This recall is for Series 503 Flushmate III Pressure Assist flushing systems installed inside toilet tanks that were manufactured from July 2009 through April 2011. The units are rectangular, black, two-piece vessels made of injection molded plastic. Previously recalled systems were manufactured from October 1997 through June 2009. Recalled units have a date code/serial number that is 16 characters long and is located on the label on the top of the Flushmate III. The first six numerals of the serial number are the date code. The date code range for units included in this recall in MMDDYY format is 070109 (July 1, 2009) through 043011 (April 30, 2011). The date code range for previously recalled systems in MMDDYY format was 101497 (October 14, 1997) through 063009 (June 30, 2009). Units included in this recall were sold individually and installed in toilets manufactured by American Standard, Copperfit, Crane, Ecotech, Eljer, Gerber, Kohler, Lamosa, Mancesa, Mansfield, Orion, St. Thomas, Universal Rundle, Vitra, Vitromex, Vortens and Western Pottery.
This recall involves IKEA PATRULL, PATRULL FAST and PATRULL KLÄMMA safety gates and safety gate extensions. The gates and gate extensions are white, made of beech wood or steel and plastic. The pressure-mounted gates open both inward and outward. They measure about 28 ¾ inches high with an adjustable width from about 28 ¾ inches to 34 ¼ inches. The gate extensions measure about 28 ¾ inches high with an adjustable width from about 2 ½ inches to 5 inches. A permanent label is attached to the metal bar at the bottom of the safety gate containing "IKEA" and the article number. This recall involves Black Cat Glitter fountain cones sold in packages of three. The cones have model number BC269 printed on the bottom of the packaging. The cone-shaped fireworks devices are approximately 6 inches tall and have the Black Cat logo on the packaging. The words "Crackling Glitter," "Colorful Glitter" or "Gold Glitter" is printed in black type on the front of the cone.
This recall involves OMP M-100 digital hanging bow scales that measure the user's peak or holding weight in archery and bow hunting. The orange plastic pocket-size, portable scales measure about 8 inches long by 4 inches wide by 1.2 inches deep. They have three buttons on the front. On one end of the scale there is a hook and a metal ring on the other. M-100 and the OMP logo are printed in black lettering on the front of the scale.
This recall involves the Ecovent Room Sensors, which plug into an electrical outlet, measure room temperature, humidity and barometric pressure. The unit also provides electrical outlets in the center of the device and USB ports on the sides. "Ecovent," model number "SS1," part number "901-00001" and date numbers "4415-5215" and "0116-0916" are printed on a label on the back of the device.
This recall involves The Land of Nod's Octo-rattles, sold individually or as part of a gift bag set. The knit fabric rattles are yellow and shaped like an octopus with multi-colored felt discs sewn into the tentacles. The rattles measure about 9 inches tall, 4 inches wide and 3 inches deep and can attach to strollers or activity gyms. "The Land of Nod," "Morton Grove, IL 60053," "951-3072461," "Made in India" and SKU number 502882 are printed on a label attached to the underside of the rattle.
This recall involves 4-inch air angle grinders with model number 20687. The model number is printed on the packaging. The grinders have a blue handle and some models have the ASC logo on the top.
This recall involves three models of surge protectors, Idealist (360320), Idealist 2.4 (360321) and Agent (360322). The model number is located on the underside of the unit. The units have seven outlets, come in black or white and measure 14.5 inches long by 2.5 inches wide by 1.5 inches tall. "360electrical" is printed on the front of the units. Recalled surge protectors do not have a single check mark engraved on the underside of the unit, right next to the model number.
This recall involves all Bighorn P-20 handgun security safes. The gray and black safes are made out of metal, measure about 3.25 inches high by 12 inches wide by 10.5 inches deep and weigh about 15 pounds. The Bighorn logo in blue and a four-button, back-lit electronic key pad are located on the top of the safe. The handgun security safes have serial numbers 1412160001 through 1412163000 printed on a sticker on the right hand side.
This recall involves AR500 Armor Level III body armor, including chest plates, side plates and ABS panels. The black, steel-core body armor is rated for protection against rifle calibers up to 7.62X51 M80 Ball (.308) at 2,800 feet per second. AR500 Armor is printed on a white label on the back side of the armor. Only AR500 Level III body armor with the manufacture date code of February 2016 and March 2016 are included in the recall.
This recall involves the Hammerhead sleds, which were sold in blue, green, orange and yellow. The front handle bars have a steering system with polycarbonate skis. The rear HDPE skis are attached to the main aluminum frame. The units measure 23 inches wide by 9 inches high by 51 inches deep and weigh about 10 pounds. "Hammerhead Pro" is written on the top of the sled.

This recall involves four models of Tonal Thirst ceramic mugs. The 12-ounce mugs have a glossy two-tone finish with a white exterior and colored interior with matching colored handles. The mugs were sold in white/black, white/blue, white/lime and white/red color combinations. Some mugs can have promotional logos printed on them when distributed as giveaways.
This recall involves all models and serial numbers of the Mohu BeBox smart boombox with built-in Android touchscreen tablet. The unit is a black, portable speaker that measures approximately 13 inches in height, 19 inches in width, 5 inches in depth and weighs about 14.5 pounds. It included a power supply 18V@3.5A and 3.5mm shielded stereo auxiliary cable. The front surface is covered in a black, metallic grill and the back of the unit is black plastic. It features a small Mohu logo on the middle, lower front and an embossed Mohu logo on the middle, center back.

This recall involves 3-inch, 5-inch and 6-inch Mirka CEROS compact electric random orbital sanders. The sanders are yellow and black with "Mirka" on the front. A speed control lever is on the top of the sanders and a vacuum hose connector on the rear. The recalled sanders came with either a cardboard box or plastic carrying case, a 12-foot power cord, DC transformer (only in certain sanders), a wrench, a multi-hole backup pad and assorted abrasives. The 3-inch sander is model CEROS 325. The 5-inch sander is model CEROS 550. The 6-inch sander is model CEROS 650. Model names are on a white sticker on the back of the sander housing beneath the hand grip, and in certain sanders, the serial number can only be found on this sticker. Serial numbers are engraved on the side of the sander housing just above the dust shroud. This recall involves five types of spill-proof Tommee Tippee Sippee cups all with a removable, one-piece white valve. The five types include: First Sips Transition cup, Trainer Sippee cup, Sippee cup (including Cute Quips), Sportee bottle and Insulated Swiggle/Sippee tumblers. The plastic drinking cups have spill-proof tops, easy-grip sides with detachable handles and were sold in clear with colored lids and a variety of colors and character prints. Some of the cups include short phrases on the front of the cups including: "You Pour I'll Fling," or "Drink Drop Repeat." The cups have a 7 to 10 oz. capacity and were sold for children ages 4 to 12 months. Only Tommee Tippee Sippee cups with a removable, one-piece white valve are included in this recall.
This recall involves the Poco AG, Poco AG Plus and Poco AG Premium model child backpack carriers. These nylon carriers come in seaside blue, ivy green and black. They have a metal frame and a gray padded child's seat inside. The back of the carrier also has an upper zippered pocket and a lower zipped compartment, along with a stretch pocket in the middle. The production date code identifying when the carrier was manufactured is stamped on a black label sewn in the interior of the large lower zippered compartment in the back of the carrier. Recalled carriers have a production date code of S16SB03, S16SB04, S16SB05, S16SB06 or S16SB07. "Osprey" is printed on the back at the top of the carrier. The model name is printed on the back at the bottom.
This recall involves an eight-light crystal chandelier. The frame is silver with crystal ornaments and eight candle-shaped lights. The loop at the top of the chandelier is made of zinc or steel. The model number 093682 can be found on the inside of the canopy and on the outside of the original carton.
This recall involves Honda FG110K1AT Mini Tillers. The mini tillers have gasoline engines that power rotating tines to dig into the ground and cultivate the soil. The mini tillers are red and black with "HONDA" printed on the front tine guard and are about 14 inches wide by 39 inches tall. The serial number range included in this recall is from FAAA-1200001 through 1540521. The serial number is printed on the top side cover of the tillers.
