces should call York at 1-877-874-7378 to obtain a free replacement furnace. York is providing a $200 allowance toward the cost of installing the replacement furnace. WASHINGTON, DC -- CPSC today announced that York International Corporation of York, PA has voluntarily offered to replace all Borg-Warner, Luxaire, Fraser-Johnson, and Moncrief "HeatPipe" furnaces. York initiated this warranty enhancement program because certain parts of the furnace and vent system may fail prematurely. Failure of certain furnace and vent components could allow potentially hazardous carbon monoxide (CO) gas to leak into the home. York manufactured approximately 10,000 HeatPipe furnaces between 1984 and 1988 under the brand names Borg-Warner, Luxaire, Fraser-Johnson, and Moncrief. These mid-efficiency furnaces can be identified by the following model numbers: Luxaire/Frasier- Johnson/Moncrief (models PAUT-LD08N073, PAUT-LD12N073, PAUT- LD12N105, and PAUT-LD16N105); Borg-Warner/York (models P1NUD08N06301, P1NUD12N06301, P1NUD12N08901, and P1NUD16NO8901). York estimates approximately 4,000 HeatPipe furnaces remain in use. Under York's warranty program, HeatPipe furnace owners may obtain a free replacement furnace of similar capacity and efficiency to their HeatPipe. Replacement of the HeatPipe furnaces will require professional installation and replacement of some or all of the furnaces' venting components. York will provide a $200 allowance toward the cost of installing the replacement furnace. CPSC urges anyone with a HeatPipe furnace to participate in the HeatPipe replacement program. Consumers can call York at 1-877-874-7378 or contact an authorized York, Fraser-Johnson, or Luxaire dealer to obtain a new replacement furnace. York is sending a letter to all known HeatPipe owners explaining the warranty program. Additionally, CPSC advises all owners of furnaces and fuel- burning appliances to have a competent service technician inspect their appliances and furnaces before the heating season begins. This inspection should include checking the furnace's vent system for corrosion or openings through which carbon monoxide could leak, as well as checking to make sure the furnace is operating properly. Only a qualified service technician should perform repairs or maintenance. CPSC recommends that consumers also purchase and install carbon monoxide detectors that meet the requirements of Underwriters Laboratories (UL) Standard 2034. These detectors warn consumers before hazardous levels of carbon monoxide are present. CO is a colorless, odorless gas that can cause serious illness or death. September 1, 1993 Release # 93-106 : Modular playground equipment with solid metal decks manufactured by Miracle Recreation Equipment Company and installed from 1950 to the present at parks, schools and fast-food restaurants. PROBLEM: If the decks are located in the direct sun, the metal can heat to temperatures in excess of 120 degrees and young children who climb or sit on the hot decks can receive severe burns. The U.S. Consumer Product Safety Commission knows of 201 children whose hands, legs, or buttocks were seriously burned on the hot metal play equipment. WHAT TO DO: If you own or think you recognize one of these decks, call Miracle at 1-800-523-4202 to identify the piece of equipment and to receive a repair kit designed to cover the metal deck. WASHINGTON, DC -- Miracle Recreation Equipment Company, Monett, MO in cooperation with CPSC is offering "deck-coverings" to owners of its modular playground equipment with solid metal decks, to prevent the metal from becoming hot and causing serious burns. Young children can receive severe burns from contact with the bare metal decks, if the decks are located in the direct sun. The solid metal decks on modular play equipment in direct sun can heat to temperatures in excess of 120 degrees, even on a 70-80 degree day. The Commission staff has received reports from consumers of 201 children who suffered serious burns to their hands, legs, or buttocks when they climbed or sat on the hot metal play equipment. The injuries occurred on bare metal play equipment installed from 1950 to the present at parks, schools and fast-food restaurants. To prevent young children from being burned on bare metal decks, the Miracle Recreation Equipment Company is offering owners of Miracle modular play equipment a covering for all its solid metal decks. Parks, schools and restaurant owners/managers who have Miracle modular play equipment with solid metal decks should call Miracle at 1-800-523-4202 to receive a covering which is designed to prevent young children from being burned on the metal deck. Also, consumers who notice unshaded bare metal decks on public play equipment should contact Miracle to identify equipment that needs to be covered. AUGUST 27, 1993 Release # 93-100 PRODUCT: Five toys/children's jewelry items distributed by Oriental Trading Company, Inc. The items include plastic toy maracas, item #16/008; plastic toy baby bottles, item #16/16; plastic heart-shaped stetchable bracelets; plastic stretchable charm bracelets; and beaded charm necklaces. PROBLEM: During CPSC testing, small parts detached from each of these toys and jewelry items, presenting a choking hazard to young children. WHAT TO DO: Consumers are urged immediately take the recalled products away from young children and return them to the stores where purchased for a full refund. Each bracelet consists of 12 various colored hearts strung together on a elastic cord. The bracelets were sold 12 to a package and were sold in a clear, plastic bag with an affixed adhesive label reading, in part "***Made in China***." - Item #24/394, 15-inch plastic beaded charm necklaces, 39,600 were sold between May 1991 and July 1992. Each necklace consists of a beaded strand with 10 various colored shaped hearts, stars, and other similar items attached. The necklaces were sold 12 to a package and were sold in a clear, plastic bag with an affixed adhesive label reading, in part "***Made in China***." - Item #24/395, 6-inch stretchable charm toy bracelets, 42,000 were sold between June 1991 and July 1992. Each bracelet consists of a beaded strand with various shaped objects, such as hearts, attached. The charm bracelets were sold 12 to a package and were sold in a clear, plastic bag with an affixed adhesive label reading in part "***Made in China***." Neither CPSC nor Oriental Trading Company, Inc. is aware of any injuries involving these products; this recall is being conducted to prevent the possibility of injury. During CPSC testing, small parts detached from each of these toys presenting a potential choking hazard to young children. These toys were found during an inspection of the firm by CPSC. The recalled toys were sold in toy stores, discount stores, and various other retail stores nationwide and ranged in price from 20 to 80 cents each. AUGUST 19, 1993 Release # 93-104 PRODUCT: Approximately 13,000 BISSELL Trio Vac model 3005 (single speed) and model 3005w (canister kit) vacuums manufactured during 1992 and sold nationwide by major retailers through June 1993. PROBLEM: The vacuum's plastic fan may break into pieces causing the housing to crack open and the fan pieces to propel from the vacuum's housing. BISSELL has received 40 reports of such incidents including 12 reported injuries. WHAT TO DO: Consumers who bought BISSELL Trio Vacuums through June 1993 should immediately stop using them and call BISSELL Customer Service at 1-800-237-7691, Monday through Friday, between 8:00 a.m. and 5:00 p.m. (Eastern Time). WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), BISSELL, Inc., Grand Rapids, MI, is voluntarily recalling the BISSELL Trio Vac model 3005 (single speed) and model 3005W (canister kit) vacuums manufactured in 1992. These models convert from full-length to hand-held vacuums and have plastic fans. The vacuum's plastic fan may break into pieces causing the housing to crack open and the hard, sharp fan pieces to be propelled from the vacuum's housing. Models with plastic fans were sold through June 1993. BISSELL has received 40 consumer reports of product failures including 12 reports of injuries. While most injuries were minor cuts and bruises to the lower body, two injuries occurred at eye level. Approximately 13,000 vacuums were sold nationwide through major retailers for about $50 or $60, depending on the model. After receiving retailer returns and direct calls from consumers regarding failed fans, BISSELL began a product recall and reported the problem to CPSC. Consumers who bought BISSELL Trio Vacuums through June 1993 should immediately stop using them and check the date stamp. To check the date stamp, remove the dirt collection tank and read the six digit number which appears on the inside of the bottom motor housing. The first two numbers are the year. If the numbers are 93, the vacuum has a metal fan design and does not need to be returned. If the numbers are 92 or no numbers are visible, the vacuum must be returned to BISSELL Inc. Please call BISSELL Customer Service: 1-800-237-7691, Monday through Friday, between 8:00 a.m. and 5:00 p.m. Eastern Time. BISSELL will replace the 1992 vacuums with 1993 vacuums containing a metal fan or provide a full refund of the purchase price. August 17, 1993 Release # 93-103 PRODUCT: Approximately 100,000 "Fisher-Price Kiddiecraft Racing Rover" cars sold nationwide during 1991 and 1992. The cars are bright yellow with a white and black dog as the driver. The car is propelled forward when the dog's head is pushed down and released. PROBLEM: Some children have removed the top part of the dog's head from the toy, creating a small part that could become a choking hazard. WHAT TO DO: Consumers should cut the plastic ears off of the dog and discard the rest of the toy. Both ears should then be sent to: Fisher-Price, Consumer Affairs, 636 Girard Avenue, East Aurora, NY 14052. Consumers will receive a $10.00 refund. WASHINGTON, DC -- CPSC, Fisher-Price, Inc., East Aurora, NY is voluntarily recalling its Kiddicraft Racing Rover car after learning of several instances in which children removed the top part of the dog's head from the product. This creates a small part that could become a choking hazard. The recall affects approximately 100,000 Racing Rover cars sold nationwide during 1991 and 1992. The Racing Rover toy is a bright yellow race car with a white and black dog as the driver that propels the car forward when its head is pushed down and released. The toy is recommended for children ages one to four years. Fisher-Price has learned of two instances of children choking on the small part created after the top part of the dog's head was removed. CPSC received reports of two additional incidents. No injuries were reported as a result of this problem. To eliminate the potential choking hazard, Fisher-Price is asking consumers to cut the plastic ears off of the dog and discard the rest of the toy. Consumers should then send both ears to Fisher-Price, Consumer Affairs, 636 Girard Avenue, East Aurora, NY 14052, for a $10.00 refund, the approximate retail price. July 22, 1993 Release # 93-095 PRODUCT: "Swing-N-Slide" molded plastic Teddy Bear Swing manufactured by Newco, Inc. of Janesville, WI. The swing is designed for children under 45 pounds. PROBLEM: The swing's waist and crotch straps can pull out of the seat. Children may then slide out of the swings and be injured. WHAT TO DO: Consumers should cut off the Teddy Bear Swing straps and plastic safety bar and send them to Newco to receive a free redesigned Teddy Bear Swing. Consumers may call Newco at 1-800-888-1232 to request postage-paid return packaging or further information. WASHINGTON, DC -- CPSC, Newco, Inc. of Janesville, WI is recalling its "Swing-N-Slide" molded plastic Teddy Bear Swing. The plastic swing seat is shaped like a brown teddy bear and is designed for children under 45 pounds. It has a waist strap and crotch strap that may pull out of the seat, allowing an infant or toddler to fall from the swing and be injured. Newco, Inc. and CPSC learned of this problem from consumers who reported three incidents where toddlers slid out of the swing when the straps pulled out from the seats. The children who fell from the swings received minor cuts and bruises. The Teddy Bear Swings were sold nationwide from February to June 1993 in hardware and home center stores. Approximately 13,000 swings were sold for about $34 each. Consumers should immediately stop using the Teddy Bear Swings. Newco asks that consumers cut off the waist and crotch straps and the plastic safety bar, and return these parts to Newco for a free redesigned Teddy Bear Swing. JUNE 28, 1993 Release # 93-086 WASHINGTON, DC -- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), the B.J. Alan Company, Youngstown, OH is voluntarily recalling its Cosmic Eruption seven-shot rack fireworks display devices. The fireworks display devices may tip over during operation and fire their charges horizontally or at various angles, possibly resulting in serious injury to persons nearby. Although the Commission is not aware of any incidents involving the Cosmic Eruption, the Commission knows of two deaths involving other multiple-shot devices, not distributed by this company, that tipped over, struck, and killed the victims. The Cosmic Eruption devices were imported from January 1993 to June 1993. Each device consists of seven 10-inch mortar tubes with a connecting single fuse. The device is 6-1/2 inches in diameter. The devices are labeled "PHANTOM BRAND COSMIC ERUPTION*** Distributed by Alexander Rori Co., Ltd., Hong Kong, MADE IN CHINA". Approximately 2,600 of the devices were distributed nationwide from January 1993 through distributors in Hawaii, Indiana, Kansas, Ohio, Pennsylvania, Tennessee, and Wisconsin. They retailed for approximately $30.00 each. Consumers are urged to stop using these fireworks devices immediately. Consumers are encouraged to return the devices to the stores or stands where purchased for a full refund. If the stands are no longer open or if consumers have other questions, contact the B.J. PROBLEM: The WHAM-O slides are designed for use by children only. Use by adults and teens has the potential to result in neck injury and paralysis. Because of their weight and height, adults and teenagers who dive onto the water slide may hit and abruptly stop in such a way that could cause permanent spinal cord injury, resulting in quadriplegia or paraplegia. The slider's forward momentum drives the body into the neck and compresses the spinal cord. Kransco reports that seven adults and a 13-year-old teenager suffered neck injuries or paralysis while using WHAM-O slides. WHAT TO DO: The U.S. Consumer Product Safety Commission and Kransco urge adults and teenagers NOT to use WHAM-O backyard slides. Approximately 1.5 million sets were sold between 1973 and 1991. PROBLEM: Strings from children's clothing may become entangled in the area where the handrail attaches to the slide. This presents a strangulation hazard. This notice, originally released in August 1992, is being reissued now to reach consumers who did not receive the earlier announcement. WHAT TO DO: Hedstrom, Blazon-Flexible Flyer, Inc. and Turco are out of business. Consumers should discard or destroy these products immediately. WASHINGTON, DC -- CPSC, Hedstrom Corporation of Bedford, PA, Blazon-Flexible Flyer, Inc. of West Point, MS, and representatives of Turco, a firm that went out of business in 1986, reissued a notice to provide consumers with guards for the handrail attachment area on certain home playground slides. These guards prevent the entanglement of strings from children's clothing, which could result in strangulation. The firms renewed this offer because of low consumer response to the initial notice. Only about 5,000 consumers have called for retrofit kits so far. Altogether, about 1.5 million sets were sold between 1973 and 1991. A child's clothing may catch and possibly result in strangulation. The slides involved are on the "Hedstrom Big T" swing sets and the Blazon-Flexible Flyer "T-Type Play Centers," and on Sears brand T-style gym sets manufactured by all three firms. They should be discarded immediately. The Hedstrom Corporation received a report that a three- year-old-girl strangled when her mitten strings, which were strung through her coat, caught at the point where the handrail attaches to the Big T slide. In addition, the Commission is aware of a hood string of a child's jacket that became entangled in the handrail attachment area of an older, similarly-designed slide whose brand could not be identified. While Blazon-Flexible Flyer, Inc. and Turco are unaware of any reports of entanglement, they are offering the guards along with Hedstrom because their slide/handrail attachment is considered by CPSC to be functionally identical to that of the Hedstrom Big T slide involved in the incidents. Hedstrom Big T and Blazon-Flexible Flyer T-Type Play Center swing sets with slides were sold nationally in retail stores from 1986 through 1991. They sold for approximately $100-200 each. Turco swing sets were sold from 1973 to 1987. Hedstrom, Blazon- Flexible Flyer and Turco also sold their T-style swing sets under the Sears labels with various model numbers. Consumers are urged to stop use of this product immediately. Hedstrom, Blazon-Flexible Flyer, Inc. and Turco are out of business. The remedy is no longer available. Affected models are ASNS-16, ASNS-18, ASNS-19, ASJ-SP-14, and ASJ-SS-14. PROBLEM: Swimmer's hair may become entangled in the suction covers while the swim jets are on. Rio Plastics is taking this action to prevent the risk of hair entrapment and drowning.To date, Rio Plastics is aware of two accidents in which young female swimmers' hair became entangled in one of the suction covers while the swim jets were on. One child drowned. The other child struggled free and sustained injury to her scalp. Five models of the swim spa were sold nationally between 1986 and 1990: ASNS-16, ASNS-18, ASNS-19, ASJ-SP-14, and ASJ-SS-14. These swim spas are recreational products made of thermoplastic/fiberglass, between 14- and 19-feet long depending on the model, that combine an exercise pool with a spa. (A small number of products were sold as 14-foot exercise pools only and did not have a separate spa.) The exercise pool and spa are separated by a wall that houses the control panel, two swim jets, and two suction openings. Each suction opening is protected by a cover designed to prevent hair entrapment. Of the thousands of swim spas sold, about 1300 were equipped with two 10-inch round, white plastic suction covers that need to be replaced. The suction cover was designed and manufactured by another, unrelated company and purchased by Rio Plastics for use in the swim spas. The swim jets located in the separation wall allow the user to swim in place in the exercise pool. Suction is created as water is drawn through the two suction covers, which are designed to be located at the bottom of the separation wall. Hair can become entangled in one or both of the suction covers when the swim jets are on. Consumers who own one of these swim spas should immediately contact Rio Plastics to arrange to receive two new suction covers free, designed to prevent hair entrapment. The finger puppets were distributed free in Taco Bell Kid's Meals from January 25 through March 28, 1993 and the inflatable balls were distributed free in Taco Bell Kid's Meals from March 29 through March 31, 1993. PROBLEM: Taco Bell received complaints that the finger puppets became stuck on the tongues of three children; and several complaints that blowing up the balls caused slight dizziness.
